애완 이구아나의 피하에 발생한 건락괴사성 종류 (Subcutaneous Caseous Necrotic Mass in a Pet Iguana (Iguana iguana))

  • A subcutaneous mass foud in a nine-year-old male Iguana (Iguana iguana) was pathologically and microbiologically examined. Grossly, this mass, measuring $2 cm{\times}1.5 cm{\times}1 cm$ in size, contained white to yellow caseous-crumbly material which was surrounded by the thin connective tissue. Histopathologically, a mass consisted of central caseous necrotic debri in which heterophils and macrophages were scattered, and peripheral fibrous connective tissue in which scattered heterophils, macrophages and lymphoid cell aggregates were present. This mass composed of central necrotic core and peripheal connective tissue did not have any detectable microorganisms including acid-fast bacteria and fungi. Histopathologically, severe atrophy of seminiferous tubules in both testicles was observed. In addition, peripheri of ductus deferens was thickened by fibrous connective tissue. This is the first report of subcutaneous caseous necrotic mass of an Iguana in Korea. In addition, the bilateral severe atrophy of seminiferous tubules may be due to fibrosis of ductus deferens.

  • Caseous lymphadenitis (CLA) is a chronic and contagious disease of sheep and goats caused by Corynebacterium (C.) pseudotuberculosis. A four-year-old female Saanen dairy goat was submitted to the Animal Disease Diagnostic Center at National Veterinary Research and Quarantine Service. The clinical signs of the goat were emaciation, abortion and quadriplegia. The multifocal nodules of lymph nodes were encapsulated and filled with whitish caseous contents on the cut surface. Histopathologically, lymph nodes displayed suppurative and necrotizing granulomas. Caseous necrosis was diffusely observed in the center of the lymph nodes. Gram positive bacilli were shown in the lesions. C. pseudotuberculosis was isolated and confirmed by the biochemical tests and PCR assay. Based on clinical signs, histopathological examination and bacterial isolation, we diagnosed this case as CLA. To our knowledge, this is the first report of CLA in a Saanen dairy goat in Korea.

Cerebral Toxoplasmosis Combined with Disseminated Tuberculosis

  • A 24-year-old man presented with mental change, fever, abdominal pain, tenderness and palpable mass on the lower abdomen. He was a non-Korean engineer and did not accompany a legal guardian, so medical history taking was difficult due to his mental status. Brain magnetic resonance imaging showed multiple rim-enhanced lesions of the brain, and abdominal computed tomography showed huge paraspinal abscess. Chest X-ray and computed tomography showed poorly defined nodular opacities. We initially thought that this patient was infected with toxoplasmosis with typical cerebral image finding and immunoglobulin laboratory finding of cerebrospinal fluid and serum study. The abdominal abscess was confirmed as tuberculosis through the pathologic finding of caseous necrosis. We used anti-tuberculosis medication and anti-toxoplasmosis medication for almost 4 months, and then his clinical state and radiological findings were considerably improved.

  • Tuberculomas of the lung are rare in children and one of the more common lesions presenting a solitary pulmonary nodule roetgenorgraphically. We are reporting of a 3-year-old child with a tuberculoma in left upper lobe. The patient was initially diagnosed as the benign mediastinal tumor but in the end as tuberculoma in left uper lobe. Wedge resection including the mass was done. The tumor had brown smooth external surfaces on sectioning including the mass was done. The tumor had brown smooth external surfaces on sectioning show pale gray and soft cut surface was shown. In light electromicroscopy chronic granulomas with multinucleated giant cells and central caseous necrosis were observed which are the characteristics of tuberculoma. The postopeative course was smooth and uneventful and patient has been well for 4 months postoperatively.

병아리 Aspergillosis 1례 (Report of Aspergillosis of a chick in Korea)

  • A case of Aspergillosis is reported in a 12-day-old chick. The chick died with clinical signs such as dyspnea, gasping, and accelerated breathing. At necropsy, small white caseous nodules (approximately 1 mm in diameter) scattered throughout lung tissue. Upon H&I staining, multiple granulomas were observed in tertialy bronchi. Lesions consisted of central area of necrosis surrounded by macrophages, giant cells, lymphocytes, and some fibrous tissues. Hyphae of Aspergillus in the lesion branched into Y type, which was observed by PAS staining for fungus.

  • Comparative studies of histopathologic lesions from 23 purified protein derivative (PPD) positive cattle, 21 slaughter cattle found with tuberculosis, and 11 tuberculosis-positive elk (Cervus elaphus) were performed. PPD positive cattle did not show specific histopathologic lesions in all 23 heads that were no visible lesion reactor. Slaughter cattle found with tuberculosis revealed microscopically classical granulomatous lesion (tubercle) with central caseous necrosis surrounded by mantle of epithelioid cells and Langhan's giant cells capsuled by connective tissue in lung. Elk was noted with some different lesion patterns with classical granulomatous lesion and suppurative abscesses that was composed of fibrin, degenerated cells without having connective tissue. In addition, many Langhan's giant cells infiltration in alveoli at peripheral lesion were observed in some cases of classical granulomatous lesion and suppurative abscesses.

만성 교약성 심낭염의 외과적 치료 (Surgical Treatment of Chronic Constrictive Pericarditis)

  • Forty two patients with chronic constrictive pericarditis, who were admitted to the Yonsei University College of medicine over a period of 18 years from January, 1970 to August, 1988, were analyzed retrospectively. Mean age of the patient was 33.5 year ranging from 6.8 to 60 years old. Male to female ratio was 1.3 to 1. Twenty-one cases [50%] were tuberculous origin [based on either associated pulmonary tuberculosis and/or caseous necrosis in thickened pericardial specimen] and 17 cases [40.5%] were idiopathic [non specific chronic inflammatory change was considered to be idiopathic]. Dyspnea on exertion was evident in 30 cases [71.4%] and abdominal distention in 21 cases [50%]. On physical examination, hepatomegaly [83.3%], neck vein distention [54.8%], distant heart sound [47.6%] and ascites were found. Thirty-nine patients showed low voltage of QRS and/or T wave flattening or inversion on EKG. Thirty-one cases had undergone cardiac catheterization which showed data compatible with chronic constrictive pericarditis. Midsternostomy group [n=15] had shown the most remarkable CVP decline [12.20 mmHg] as compared with bilateral submammary incision group [n=25, 8.96 mmHg] and left thoracotomy group [n=2, 7.75 mmHg] but difference was not significant statistically There was four early death among 42 patients [9.5%] including 3 cases of left ventricular failure and one cardiac tamponade. Main postoperative complications were wound infection [6 cases] and arrhythmia [3 cases]. Follow-up of 24 patients [mean; 55.3 months, ranging from 2 months to 155 months] revealed good functional status.

원발성 흉골결핵의 외과적 치험 -1례 보고- (Primary Sternal Tuberculosis with Spontaneous Fracture Treated by Resection and Reconstruction -A Case Report-)

  • 본 강북삼성병원 흉부외과에서는 병적골절이 동반된 원발성 흉골결핵을 치험 하였다. 22세된 여자환자가 특별한 외상이없이 발생한 흉골부위의 통증 및 골절로 입원 하였다. 입원당시 시행한 흉부단순촬영 및 전산화단층촬영상 흉골의 괴사소견과 연부조직의 부종이 관찰되었다. 세침세포검사상 냉농양을 동반한 결핵성 골수염으로 진단되어 술전 항결핵 치료후 수술을 시행하였다. 수술시 흉골중앙부위의 골괴사 및 다발성 누공을 확인하였으며, 골괴사 부위를 제거한후 환자의 장골능을 골이식 하였다. 병리조직학적 결과는 결핵성 골수염의 소견 및 치즈양괴사를 보였다. 술후 경과는 양호 하였으며, 2개월동안의 추적검사상 흉골의 불안정은 관찰되지 않았다.

BCG 접종후 발생한 결핵성 농양 및 골수염 1례 (A Case of BCG Osteomyelitis)

  • BCG osteomyelitis is rare comlication and the incidence rate has been estimated to be 1/milion vaccinated neonate and infants. BCG osteomyelitis is also reported as a complication of intravesical BCG treatment for bladder carcinoma. We had experienced a 14 months old infant who presented swelling, tenderness and redness on left upper arm suspicious due to BCG vaccination. The MRI finding showed $2{\times}4{\times}4cm$ subcutaneous abscess with cortical defect on proximal humerus and axillary lymph adenopathy. The histologic finding showed diffuse caseous necrosis and Langhans type giant cell. We report A case of BCG osteomyelitis.

비복신경에 발생한 결핵종: 증례 보고 (Sural Nerve Tuberculoma: A Case Report)

  • Nearly one third of the world's population have active or latent tuberculosis, resulting in 1.5 million deaths annually. Tuberculosis involving the peripheral nerve is difficult to detect. Sural nerve tuberculoma is an extremely rare case of tuberculous involvement of the peripheral nerve that has attracted the attention of physicians. This paper reports a patient with sural nerve tuberculoma. A 58-year-old female patient presented with a palpable mass on the posterolateral calf with progressive tingling sensation on the distal area. The patient had no history of trauma and it was unclear whether the patient had any contact with individuals with active tuberculosis. The histopathologic findings revealed a granuloma-like lesion with caseous necrosis that was compatible with tuberculoma.
