This paper presents a implantable intrathecal drug infusion pump for pain control in cancer patients. This device consists of micropump module, drug reservoir module and control module. The micropump module using cam-follower mechanism composed of small-sized four cams and four followers. Each followers is driven by a cam and liquid is discharged by a sequential reciprocal motion of the followers. The advantage of this structure is that it allows the pump to be clean and valveless. The drug reservoir module composed of drug chamber, gas chamber and diaphragm. The control module composed of battery, wireless communication unit and controller. To design a small-sized, low power pump some analysis were performed to determine the design parameters. To verify the feasibility of the experiment, a prototype was manufactured and its operating characteristics were investigated. Experimental results were in accordance with the expected results obtained from analysis.
Purpose: This study was to explore the psychosocial adjustment, marital intimacy and family support of post-mastectomy patients. Methods: The subjects were 90 post-mastectomy patients (stage I or II) who were eligible and agreed to participate in the study. The data collection period was from March to May, 2008. Subjects completed a survey including demographics, psychosocial adjustment scale by Lee (a 4-point Likert scale), marital intimacy scale by Kim (a 4-point Likert scale), and family support by Shim (a 5-point Likert scale). Data were analyzed using SAS (ver 9.0) program and frequency, mean (SD), t-test, ANOVA and Pearson correlation coefficients were used. Results: The mean scores of the psychosocial adjustment and marital intimacy were 2.8 (${\pm}0.4$) and 2.5 (${\pm}0.5$). Also, the mean score of the family support was 3.6 (${\pm}0.7$). Regarding the correlation between the psychosocial adjustment, marital intimacy and family support, there was a positive correlation. Conclusion: These results showed that subjects who had higher marital intimacy and family support showed better in psychosocial adjustment. To improve psychosocial adjustment of post-mastectomy patients for breast cancer more effectively, there is an necessity to provide various strategic supports and intervention for effective communication with spouse and family.
All living cells release extracellular vesicles having pleiotropic functions in intercellular communication. Mammalian extracellular vesicles, also known as exosomes and microvesicles, are spherical bilayered proteolipids composed of various bioactive molecules, including RNAs, DNAs, proteins, and lipids. Extracellular vesicles directly and indirectly control a diverse range of biological processes by transferring membrane proteins, signaling molecules, mRNAs, and miRNAs, and activating receptors of recipient cells. The active interaction of extracellular vesicles with other cells regulates various physiological and pathological conditions, including cancer, infectious diseases, and neurodegenerative disorders. Recent developments in high-throughput proteomics, transcriptomics, and lipidomics tools have provided ample data on the common and specific components of various types of extracellular vesicles. These studies may contribute to the understanding of the molecular mechanism involved in vesicular cargo sorting and the biogenesis of extracellular vesicles, and, further, to the identification of disease-specific biomarkers. This review focuses on the components, functions, and therapeutic and diagnostic potential of extracellular vesicles under various pathophysiological conditions.
The objective of this paper is to present the implementation and process evaluation of the first computer tailored program for smoking cessation among Romanian smokers. The program targeted adult smokers who declared the intention to quit smoking in the next six months. The intervention consisted of a letter tailored to several respondent characteristics: gender, cognitive variables (attitude, social influence, and self-efficacy), intention to quit smoking, goal and relapse prevention strategies (action and coping plans), and smoking behaviour. The first 80 participants entered into the program filled in a process evaluation questionnaire one month after the intervention. The results of our study confirmed that the participants had read and remembered the letter. Moreover, new for Romania, this approach was positively appreciated by the participant and the score received for the tailored letter was high. The opinions of the participants confirmed that the tailored letter provided information that was useful, trustworthy and relevant for the individual. At the same time, the participants appreciated the polite, easy to understand content of the letter. These data underlined the premises for continuing the program and for using the information and communication technologies for healthy lifestyle promotion among Romanian population.
Purpose: The purpose of this study was to identify characteristics of patients who were recipients of decision-making DNR, to describe the situations of DNR, and to analyze the APACHE III and MOF scores. Method: Data collection was conducted through reviews of medical records of 51 patients and through interviews with families of patients who were decision-makers for DNR at C university K Hospital located in Seoul from April to September 2002. Results: The men's APACHE III and MOF scores were higher than the women's and the non cancer patients were higher than cancer patients. Some 80.4% of DNR orders was by communication, while 11.8% of consents were written. Each of APACHE III and MOF scores of patients in the intensive care unit was higher than the patients in general ward at both points of admission and decision-making of DNR. APACHE III and MOF scores positively correlated statistically with each other. Conclusions: The findings of this study suggest that APACHE III and MOF scores be useful for decision-making of DNR as a tool measuring severity.
Hypoxia, a widespread occurrence observed in various malignant tumors, results from rapid tumor growth that outpaces the oxygen supply. Tumor hypoxia precipitates several effects on tumor biology; these include activating angiogenesis, intensifying invasiveness, enhancing the survival of tumor cells, suppressing anti-tumor immunity, and fostering resistance to therapy. Aligned with the findings that correlate CMGC kinases with the regulation of Hypoxia-Inducible Factor (HIF), a pivotal modulator, reports also indicate that hypoxia governs the activity of CMGC kinases, including DYRK1 kinases. Prolyl hydroxylation of DYRK1 kinases by PHD1 constitutes a novel mechanism of kinase maturation and activation. This modification "primes" DYRK1 kinases for subsequent tyrosine autophosphorylation, a vital step in their activation cascade. This mechanism adds a layer of intricacy to comprehending the regulation of CMGC kinases, and underscores the complex interplay between distinct post-translational modifications in harmonizing precise kinase activity. Overall, hypoxia assumes a substantial role in cancer progression, influencing diverse aspects of tumor biology that include angiogenesis, invasiveness, cell survival, and resistance to treatment. CMGC kinases are deeply entwined in its regulation. To fathom the molecular mechanisms underpinning hypoxia's impact on cancer cells, comprehending how hypoxia and prolyl hydroxylation govern the activity of CMGC kinases, including DYRK1 kinases, becomes imperative. This insight may pave the way for pioneering therapeutic approaches that target the hypoxic tumor microenvironment and its associated challenges.
Purposes: The purpose of this study is to analyze cases of development and operation of a mentoring program that provides psychological support to new employees and helps them adapt to work, thereby applying it to actual work and laying the foundation for follow-up research. Methodology: We explored the development and application process of A Hospital mentoring program by applying the mentoring program model developed according to the procedures of the ADDIE model, and confirmed the perceptions of participants who participated in the training course through analysis of activity logs and in-depth interviews. Findings: The main results of the case analysis are as follows. First, the curriculum was developed according to the stages of analysis, design, development, implementation, and evaluation. As a result of activity log and in-depth interview analysis, participants recognized that the mentoring program was helpful in forming social relationships, organizational adaptation, and preventing job turnover, and recognized difficulties in communication. Participants mentioned supplementing the operating system. Practical Implication: The results of a systematic review of the application and effectiveness of mentoring programs for new employees can serve as reference material for practical program design.

With the development of information and communication technology, the Internet is more important in the social and economic influence rapidly, and it is no different in the field of health care. As health information on the Internet increasing, the availabilities of health information from the Internet becomes more important with health care professionals and information specialists. the quality of health information on the Internet are continually being presented without any guarantee or judge on the quality. It is needed to provide the right to use of qualified health information through Internet. HONCODE has been established and managed by HON (Health On the Net) Foundation. In this paper, Web sites of thyroid cancer Information are evaluated using HONCODE. They provide domestic medical information on the Internet. Through this, more accuracy and evaluated information could be provided on the Internet about the thyroid cancer.
The purpose of this study was to demonstrate the feasibility of VIA screening with cryotherapy and to record normative values for indicators anticipated in similar low resource settings. Women aged 30-49 years were targeted, resulting in 1961 women screened and treated at two primary health care (PHC) centres near Suva, Fiji. Recruitment was through provision of information, education and communication (IEC). Referrals to a gynaecology outpatient department (OPD) at a referral hospital occurred throughout the screening pathway. Participation was 32% (95%CI 31-33%), higher in iTaukei (Melanesians) women (34%, 95%CI 33-36) compared to Fijians of Indian descent (26%, 95%CI 24-28). Regression analysis, adjusted for confounders, indicated significantly lower participation in those of Indian descent, and age groups 35-39 and 45-49 years. Of those examined by VIA, 190 were positive with aceto-white lesions (9.9%), within the expected range of 8-15%, with minor geographic and ethnic variation. Positive VIA results were more common in the peri-urban area, and in those aged 35-39 years. Of women aged 30-49 years, 59 received cryotherapy (none of whom had significant complications), 91 were referred to OPD, two cervical carcinomas were identified and eight cervical intra-epithelial neoplasms (CIN) II-III were diagnosed. These results provide normative findings from a community-based VIA screening program for other similar low resource settings.
Background: Evidence based medicine has become main tools for medical practice. However, conducting a highly ranked in the evidence hierarchy pyramid is not easy or feasible at all times and places. There remains a room for descriptive clinical outcome measure studies with admitting the limit of the intepretation. Aims: Presents three Korean clinic based outcome measure studies with a view to encouraging Korean clinicians to conduct similar studies. Methods: Three studies are presented briefly here in치uding 1) Quality of Life of liver cancer patients after 8 Constitutional acupuncture; 2) Developing a Korean version of Measuring yourself Medical Outcome profile (MYMOP); and 3) Survey on 5 Shu points: a pilot In the first study, we have included 4 primary or secondary liver cancer patients collecting their diagnostic X-ray film and clinical data f개m their hospital, and asked them to fill in the European Organization Research and Treatment of Cancer, Quality of Life Questionnaire before the commencement of the treatment. The acupuncture treatment is set up format but not disclosed yet. The translation and developing a Korean version of outcome measures that is Korean clinician friendly has been sought for MYMOP is one of the most appropriate one. The permission was granted, the translation into Korean was done, then back translated into English only based on the Korean translation by the researcher who is bilingual in both languages. The back translation was compared by the original developer of MYMOP and confirmed usable. In order to test the existence of acupoints and meridians through popular forms of Korean acupuncture regimes, we aim at collecting opinions from 101 Korean clinicians that have used those forms. The questions asked include most effective symptoms, 5 Shu points, points those are least likely to use due to either adverse events or the lack of effectiveness, theoretical reasons for the above proposals, proposing outcome measures, and the time from insertion to be effective. The questionnaire is posted on the virtual notice board of the homepage of the communication KOMA (The official tool of Association of Korean Oriental Medicine).
