항암 치료가 불가한 고령의 4기 담낭암 환자 한방치험 1례 (Single Case Report of a Stage IV Geriatric Gallbladder Cancer Patient treated with Korean Medicine Who Could Not Receive Conventional Therapies)

  • Background : This case is to report a gallbladder cancer patient treated with Korean Medicine who could not receive conventional cancer therapies such as operation and chemotherapy due to her underlying diseases, old age and poor performance. Method and Results : An eighty-year-old female patient was diagnosed with stage IV gallbladder cancer on Feb. 2014 and she already had hypertension, diabetes and Parkinsonism. Comprehensive Korean Medicine (KM) treatments instead of operation and chemotherapy were administrated due to her poor condition and old age. KM treatments were done focused on improving symptoms and quality of life combined with best supportive care. During KM treatments her abdominal pain was generally managed and she survived for 22 months. Conclusion : Further studies should be performed in the future to clarify the survival benefit and symptom management of comprehensive KM treatments.

간 전이를 동반한 결장암 환자에서 한방치료의 효과 : 8년간의 추적조사를 통한 증례보고 (Constant State of Sigmoid Colon Cancer with Liver Metastasis by Herbal Medicine : A Case with an 8-year Follow-up)

  • Objectives : The aim of the present study is to derive further studies evaluating the effectiveness of oriental medical treatment on colon cancer patients. We present a case of a patient diagnosed with colon cancer with colon cancer with liver metastasis who has survived more than 8 years. Methods : We followed all of treatments and examination. We prescribed the patient to take HangAm-dan(HAD) three times a day for 5 years and 1 month from January, 2000 to February, 2005. Abdominal computed tomography(CT) was performed to evaluate the therapeutic efficacy and measuring concentration of carcinoembryonic antigen(CEA) in blood serum was also performed to monitor therapeutic response. Results : The patient was diagnosed with liver metastasis in September of 1999 but has survived for over 8 years since. Abdomen CT show no interval change. Conclusions : This case may give us a possibility that oriental medical treatment offers potential benefits for patients with colon cancer.

추가 항암 치료를 거부하는 국소 진행형 췌장암 환자 1례 (A Patient with Locally Advanced Pancreatic Cancer Who Refused Additional Chemotherapy)

  • The prognosis for pancreatic cancer patient is very poor. Patients with locally advanced disease have a median survival time of 8 to 12 months, and patients with distant metastases have significantly worse outcomes, with a median survival time of only 3 to 6 months. Approximately 30% of patients with pancreatic cancer present with locally advanced disease defined as unresectable pancreatic cancer without evidence of distant metastatic disease. Primary treatment options in locally advanced pancreatic cancer include chemotherapy and radiotherapy. Here, we reported a patient with locally advanced pancreatic cancer who does not want further chemotherapy because of chemotherapy induced nausea and vomiting. Irreversible electroporation was performed. Irreversible electroporation was well tolerated in this case, and may be a therapeutic modality for selected patients with locally advanced pancreatic cancer.

위암환자를 위한 간호 데이터베이스 개발 (Development of the Nursing Database for Gastric Cancer Patients)

  • Purpose : This study was to develop the nursing database for gastric cancer patients for clinical application. Method : Nursing data that development of this data base is comprehensive connected with gastric cancer patient nursing process frame to foundation as classification. Result : Each stage was processed based on the System Development Life Cycle. At the Strategy Planning stage, gastric cancer patient nursing process were analyzed. At the system Analysis Stage, database flowchart was drawn up based on frame of nursing process was drawn up. At the system Design Stage, a system was developed based on the flowchart and named the Nursing Database. The Nursing Database consisted of the patient's Basic Information, Patient's Nursing History, Discharge summary, Nursing Assessment, Nursing Diagnosis, Nursing Intervention/activity, Nursing Evaluation, Statics, Code Registration. Each element in flowchart was coded and made into a database. Nursing Assessment classified according to Gorden's Health Pattern Typology, and nursing diagnosis draws the standard 27 name of Hanguls and connected with nursing assessment. Nursing intervention and nursing activity draw 192 of thing that present in NIC, connected this with nursing assessment. Nursing evaluation is linked with nursing assessment, diagnosis and intervention by achievement availability of nursing goals. Conclusion : The biggest advantage of this database nursing process that can manage nursing information exactly and rapidly to foundation be.

간호결과에 대한 환자 특성의 영향 - 위장관계 수술 환자를 중심으로 - (The Effect of the Patient's Characteristics on Nursing Outcomes in Gastrointestinal Surgery Patients)

  • Purpose: This study was performed to identify the patient characteristics significantly affecting nursing outcomes and their predictability in gastrointestinal surgery patients. Method: The subjects were 149 abdominal surgery patients from 3 general surgical nursing units of 3 general hospitals. Two instruments were used to measure nursing outcomes and acuity of the subjects. The nursing outcomes were measured at post-operation 4 and 7days using review of patients' records, observation of patients, and interviews with patients by a trained nurse. For data analysis, T-test or ANOVA, Pearson Correlation and Stepwise Multiple Regression were done. Result: Age, severity score, diagnosis, cancer or not, operation site were the subjects' characteristics that were significantly related to the nursing outcomes in both post-operation 4 and 7days. Cancer or not, age, diagnosis and severity score were the significant predictors for the scores of nursing outcome in post-operation 4days and the predictability was 34.9%. The predictability of cancer or not was highest, 22.6%. Age, diagnosis and cancer or not were the significant predictors for the scores of nursing outcome in post-operation 7days and the predictability was 27.8%. The predictability of age was highest, 17.3%. Conclusions: The patient characteristics affecting nursing outcomes should be considered when nursing care is planned and provided. Especially, careful attention should be given to the patients with cancer and older age. And, these patient characteristics should be adjusted for correct estimation of the effectiveness of nursing interventions on nursing outcomes.

간암환자(肝癌患者) 1례(例)에 대한 증례보고(證例報告) (Case study on one case of hepatoma)

  • This study was performed on the basis of clinical consideration about patient who has hepatoma, cholangiocarcinoma and pancreatic cancer. Cancer has gradually increased morbidity and mortality all over the world including Korea. Western medicine has treated with surgical therapy, chemotherapy, radiotherapy and immunotherapy, but has not obtained outstanding results and has a difficulty due to side effects by those therapies. In this study, we recognized that symptom including pain by cancer are rapidly decreased with oriental medicine treatment. Oriental medicine treatment is applied throughly on the basis of the oriental medicine principle and the prescrptions were used Bulwhankumjunggisan(不換金正氣散), Sojukbaekchulsan(消積白朮散) Banhabackchulchunmatang(半夏白朮天麻湯). The communitial effect of those prescriptions are invigorating the spreen and benefiting vital energy(健脾益氣), supporting healthy energy to eliminate evils(扶正祛邪) and those treatments are corresponed with traditional oriental treatment principle. Also we can know this patient has a little improvement in the general test including biochemistry, blood test, urine analysis and etc. in spite of outstanding improvement of patient's symptom on administration of oriental medicine. According to the above results, though we has not got satisfaction in getting rid of cancer completely on administration of oriental medicine, it is helpful in decreasing symptoms including pain by cancer and in improving quality of life of patient and in improving quality of patient life.

외상성 요도 손상으로 오인된 요도암 (Traumatic urethra injury presenting as urethral cancer : A case study)

  • Purpose: The purpose of the study was to investigate traumatic urethral injury in a 63-year-old patient with hematuria. Methods: A hematuria patient was transferred by paramedics. At the time of the visit, the patient's blood pressure (151/91mmHg), pulse rate (86/min), body temperature (37.1℃), and other vital signs were stable. Their KTAS (Korean Triage and Acuity Scale) was Level 4. The patient had no damage to the injured area, but a large contrast defect was observed between the prostate urethra and the bladder in urethral angiography performed due to persistent hematuria and pain in the injured area. Results: Following radiological evaluation of a suspected liposarcoma or neuroma mass of the prostate urethra, the mass was removed through urethral tumor resection. The result of histologic evaluation provided a diagnosis of highly differentiated invasive urethral cell carcinoma that had invaded the muscle layer. The patient was given additional treatment for urethral cancer but was rejected and is currently being followed. Conclusion: The prognosis for urinary tract cancer has distinct differences for patients with lymph node metastasis and tumor characteristics. The presence or absence of urethral cancer should be confirmed through angiography, CT, MRI, and cystoscopy.

The Meaning of Death for Korean in View of Novel and End Stage Cancer Patient

  • Every one experiences death one day, however no one can hows exactly what it is because people can not experience death until it comes, it is therefore impossible to judge correctly on the phenomenon of the death. On the whole, man experiences indirect death through the mass communications such as TV drama, fiction, magazine etc because those methods can easily access by every one. In addition to this, people usually acquire the negative awareness of death through the dramatic change of story like dying of cancer for dramatic effect by giving scare and fear to the cancers. The purpose of this study is to provide basic information on the spiritual care that enables the facing death patients to accept death as a part of life and divert hope from scare about after death by comparing and analyzing of two aspects of death meaning i.e, Korean fiction and the end stage cancer patients. Additionally, for medical staff to understand the facing death cancer patients by making to aware patients correctly and provide the better quality of care. The study was performed from September 28, 2002 to February, 28, 2003. The materials of this study were collected by direct data obtained from observation, interviews, note and diary of end stage of cancer patients and written materials acquired from Korean contemporary fiction. Participants of this study were 4 end stage cancer patients including 2 lung cancer patients, 1 liver cancer patient and 1 esophagus cancer patient. The methodology used in this study was divided into two types; Huberman & Miles methodology was used for fiction to find and categorize subject, and Colaizzi, one of phenomenological methodology was used for end stage cancer patients to find the major meaning, subject and categorization.   • 암환자에게 부부적응은 심리적인 안녕 뿐만 아니라 치료경과 및 생존율에 영향을 미치는 것으로 보고되고 있어 이들의 부부적응을 도모하기 위한 사회복지 노력과 관심이 절실히 필요하다 하겠다. 이에 본 연구는 암환자와 배우자의 커플자료를 수집하여 이들의 부부적응에 대해 조사하였으며, 기초자료가 미비한 실정에서 부부관계의 지표로 부부적응에의 필수고려 요인으로 지목되고 있는 부부의사소통의 영향을 조사하였다. 또한 암환자와 배우자의 상호의존성을 고려하여 부부의사소통이 부부적응에 미치는 영향을 자기 자신에게 미치는 영향(자기효과), 상대방에게 미치는 영향(상대방효과)을 고찰하였다. 이를 통해 부부적응에 긍정적 부부의사소통의 중요성을 제시하였으며, 부정적 의사소통 유형인 요구-철회의사소통에서 환자의 배우자에 대한 상대방효과가 유의한 것으로 나타나 환자에 대한 의사소통 교육에의 필요성을 제시하였다. 이러한 연구결과를 바탕으로 실천적 함의 및 후속연구에 대한 제언을 제시하였다.

고주파 온열치료와 소라페닙을 병용하여 호전된 재발성 간암환자 1례 (A Case Report of Recurred Hepatocellular Carcinoma Patient Treated with Radio-Frequency Hyperthermia in Conjunction with Sorafenib)

  • Objective : The purpose of this study is to report the effects of radio-frequency hyperthermia cancer treatment in conjunction with Sorafenib on hepatocellular carcinoma patient. Method : The patient was diagnosed with hepatocellular carcinoma at S6/7 and treated with right posterior sectionectomy. After 4 months, tumor recurrence was found at S4, 5 and 8. After transarterial chemoembolization, the patient was prescribed Sorafenib (proprietary name $Nexavar^{(R)}$ ) as well as proceeding with radio-frequency hyperthermia. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (AFP, PIVKA-II), natural killer (NK) cell activity, and numeric rating scales (NRS). Results : After the treatment, tumor size was decreased accompanying by reducing the level of tumor markers (AFP, PIVKA-II). Major clinical symptoms were improved with increasing NK cell activity. There were no adverse events based on National Cancer Institute Common Terminology Criteria for Adverse Events version 4.0. Conclusion : This case suggests that radio-frequency hyperthermia has synergistic effect for the treatment of hepatocellular carcinoma patient in conjunction with Sorafenib.
