Performance of Occupational Health Services by Type of Service (사업장 보건관리사업의 형태별 수행성과 분석 -실적, 수혜도, 영향요인을 중심으로-)

  • Occupational health services in Korea have been operated as dual types: one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative. health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area: 154 places (39.4%) managed by designated. health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation due to the characterstics of each variable and analyzed for impacting factors with relation to the using multiple regression analysis using SPSS PC program, especially using t -test method in order to compare each type of health care management. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. The common sickness management is the most accomplished item in health care area of occupational health care services, while the preventive care and control for the workers who have serious health problems are insufficient in workers health care area. 3. An insufficient accomplishment of overall health education has been shown because it is difficult to perform health education due to almost no chance of the direct introduction at workplaces. Therefore a strong support system for making and supplying the media is necessary in order to activate indirect health education by means of media. 4. Because health care managers and the agencies managers where take the workplaces for this study are almost nurses who have been comparatively high work site rounding rate about an environmental management at the workplaces, that non-profession can also do it, the activities about the professional area not enough. Therefore, an appropriate referral system should be established in order to complement professional area. 5. Two factors which have an effect on the coverage rate of occupational health care services are : one is those from the workplaces such as type of services, the number of workers, the number of harzadous factors and safety & health committee, the other from health care organization about whether there is its own manager or not.

Multifunctional services and space composition in small elderly care facilities - Analysis of pioneering care facilities in Japan (takurosho) - (소규모 고령자 복지시설에서의 서비스 다기능화와 공간구성 - 일본의 선진사례 택로소를 중심으로 -)

  • This study aimed to clarify the multifunctional services and space composition in the process of developing a multifunctional long-term care program in small elderly care facilities in Japan. We collected data about multifunctional long-term care at small facilities from the Community Life Support Center (CLC), a Japanese non-profit corporation, and conducted an interview survey of the members of the CLC's secretariat in 2014. Furthermore, we selected 3 Japanese pioneering care facilities (known as takurosho), and conducted interview surveys and data collection to clarify in detail the space composition and process of development of multifunctional long-term care at small facilities. Four distinct results were found. First, the facilities had gradually increased non-institutional services, including visitation, overnight stays, and long-term stays, to fit the needs of users and their families. Secondly, in the 1990s, they could offer both non-institutional and institutional services at the same facility, but after the long-term care insurance system began in 2000, non-institutional long-term stay services were not allowed. Third, the facilities had built extensions or extra rooms in response to increases in multifunctional services and users. These rooms had common characteristics, with sitting rooms at the center of the facility. Lastly, the maximum number of service users at each of the 3 facilities was limited to 15, to maintain a small scale. However, as the size of facilities was increased through building extensions or remodeling, the overall amount of area available to users increased.

Quality Improvement in Patient Care Services : Obstacles and Approaches (진료의 질관리에 대한 시론 -장애와 접근-)

  • Patient care services are provided to individual patients in response to their health needs produced by illnesses or injuries. The services are often addressed to very serious conditions, and also they constitute the most expensive component of health care services. Therefore, the importance of quality is emphasized, but there are many indications that patient care quality is far from a satisfactory state in most of the countries. Based upon this observation, it is attempted to examine obstacles and approaches to quality improvement in patient care services. In doing so, following Taguchi's(1986) definition of product quality, quality of patient care services is conceived of as better when the less is the sociental loss attributalbe to variability of intended function and harmful side effects they emhibit after being delivered. Some distinguishing features of medical care sector pose difficulties in implementing effective quality improvement programs in patient care services. Nevertheless, newly proposed method of quality management, based on industrial quality management approach, seems to have a great deal of potential to effectively cope with such difficulties. This method, unlike the traditional approach to quality assurance, focuses on total organisational processes, not individuals, as the obproach to quality assurance, focuses on total organizational processes, not individuals, as the objects of quality improvement; variation, not comparison with standards, in quality measurement; and continuous improvement, not removing only bad quality care, as an ideal. Prerequisite to a successful use of any quality mangement method is motivating providers to improve quality. Conceivable approaches for such motivation are self-regulation, external controls and promotion of competition. Since these approaches are not mutually exclusive, they may be employed in an appropriate combination. In Korea, medical care providers are now functioning under the circumstances where they have little reason for making efforts to improve quality of their services. Once these circumstantial conditions are changed to exert pressures on providers to improve quality, the use of adequate quality management method becomes an issue. In this connection, much attention shoould be directed to the newly proposed method described above. In all these efforts for improving quality of patient care services, health insurance would be able to play a pivotal role. Poviders of medical care, buth indiciduals and organizations, are usually very responsive to the measures that affect their financing, and thus health insurance can be a strong instrument for motivationg providers to improve quality. Also, the insurance continuously acquires data on patient care, which could be processed to produce information required to effective quality control.

Moon Jae-in Government Health Policy Evaluation and Next Government Tasks (문재인정부의 보건의료정책 평가와 차기 정부의 과제)

  • Moon Jae-in Care can be seen as a 2.0 version of Roh Moo-Hyun Care. Just as Roh Care failed to achieve its coverage rate goal and 30% share of public beds, Moon Care also failed to achieve its expected goal. The reason is that it followed Roh Care's failed strategy. Failure to control non-covered services has led to a long way to achieve a 70% coverage rate and induced the expansion of voluntary indemnity insurance, resulting in increased public burden. The universal coverage of non-covered services caused an immediate backlash from doctors. And Moon government also failed to control the private insurance market. The expansion of publicly owned beds has not become realized and has not obtained public support. Above all, it failed to overcome the resistance of doctors and failed to obtain consent from budget power groups in the cabinet for public investment. It was also insufficient to win the support of civic groups. Communication with interested groups failed and the role of private health care providers was neglected. The next government should also continue to strengthen health care coverage, but it should prioritize preventing medical poor and create a consensus with both medical providers and consumers for the control of non-covered services. Ahead of the super-aged society, the establishment of linkage between medical services and long-term care and visiting health care or welfare services is an important task. All public and private provisions and resources should be utilized in the view of a comprehensive public health perspective, and public investment should be input in sectors where public medical institutions can perform more effective functions. The next government, which will be launched in 2022, should design a new paradigm for health care in the face of a period of transformation, such as the coming super-aged society in 2026 and the Fourth Industrial Revolution, and recognize that the capabilities of the health care system represent the nation's overall capacity.

Functional Status Evaluation of Disabled Persons in Urban and Rural Areas (도농복합지역 재가복지대상자의 기능상태평가)

  • Purpose: The purpose of this study was to provide the basic techniques in utilizing the systematic home-rehabilitation service with a Minimum Data Set for Home Care (MDS-HC) 2.0 navigator system. Methods: The present study was conducted with 50 persons receiving home-care services from a Welfare Center. Respondents were selected from urban and rural areas in and around the Wonju area. Results: The results showed that MDS-HC2.0 was useful to evaluate the functional status of disabled persons in the fields of health and home-care services. Furthermore, this navigator system provided basic information about the functional health problems of respondents and therefore can provide guidance for health and home care services for disabled persons. Conclusions: Through the present study, a comprehensive model for health and home-rehabilitation was developed. The author believes that respondents will have the satisfaction of high quality service if the developed model is used as the standard in planning and providing home-rehabilitation services.

A Study on the Perception of Quality of Care Services by Care Workers using Big Data (빅데이터를 활용한 요양보호사의 서비스질 인식에 관한 연구)

  • Background: This study was conducted to confirm the service quality management of care workers, who are direct service personnel of long-term care insurance for the elderly, using unstructured big data. Methods: Using a textome, this study collected and analyzed unstructured social data related to care workers' service quality. Frequency, TF-IDF, centrality, semantic network, and CONCOR analyses were conducted on the top 50 keywords collected by crawling the data. Results: As a result of frequency analysis, the top-ranked keywords were 'Long-term care services,' 'Care workers,' 'Quality of care services,' 'Long term care,' 'Long term care facilities,' 'Enhancement,' 'Elderly,' 'Treatment,' 'Improvement,' and 'Necessity.' The results of degree centrality and eigenvector centrality were almost the same as those of the frequency analysis. As a result of the CONCOR analysis, it was found that the improvement in the quality of long-term care services, the operation of the long-term care services, the long-term care services system, and the perception of the psychological aspects of the care workers were of high concern. Conclusion: This study contributes to setting various directions for improving the service quality of care workers by presenting perceptions related to the service quality of care workers as a meaningful group.

The Effects of After-school Service Use on Self-care of Children: Focusing on the Types of Service, the Number of Types, and the Patterns of Service Use (방과후 서비스 이용이 아동의 자기보호에 미치는 영향: 이용하는 방과후 서비스의 유형, 유형의 개수, 혼합이용 패턴을 중심으로)

  • This study explored the effects of after-school services on whether they reduced the possibility of self-care of children. In terms of characteristics of the use of after-school services, it focused on the 1) types, 2) diversity, and 3) use patterns of the after-school services. For the analysis, it used data of the cohort of elementary-school children from the Korean Children and Youth Panel Survey. The main findings are as follows. First, private mentoring and after-school services provided by welfare centers reduced the possibility of self-care. Second, when the total amount of educational time was controlled, the diversity of participating after-school services did not explain the possibility of self-care. Third, when the total amount of educational time was controlled, the use patterns of after-school services did not explain the possibility of self-care. Based on these findings, it suggested policy implications and future research.

A Study of Nurses' Perception of the Visiting Nursing Services of Long-term Care Insurance (노인장기요양보험 재가간호서비스에 대한 방문간호사의 인식)

  • Purpose: The purpose of this study is to understand nurses' perception of visiting nursing services of long-term care insurance. Method: The descriptive survey study involved 188 nurses selected by the convenient sampling of visiting nurses who participated in professional education sessions. Results: Of the 188 participants, 149 (79.3%) were aware of long-term care insurance. Awareness of aspects of long-term care services was 78.7% for facility service, 77.7% for ordinary visiting care service, 85.1% for visiting nursing service and 77.7% for visiting bathing service. Concerning visiting nursing service provision, the majority of the study subjects considered nurse-aid not to be the appropriate route for delivery of services including nasogastric tube exchange, tracheostomy tube management and stitch removal. Conclusion: Continuous evaluation and research on the standards and requirements of the nursing workforce is needed to secure and maintain the high quality of visiting nursing services. Exhaustive studies concerning task division and workforce separation according to nursing services type and level of difficulty should be done to develop the appropriate job description for visiting nursing service staff.

Self-care Education Experience, Self-care Performance, and Home Care Service Needs of Patients With Rare and Incurable Diseases Who Use a Home Ventilator (가정형 인공호흡기를 적용하고 있는 희귀, 난치성 질환자의 자가관리 교육 경험과 수행정도, 가정간호 요구도)

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.

The Study on Supporting Situation of the Community Child Care Service for the Work-Family Balance - Focused on Dual Income Families Living in Seoul and Gyeonggi Province (일-가정 균형을 위한 지역사회의 자녀 돌봄 지원 실태에 대한 연구 -서울, 경기지역 맞벌이 가정을 중심으로)

  • The purpose of this study is to research support systems of the community child care services for optimum work-family balance. For this purpose, the questionnaire to be completed combined four areas: the workplace, school, community and home. Then questions concerning demographic factors and community-related aspects were analysed for this article. After analyzing 197 questionnaires, supplementary questions were asked, by interview to 10 samples. Full-time working wives (eg. working from 9am to 6pm) who have 1 or more children under 10 years old, living in Seoul and Gyeonggi province were collected as samples. The results were as follows: First, child care from family and relatives, and community services was properly used during the week, but dual working parents took complete charge of child care on Saturdays. Second, emergency child care was usually provided by the family, while relatives and community child care services were used less. Third, parents spent 17% of their monthly income on total child care expenditure. Fourth, community child care services were not used often, but the level of satisfaction with them was very high. Finally, they used relatives' services for safety reasons and community services for location, and only based on informed decisions. Therefore, Saturday child care programmes need to be initiated to help physically and mentally tired parents, with more general provision of positive and diverse public support systems.
