The Relationship between Metabolic Syndrome and Erythrocyte Deformability in Small Vessel Disease Stroke Patients (중풍환자에서 대사증후군과 적혈구변형능의 관련성)

  • Objectives : The aim of this study was to assess the relationship between metabolic syndrome and erythrocyte deform ability in acute stroke patients. Methods : Among 88 of the recruited patients, 52 were diagnosed as metabolic syndrome. We assessed their general characteristics, risk factors. We compared the assessed variables between metabolic syndrome and control group. We analyzed the relationship between metabolic syndrome and erythrocyte deform ability. We analyzed relationship between cardiovascular risk factors and erythrocyte deformability. Results : The general characteristics waist and hip circumference, waist/hip ratio were higher in metabolic syndrome group. The metabolic syndrome group was also diagnosed with hypertension, DM, and hyperlipidemia more often than the control group. The blood test metabolic syndrome group showed higher triglycerides, total lipids, fasting blood sugar, and 2 hours postprandial plasma glucose level and lower HDL-cholesterol than the control group. There were more patients diagnosed with Dampness-Phlegm in the metabolic syndrome group. There were more patients showing lower erythrocyte deform ability in the metabolic syndrome group. The plasma homocysteine level was negatively correlated with erythrocyte deform ability. Conclusion : The results reconfirmed that the risk factors are more in metabolic syndrome group. The results indicated that metabolic syndrome lead to a lower erythrocyte deform ability in small vessel disease stroke patients. The Plasma homocysteine level was negatively correlated with erythrocyte deform ability.

  • Objective : The aim of this study was to examine the characteristics of the acute stroke patient who take Gwakhyangjeonggisan, and provide the basis of Gwakhyangjeonggisan prescription Method : We studied hospitalized patients within 4 weeks after their ictus who were admitted at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Oriental Medical Center, Semyung University Oriental Medical Center from February 2010 to July 2010 We compared the general characteristics of acute stroke patient according to herb medicine Result : The patient who take Gwakhyangjeonggisan show significant difference for sex, family history of Stroke, Face color, Tongue color, Pulse condition, HDL Cholesterol, Fastfood eating, Waist circumference. Conclusion : The above result show that Gwakhyangjeonggisan can be prescribed to stroke patient whose complaining gastrointestinal symptoms, & Oriental Medical Diagnosis is Cold & Deficiency type. Further studies will be needed to better understand the difference between Gwakhyangjeonggisan group and Other herb medicine among acute stroke patients.

Method : We studied hospitalized patients within 4 weeks after their occur who were admitted at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Oriental Medical Center, Semyung University Oriental Medical Center from February 2010 to July 2010. We compared the general characteristics of acute stroke patient with herbal medicine. Result : The patient who took Chungpyesagan-tang showed significant difference for age, family history of hypertension, ALT, coffee drinking, fastfood eating, stool, Oriental Medical Diagnosis. Conclusion : The above result show that Chungpyesagan-tang can be prescribed to stroke patient whose stool is hard, whose Oriental Medical Diagnosis is fever type. Further studies will be needed to better understand the difference between Chungpyesagan-tang group and Other herbal medicine among acute stroke patients.

  • Objective : The purpose of this study was to investigate the characteristics of the acute stroke patient who take Yanggyeoksanhwa-tang, and arrange the indication of Yanggyeoksanhwa-tang. Method : We studied hospitalized patients within 4 weeks after their accident who were hospitalized at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Oriental Medical Center, Semyung University Oriental Medical Center from February 2010 to July 2010. We classified and found out the characteristics of acute stroke patient according to herb medicine. Result : The patient who take Yanggyeoksanhwa-tang show meaningful difference for TG, HDL-cholesterole, total lipid, AST, homocysteine, chest burning sensation, and oriental medical diagnosis. Conclusion : The result of this study show that Yanggyeoksanhwa-tang can be applied to acute stroke patient who feels chest burning sensation, and whose oriental medical diagnosis is fever type. Additory evaluations would be needed to better understand the difference between Yanggyeoksanhwa-tang group and other herb medicine group on acute stroke patients.

  • Recently, arsenic-toxicity has become the major focus of strenuous assessment and dynamic research from the academy and regulatory agency. To elucidate the cause and the mechanism underlying the serious adverse health effects from chronic ingestion of arsenic-contaminated drinking water, numerous studies have been directed on the investigation of arsenic-toxicity using various in vitro as well as in vivo systems. Neverthless, some questions for arsenic effects remain unexplained, reflecting the contribution of unknown factors to the manifestation of arsenic-toxicity. Interestingly, very recent studies on arsenic metabolites have discovered that trivalent methylated arsenicals show stronger cytotoxic and genotoxic potentials than inorganic arsenic or pentavalent metabolites, arguing that these metabolites could play a key role in arsenic-associated disorders. In this review, recent progress and literatures are summarized on the metabolism of trivalent methylated metabolites and their toxicity on body systems including cardiovascular system in an effort to provide an insight into the future research on arsenic-associated disorders.

  • Quitting smoking helps smokers maintain their health and extend their lifespan by 10 or more years. Treatment strategies for smoking cessation should be tailored to individual smokers with special needs based on their specific circumstances. It is recommended that pregnant women adopt smoking cessation through counseling and behavioral interventions because the safety of medications has yet to be established. Counseling is the main strategy for smoking cessation in adolescents and nicotine replacement therapy can be used with caution in individuals with serious nicotine dependence. It is important for smokers with psychiatric diseases to quit smoking following accurate assessment of their depression status. Nicotine replacement therapy, varenicline, and bupropion can be used for smoking cessation in smokers with psychiatric disorders. The incidence of cardiovascular disease decreased according to the smoking status and the duration of smoking cessation. In smokers with chronic obstructive pulmonary disease (COPD) who used a combination of counseling and pharmacotherapy the quitting rate was more than twice as high as subjects who used behavioral interventions alone. Varenicline can be used as the most effective anti-smoking drug by most smokers including those with psychiatric disorders, cardiovascular disease, and COPD.

  • This is a retrospective study on the care of odontogenic infections in admission patients with major bleeding disorders. The study was based on a series of 514 patients treated at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, from Jan. 1, 2000, to Dec. 31, 2002. The obtained results were as follows : 1. The cardiovascular disease was the most frequent cause of the systemic diseases with major bleeding disorders, and liver disease, cerebrovascular disease and renal failure were next in order of frequency. But, there was the most frequent dental consultation in the liver disease, owing to the many odontogenic infectious diseases. 2. Male prediction (66.3%) was almost existed in the odontogenic infectious patients with major bleeding disorders. But, there was slight female prediction (53.4%) in the cardiovascular disease. 3. The most common age group of the odontogenic infectious patients with major bleeding disorders was the fifty decade(27.2%), followed by the forty, sixty & thirty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with major bleeding disorder, peak incidence was occurred as toothache (42.2%), followed by intraoral bleeding, ulcer pain, dental extraction in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis and periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(44.2%) was showed in primary endodontic drainage(pulp extirpation, occlusal reduction & canal opening drainage) and followed by the incision & drainage, the medications & oral hygiene instruction, scaling, indirect pulp capping in order.

  • G protein-coupled receptor(GPCR) family is a cell membrane protein, and plays an important role in a signaling mechanism which transmits external signals through cell membranes into cells. Now, it is estimated that there may be about 800-1000 GPCRs in a human genome. But, GPCRs each are known to have various complex control mechanisms and very unique signaling mechanisms. GPCRs are involved in maintaining homeostasis of various human systems including an endocrine system or a neural system and thus, disorders in activity control of GPCRs are thought to be the major source of cardiovascular disorders, metabolic disorders, degenerative disorders, carcinogenesis and the like. As more than 60% of currently marketed therapeutic agents target GPCRs, the GPCR field has been actively explored in the pharmaceutical industry. Structural features, and class and subfamily of GPCRs are well known by function, and accordingly, the most fundamental work in studies identifying the previous GPCRs is to classify the GPCRs with given protein sequences. Studies for classifying previously identified GPCRs more easily with mathematical models have been mainly going on. Considering that secondary sequences of proteins, namely, secondary binding structures of amino acids constituting proteins are closely related to functions, the present paper does not place the focus on primary sequences of proteins as previously practiced, but instead, proposes a method to transform primary sequences into secondary structures and compare the secondary structures, and then detect an unknown GPCR assumed to have a same function in databases of previously identified GPCRs.

Chunghyul-dan acts as an anti-inflammatory agent in endothelial cells by regulating gene expression

  • Chunghyul-dan (CHD) is a combinatorial drug known to exert anti-inflammatory effects in endothelial cells. In this study, we employed global transcriptional profiling using cDNA microarrays to identify molecular mechanisms responsible for the anti-inflammatory activity of CHD in endothelial cells. An analysis of the microarray data revealed that transcript levels of monocyte chemotactic protein-1 (MCP-1), vascular cell-adhesion molecule-1 (VCAM-1) and activated leukocyte cell-adhesion molecule were dramatically altered in CHD-treated endothelial cells. These changes in gene expression were confirmed by RT-PCR, Western blotting and ELISA. Chronic CHD treatment also appeared to decrease MCP-1 secretion, probably as a result of decreased MCP-1 expression. In addition, we determined that chronic CHD treatment inhibited lipopolysaccharide-stimulated adhesion of THP-1 leukocytes to endothelial cells. The inhibitory effect of CHD on LPS-stimulated adhesion resulted from downregulation of VCAM-1 expression. Transmigration of THP-1 leukocytes through endothelial cells was also inhibited by chronic CHD treatment. In conclusion, CHD controls a variety of inflammatory activities by regulating MCP-1 and VCAM-1 gene expression.

The Relationship between Coronary Risk Factors and Coronary Calcium Score Detected by Computed Tomography Coronary Angiography in Korean Middle Aged Men (다중절편 방사선단층촬영 결과에 기반한 한국 중년 남성에서의 관상동맥 석회화와 심혈관질환 위험인자와의 연관성)

  • Objectives: The objective of this study was to investigate the relationship between coronary artery calcification and risk factors of cardiovascular disease with multidetector computed tomography. Methods: Data were collected from 5,899 males between 30 and 59 years old by interview, survey, physical examination, blood test, and multidetector computed tomography in the university hospital from January 2010 to December 2011. We confirmed the coronary artery calcium scores of subjects by multidetector computed tomography and identified risk factors of cardiovascular disorders. We investigated the relationship between coronary artery calcification and risk factors of cardiovascular disorders. Results: Mean calcium score of the coronary arteries in 5,899 participants was 8.20, and 773 participants (13.1%) exhibited coronary artery calcification. The presence of coronary artery calcification was correlated to risk factors of cardiovascular disease (age, blood pressure, triglyceride, HDL cholesterol, LDL cholesterol, glucose, Apolipoprotein A-1, Apolipoprotein B, body mass index, waist circumference) and risk assessment tools of cardiovascular disorders. Significant predicted factors of coronary artery calcification had different patterns in each age group (30-39, 40-49, 50-59 years old). Conclusions: We confirmed the relationship between coronary artery calcification and either typical risk factors of cardiovascular disease or risk assessment tools of cardiovascular disease. In addition, we also observed that the pattern of these factors varied according to age. Therefore, age-related variation needs to be considered in management strategies to prevent cardiovascular disease.
