목적: 본 연구는 오늘날 증가추세에 있는 폐암 환자에게 보다 체계적이고 효율적인 호스피스 완화의료를 위한 기초자료를 제공하고자 호스피스 병동에서 임종한 말기 폐암 환자의 임상적 특성을 조사하였다. 방법: 2003년 3월 1일부터 2006년 12월 31일까지 샘 안양병원 호스피스 병동에서 말기 폐암으로 임종한 129명의 환자를 대상으로 하여 이들의 의무기록을 후향적으로 조사하였다. 수집된 자료는 실수와 백분율, 빈도와 중간 값으로 제시하였으며 호스피스 완화의료 동안의 생존기간은 SPSS 13.0의 Kaplan-Meier방법으로 분석하였다. 결과: 129명의 말기 암 환자 중 남자 93명(72%), 여자 36명(28%), 연령의 중간 값은 68세(범위 $37{\sim}93$세)이었다. 진통제를 복용하고 있었던 환자는 82명(64%)이었으며 그렇지 않은 환자는 47명(36%)이었다. 입원 이유는 호흡곤란이 47명(36%)으로 가장 많았고 이는 일반적으로 말기 암환자의 입원이유가 통증인 것과 다르게 나타났다. 호스피스 병동 입원 후 120명(93%)의 환자에게 마약성 진통제를 사용하였고 주사용 모르핀(103명, 80%)이 가장 많이 처방되었다. 진정제를 사용한 환자는 87명(67%)이었으며 미다졸람을 가장 많이 사용하였다(68명, 53%). 호스피스 완화의료 동안의 생존기간의 중간 값은 35일, 입원 일수의 중간 값은 24일이었다. 결론: 말기 폐암환자에서 호흡곤란의 조절은 매우 중요하며 말기 폐암 환자에 대한 호스피스 완화의료의 기간이 짧아서 효과적인 호스피스 완화의료를 위해서는 가족과 환자에게 지속적인 교육과 홍보가 필요하다고 생각된다.

Purpose: To Study the dosimetric advantage of the Jaw tracking technique in intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc therapy (VMAT) for Head and Neck Cancers. Materials and Methods: We retrospectively selected 10 previously treated head and neck cancer patients stage (T1/T2, N1, M0) in this study. All the patients were planned for IMRT and VMAT with simultaneous integrated boost technique. IMRT and VMAT plans were performed with jaw tracking (JT) and with static jaw (SJ) technique by keeping the same constraints and priorities for a particular patient. Target conformity, dose to the critical structures and low dose volumes were recorded and analyzed for IMRT and VMAT plans with and without JT for all the patients. Results: The conformity index average of all patients followed by standard deviation (${\bar{x}}{\pm}{\sigma}_{\bar{x}}$) of the JT-IMRT, SJ-IMRT, JT-VMAT, and SJ-VMAT were $1.72{\pm}0.56$, $1.67{\pm}0.57$, $1.83{\pm}0.65$, and $1.85{\pm}0.64$, and homogeneity index were $0.059{\pm}0.05$, $0.064{\pm}0.05$, $0.064{\pm}0.04$, and $0.064{\pm}0.05$. JT-IMRT shows significant mean reduction in right parotid and left parotid shows of 7.64% (p < 0.001) and 7.45% (p < 0.001) compare to SJ-IMRT. JT-IMRT plans also shows considerable dose reduction to thyroid, inferior constrictors, spinal cord and brainstem compared to the SJ-IMRT plans. Conclusion: Significant dose reductions were observed for critical structure in the JT-IMRT compared to SJ-IMRT technique. In JT-VMAT plans dose reduction to the critical structure were not significant compared to the SJ-IMRT due to relatively lesser monitor units.
Self-organized nanogels were prepared from pullulan/biotin conjugates (PU/Bio) for the development of an effective anticancer drug delivery system. The degree of biotin substitution was 11, 19, and 24 biotin groups per 100 anhydroglucose units of pullulan. The physicochemical properties of the nanogels (PU/Bio1, 2 and 3) in aqueous media were characterized by dynamic light scattering, transmission electron microscopy, and fluorescence spectroscopy. The mean diameter of all the samples was less than 300 nm with a unimodal size distribution. The critical aggregation concentrations (CACs) of the nanoparticles in distilled water were $2.8{\times}10^{-2},\;1.6{\times}10^{-2}$, and $0.7{\times}10^{-2}mg/ml$ for the PU/Bio1, 2, and 3, respectively. The aggregation behavior of the nanogels indicated that biotin can perform as a hydrophobic moiety. To observe the specific interaction with a hepatic carcinoma cell line (HepG2), the conjugates were labeled with rhodamine B isothiocyanate (RITC) and their intensities measured using a fluorescence microplate reader. The HepG2 cells treated with the fluorescence-labeled PU/Bio nanoparticles were strongly luminated compared with the control (pullulan). Confocal laser microscopy also confirmed internalization of the PU/Bio nanogels into the cancer cells. Such results demonstrated that the biotin in the conjugate acted as both a hydrophobic moiety for self-assembly and a tumor-targeting moiety for specific interaction with tumor cells. Consequently, PU/Bio nanogels would appear to be a useful drug carrier for the treatment of liver cancer.
This paper reports on a Realtime OS based motor control system for laparoscopic surgery robot which enables telesurgery and overcomes shortcomings with conventional laparoscopic surgery. The system has a conventional master-slave robot configuration and the control system consists of joint controllers, host controllers, and power units. The robot features (1) a compact slave robot with 5 DOF (Degree Of Freedom) expanding the workspace of each tool and increasing the number of tools operating simultaneously, and (2) direct 1:1 correspondence in the joint of master and slave robot that simplifies control algorithm and enhances reliability. Each master, slave and GUI (Graphical User Interface) host has a dedicated RTOS (RealTime OS), RTLinux-Pro (FSMLabs Inc., U.S.A.) Each master and slave controller set pair has a dedicated CAN (Controller Area Network) channel for control and monitoring signal communication. Total 4 pairs of the master/slave manipulators as current are monitored by one host controller for operation monitoring and higher level motion control. The system showed acceptable performance in both position control precision and master-slave motion synchronization and is now under further development for better safety and control fidelity for clinically applicable prototype.
The visiting nursing service is an essential part of public health. The purpose of this study was to analyze the visiting nursing service in primary health service centers. The data were collected from visiting nurse records in Wonju City Health Center, Myun Health Center and Community Health Subcenters. The period of data collection was from April 6 to July 15, 1998. The major findings were as follows: 1. Characteristics of 36 service providers. 1) Age : Over 40 years old - 66.7% 2) Educational level: The proportion of registered nurses was 47.2% and nurse aids 52.8% 3) Career: The proportion of providers who worked over 11 years in Public Health Service was 7.8% 2. Characteristics of Subjects 1) The major health problems were cerebro vascular attack, hypertension, D. M., arthritis, gastrointestinal problems and psychiatric problems. The prevalence of chronic health problems increased with age, except for mental illness. 2) The prevalence of cancer was 4.3/1000. 3. Contents of Home Visiting Nursing Services. 1) The major service was education and counseling. 2) The other services were Direct Care(ROM exercise, wound care, physical therapy, basic nursing care etc,) as 56.5%, of the work involved Indirect Care(teaching, counseling, emotional support, etc,) 30.3%, medication - 11.7%, and referral to hospitals - 1.5%.
New Dietary Reference Intakes (DRIs) forthe United States and Canada have recently been set for both macronutrients and micronutrients, and are likely to be of interest to health professionals in Korea as well. DRIs are now available for nutrients that did not have Recommended Dietary Allowances set in the past (amino acids, n-3 and n-6 fatty acids, total fiber, added sugar, choline, boron, nickel, and vanadium). Furthermore, the units for the DRIs do not always match those traditionally carried on food composition tables (FCTs). FCT developers will also need to consider carrying new variables to allow the calculation of folate intake in $\mu$g of dietary folate equivalents, vitamin E intake as mg of a-tocopherol (not as mg of a-tocopherol equivalents), and vitamin A intake as $\mu$g of retinol activity equivalents (not as $\mu$g of retinol equivalents). Because the new recommendations for upper levels of intake sometimes refer to a specific form or source of a nutrient, nutrients occurring in foods must be separated from added or supplemental forms for vitamin E, niacin, and folate; pharmacological magnesium must be carried as a separate variable; and preformed vitamin A must be separated from vitamin A from carotenoids. Forest bathing has beneficial effects on human health via showering of forest aerosols as well as physical relaxation. Terpenes that consist of multiple isoprene units are the largest class of organic compounds produced by various plants, and one of the major components of forest aerosols. Traditionally, terpene-containing plant oil has been used to treat various diseases without knowing the exact functions or the mechanisms of action of the individual bioactive compounds. This review categorizes various terpenes easily obtained from forests according to their anti-inflammatory, anti-tumorigenic, or neuroprotective activities. Moreover, potential action mechanisms of the individual terpenes and their effects on such processes, which are described in various in vivo and in vitro systems, are discussed. In conclusion, the studies that show the biological effectiveness of terpenes support the benefits of forest bathing and propose a potential use of terpenes as chemotherapeutic agents for treating various human diseases.

Although most patients prefer dying at home, patients whose condition rapidly becomes critical need care in the intensive care unit (ICU), and it is rare for them to die at home with their families. Therefore, interest in hospice and palliative care for patients in the ICU is increasing. Hospice and palliative care (PC) is necessary for all patients with life-threatening diseases. The following patients need palliative care in the ICU: patients with chronic critical illnesses who need tracheostomy, percutaneous gastrostomy tube, and extracorporeal life support; patients aged 80 years or older; stage 4 cancer patients; patients with specific acute diseases with a poor prognosis (e.g., anoxic brain injury and intracerebral hemorrhage requiring mechanical ventilation); and patients for whom the attending physician expects a poor prognosis. There are two PC models-a consultative model and an integrative model-in the ICU setting. Since these two models have advantages and disadvantages, it is necessary to apply the model that best fits each hospital's circumstances. Furthermore, interdisciplinary decision-making between the ICU care team and PC specialists should be strengthened to increase the provision of hospice and palliative care services for patients expected to have poor outcomes and their families.
Covalent organic frameworks (COFs) are porous crystalline polymers in which organic units are linked by covalent bonds and have a regular arrangement at the atomic level. Recently, the COFs have been much attention in bio-medical area such as bio-imaging, drug delivery, and therapeutics. These 2D nanoparticles are proving their value in nanomedicine due to their large surface area, functionalization through functional groups exposed on the surface, chemical stability due to covalent bonding, and high biocompatibility. The high ω-electron density and crystallinity of COFs makes it a promising candidate for bioimaging probes, and its porosity and large surface area make it possible to be utilized as a drug delivery vehicle. However, the low dispersibility in water, the cytotoxicity problems of COFs are still challenged to be solved in the future. In this regard, several efforts that increase the degree of dispersion through functionalization on the surface of COFs for the application to the biomedical field have been reported. In this review, we would like to describe the advantages and limitations of COFs for bio-imaging and anti-cancer treatment.
