본 연구의 목적은 노인요양시설 종사자의 근무환경이 요양서비스품질에 미치는 영향과 사회적 지지의 매개효과를 분석하는 것이다. 연구결과, 첫째 노인요양시설 종사자의 근무환경 변인에서 월 평균 급여와 급여 만족도가 높을수록 요양서비스품질이 향상되는 정(+)의 상관관계를 확인하였다. 둘째, 노인요양시설 종사자의 근무환경 변인의 월 평균 급여와 급여 만족도 변인이 매개변인인 사회적 지지와 정(+)의 상관관계를 확인하였다. 셋째, 노인요양시설 종사자의 근무환경에서 월 평균 급여와 급여 만족도 변인이 요양서비스품질에 미치는 영향에서 사회적 지지의 부분매개효과를 확인하였다. 이에 본 연구는 고령사회의 사회적 상황을 적극 반영하여 노인요양시설의 운영방향을 제시할 수 있는 '자가점검관리척도'를 사용하여 노인요양시설 서비스품질을 측정하였으며 또한 노인요양시설 종사자의 근무환경 개선 방안을 제언 하였다.
Nowadays, a proportion of healthcare service for the elderly has been improved due to the factors, such as a prolonged life expectancy, a growth of aging population and a change of member of family. For these factors, it is necessary for the elderly to receive treatment for multidisciplinary diseases, associated with psychological care of sociological concept. It is quite difficult in an acute-care hospital to accommodate the elderly patient because of the fact that an acute-care hospital is required to maintain a high level of medical care and technical standard. That is why specialized medical service is needed for the elderly. In the case of Germany, they are at the stage of the change from large scale facilities to specialized facilities applying the integrated concept. This paper addresses the medical environmental factor and distinction of medical facilities for the elderly through survey and analysis relating to all change in Germany. Therefore, it aims to suggest a fundamental resource for architectural planning and network of medical facilities for the elderly.
Objectives: The challenge of an increasing elderly population has coupled with everpresent social concerns in Korea. A major problem in health center for the frail older people is that medical, healthcare, and welfare services are often fragmented in terms of providers and settings without appropriate coordination. The purpose of this study was to investigate the need of health center-based integrated healthcare services and its related factors for the elderly. Methods: A total of 110 elderly people who had visited at a county Health Center were interviewed using a self-administered questionnaire from November to December, 2005. The questionnaire consists of five domains according to the Program of All-inclusive Care for the Elderly. Results: Respondents had high need (total mean score with the 5-point Likert-type sacle: 3.67) of health center-based integrated healthcare services including home visiting service (mean: 4.08), chronic disease care service (mean: 4.06), and transportation service (mean 4.05). According to the results of hierarchical multiple regression analysis, among three regression models the magnititude of the variance of full model that is explained by the need of welfare-domain service was significantly larger than two reduced model. Income was a significant variable in increasing the need of health care and welfare services. Conclusions: This study suggests that the health center-based integrated healthcare services for the elderly must be continuously developed and provided for the health promotion and improved the quality of life of the elderly who live in rural area in Korea.
Introduction of new longterm care policy for elderly in Korea would change many aspects of elderly care service facilities. Especially elderly home care services like adult daycare centers will expand drastically after beginning of longterm care insurance. The purpose of this study is to estimate demand of adult daycare centers by comparing with the U.S and Japanese cases. Korean government is expecting that adult daycare centers will expand ten times within 4 years. This estimate is exceeding the facility demand estimate of the U.S. and Japan. The results of population study and expecting growth rate of adult daycare centers in Seoul indicate that more than 300 centers, 4 times of the number of existing centers, are in need based on Seoul elderly population in 2004. To supply these numbers of facilities in short period, more in depth study should be followed. Existing adult daycare facilities' in Seoul were analyzed by their building and management types. Interior spaces of adult daycare centers in Korea are similar to the U.S. and Japan in space arrangements but much smaller in size. In depth study of space programming as well as overall demand survey of adult daycare centers is urgently in need for more realistic expansion of adult daycare centers.
Purpose: The purpose of this study was to examine satisfaction with counseling in long-term care service, and to compare the scores of counseling satisfaction according to variables among beneficiaries of Korean long-term care services. Methods: Questionnaires were completed by 445 beneficiaries of long-term care insurance to measure satisfaction with counseling. Research design was cross-sectional descriptive design. Data were analyzed using descriptive statistics, t-test and ANOVA for evaluating differences in satisfaction with counseling according to variables including economic status, the level of long-term care insurance approval, duration of long term care service, and conditions of counseling. Results: The score of satisfaction with counseling was somewhat high as 71.67. The score of counselor's attitude was highest among the subcategories of satisfaction. The factors that influenced satisfaction with counseling were frequency and time of counseling (F=12.19, p<.001). Conclusion: Home-based individual counseling is necessary for the elderly who need long-term care service. The National Long-term Care Insurance Corporation should offer counseling and assistance to elders and their caregivers about long term care insurance.
본 연구는 한국 노인장기요양 서비스 이용 상태의 결정요인과 상태 의존성을 파악하고자 하였다. 이를 위해 한국복지패널 자료를 이용하여 시간이 지남에 따른 서비스 이용 상태간 전환 패턴을 랜덤효과 다항로짓 모형을 이용하여 분석하였다. 그 결과 노인장기요양 서비스 이용 상태에 있어 강한 상태 의존성을 확인하였다. 특히 초기 상태에서 노인장기요양보험 이용자는 상태가 지속되는 경향이 강한 것으로 나타났다. 개인의 인구통계학적 특성 중 연령이 높을 수록 노인장기요양보험 이용 상태일 확률이 높아지는 반면 혼인상태에 있는 경우 유의하게 낮았다. 거주지역 경우 도농 복합군 거주자는 준거지역에 비해 노인장기요양보험 이용 상태일 확률이 유의하게 높아지는 것으로 나타났다. 본 연구의 결과 노인장기요양 서비스 이용자가 강한 상태 의존성을 가진다는 사실은 향후 수요예측에 있어 기존 이용자의 이용 기간 증가도 충분히 고려하는 것이 중요함을 시사한다.
Facing the aging society in Korea, the supply of elderly care services has to be increased. Among these services short-stay care service is one of the alternative service connecting home and communities and this service is at the beginning stage in Korea. This paper aims to identify the current status and characteristics of short-stay care facilities in Korea. On the bases of survey and analysis the basic data for architectural planning and design of short-stay care facilities for elderly have been proposed.
Purpose: The purpose of this study was to develop an elderly care volunteer program for nursing students and to evaluate its effects. Method: An elderly care volunteer program was developed and then used with 77 nursing students for 6 months. Before and after the program, students responded with a questionnaire on knowledge, attitude and behavior toward the elderly. Data was analyzed by descriptive statistics, $x^2$-test, t-test, and Repeated measure ANOVA. Result: The elderly care volunteer program significantly improved knowledge toward elderly between the experimental and control groups but not in attitude and behavior toward elderly. Conclusion: This study shows that the elderly care volunteer program was an effective service-learning program to improve knowledge toward elderly for nursing students.
이 연구는 노인의료복지서비스의 질적 제고를 위해 노인의료복지시설 소속 종사자들을 대상으로 관리의 변수들 간의 구조적인 관계를 살펴보기 이전에 시설유형에 따른 자기효능감과 직무만족에 대한 요양보호사들의 주관적 인식의 차이를 분석해 보고자 하였다. 실증분석결과 요양보호사들이 근무하는 노인의료복지시설 유형에 따른 인식의 차이는 없었지만 전체적으로 요양보호사들의 자기효능감과 직무만족의 충족이 노인의료서비스 제공에 있어서 중요하다는 것을 강조하였다. 따라서 이 연구는 노인의료복지시설 종사자 관리의 효율화 방안을 위한 이론적 기초를 제공하였다는데 그 의의가 있다.
Purpose: Through a thorough examination of the CCSC (Community Comprehensive Support Center) system in Japan, this study suggests a scheme to provide community-based preventive health care services for the elderly in Korea. Methods: The study inquired into the applicability of the Japanese model by reviewing the data related to the CCSC project, aided by both in-depth interviews with staff in the field and consultations with specialists. Results: Rearrangement of the Visiting Health Management Project system is needed to manage the collective or individual visiting care management for frailty prevention of the elderly in communities. The delegated service system for preventive care in the community, including direct management by one of the public health centers, also needs to be reviewed and the application of stricter standards for the selection of the agency or corporation to run the delegated service is necessary. Long-Term Care Insurance, along with national and local grants, is to be considered as a financial resource for the community-based preventive health care model for the elderly. By making active use of education rooms at district offices, senior citizen centers in neighborhoods for the elderly with easy access can be created. The project needs to raise active supports from communities, develop programs which can be absorbed into particular local cultures, and promote the understanding of the preventive project in local communities. The preventive program should focus on first solving the problems of depression, seclusion, and lack of mobility of the elderly. Second, the program should instruct physical self-management for exercise-nutrition-dental maintenance, and third, the program should strengthen the cognitive abilities of the elderly. In addition, it is necessary to systematize and implement counter-plans of the family and community to protect the elderly who has mental and cognitive problems. Finally, by establishing a network of public health welfare resources based upon research on a community level, assessment and planning for the health of the elderly should be one with their family, and comprehensive consultation and recommendations should be provided to the family. Conclusion: Taking into consideration the experience Japan has had with respect to a similar project, it is appropriate to develop and implement a service system which would combine the Visiting Health Management Project system which has already been established and a preventive health care model for the elderly on a community level.
