Regionalization of neonatal care and neonatal transport system (신생아 괸리의 지역화 및 전원시스템)

  • In the United States, The concept of designation for hospital facilities that care for newborn infants according to the level of complexity of care provided was first proposed in 1976. The extent of perinatal health care regionalization varies widely from one area to the other. facilities that provide hospital care for newborn are classified into three categories on the basis of functional capabilities; level I-primary or basic care, level II-secondary or specialty care, level III-tertially or subspecialty care. These facilities should be organized within a regionalized system of perinatal care. The transport system of newborn infants should be organized for referral of high risk newborn to centers with the personnel and resources needed for their degree of risk and severity of illness. In Korea, The korean society of neonatology was established and articulated in the 1994. During the past decade, the number of neonatologist has increased and neonatal intensive care units have proliferated in Korea. However, no standard definitions exist for the graded levels of complexity of care that neonatal intensive care units provide and no uniform guideline or recommendation for regionalization and referral system of high risk neonate have been established. With the rapid changing neonatal care system in Korea, the optimal neonatal care demands regionalization of care in utilization of manpower resources and in efficient use of advanced technology and facility.

The Relationship between Mothers' Attachment Levels, Types of Verbal Control, and Infants' Language Development (어머니 애착수준 및 언어통제유형과 영아의 언어발달 간의 관계)

  • This study aims to examine the relationship between mothers' attachment levels, types of verbal control, and infants' language development. The selected participants comprised 224 infants, aged 24-35 months and their mothers (224) at 25 long day care centers located Goyang-si, Gimpo-si in Gyeonggi-do, Incheon and Seoul. The major findings of this study were as follows. First, there were significant differences in mothers' attachment levels, types of verbal control, and infants' language development depending on the mothers' employment status. Secondly, to assess the relative influences of two variables which were significantly associated with infants' language development, the sociodemographic variables of mothers and infants, including infants' age and mothers' employment status, were controlled in order to conduct hierarchical regression analysis. The results revealed that imperative-oriented verbal controls, person-oriented verbal controls, and contact seeking all influenced infants' overall language development.

The Types of Infant Care Curricula Based on Analysis of Infants Care Plans (영아반 보육계획안 분석에 따른 프로그램 유형)

  • The purpose of this study was to investigate the actual conditions and characteristics of weekly and daily child care programs for infants, to provide a basis for the improvement of infant care quality. Weekly and daily curricula of 18 classes for one and two-year-olds classes (9 for each ages) from childcare centers in D city were collected from Nov. 2009 to February 2010. The results are as following: first, classes for one-year-olds plan 6-25 activities and classes for two-year-olds plan 12-30 activities weekly, with differences in the number of weekly activities according to the childcare centers. The number of weekly activities should be planned considering infants' interest as well as educational continuity of the activities. Second, the investigation of daily activities showed that the routine activities planned for one-year-olds were for total of 310-600 minutes and for two-year-olds 310-540 minutes. Among these, on average of 66% of the time was planned for everyday life such as eating and napping. The rest of time was dedicated to indoor and outdoor free play. However a few centers allocated more time for group activities according to subjects compared to free play. A curriculum composed of group activities ignoring infants' individual developmental differences should not be encouraged. Last, the results of cluster analyses on weekly and daily curricula in terms of the number of activities and time, duration, number of group activities and time showed that curricula for infants could be characterized into two types: "free play" and "group activity" curricula.

Development of a Premature Infant Pain Scale (PIPS) (미숙아 통증지표(Premature Infant Pain Scale) 개발)

  • Purpose: To develop and validate a scale suitable and efficient scale for use in clinical practice as to assess pain in premature infants. Method: Pain indicators identified by observation of preform infants. A cohort of preform infants was studied prospectively to determine the construct validity, inter-rater reliability, and internal consistency of the scale. The PIPS uses four indicators of pain: corrected gestational age, heart rate, oxygen saturation, behavioral state. The validation study included 45 premature infants with gestational age of 37 weeks or less. Results: The inter-rater reliability of the PIPS was acceptable, with Pearson correlations ranging from.720 to.970. Internal consistency was high: Cronbach's alpha coefficients ranged from.551 to.653. There was a strong correlation between the PIPS and PIPP scores (each researcher's r=.743, each indicator's r=.914). Although gestational age showed no association between these factors and the sum, the other variables were positively associated with the sum. Time needed to calculate PIPS scores is was less than Premature Infant Pain Profile (PIPP) scores(p<.000). Conclusion: The validation data suggest that the PIPS is appropriate and efficient for assessing pain in premature infants. Further studies are required about to determine appropriate interventions for each pain score on the PIPS.

The Usefulness of the Evaluation of Gastric Residuals in Premature Infants (미숙아의 장관영양 시 위 잔류 확인의 유용성 평가)

  • Purpose : The routine evaluation of gastric residuals (RGR) is considered standard care for premature infants. This study evaluated the usefulness of RGR in premature infants. Methods : The study retrospectively investigated 208 premature infants (gestational aged under 34 weeks) who underwent gavage feeding in a neonatal intensive care unit at a tertiary hospital. The patients were divided into two groups: RGR (n=104) and no-RGR (n=104). Those in the no-RGR group had their gastric residuals checked only if signs of feeding intolerance were present. Clinical outcomes, including the time to reach full enteral feeding (FEF) and the incidences of gastrointestinal disorders such as feeding intolerance (FI) and necrotizing enterocolitis (NEC), were compared. Data were analyzed with SPSS ver. 21, using a Mann-Whitney U test, chi-squared test, and Fisher's exact test. Results : There was no statistically significant difference for the time to FEF (z=-0.61, p=.541), incidence of FI ($X^2=0.38$, p=.540), and NEC ($X^2=1.42$, p=.234) between the two groups. Conclusion : No-RGR did not increase the risk for FI or NEC. These results suggest that RGR evaluation may not improve nutritional outcomes in premature infants. Recommendations for further research and practice guidelines will be provided.

Effects of Postpartum Depression and Temperament of Infant on Child-care Stress among Mothers of Newborn Infants (영아기 어머니의 산후 우울과 아기 기질이 양육 스트레스에 미치는 영향)

  • Purpose: This study was designed to identify mother and infant related factors that influence child-care stress among the mother of newborn infants. Methods: Data were collected through self-administered questionnaires and descriptive statistics, correlation and multiple regression were used to analyze the data. The data survey was conducted with 957 conveniently selected mothers of infants when they visited a public health center in Seoul to have their children immunized. Results: The average item score for the Childcare Stress Inventory was 38.03, for the Edinburgh Postnatal Depression Scale, 9.31 and for the Degree of Bother Inventory, 23.42. The CSI was positively correlated to EPDS (r=.44, p<.001) and DBI (r=.40, p<.001). Also these two variables explained 30.0% of CSI in infants' mothers. Conclusion: These findings are expected to expand the understanding about postpartum mothers' child-care stress and can contribute to the development of comprehensive interventions based on community health nursing.

Central line-associated bloodstream infections in neonates

  • Newborn infants, including premature infants, are high-risk patients susceptible to various microorganisms. Catheter-related bloodstream infections are the most common type of nosocomial infections in this population. Regular education and training of medical staffs are most important as a preventive strategy for central line-associated bloodstream infections (CLABSIs). Bundle approaches and the use of checklists during the insertion and maintenance of central catheters are effective measures to reduce the incidence of CLABSIs. Chlorhexidine, commonly used as a skin disinfectant before catheter insertion and dressing replacement, is not approved for infants <2 months of age, but is usually used in many neonatal intensive care units due to the lack of alternatives. Chlorhexidine-impregnated dressing and bathing, recommended for adults, cannot be applied to newborns. Appropriate replacement intervals for dressing and care sets are similar to those recommended for adults. Umbilical catheters should not be used longer than 5 days for the umbilical arterial catheter and 14 days for the umbilical venous catheter. It is most important to regularly educate, train and give feedback to the medical staffs about the various preventive measures required at each stage from before insertion to removal of the catheter. Continuous efforts are needed to develop effective and safe infection control strategies for neonates and young infants.

Factors Influencing Post-traumatic Growth in Mothers with Premature Infants Admitted to the Neonatal Intensive Care Unit (신생아집중치료실에 입원 경험이 있는 미숙아 어머니의 외상 후 성장에 영향을 미치는 요인)

  • Purpose: Caring for a vulnerable premature baby is a challenging task, but some mothers experience growth through that process. The purpose of this study was to investigate the factors influencing post-traumatic growth in mothers with premature infants admitted to the neonatal intensive care unit. Methods: A correlational research design was used and 105 mothers of premature infants were recruited from an online community. Data were collected from January 15 to January 25, 2019. Post-traumatic growth was measured using the Korean version of the Posttraumatic Growth Inventory. Data were analyzed using descriptive statistics, the t-test, analysis of variance, the Scheffé test, Pearson correlation coefficients, and hierarchical multiple regression. Results: The final model developed in this study explained 45.5% of post-traumatic growth (F=13.66, p<.001). Resilience (β=.54, p<.001) was the strongest predictor of post-traumatic growth, followed by the age of the mother when giving birth (β=.17, p=.028) and current employment status (β=.17, p=.049). Conclusion: For mother with premature infants to grow psychologically after their experience, it may be needed to support them to develop and strengthen their resilience through either education or their own support network.

Analysis of the Prospective Child Care Teachers' Discourse on the Professionalism of Music Classes for Infants and Children (영유아 음악수업 전문성에 대한 예비보육교사들의 담론 분석)

  • Purpose: The purpose of this study is to analyze the prospective child care teachers' discourse on the professionalism of music classes infants and children. For this purpose, in-depth interviews were conducted targeting second-year students in the department of child care and welfare at the H university in S city. Methods: The results were analyzed according to the methods of Bogdan and Biklen (2006). Results: The results of interviews showed, the prospective child care teachers felt that it was too soon to talk about their professionalism and teachers' professionalism needed further education and experience in the field. Additionally, regarding the professional music classes, the prospective child care teachers were suggested to require the knowledge of infants' individual development, difference and interest. Secondly, to be able to comply with the national curriculum and build an integrated education plan based on the themes that of infants and children' interests. Thirdly, to be familiar with the teaching methods guiding what to teach and how to teach. And in order to possess such expertise, the discourse suggested that the teachers should enjoy music themselves, learn musical instruments to develop their musical abilities, and have a variety of artistic experiences. In spite of their efforts, if they do not meet the needs, they should overcome by co-operating with their colleagues rather than giving up the music classes. Conclusion/Implications: This study presents the practical direction of music education for the prospective child care teachers.

Open Heart Surgery in Infants Weighing Below 10 kg. (영아 [10 kg] 개심술 환아의 임상적 고찰)

  • The principal aim of surgery for congenital heart anomalies is the establishment of normal hemodynamic function. Palliative and corrective operations are selected with time to attain this end with minimal risk. In recent years, as operative mortality after primary total correction is lower than the mortality after early palliation and delayed correction, corrective operations in infants have increasingly supplanted palliative ones. Two hundred and eighteen infants below 10 kg with congenital heart anomalies underwent primary surgical intervention at Yonsei Medical Center from March 1979 to June 1985. There were 155 infants with VSD, 35 Infants with TOF, 5 infants with ECD, 4 infants with TGV, 3 infants with DORV, 3 infants with Pulmonary atresia, 3 infants with ASD and PDA, 2 infants with DOLV, and the remainders were Sinus Valsalva rupture, residual mitral regurgitation after total correction of ECD, PAPVR, Cor triatriatum, Truncus arteriosus, and Tricuspid atresia. The overall surgical mortality was 15.1%. In the acyanotic group, 13 infants died among 168 infants, and mortality was 7.7%. But in the cyanotic group, the mortality rate was very high and 20 infants died among 50 infants raising the mortality to 40.0%. These poor surgical results in the cyanotic or complicated group was due to inaccurate diagnosis, improper surgical methods and inadequate post-operative care which should be improved.
