Late Migration of Amplatzer Septal Occluder Device to the Descending Thoracic Aorta

  • Percutaneous closure of atrial septal defect (ASD) has become an increasingly common procedure. Serious complications of the procedure, such as cardiac migration, are rare, and usually occur <72 hours after device placement. In this report, we present the case of a patient who underwent successful surgical treatment for the migration of an ASD occluder device to the thoracic aorta 12 months after ASD closure.

  • The purpose of this paper is to develop diaphragm type pneumatic Left Ventricular Assist Device(LVAD) for clinical application and to evaluate its performance through the mock circulation and animal experiment. The blood housing and diaphragm are made by polyurethane. The relations of cardiac output us. beat rate and cardiac output vs. systolic-to-diastolic rate was estimated through the mock test and hemodynamic waves are recorded for the evaluation of VAD. We performed animal experiment and 4 animals survived more than 24hrs. As a result, the hemodynamic data and waves showed this system can be applicable to the animal experiment.

이식형 심장 모니터링 장치 관련 감염의 위험요인 (Risk Factors for Cardiac Implantable Electronic Device-Related Infections)

  • Purpose: This study aimed to investigate the risk factors for cardiac implantable electronic device (CIED)-related infections within the first post-procedural year after CIED insertion. Methods: This study included 509 adult patients undergoing CIED implantation procedures between January 1, 2011 and December 31, 2015. The data were analyzed by t-test, chi-square test, Fisher's exact test, and logistic regression analysis using SPSS/WIN 23.0. Results: Fifteen infections and 494 non-infections were examined. The CIED-related infection rate was 2.9%; patients with 14 pocket infections and one bacteremia were included in the CIED-related infection. The risk factors of CIED-related infections were the estimated glomerular filtration rate (eGFR) of ≤ 45 mL/min/1.73 m2 (Odds ratio [OR]= 4.03, 95% confidence interval [CI],1.15-14.10) and taking a new oral anticoagulant (NOAC) (OR = 4.50, 95% CI 1.09-18.55). Conclusion: These results identified the CIED infection rate and risk factors of CIED-related infection. It is necessary to consider these risk factors before the CIED implantation procedure and to establish the relevant nursing interventions.

심부전 정도에 따른 좌심실보조장치의 박동효율예측을 위한 심장의 전기역학적 유한요소 모델의 응용 (Application of Cardiac Electromechanical FE Model for Predicting Pumping Efficacy of LVAD According to Heart Failure Severity)

  • 좌심실보조장치(LVAD)가 심실부하감소에 미치는 영향을 극대화 하기 위해, 심실보조장치 치료를 위한 최적의 심부전 심각도 단계를 찾는 것은 중요하다. 우리는 심부전 정도에 따른 LVAD 의 박동효율을 이론적으로 예측하였다. 우리는 혈관시스템의 6 컴파트먼트의 Wind-kessel 모델과 연동된 심실의 삼차원 유한요소모델을 사용하였다. 이 모델을 이용하여, LVAD 치료 하에서 심부전의 정도에 따라 심실의 수축성 ATP 소모율, 좌심실압력, 심박출량, 심박출 분획, 1 회심박출량 등과 같은 심장응답을 예측하였다. LVAD 치료 중에 에너지학적 부하조건을 암시하는 수축성 ATP 소모율은 5 단계 심부전 조건에서 가장 크게 감소하였다. 따라서, 우리는 LVAD 를 회복으로의 가교로서 고려하고 있을 때, 심부전 5 단계에서 LVAD 치료를 시작하는 것이 가장 적절하다는 결론을 내린다.

디지털 센서 장비를 적용한 가슴 압박의 질 변화 (Changes in the quality of chest compressions applying a digital sensor device)

  • Purpose : The purpose of this study is to demonstrate the effectiveness of using a digital sensor device during CPR by analyzing the results from that chest compressions with a digital sensor device are applied to cardiac arrest patients. Methods : This study analyzed the results from the experiment that 42 people were selected randomly among Korean 119 rescuers, and they divided into the experimental group using a digital sensor device and the control group only using their hands, then they had been observed to conduct chest-compressions to mannequins for 10 minutes. Results : The results were found that compression depth in both the control and experimental group was gradually decreased over time, but the experimental group not only kept the depth but also maintained the speed of chest-compressions close to 100 times a minute. In addition, due to the use of the digital sensor device, the insufficient recoil ratio of chest-compressions was significantly reduced. Conclusion : The results show that conducting chest-compressions with a digital sensor device keeps the compression-death, maintains the speed of chest-compression properly and makes the insufficient relaxation ratio of chest-compressions reduce significantly.

공압식 심실 보조기의 동물실험 (Animal Experiment of the Pneumatic Ventrivular Assist Device)

  • Background : Ventricular assist devices(VADs) are being used for patients in postcvardiotomy cardiogenic shock status bridge to cardiac transplant settings and in post-myocardial infarction cardiogenic shock. The VAD which was developed at the Deparment of medical engineering in Dankook University College of Medicine was a pneumatically driven device and can maintain pulsatile flow. The goal of this study is to develop animal experimental models using the VAD and to clarify the reliability and hemodynamic property adequacy of end organ perfusion durability and severity of thrombotic-hemolytic tendency of the device. Material and Method : The pneumatic VAD was applied to 8 adult female lambs, We examined some hemodynamic parameters such as arterial blood pressure pulmonary capillary wedge pressure(pcwp) pulmonary artery pressure(PAP) left atrial pressure hour urine output cardiac index VAD flow EKG to determine the reliability of the VAD and hemodynamic compatibility of the experimental animals within 24 hours of experiment. We also observed the end organ perfusion durability of the VAD and thrombotic-hemolytic property of the VAD after 24 hours of VAD insertion. Result: We could monitor all hemodynamic parameters including pcwp PAP cardiac index EKG, adn hour urine as true clinical settings. We observed that the reliability of the VAD was excellent and the hemodynamic property of the experimental animal and end organ perfusion were adequate within 24 hours of experiment. In four lambs surviving 24 hours after insertion the reliability of the VAD and end organ perfusion were excellent and no thrombotic-hemolytic tendency was noted. However after 15 days of experiment the diaphragm of the VAD was torn and it was recommende that the durability of the VAD should be extended. Conclusion : e conclude that the pneumatic VAD developed at Dankook University Biomedical Engineering has good hemodynamic property and low thromboembolic tendency and presents adequate end organ perfusion but we noted that the durability of the device should be expanded further. It will be possible to do more reliable experiment in the future according to the animal experimental method developed in this study especially with the heart failure models.

MCG 영상진단 검사에 관한 연구 (A Study on MCG Imaging)

  • Magnetocardiography (MCG) is the measurement and analysis of the magnetic component of the electro-magnetic field of the human heart, usually conducted externally, using extremely sensitive devices such as a Superconducting Quantum Interference Device (SQUID). MCG is a totally noninvasive method, it uses neither radiation nor ultrasonics. The magnetic activity of the heart is registered from outside the thorax. MCG has a very high sensitivity and a high spatial resolution for very a small, local myocardial current. In comparison to the electrical signals measured by an ECG, the magnetic signal does not disturb the boundaries of tissues with different electrical properties. MCG measures the myocardial function rather than describing the morphology. MCG is a relatively new technique that promises good spatial resolution and extremely high temporal resolution, thus complementing other heart activity measurement techniques such as Electrocardiography (ECG). The clinical uses of MCG are in detecting various cardiac disorders including myocardial infarction, ventricular hypertrophy, ventricular conduction defects, Wolff-Parkinson-White (WPW) syndrome, sudden cardiac death and fetal magnetocardiography. Magnetocardiography may be used alone or together with electrcardiography for the measurement of spontaneous or overloaded activity and for research or clinical purposes.

A Systematic Review of the Mechanical CPR and Manual CPR on Out-of-Hospital Cardiac Arrest Occurring in High-rise Building

  • CPR in High-rise building is one of the challenging tasks to 119 paramedics, evacuating patient from the narrow and vertical area. This study was built to compare the method of mechanical CPR and manual CPR is to maximizing on-scene treatment time, and minimizing the hand-off time in cardiac arrest, transporting patient as fast as possible. The electronic data research (Science, Pubmed, Medline, Medline and 55 academic DB interworking) was conducted, and five articles were included by reviewing and excluding through the Covidence program and Review Manager version 5.4(Cochrane Collaboration). OHCA occurring on the higher floor indicates lower in survival. A total studies uniformly reported mechanical CPR is more effective during the high-rise building evacuation, than manual CPR in rate, depth, and hands-on time of chest compression. Use of mechanical CPR device is more suitable in case of High-rise building OHCA to improve the survival rate which is affected by high-quality CPR.

웨어러블 기기를 사용한 심정지 환자 지원 시스템의 설계 및 구현 (Design and Implementation of a Cardiac Arrest Supporting System Using Wearable Device)

  • 급성 심정지는 갑작스러운 심장 기능의 상실로 인하여 수 분 안에 뇌를 비롯하여 온 몸에 혈액 공급이 되지 않아 사망으로 이어지는 대표적인 중증응급질환이다. 특히 병원 밖에서 발생하는 병원외 심정지 환자는 생존율이 매우 낮다. 그 이유는 갑작스런 발병으로 인해 주변사람들이 심정지 환자라는 것을 인식하지 못할 뿐만 아니라 인식한다 하더라도 전문 의료지식이 없어 정확한 응급조치를 취하는 것이 어렵기 때문이다. 기존의 심정지 관련 연구는 대부분 심정지 발생 후의 치료법 방안에 집중되어 있었으나, 이는 근본적인 문제해결이 어려워 생존율을 높이는데 한계가 있었다. 이러한 문제를 해결하기 위해 본 논문에서는 널리 보급된 스마트폰을 활용하여 급성 심정지 환자를 위한 알림, 사전예방, 응급조치 등의 기능을 갖춘 솔루션을 개발하였다. 이 솔루션을 활용하면 급성 심정지를 사전에 예방할 수 있고, 설사 발병한다고 하더라도 주변사람에게 신속하게 알려 최대한 골든타임(4분)을 지킬 수 있으며, 1차 발견자가 신속하고 정확한 심폐소생술을 실시할 수 있어 생존율을 한층 더 높일 것으로 기대한다.

Transfromation of Percutaneous Extracorporeal Life Support to Paracorporeal Ventricular Assist Device: A Case Report

  • Percutaneous extracorporeal life support (P-ECLS) is a useful modality for the management of refractory cardiac or pulmonary failure. However, venoarterial P-ECLS may result in a complication of left ventricular distension. In this case report, we discuss a patient with drug-induced dilated cardiomyopathy managed with venoarterial P-ECLS and a left atrial vent catheter. The venoarterial P-ECLS was modified to a paracorporeal left ventricular assist device (LVAD) by removing the femoral venous cannula. After 28 days of hospitalization, the patient was successfully weaned from the paracorporeal LVAD and discharged home from the hospital.
