  • As, in Korea, rural economy has a rapid growth and medical health care becomes more necessary for rural inhabitants, we are planning to enlarge the medical care networks and aid childbirth free of charge and perform lower cost medical care in order that more inhabitants are benefited and more available for medical care in rural society. Further, we will make it a basement of the policy to equalize benefits of medical care and medical insurance system.

  • The purpose of this study is to analyze the contents and assessment items in the certification criteria for Child Care Support Apartment program in Japan that is implemented mainly by local authorities with a view to provide information that could be used as reference in enhancement of the residential environment. For this purpose, the architectural planning elements for individual residence units and the public spaces of the Child Care Support Apartment Certification programs in effect in Yokohama city were identified and analyzed. In spite of the recent trends in Korea, in which the residential services are more diversified and segmented in consideration of the needs of the residents and the increased interest in child care in general, the literatures that addressed the direct relationship between childbirth and residences are scarce, and it is anticipated that the result of this study would be of contribution to provide a hint in direction for improvement of residential environment with child support programs.

The Effect of Health Care Program for Newly Married Couple in a Community Health Care Center (일 보건소의 신혼부부 건강프로그램의 효과)

  • Purpose: The research purpose was analysis of the effect of a health education program for newly-married executed from 2003 to 2005 as a part of health care service from community health center and providing the fundamental source for health education needed during newly-married. Method: The data collection with structured questionnaires was conducted during October to December in 2006. From the health center 106 brides who participate the program and 130 brides who didn't participate. Health care program for newly married couple were composed health examinations and health educations. Health examinations were CBC, LFT and Ag & Ab prevallence rates of hepatitis B, rubella. Health educations were done two times, the first individual health education was done at enrolled in health center for establishment of desirable couple relationship, family planning, contraception method and management of pre-pregnancy. The second individual health education was done for explanation of blood examination results, prevention of the congenital deformity and the vertical transmission of hepatitis B, management for pregnancy, breast feeding method, introduction to safety delivery method and encouraging self-study using by materials made by health center after two weeks at revisiting health center. SPSS/PC(ver. 12.0) and $X^2$-test, t-test was used to analyze the collected and tabulated data. Socio-demographic characteristics and regional characteristics of residence area of two groups shows no significant difference. Result: As a variables of experimental effect. The natural childbirth rates of participants group was significantly higher than Non-participants group(p=.012). Breast feeding rates for participants group was also statistically significant higher than Non-participants group. Contraception rates showed no significant difference. The rubella vaccination rates (p=.001) and The hepatitis B vaccination rates (p=.012) shows statistical differences. Conclusion: Therefore health care program for newly married couple in a community health care center was effective generally. We supposed that explore participation ways for nonparticipants, expand the program to community health care center in whole country.

Adaptation in pregnant women: a descriptive phenomenological study using Giorgi's approach (임신 여성의 적응에 관한 Giorgi의 기술적 현상학 연구)

  • Purpose: This descriptive phenomenological study aimed to explore the lived experience and meaning of pregnant women's adaptation. Methods: Ten pregnant women from an ongoing Pregnant Couples' Cohort Study agreed to participate in this study. The data were collected through telephone in-depth interviews regarding what they experienced and felt about pregnancy adaptation. The qualitative data were analyzed using Giorgi's method of descriptive phenomenology. Results: Five core situation components were extracted from the raw data, along with 12 themes and 33 focal meanings. The five core situations were 1) first recognizing the pregnancy, 2) pregnancy-related changes, (3) the upcoming birth, 4) the postpartum period, and 5) parenting. The 12 themes were as follows: "anxiety, pressure, and embarrassment due to pregnancy," "efforts to adapt to physical changes," "efforts to adapt to the psychological difficulties of pregnancy," "efforts to adapt to the financial burden and role changes caused by pregnancy," "connecting with the fetus," "adapting to a new marital relationship centering on the baby," "the frustration of childbirth," "fear of childbirth," "postpartum care, need help with lactation planning," "parenting beyond what I imagined," "dad's willingness to participate in parenting," and "career disconnect and consideration of workplace needs." Conclusion: We identified that pregnant women experience adaptation in physical, psychological, relational, and social aspects. The thematic clusters identified can be used to develop nursing interventions to promote women's adaptation to pregnancy.

A Study on the Space Composition for Department of Delivery in Regional Public Hospital (지역거점 공공병원의 분만부 공간구성에 관한 연구(1))

  • Purpose: This study presents the analysis on space usage of delivery departments in regional public hospitals. The results intention is to achieve improvement of the delivery environment for the mothers and newborns regarding exposure prevention and efficient infection control. The purpose of this study is to provide fundamental data for architectural plans and guidelines for the delivery department. Method: The investigation and analysis were based on research papers, legal systems, public medical statistical data, and the architectural floor plan drawing. For research, 20 regional public hospitals with an operating delivery room were excluded. Regarding data accessibility, 15 regional public hospitals were selected. Results: To overcome the increased vulnerability of the delivery department, the research results of basic data is provided for the establishment to address urgent needs and rapid response. Thus, the research results are as follows: Firstly, the delivery department needs to respond promptly according to the type of patients. For example, in a case of emergency surgery, a connected circulation plan with the related departments is needed. Secondly, for the environment of the delivery area, alleviating anxiety is imperative for pregnant patients and guardians, labor, childbirth, and recovery. Therefore, these needs must be addressed for treatment space and circulation. Lastly, the delivery department is classified into three areas for analysis: access area, treatment area, and support area. In most of the delivery departments of the 15 selected hospitals, there is no space for the access and support area except for the labor and delivery rooms in the treatment area. For the access area, a waiting area, changing room for pregnant women and guardians, and a storage space for contaminated linens are required for infection prevention, safety, and efficiency. For the treatment area, childbirth processes and circulation should have space reserved for labor, delivery, recovery, examination, and treatment. In preparation for an emergency during childbirth, emergency response measures and supporting space needs to be established. For the support area, circulation and rooms are to be designed for medical staff support, activity space, storage and transportation of equipment, and urgent medical treatment. Implications: Along with the low fertility rate and the decrease of medical institutions that operate delivery departments, for the purpose of establishing a public medical service system and a healthy medical environment for mothers and newborns, the researched information demonstrates basic data on space plan of delivery departments in regional public hospitals.

A Study on Mother's Knowledge Level of Breastfeeding's Benefit - Breastfeeding's Benifit and Rationale of Breastfeeding's Benefit - (모유수유에 대한 산모의 지식수준 정도에 관한 연구 - 모유수유 장점과 합리적 근거를 중심으로 -)

  • The purpose of this study was to investigate the mother's knowledge level of breastfeeding's benefit and rationale of breastfeeding's benefit. The subjects were 98 mothers who within 3 days after childbirth in 2 obstetric clinic in K city and N city from April 7 to May 10, 1996. Data were collected by a questionnaire developed by researchers. This study was statistically analyzed according to Frequency, ANOVA, Pearson Correlation Coefficient. The result of this study was as follows : 1. Mean score of mother's knowledge level of breastfeeding's benefit was ranged from $0{\sim}19$. Mean score of breastfeeding's benefit was 10.28. Mean score of rationale of breastfeeding's benefit was 8.57. Mean score of rationale of breastfeeding's benefit and rationale of breastfeeding's benefit was not high. 2. General characteristics and feeding characteristics were significantly related to mother's knowledge level of breastfeeding's benefit as follows : mixed feeding method in planning feeding method(F=3.33, p<.05) 3. General characteristics and feeding characteristics were significantly related to mother's knowledge level of rationale of breastfeeding's benefit as follows : mixed feeding method in planning method(F=3.96, p<.05), mixed feeding method in previous feeding method(F=3.16, p<.05). 4. A positive correlation between mother's knowledge of breastfeeding's benefit and rationale of breastfeeding's benefit was statistically significant(r=.86, p<.001).

A Study on the Size Planning of Parking-Block for Pregnant Women (임산부를 위한 주차구획의 규모설정에 관한 연구)

  • Purpose and Methods: Currently, Korea has the lowest fertility rate among OECD countries. It is also true that from the aspect of encouraging childbirth policy, as the use of automobiles has increased, the interest in securing parking spaces has also become more important. However, there are efforts and researches on the supplying parking spaces for the general public in society not to the positive discrimination such as elders or pregnant women. On the other hand, there is a growing need for a dedicated parking space for pregnant women due to the changing social environment and increasing number of female drivers. At this point, proper design of the maternity parking spaces is essential. Therefore, this study is analyzing the characteristics of pregnant women such as measurements and certain actions and case studies of domestic and oversea. Through the analysis of these factors, the following results were derived. Results: First, it was determined that the important factor in the planning of the parking-block for pregnant women was the human body size of pregnant women. And the required dimensions of movement of pregnant women were derived. Second, the dimensions of automobile were analyzed and applied from the most used vehicles in the family composition. Third, after the observations, the status of the parking spaces of the pregnant women and observations showed that the drivers not only the pregnant women but others were using the spaces improperly. Fourth, at certain conditions of the parking spaces, in order to improve the efficiency of the parking spaces, it is necessarily considered that the usage of designated accessible parking spaces can be used together by not only the disabled but also to the passengers who are classified as 'transportation weak' such as pregnant women. Implications: In this study, the researcher will propose the shape and size of the parking-block for pregnant women considering the elements derived above.

The Planning Characteristics Analyzed by Spatial Composition of Domestic Share House

  • Purpose: Recently, a sudden increase in one household in Korea has become an important social problem. Changes in lifestyles such as population change, marriage, childbirth, and divorce are becoming increasing factors for single-family households. As a result of these changes, the government has implemented policies for one accredited state. However, the policy for one domestic applicant is insufficient compared to other countries. As a measure to cope with the increase in the number of applicants, Shared House has emerged and research on Shared House is necessary. Method: First, we analyze the overall characteristics of domestic share house. And it does a spatial analysis of domestic share house. Especially, it analyzes the relationship between private space and public space. Finally, the plan characteristics are derived based on the analysis results. Result: The results are as follows. First, the type of share house is classified according to the combination of the arrangement of private space and the public space. Second, the larger the scale, the more vertical arrangement than horizontal arrangement. Finally, the character of the share house changes according to the characteristics of the resident.

A Study on the Location and Functional Arrangement Characteristics of the Neonatal Intensive Care Unit(NICU) in a Certified Tertiary General Hospital (상급종합병원의 신생아 집중치료실(NICU) 설치 위치 및 기능 배치 특성 연구)

  • Purpose: The purpose of this study is to present basic data on installation location, functional connection, and arrangement to be considered when planning a neonatal intensive care unit in a certified tertiary general hospital. Method: Review the facility standards related to the neonatal intensive care unit, such as laws and guidelines, and select a Certified Tertiary General Hospital as a case hospital among the Neonatal Intensive Care Regional Centers designated by the Ministry of Health and Welfare to analyze the placement characteristics of major NICU components such as the location of the NICU installation, the connection of movement between adjacent facilities and functions, and the treatment room and accessory room. Results and Implications: NICU is a facility for critically ill babies among newborns and needs to consider more about infection and patient management compared to adult intensive care units, which not only install close to delivery-related facilities to enable immediate action after childbirth, but also divide the treatment room into open rooms and allow access to the attached rooms through dedicated corridors to minimize movement in the treatment room.

Application of NANDA and HHCC to Classification of Nursing Diagnosis in a Hospital-Based Home Health Care (일개 종합병원중심 가정간호 간호진단분류를 위한 NANDA와 HHCC의 적용 비교)

  • This study examines that North American Nursing Diagnosis Association(NANDA) and Home Health Care Classification(HHCC) is appropriate to classify home health care client's nursing problems and suggests a modified nursing diagnosis classification system. Two hundred and forty-nine clients' records at a general hospital were reviewed and nursing problems were diagnosed according to each classification system. Results of this study are as follows. The major client's medical diagnosis are pregnancy, childbirth and puerperium, malignant neoplasm, and benign neoplasm. Of four hundred and sixty-three nursing problems, all nursing problems made a diagnos according to HHCC, while three hundred and eighty-five made a diagnosis according to NANDA. The HHCC diagnosis included 78 more nursing problems than NANDA. The discrepancy in the results may indicate a significant advantage to HHCC diagnosis because HHCC nomenclature was created empirically from hard data. However, this may be due to limitations in the data collection method so determination of which classification system is more useful is difficult to judge. However, nursing components of the HHCC are more concrete and clearer than human response patterns of the NANDA. Also the HHCC facilitates the documentation of patient care by computer, while using a conceptual framework consisting of 20 Care Components based on the nursing process: assessment, diagnosis, outcome identification, planning, implementation and evaluation. Accordingly, the practical application of HHCC is more useful than NANDA. Limitations of this study include a retrospective data collecting method and universality of samples. Further research for various samples that use prospective data collection method is recommended.
