The Scenario Approach Method for Family Nursing Intervention Based on the Movie 'Mabin's Room' (영화 '마빈스룸' 가족간호중재 -시나리오 접근법-)

  • Caring is an essential element of nursing. In a family with a patient. a critical situation may happen not only for the patient but also for the members of the family. Therefore, a caring service from the healthcare providers can also be requested for the family members. The movie 'Mabin's Room' deals with family problems. In this study, the assessment, problem, diagnosis and planning for the family nursing situation for family problems were made focusing on the role of characters in the movie 'Mabin's Room'. Regarding family nursing intervention, the framework of the story 'Mabin's Room' was reorganized to solve the problems based on the role of community health nurse. In this scenario approach method, the situation in the movie is used without change but the scenario related to the roles (or communication) of community health nurse for solving the family problems is added. It is a problem solving oriented method by reorganizing the scenario in a movie story situation. The reorganized scenario in this study is just an example of scenario approach method. Community Health Nurses can cultivate their creativity by solving various problems in the community by adopting or modifying this simple scenario in practice.

Analysis of the supportive care needs of the parents of preterm children in South Korea using big data text-mining: Topic modeling

  • Purpose: The purpose of this study was to identify the supportive care needs of parents of preterm children in South Korea using text data from a portal site. Methods: In total, 628 online newspaper articles and 1,966 social network service posts published between January 1 and December 31, 2019 were analyzed. The procedures in this study were conducted in the following order: keyword selection, data collection, morpheme analysis, keyword analysis, and topic modeling. Results: The term "yirundung-yi", which is a native Korean word referring to premature infants, was confirmed to be a useful term for parents. The following four topics were identified as the supportive care needs of parents of preterm children: 1) a vague fear of caring for a baby upon imminent neonatal intensive care unit discharge, 2) real-world difficulties encountered while caring for preterm children, 3) concerns about growth and development problems, and 4) anxiety about possible complications. Conclusion: Supportive care interventions for parents of preterm children should include general parenting methods for babies. A team composed of multidisciplinary experts must support the individual growth and development of preterm children and manage the complications of prematurity using highly accessible media.

A Study on Patients' Satisfaction and Service Utilization in the DRG Based Payment System - Patients who Experienced Cesarean Section Before and After the Demonstration Program - (DRG 지불제도에서 환자의 의료서비스 만족도와 제공량에 관한 연구 - 시범사업 전.후 제왕절개 분만 경험 산모를 대상으로 -)

  • Background : The objectives of this study were to examine patients' satisfaction with the DRG based payment method and its association with their awareness of the method, to examine patient reported changes in doctors' caring attitude, level of their out-of-pocket payments, providers' acceptance of patients' request for additional services after the program, and to examine changes in service utilization recorded in medical records. Method : One hundred-four patients who had cesarean sections before and after the demonstration program at two hospitals located in Seoul participated in the study. Patients were surveyed before discharges when their charges were finalized. Their medical records were reviewed as well to collect data for service utilization during hospital stays. The association between patients' satisfaction with the payment method and their awareness of the method was analyzed by ${\chi}^2$-test, and the significance of changes in providers' acceptance of patients' request for additional services and service utilization after the program were examined by ${\chi}^2$-test and t-test, respectively. Results : A large proportion of patients did not know of the DRG based payment method at the time of survey and a significantly larger proportion of patients who came to the hospitals with the knowledge satisfied with the method. About the same proportion of patients reported improvement and deterioration in doctors' caring attitude compare to the previous hospitalizations and a similar result was found concerning out-of-pocket payments. Providers' acceptance of patients' request for medication, PCA and painless delivery decreased significantly after the program whereas the acceptance for additional hospital days and laboratory and radiology tests did not. Length of stay, the numbers of days on antibiotics and antianemic medication, and the number of blood tests decreased significantly after the program, however, decreases in the rate of antianemic medication and the number of urine analyses were not statistically significant. Re-operation, in-hospital death, and complications were not observed before and after the program. Conclusion : The study findings indicated a need for better patient education and publicity about the newly introduced payment method to improve their satisfaction with the system. Other study findings concerning service utilization and quality of care indicators were consistent with the government funded evaluation studies.

  • This study is to propose the dwelling planning of collective housing for the senior citizen who lives alone in rural and fishing villages. and this paper analyzed a change element regarding the field of care service for the elderly. The results of research are as follows: First, the range a village unit of "Home for the Elderly" was investigated most primarily. Second, on the lifestyle of a bedroom, proper using person per a room was investigated to two people to use with one room and life style of using bed preferred more in life style of sitting on the floor(ondol). Third, most necessary facilities were examined into bathroom and steam-room, and the separation of man and woman space, toilet was investigated to 1 ranking. Fourth, to be most difficult care service was investigated by long-distance circulation. This is the item which can be reduced naturally in case of service activity caring for a visit in a collective housing.

Impact of Work Environment and Job Satisfaction on Service Quality among Staff in Elderly Facility (노인요양시설 종사자의 업무환경, 직무만족이 서비스 질에 미치는 영향)

  • Purpose: This study examined the effect of work environment and job satisfaction on service quality among staff caring for the elderly. Methods: A convenient sample was 192 staff members from 95 elderly facilities. Data collection occurred from June to July, 2011 using a self-administered questionnaire. Data was analyzed using SPSS/WIN 18.0. Results: Among the subscales of Work Environment Scale, the self-realization score was the highest followed by workload, nervousness, and conflict. The mean score of job satisfaction was 71.43. Reliability was the highest followed by assurance, empathy, responsiveness, and tangibles. Predictive factors of service quality included self-realization work environment and job satisfaction. Conclusion: Exploration of strategies to improve the recognition of self-realization work environment and job satisfaction will be necessary in order to increase of service quality among staffs in elderly facility.

A Study on Human-Centered IT Utilization in Caring for Elderly People Who Live Alone (독거노인 돌봄에 있어 인간중심의 IT 활용방안에 관한 연구)

  • This study was conducted to find ways to utilize human-centered IT in caring for elderly people who live alone. Through focus group interviews with experts, this study investigated the problems with delivery system, and ethical issues. Problems such as lack of trust, supplier-centered care, and uniform service provision were derived as major problems in the delivery system. These findings indicate that IT should be used as an auxiliary means of face-to-face services and to be controllable and convenient. Issues such as "guaranteeing the right to self-determination," "protecting privacy," "sufficiently guaranteeing the right to know," and "encompassing blind spots" were raised as important ethical issues related to human-centered IT utilization. Based on the research results, this study presented the necessity of designing user-centered information technology and the necessity of developing ethical indicators for the use of human-centered technology.

The Causal Relationship of Hospital Inpatient's Perceived Quality, Satisfaction, Service Value, and Intention to Revisit (병원입원환자가 인지하는 의료서비스 질, 만족도, 서비스가치, 병원 재이용 의사간의 인과관계분석)

  • The objective of this study is to analyze the causal relationship of hospital inpatient's perceived quality, overall satisfaction, service value, and future intention to revisit. To carry out this objective, first we analyzed the dimensions of inpatient care service quality using SERVQUAL scale. The SERVQUAL scale is based on the gap theory, that is, the difference of patients' expectations and the actually received medical care service in hospital. On the basis of this theory, we measured the inpatient's perceived service quality and overall patient satisfaction. Data was gathered from a self-administered questionnaire at a 980 bed university hospital in Inchon City. These questionnaire measuring the service quality were distributed to 250 inpatients. The response rate was 66.4%. A total of 166 questionnaires was finally analyzed. To categorize medical service quality, the factor analysis was performed on 42 items. The reliability and validity of these items was evaluated. Finally to test 6 hypotheses, we analyzed the causal relationship of service quality, overall satisfaction, service value, and intention to revisit through the structural equation modeling(SEM). The major results of this study are as follows. First, the dimension of inpatient service quality was categorized into 7 dimensions, that is, personal caring, communication, access, physical environment, facilities and equipment, cleanliness, appropriateness and health status. Second, the reliability and validity of inpatient service quality items was satisfied. Third, as a result of structural equation modeling, the effect of inpatient's perceived service quality on overall satisfaction, service value, and intention to revisit was statistically significant. And total effect on intention to revisit as the core endogenous variable was perceived service quality(1.100), patient satisfaction(0.006), and service value(0.605).

The Effects of Job Stress and Mental Health of Care Service Workers on Self-diagnosed Oral Symptom in Seoul (서울시 돌봄 서비스 노동자의 직무스트레스와 정신건강이 주관적 구강 증상에 미치는 영향)

  • This study examined the effect of job stress and mental health factors on the oral symptoms of caring service workers in Seoul. For the research method, a survey was conducted from July 24 to August 6, targeting 150 caring service workers. The survey items were job stress, mental health (physical symptoms, anxiety, and insomnia), and self-diagnosed oral symptoms. The results of the study were as follows. Job stress (p=0.001) had a significant effect on the oral symptoms. In addition, physical symptoms (p<0.001), anxiety symptoms (p<0.001), and insomnia symptoms (p<0.001) all had significant effects on the oral symptoms. Among the risk factors, physical symptoms had the greatest influence on oral symptoms. In addition, according to the correlation result of job stress, anxiety symptoms, insomnia symptoms, oral symptoms, as job stress increases, physical symptoms (p<0.001), anxiety symptoms (p<0.001), insomnia symptoms (p<0.001), oral symptoms (p<0.01) showed an increasing positive correlation and a statistically significant difference. Physical and psychological labor power is greatly exhausted by care service workers performing tasks that require physical and mental care for those who are being cared for. Therefore, there is a need for systematic expansion measures for the mental health of workers and the introduction of customized oral health education programs to improve oral health because improved health and oral health conditions can increase work performance.

Facts and Needs of Comprehensive Child Edu-care Services in Daejeon (대전지역 보육서비스의 실태와 요구 분석 - 어린이집 교사를 대상으로)

  • The purpose of this study was to provide the basic data needed for developing comprehensive child edu-care programs that fits the regional needs. Subjects of the survey were the teachers of child edu-care centers in Daejeon. 95 teachers were selected from 20 child edu-care centers, using stratified random sampling method, from all of five municipal districts of the city. Gathered data were analyzed using SPSS 10.0 for reliability, factor analysis, frequency and percentile, mean, standard deviation, paired samples t-test and F-test. The summary of the findings were as follows: First, the result of survey on the child edu-care facts showed that the most child edu-care centers had the focus on caring and education programs of children, while emotional supports for family services and community social services were rather secondary importance. Therefore, to achieve the comprehensive service programs, child edu-care services should be harmonized by the education and welfare approaches. Second, centered around teacher's request, comprehensive child edu-care program that fits the regional characteristics focuses on family service; thus the program requires family group consulting, parents education etc., as well as focusing on community service to utilize local community's human, physical resources and to strengthen the tie between local community and child care services.

The effects of a fashion store saleswomans clothing and store type on elderly female consumers likability and intention to purchase (패션점포 판매원의 의복과 점포유형이 노년여성소비자들의 호감과 구매의도에 미치는 영향)

  • The purpose of this study was to investigate the effects of a saleswomans clothing and store type on elderly female consumers likability to a saleswoman. Also it was to study the effects of the factors consisting likability to a saleswoman on intent to purchase among elderly female consumers. 3(clothing formality: low, medium, and high)$\times$2(clothing style: clothing looking old vs. young)$\times$2(store type: high priced specialty store vs. low priced open market) complete randomized between subjects design using field experiments were conducted. Subjects were 240 elderly females aged 54-69. The results showed that likability to a saleswoman consisted of impression factor, service ability factor and caring factor. There was the significant three way interaction among three independent variables in the service ability factor(F(sub)2,228=15.62, P<.001). Regression Analysis showed that the impression factor($R^2$=0.29, F<.001) and the service ability factor($R^2$=0.06, F<.001) influenced the elderly consumers intention to purchase significantly. In conclusion, favorable appearances of a saleswoman is considered a crucial service quality influencing potential elderly consumers to purchase.
