Analysis of Women with Low Back Pain and Bone mineral density (일부 여성의 요통과 골밀도에 관한 분석)

  • Objectives: The objective of this study was to investigate analysis of women with low back pain and osteoporosis were measured for 40 normal in the women from July 20, 2000 to October 20, 2000. Methods: Bone mineral density(BMD) of lumbar spine was measured using energy absorptiometry and were correlated with age, calcium. alkaline phosphatase. bone mineral density standard T scores(p<0.05). Results: The bone mineral density of the lumbar spine decreased with aging, The bone mineral density of the lumbar spine decreased with the serum calcium and phosphate increased. The mean bone mineral density of the lumbar spine of healthy women in age($50\sim59$) was $0.83g/cm^2$, the lumbar spine of women low back pain in age($50\sim59$) was 0.75 glad. Conclusion: In the multiple regression of risk factors to bone mineral density(BMD) of lumbar spine were correlated with age, of abortion, calcium, bone mineral density standard T scores(p<0,05). In the prevention and early diagnosis and treatment of osteoporosis, the physician should consider the risk factors.

Effects of Nutrients Intakes on Development of Osteoporosis in Korean Postmenopausal Women (폐경후 우리나라 여성의 영양 섭취 상태가 골다공증 발병에 미치는 영향)

  • The relationship of nutrients intake and bone mineral density (BMD) was investigated in 285 Korean postmenopausal women (age 40-70 y) consisted of 65 normal women and 159 osteopenia patients who don't have other diagnosed disease. BMD was measured at the spine (vertebrae L2-4) and femur (neck, Ward's triangle and trochanter). Height, weight, body mass index(BMI), lean body mass(LBM), body fat(Bfat) and dietary intakes of animal calcium(animal Ca), protein and phosphorus per 1,000kcaI intake were correlated with BMD of the spine and the femur positively(p<0.05). Women with an animal calcium intake < 315mg/d (mean % animal Ca/total Ca intake > 50%) exhibited significantly lower BMD of the spine and Ward's triangle than that of women with an intake > 315mg/d, p<0.OS. When subjects were grouped by diagnosis into 3 groups (normal, osteopenia and osteoporosis group), animal Ca intake of osteoporosis group (mean animal Ca intake 261mg) was significantly lower than those of the other two groups (mean animal Ca intake 306mg and 297mg, respectively), p < 0.05. These results suggest that balanced nutrients intake and increased animal Ca in the diet is likely to be beneficial in reducing bone loss in postmenopausal women.

Treatment of Osteoporosis after Hip Fracture: Survey of the Korean Hip Society

  • Purpose: To assess current practice in the treatment of osteoporosis in patients who underwent treatment for hip fracture in South Korea. Materials and Methods: A survey of 97 members of the Korean Hip Society, orthopedic hip surgeons who administer treatment for hip fractures in South Korea, was conducted. The survey was conducted for assessment of demographic data and perceptions regarding the management of osteoporosis in patients who have undergone treatment for hip fracture. Analysis of the data was performed using descriptive statistical methods. Results: The majority of participants were between the age of 41 and 50 years, and 74% were practicing in tertiary hospitals. Testing for serum vitamin D levels (82%) was the most commonly performed laboratory test. Calcium and vitamin D were prescribed for more than 80% of patients by 47% and 52% of participants, respectively. Denosumab was the most commonly used first-line treatment option for osteoporosis in hip fracture patients. Bisphosphonate was most often perceived as the cause of atypical femoral fractures, and the most appropriate time for reoperation was postoperative 12 months. Teriparatide was most preferred after cessation of bisphosphonate and only prescribing calcium and vitamin D was most common in high-risk patients for prevention of atypical femoral fracture. Conclusion: The results of this study that surveyed orthopedic hip surgeons showed that most participants followed the current strategy for management of osteoporosis. Because the end result of osteoporosis is a bone fracture, active involvement of orthopedic surgeons is important in treating this condition.

A comparative study on the osteoporotic animal models in ddY mouse: ovariectomized and neurectomized models (ddY 마우스를 이용한 골다공증 모델에 대한 비교연구: 난소적출 및 신경절단 모델)

  • In order to compare the induced time of osteoporosis between ovariectomized and neurectomized models in ddY mice. Experimental groups were divided into Sham, ovariectomized (OVX group) and neurectomized (NX group) group. The changes of body weight, tibia weight and histomorphometry of epiphyseal regions of tibia that were generally used as criteria index in osteoporosis, were evaluated at 2 and 4 weeks after operations with other generally used index-changes of serum osteocalcin. Also, calcium and phosphorus levels in the ash tibia were demonstrated with their ratio (Ca/P ratio). From the result of this study, evidences which reflect osteoporotic states of animals such as decrease of absolute and relative tibia weight, histomorphometrical index of epiphyseal region of tibia including trabecular bone volume %, and calcium and phosphorous contents in tibia, were generally detected from 4 weeks after ovariectomy and 2 weeks after neurectomy with increase of serum osteocalcin levels. In conclusion, it is considered that more rapid and favorable osteoporosis was induced in neurectomized model compared to that of ovariectomized model.

AN EXPERIMENTAL STUDY OF MANDIBULAR FRACTURE WOUND HEALING IN THE CALCIUM DEFICIENT RAT (저칼슘식이가 백서의 하악골 골절치유에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study was to investigate effects of osteoporosis on fracture wound healing in the calcium deficient rat. To research the experiment some ten-week old Wistar strain rats with approximately 300 gms weight were selected. Then, the rats were divided into two groups: Normal diet group(rats given a normal diet before and after bone fracture) and Low calcium diet group(rats given a low calcium diet before and after bone fracture). Both groups had been provided with each diet for three weeks. When the rats became thirteen weeks old, the mandibular angle of rats in both groups was artificially fractured for test. The healing of fracture wounds was reviewed by using soft x-ray radiography and /sup 99m/Tc-MDP bone scan and also histopathologic examination. The obtained results were as follows : 1. The radiolucency of the fracture site for the Normal diet group started to decrease from the 14th day since the experiment was made, while the Low calcium diet group began decrease in the radiolucency from the 21st day of the experiment. The radiolucency for the Normal diet group disappeared at the 42nd day, but one for the Low calcium diet group disappeared at the 56th day of the experiment. 2. The highest uptake rate of /sup 99m/Tc-MDP stood at the 14th day of the experiment in the Normal diet group and the Low calcium diet group's maximum rate was recorded at the 21st day of the experiment. These both groups were gradually experiencing decrease in the uptake rate as the experiment time was going on. However, the uptake rate in the Low calcium diet group was lower than one in the Normal diet group. 3. For the Normal diet group, the newly formed trabeculae, which were similar to one of the surroundung bone, were seen at the 42nd day of the experiment. On the other hand, the Low calcium diet group showed at the 56th day of the experiment that the osteoporotic findings looked weak, irregular trabeculae, and also large bone marrow space were observed clearly. As a result of the above experiment, it is said that the healing of the fracture can be completed for both groups, the Normal diet group and the Low calcium diet group. However, the amount of the newly formed bone wound in the Low calcium diet group is rather decreased compared to one in the Normal diet group and at the same time the healing of the fracture is delayed in the Low calcium diet group. Consequently, for the successful healing of fracture in osteoporosis, it is considered that the management of the etiologic factors of osteoporosis must be preceded. The more study of calcium metabolism and functions of osteoblast and osteoclast needs to proceed on.

Knowledge, Health Belief, and Self-efficacy Related to Osteoporosis (일개 신도시 지역의 골다공증 지식, 건강신념, 자기 효능감에 관한 연구)

  • Purpose: This research was conducted to examine the relationship of knowledge, health beliefs, and self-efficacy on osteoporosis. Method: The design of this study was a correlative design. The subjects were 1,615 Korean adults over the age of 20 registered in a new town and participants were asked to fill in a questionnaire developed by Kim, Horan & Gendler (1991), translated into Korean. Results: 1) The subjects recorded an average score of 1l.10 on osteoporosis knowledge. The average scores on the osteoporosis health beliefs was 15.68 for perceived susceptibility and the respondents recorded an average score of 40.40 for osteoporosis self-efficacy. 2) There were statistically significant differences in the degree of osteoporosis knowledge, self-efficacy and health belief according to gender, age, scholastic achievement, marital state, and jobs. 3) There were statistically significant positive correlations between osteoporosis knowledge and self-efficacy. Conclusion: According to these results, an osteoporosis education program improving not only knowledge but also self-efficacy and health beliefs should be developed and applied to decrease the perception of barriers to exercise and intake of calcium.

Protective effect of dietary oils containing omega-3 fatty acids against glucocorticoid-induced osteoporosis

  • Purpose: Glucocorticoids (GCs) are implicated in secondary osteoporosis, and the resulting fractures cause significant morbidity. Polyunsaturated fatty acids (PUFAs) play a vital role in bone metabolism. However, few trials have studied the impact of omega-3 PUFA-containing oils against GC-induced osteoporosis. Therefore, the present study was undertaken to determine whether supplementation with omega-3 PUFA-containing dietary oils such as fish oil, flaxseed oil or soybean oil can impede the development of GC-induced osteoporosis. Methods: The fatty acids (FAs) content of oils was determined using gas chromatography. Male rats were subdivided into 5 groups (8 rats each): normal control (balanced diet), prednisolone control (10 mg/kg prednisolone daily), soybean oil (prednisolone 10 mg/kg + soybean oil 7% w/w), flaxseed oil (prednisolone 10 mg/kg + flaxseed oil 7% w/w), and fish oil (from cod liver; prednisolone 10 mg/kg + fish oil 7% w/w). Results: The study data exhibited a significant depletion in bone mineral density (BMD) and femur mass in the prednisolone control compared to the normal control, accompanied with a marked decrease in the levels of plasma calcium and 1,25-$(OH)_2$-vitamin $D_3$, and elevated levels of C-terminal telopeptide (CTX), tumor necrosis factor-alpha (TNF-${\alpha}$) and malondialdehyde (MDA). Supplementation with fish oil, soybean oil or flaxseed oil helped to improve plasma calcium levels, and suppress oxidative stress and inflammatory markers. Additionally, bone resorption was suppressed as reflected by the decreased CTX levels. However, fish oil was more effective than the other two oils with a significant improvement in BMD and normal histological results compared to the normal control. Conclusion: This study demonstrated that supplementation with dietary oils containing omega-3 PUFAs such as fish oil, soybean oil or flaxseed oil can play a role in the prevention of bone loss and in the regulation of bone metabolism, especially fish oil which demonstrated a greater level of protection against GC-induced osteoporosis.

  • Osteoporosis is a major underlying cause of bone fracture in postmenopausal women and older person in general, and so it is a major public health problem. It is a condition in which bone mass decreases, causing bones to be more susceptible to fracture. A trivial trauma can easily cause one or more bones to break in a person with severe osteoporosis. Physicians and patients alike are concerned with the optimum approach to the treatment and prevention of osteoporosis. The diagnostic method and proper use of agents, such as calcium, vitamin D, estrogen, and fluorides, as well as the role of exercise are issues that have generated major research efforts. So these are to be mentioned in this paper.

A study on the recognition about osteoporosis and Traditional Korean Medicine(TKM) treatment for osteoporosis. (한방병원 내원 여성의 골다공증과 한방 치료에 대한 인식 조사)

  • Purpose : A study on the recognition about osteoporosis and TKM treatment for osteoporosis of women who visited Sangji Oriental Medical Hospital. Methods : We studied the recognition and realities of TKM treatment for osteoporosis with questionnaire from 14th september, 2005 to 14th October, 2005 in Sang-ji Oriental medical clinic. A questionnaire was given to 184 women and 171 women answered. Results : 1) In 171 women, 169(98.8%) women have heard about osteoporosis, 1(0.6%) woman hasn't heard about it, and 1(0.6%) woman doesn't know. 2) In 171 women, 85(49.7%) women checked up for osteoporosis. 31(36.1%) women of them were diagonosed as osteoporosis, 50(58.1%) women of them were not diagonosed as osteoporosis, and 5(5.8%) women of them didn't know.3) In 31 women, 18 women is taking none medical care, 5 women calcium supplement, 4 women HRT, and 4 women both therapy. 4) In 171 women, 74(43.3%) women know about TKM treatment for osteoporosis, 96(56.1%) women don't know. 5) In 171 women, 140(81.9%) women have intention of TKM treatment for osteoporosis, 17(9.9%) women don't have, and 14(8.2%) women don't know. Conclusion : The guide that treats osteoporosis with T]U is needed on purpose to make intention of TKM treatment for osteoporosis to real demand. If the guide be informed to oriental doctors, It can expand the range of oriental gynecology.

The Role of Milk Product in Nutritional Intake and Chronic Disease Reduction (영양 섭취와 만성질환 예방을 위한 우유의 역할)

  • The milk product is one of the major foods which provide calcium for nutritional requirement. In addition to calcium, it supplies diverse nutrients including protein, potassium, vitamin A, vitamin $B_1$, and vitamin $B_2$. Adequate calcium intake throughout lifetime ensures optimum bone density and prevents osteoporosis in old age by reducing loss of bone mineral during growth and aging. The nutritional deficiency of calcium intake and spreading of osteoporosis in Korean population accentuated necessity of adequate calcium intake through milk consumption. Obstruction of milk consumption due to lactose intolerance should be overcome by educational programs for adequate milk consumption and low-lactose milk product developments. Consumption of milk products was reported to prevent high blood pressure in clinical studies and epidemiological investigations. As the feeding of milk increased, the body weight and fat of animal decreased in animal studies. The epidemiological investigations revealed that there was a negative correlation between the amount of milk consumption and the body fat. When calcium was provided as dietary supplement in the clinical trials, the body weight and body fat of subjects decreased. When calcium was particularly supplied as a part of milk, the body weight reduction rate was augmented. Calcium, vitamin D, and conjugated linoleic acid have been reported to exert a role in reducing cancer. The paramount importance of milk in nutrition as a source of calcium and other invaluable nutrients emphasizes consistent supply of milk products to adults as well as adolescents by dairy industry.
