A study on the medical system and policies of Jeju-mok in the Joseon Dynasty (조선시대 제주목의 의료제도 및 의정(醫政))

  • During the Joseon Dynasty, Jeju had a unique aspect that differentiated it from other regions in terms of their medical system, such as the exclusive deployment of shimyak dispatched to Gamyeong and Barracks units due to the uniqueness of being an island. This study uses various historical sources to verify that these differences existed throughout the medical system and procedures of Jeju in the late Joseon Dynasty. The following significant conclusions were drawn: 1) Looking at the work and characters of Jejushimyak reveals the inherent limitations of Jeju medical care in the Joseon Dynasty. Compared to other regions' shimyak, Jejushimyak had two limitations: it was difficult to engage in only medicine and the quality of medical doctors declined due to the avoidance of major medical doctors' households. 2) The establishment of public health care in Jeju through Medical Cadets failed. Jeju medical science obviously played an essential role in public health care in the early 18th century. However, there was no continuity in the garden, etc. Hyangri, who was in charge of Medical Cadets, was in charge of various fragrances as needed. Thus expertise in medicine was lacking. 3) The cultivated herbs of Jeju's herb field show the failure to supply herbs for institutional medicine. It was impossible to supply enough herbs to implement institutional medicine in Jeju. In that case, it would have been necessary to discover alternative local herbs or to bring them in from outside, but there was barely any such effort. In conclusion, in the late Joseon Dynasty, Jeju failed to establish a foundation for centrally administered institutional medicine. There was a lack of all the entities that provided medical care and herbs that could be used for medical care. The reason that Jeju continued to follow traditional shaman medicine in the late Joseon Dynasty was because there was no other alternative.

TEMPERATURE CONTROL AND COMPRESSIVE STRENGTH ASSESSMENT OF IN-PLACE CONCRETE STRUCTURES USING THE WIRELESS TEMPERATURE MEASURING SYSTEM BASED ON THE UBIQUITOUS SENSOR NETWORK

  • The temperature control of in-place concrete is the most important factor for an early age of curing concrete. Heat stress of mass concrete caused by the heat of hydration can induce the crack of concrete, and a frost damage from cold weather casting concrete results defect on compressive strength and degradation of durability. Therefore, success and failure of concrete work is dependant on the measurement and control of concrete temperature. In addition, the compressive strength assessment of in-place concrete obtained from the maturity calculated from the history of temperature make a reduction of construction cycle time, possible. For that purpose, wireless temperature measuring system was developed to control temperature and assess strength of concrete. And, it was possible to monitor the temperature of concrete over 1km apart from site office and to take a proper measure; mesh-type network was developed for wireless sensor. Furthermore, curing control system that contains the program capable to calculate the maturity of concrete from the history of temperature and to assess the compressive strength of concrete was established. In this study, organization and practical method of developed curing control system are presented; base on in-place application case.

Surgical Treatment of Pulmonary Aspergillosis[II] (폐 Aspergollosis의 외과적 치료[제 2보])

  • This investigation is designed to illustrate the clinical features '||'&'||' preoperative diagnosis, surgical role in the management of pulmonary aspergilloma, '||'&'||' compare with the previous study. We reviewed 42 cases of surgically treated pulmonary aspergillosis from Jan. 1984 to July 1992. The peak age incidence laid in the 2nd '||'&'||' 3rd decade of 25 cases[59.5%]. The 41 cases[97.6%] had a history of treatment with anti-tuberculous drugs under impression of pulmonary tuberculosis. The most common complaint was hemoptysis in 28 cases[66.7%]. The 21 cases showed so called "Air-meniscus sign" on the preoperative chest X-ray. As a preoperative diagnostic modality, the positivity was 30.0%, 57.8%, 88.5% on the fungus culture of sputum for Aspergillus, Chest CT, serum immunodiffusion test for A. fumigatus, respectively. The 37 cases[88.1%] can be diagnosed as pulmonary aspergillosis pre-operatively by any diagnostic method. The anatomical location of aspergilloma was mainly upper lobe in 17 cases[40.5%] '||'&'||' the majority of cases were managed by lobectomy. The postoperative pathologic findings showed that 18 cases[41.0%] were combined with tuberculosis '||'&'||' 15 cases[34.1%] were not combined with any other disease[Pr imary Aspergillosis]. The 6 cases showed postoperative empyema including 4 cases of bronchopleural fistula, 3 cases showed postoperative bleeding. One case was died postoperatively due to respiratory failure. In conclusion, when the patient who has longstanding history fo pulmonary tuberculosis '||'&'||' hemoptysis, must be suspected fungal super infection. The resectional surgery is the treatment of choice for symptomatic localized disease. And compare with the previous study, preop. chest CT '||'&'||' immunodiffusion test were more commonly available '||'&'||' showed high positivity.

Slope Stability Analysis and Suggestion of Stabilization Methods on Failed Cut-Slopes Interbedded with Weakness Layer (연약층이 협재된 절개면의 안정성 해석 및 대책)

  • This study is performed to investigate the cause of slope failure and to suggest suitable stabilization methods for the failed rock slopes. The slope which is located along the national highway between Maesan and Kakok in Dangjin-gun failed during the construction of roads. Site investigation, drilling program, field measurements, rainfall records, and stability analyses are used to investigate the cause of the slope failure. The problem determining the cut-slope angle based on the existing design manual for the construction of roads is reviewed based on the case history given in this paper. If weakness layer and geological structures such as folds and faults are developed in a slope, slope failure is possible even though the direction of slope and the direction of discontinuities depart more than $30^{\circ}$.

Disseminated Histiocytic Sarcoma with Ureteric Involvement in a Jindo Dog

  • We describe the case of a Jindo dog that presented with a 2-month history of an abscess-like mass on the skin of the left hip. The patient was initially diagnosed with a suspected insect bite and was treated with a topical dressing. After observing no improvements, surgical debridement was done to remove the abscess. Nine days after surgery, the patient started showing severe anorexia, vomiting, lethargy, and oliguria. Azotemia was detected, and the patient was diagnosed with acute kidney injury. Ultrasonography depicted an abdominal mass (4.6 × 7.5 cm) in the right ureter, and severe hydronephrosis in the right kidney. The patient was euthanatized. Results of the necropsy, histopathologic examination, and immunohistochemistry for tumor cells, confirmed that the patient had developed hydronephrosis and disseminated histiocytic sarcoma which had metastasized to the lungs. To the best of our knowledge, this is the first report of acute renal failure produced by ureter invasion and urinary tract obstruction resulting from the abdominal mass of disseminated histiocytic sarcoma in dogs.

Development of a nonlinear seismic response capacity spectrum method for intake towers of dams

  • The seismic-induced failure of a dam could have catastrophic consequences associated with the sudden release of the impounded reservoir. Depending on the severity of the seismic hazard, the characteristics and size of the dam-reservoir system, preventing such a failure scenario could be a problem of critical importance. In many cases, the release of water is controlled through a reinforced-concrete intake tower. This paper describes the application of a static nonlinear procedure known as the Capacity Spectrum Method (CSM) to evaluate the structural integrity of intake towers subject to seismic ground motion. Three variants of the CSM are considered: a multimodal pushover scheme, which uses the idea proposed by Chopra and Goel (2002); an adaptive pushover variant, in which the change in the stiffness of the structure is considered; and a combination of both approaches. The effects caused by the water surrounding the intake tower, as well as any water contained inside the hollow structure, are accounted for by added hydrodynamic masses. A typical structure is used as a case study, and the accuracy of the CSM analyses is assessed with time history analyses performed using commercial and structural analysis programs developed in Matlab.

A Fatal Intracerebral Hemorrhage Complicated by Compartment Syndrome of the Upper Arm (상지 구획 증후군 이후 발생한 치명적인 뇌출혈)

  • Compartment syndrome has a wide spectrum from muscle pain to a life- threatening condition, such as acute renal failure and disseminated intravascular coagulation (DIC). Intracerebral hemorrhage (ICH) due to compartment syndrome has not been reported. We report a patient who presented with ICH leading to death. A 25-year-old female with no significant past history developed extensive compartment syndrome followed by rhabdomyolysis, acute renal failure, DIC, and ICH. Although the patient underwent a fasciotomy and hemodialysis and received aggressive resuscitation with massive transfusions of blood and intravenous fluids, she died. This case stresses the importance of early diagnosis and prompt treatment of compartment syndrome to prevent devastating complications.

  • It is unavoidable to be formed slopes under special circumstance of Korea where 70% of the whole area are composed of mountains when civil engineering projects such as roads, site developments are increased with industrial development and horizontal expansions of urban area. Moreover, stability of the slopes become one of quite important issues under special meteorological characteristics that over two-thirds of annual average rainfall are concentrated in summer season and the localized torrential downpour are getting more frequent recently. Resulting in these circumstances, partial slope failures by debris flow of the high water content soils are occurred frequently in cutting soil slopes. In this case of debris flow slope failure, slope declination method are selected for their stable recovery because it is impossible to recover entirely by existing recovery methods. Seeding or special grass planting methods are followed separately without exception. The method by which entire recover with bigger stability ratio would be possible and grass planting work would be done simultaneously is developed. In this study, the results of the tests including a real construction history in a failed slope using developed method are described

Metronomic Chemotherapy with Toceranib Phosphate for Treatment of a Chemodectoma in a Dog

  • A 10-year-old, spayed, female Cocker Spaniel was referred to our hospital with a history of a cough and dyspnea. The patient was tentatively diagnosed with a chemodectoma based on clinical features evident on echocardiography and computed tomography. Metronomic chemotherapy utilizing toceranib phosphate as well as medications for congestive heart failure were administered. During the period chemotherapy was administered (nine months from the time of diagnosis), clinical improvement was noted without the development of any adverse effects. However, clinical signs recurred after chemotherapy was discontinued at the owner's request. When the patient was reevaluated via computed tomography, it was found that the size of the mass had increased. The patient developed severe dyspnea secondary to recurrent pleural effusion and was euthanized 65 days after clinical signs reappeared. A necropsy was performed and the patient was definitively diagnosed with a chemodectoma based on histopathologic and immunohistochemical analysis. This case report describes the clinical application of metronomic chemotherapy with toceranib phosphate, which is a tyrosine kinase inhibitor, in the treatment of a chemodectoma. We propose that this treatment may improve the quality of life and result in a prolonged survival time compared to treatment with medications for congestive heart failure alone.

A Development of Integrity Evaluation System Based on Elastic Plastic Fracture Mechanics(I) - Specimen Cases - (탄소성 파괴역학적 건전성 평가 시스템의 개발 I)

  • A practically useful system for elastic-plastic fracture mechanics analysis has been developed. The developed system is comprised of the deformation plasticity failure assessment diagram(DPFAD) approach and the J-integral/Tearing modulus(J/T) approach. The system contains analysis routines for five types of fracture specimens : compact tension, center cracked tension, single edge craked plate in uniform tension, single edge cracked plate in three point bending and double edge cracked plate in tension. A double interpolation scheme was adopted to interpolate J values from the EPRI developed EPFM handbook and the Newton-Raphson method was used to obtain proper loadings for displacement control conditions. A graphic output system was utilized to present numerical results. Several case studies were performed to evaluate the accuracy and the usefulness of the code. It was found that the J/T approach and the DPFAD approach yielded similar results. However, the DPFAD approach is more convenient for qick assessment of integrity of cracked structures while the J/T approach is more useful in evaluating the full history of the fracture process.
