Rhabdomyolysis after the free fibular flap operation for mandibular reconstruction: a case report

  • Background: Free fibular flap is one of the most useful methods in the hard tissue reconstruction of the maxillamandible. Free fibular flap presents some advantages in which the reconstruction of both soft and hard tissues can be done at the same time. It also provides a safe and successful bone graft for the reconstruction, along with a low rate of complications. Despite these advantages and the rarity of a postoperative complication, particularly in oral and maxillofacial surgery procedures, a prolonged operation might exhibit some complications related with rhabdomyolysis. We experienced the rare event of rhabdomyolysis after oral cancer surgery. Case presentation: In this article, we report the case of a patient who developed rhabdomyolysis after undergoing free fibular flap surgery. Conclusions: Despite the advantages of the free fibular flap operation, clinicians must be aware of the risk of complications because there are multiple factors that could result in rhabdomyolysis, such as duration of operation, position of the subject, and pre-existing conditions of diabetes and hypertension. Once the diagnosis of rhabdomyolysis is confirmed, a prompt treatment plan should be made and applied as soon as possible. This will increase the chance of a full recovery for the patient who is exhibiting symptoms of rhabdomyolysis.

치과환자에서 경동맥 석회화 유발율에 대한 전산화단층사진 연구 (A CT study of the prevalence of carotid artery calcification in dental patients)

  • Purpose : Stroke is one of the leading causes of death in Korea. Atherosclerotic disease in the carotid artery bifurcation is the most common cause of stroke. The carotid artery calcification is easily appreciated by CT (Computed tomography). CT is often taken in a dental hospital for the diagnosis of inflammation, injury, cyst or tumor on maxillofacial region. However, there was no report of carotid artery calcification on CT in dental patients. This study was to determine the prevalence of carotid artery calcification on CT in dental patients. Materials and Methods : The presence of carotid artery calcification was evaluated by an experienced radiologist on CT scans of 287 patients (166 males, 121 females, average age 42, range 6 to 86 years) and the medical history of the patient and the interpretation of CT were reviewed. Results : Carotid artery calcification was detected on CT scans of 57 patients (19.8%; 35 males, 22 females). All the male patients with carotid artery calcification were older than 50, and all the female patients with carotid artery calcification were older than 60. Among the 57 patients, 10 had Diabetes mellitus, 20 had cardiovascular disease, 3 had history of stroke and 3 underwent radiation therapy for head and neck cancer. Carotid artery calcification was not included in the interpretation of CT of dental patients except one patient. Conclusion : The prevalence of carotid artery calcification on CT of dental patients was about 20% in this study. Carotid artery calcification should be included in the interpretation of CT of dental patients.

Contralateral recurrence of necrotizing sialometaplasia of the hard palate after five months: a case report

  • Necrotizing sialometaplasia usually heals within 4 to 10 weeks with conservative treatment, and rarely recurs. When necrotizing sialometaplasia is present on the hard palate it may occur unilaterally or bilaterally. In this case, necrotizing ulceration occurred on the left hard palate of a 36-year-old woman after root canal treatment of the upper left first premolar under local anesthesia. After only saline irrigation the defect of the lesion completely healed and filled with soft tissue. After 5 months, however, a similar focal necrosis was found on the contralateral hard palate without any dental treatment having been performed on that side and progressed in similar fashion as the former lesion. We conducted an incisional biopsy and obtained a final pathological diagnosis for the palatal mass of necrotizing sialometaplasia. At the 3-year follow-up, the patient's oral mucosa of the hard palate was normal, without any signs and symptoms of the condition. We report a case of a second occurrence of necrotizing sialometaplasia on the contralateral side from the first, with a time lapse between the first and second occurrence.

  • Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is an ulcerative lesion on oral mucosa featuring as a benign mass of self-limiting growth. It can be easily misdiagnosed as squamous cell carcinoma (SCC) due to its long healing period and elevated or rolled-up margin. A 57-year old male patient who visited our department was diagnosed as SCC according to the clinical features, results of positron emission tomography (PET) and magnetic resonance imaging (MRI) of the oral lesion. However, after performing incisional biopsy, histopathologically, there were no atypical cells, but eosinophil and CD 30+ T-cells were clustered in subcutaneous and muscle tissue. It is very significant to consider this reactive lesion in the field of oral and maxillofacial surgery because of its possibility of mistaken as malignant disease. Here, we will report a couple of cases of TUGSE with references and limit of radiographic tools used for diagnosis of head and neck cancer.

유방종괴의 세침흡인세포학의 진단적 가치에 관한 연구 (A Study of Diagnostic Value on Fine Needle Aspiration Cytology of the Breast Masses)

  • This study was performed in order to evaluate the accuracy and the usefulness of the fine needle aspiration cytology (FNAC) on the breast lesions, to compare the FNAC findings between fibroadenoma and fibrocystic disease, and to determine the accuracy of cytologic Black's nuclear grading. The subjects in this study were 110 cases of FNAC, later confirmed by biopsy, between January 1988 and December 1991. The results are as follows ; 1 Comparison between the results of the FNAC and the histologic findings revealed that FNAC had a sensitivity of 96.6%, a specificity of 100%, a false negative rate of 3.4% a false positive rate of 0.0%, and an overall diagnostic accuracy of 98.2%. 2 Semi-quantitative evaluation of epithelial celluarity, stroma, and naked nuclei in the smears of aspirate showed high celluarity in 56.7% of the aspirates from fibroadenoma and in 0% of those from fibrocystic disease. Abundant stroma was found in 46.7% of the fibroadenoma and none of fibrocystic disease. Numerous naked nuclei were found in 60% of the fibroadenoma and 4.5% of the fibrocystic disease. The overall diagnostic accuracy was 98% 3. In order to determine the accuracy of Black's nuclear grading of FNAC on breast carcinoma, we retrospectively studied 38 cases of ductal carcinomas diagnosed by FNAC with subsequent histologic confirmation. The concordance rate with histology was 94.7%. These results suggest that FNAC of breast is a diagnostically accurate method, and provide for the preoperative differential diagnosis between fibroadenoma and fibrocystic disease. Our results also suggest that the evaluation of nuclear grading of FNAC can predict clinical outcome and decide the way of management of breast cancer.

KOTRON-13과 상용 PET 사이클로트론의 최근 기술 동향 (Recent Status of Commercial PET Cyclotron and KOTRON-13)

  • 상용 PET사이클로트론의 기술적인 변화는 PET의 수요와 밀접한 관계가 있다. PET활용이 일반화 되어 미국을 중심으로 PET의 수요가 급증하게 됨에 따라 사이클로트론의 수요도 증가 하였다. 기존의 사이클로트론은 크기와 무게로 병원에 설치에 공간과 전원 공급 등 여러 문제가 있어 각 사이클로트론 제작사들은 새로운 모델을 출시하게 되었다. 새로운 모델의 특징은 다음과 같다. 첫째 기존의 모델이 양성자와 중양자 등의 2가지 이온을 인출 하였으나 시스템의 복잡성 등의 문제와 양성자로 O-15 생산이 가능하여 양성자 빔만을 인출 하거나 중양자는 선택 사양으로 전환 하였다. 둘째 기존 모델의 경우 양성자의 빔 에너지는 약 16 MeV 이상 가속 시킬 수 있으나 신 모델은 13 MeV 이내로 대폭 낮추어 PET 용 Rl생산에 최적화하였다. 셋째 전자석, RF 장치등의 최적화를 이룩하여 전력소모를 대폭 개선하였으며 외양도 대폭 작아져 무게를 20톤 이내로 대폭 낮추었다. 넷째 새로운 모델 사이클로트론들은 자체 차폐기능이 일반화 되었으나 IBA제품이나 삼영유니텍 제품은 사용자의 견지에 따라 선택 사양이 가능하다.

분자영상용 방사성 금 나노입자 합성 (Synthesis of 125I-Labeled Gold Nanoparticles for a Molecular Imaging)

  • Gold nanoparticles (GNPs) have led to the development of a new field in the diagnosis and treatment of diseases such as cancer. An efficient synthesis of gold nanoparticles within the range of 8~57 nm was established by ${\gamma}-ray$ irradiation. The good point of a radiation-based method is the production of gold nanoparticles with a higher concentration and narrower size distribution compared with conventional methods. The size of gold nanoparticles was controlled using two methods. : (i) varying the ${\gamma}-ray$ irradiation dose of 10 to 25 kGy and (ii) varying the concentration of $HAuCl_4$ solution from 4 to 40 mM. In addition, the GNPs were radiolabeled using $[^{125}I]NaI$ in a simple and fast manner with high yields. The produced gold nanoparticles were characterized using a transmission electron microscopy (TEM), a UV-visible spectrophotometer, and a radio-TLC imaging scanner. From these results, these radiolabeled GNPs can be applicable for a radioisotope tag of biomolecules.

공통데이터모델 기반의 임상의사결정지원시스템에 관한 연구 (A Study on Clinical Decision Support System based on Common Data Model)

  • 최근 의료IT 분야 솔루션들이 분산 환경 기반으로 제공되고 있는 추세이다. 국내에서도 분산 환경에서 의료정보를 공유할 수 있는 임상의사결정지원시스템 개발의 필요성이 인식되어 연구되고 있다. 기존 임상의사결정지원시스템은 병원 내의 자체적인 의료정보만을 사용하여 구축되고 있다. 이로 인해 기존의 시스템은 의사결정지원의 효율성 및 정확성 측면에서 좋은 결과를 얻기 어렵다. 이러한 한계점을 해결하기 위해 이 논문에서는 의료분야의 공통 데이터 모델을 기반으로 하는 임상의사결정지원시스템 모델을 설계하고 구축방안을 제시한다. 제안 모델의 적용 과정을 설명하기 위해 대장암 진단을 위한 임상의사결정지원시스템의 개발 시나리오를 기술한다. 또한 성공적인 임상의사결정지원시스템 개발을 위한 필수 요구사항을 제시한다. 이를 통해 여러 병원에서 공통으로 사용이 가능하고 시스템의 효율성과 정확성을 높일 수 있는 임상의사결정지원시스템 개발이 가능할 것으로 기대한다.

MicroRNAs and periodontal disease: a qualitative systematic review of human studies

  • Purpose: MicroRNAs (miRNAs) are epigenetic post-transcriptional regulators that modulate gene expression and have been identified as biomarkers for several diseases, including cancer. This study aimed to systematically review the relationship between miRNAs and periodontal disease in humans, and to evaluate the potential of miRNAs as diagnostic and prognostic biomarkers of disease. Methods: The review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines (reference number CRD42020180683). The MEDLINE, Scopus, Cochrane Library, Embase, Web of Science, and SciELO databases were searched for clinical studies conducted in humans investigating periodontal diseases and miRNAs. Expression levels of miRNAs across the different groups were analysed using the collected data. Results: A total of 1,299 references were identified in the initial literature search, and 23 articles were finally included in the review. The study designs were heterogeneous, which prevented a meta-analysis of the data. Most of the studies compared miRNA expression levels between patients with periodontitis and healthy controls. The most widely researched miRNA in periodontal diseases was miR-146a. Most studies reported higher expression levels of miR-146a in patients with periodontitis than in healthy controls. In addition, many studies also focused on identifying target genes of the differentially expressed miRNAs that were significantly related to periodontal inflammation. Conclusions: The results of the studies that we analysed are promising, but diagnostic tests are needed to confirm the use of miRNAs as biomarkers to monitor and aid in the early diagnosis of periodontitis in clinical practice.

Development of a Novel Multiple Cross-Linking Spiral Amplification for Rapid and Sensitive Detection of HPV16 DNA

  • There has been increasing interest in the head and neck squamous cell carcinoma (HNSCC) that is caused by high-risk human papillomavirus (HR-HPV) and has posed a significant challenge to Otolaryngologists. A rapid, sensitive, and reliable method is required for the detection of HR-HPV in clinical specimens to prevent and treat HPV-induced diseases. In this study, a multiple cross-linking spiral amplification (MCLSA) assay was developed for the visual detection of HPV-16. In the MCLSA assay, samples were incubated under optimized conditions at 62℃ for 45 min, and after mixing with the SYBR Green I (SGI) dye, the positive amplicons showed bright green fluorescence while the negative amplicons exhibited no obvious change. The specificity test revealed that the developed MCLSA technique had high specificity and could effectively distinguish all five HPV-16 strains from other pathogenic microorganisms. In terms of analytical sensitivity, the limit of detection (LoD) of MCLSA assay was approximately 5.4 × 101 copies/tube, which was 10-fold more sensitive than loop-mediated isothermal amplification (LAMP) and RT-PCR. The detection results of laryngeal cancer specimens collected from 46 patients with suspected HPV infection in the Liaoning region demonstrated that the positive detection rates of MCLSA and hybridized capture 2 kit were 32.61% (15/46). The true positive rate of the MCLSA assay was higher than that of RT-PCR (100% vs. 93.33%) and LAMP (100% vs. 86.67%). Therefore, the MCLSA assay developed in the present study could be a potentially useful tool for the point-of-care (PoC) diagnosis of HR-HPV, especially in resource-limited countries.
