  • True nursing care means total nursing care which includes physical, emotional and spiritual care. The modern nursing care has tendency to focus toward physical care and needs attention toward emotional and spiritual care. The total nursing care is mandatory for patients with terminal cancer and for this purpose, hospice care became emerged. Hospice case originated from the place or shelter for the travellers to Jerusalem in medieval stage. However, the meaning of modem hospice care became changed to total nursing care for dying patients. Modern hospice care has been developed in England, and spreaded to U.S.A. and Canada for the patients with terminal cancer. Nowaday, it became a part of nursing care and the concept of hospice care extended to the palliative care of the cancer patients. Recently, it was introduced to Korea and received attention as model of total nursing care. This study was attempted to assess the efficacy of hospice care. The purpose of this study was to prove a difference in terms of physical, emotional a d spiritual aspect between the group who received hospice care and who didn't receive hospice care. The subject for this study were 113 patients with advanced cancer who were hospitalized in the S different hospitals. 67 patients received hospice care in 4 different hospitals, and 46 patients didn't receive hospice care in another 4 different hospitals. The method of this study was the questionaire which was made through the descriptive study. The descriptive study was made by individual contact with 102 patients cf advanced cancer for 9 months period. The measurement tool for questionaire was made by author through the descriptive study, and included the personal religious orientation obtained from chung(originated R. Fleck) and 5 emotional stages before dying from Kubler Ross. The content ol questionaire consisted in 67 items which included 11 for general characteristics, 10 for related condition with cancer, 13 for wishes far physical therapy, 13 for emotional reactions and 20 for personal religious orientation. Data for this study was collected from Aug. 25 to Oct. 6 by author and 4 other nurse's who received education and training by author for the collection of data. The collected data were ana lysed using descriptive statistics, $X^2-test$, t-test and pearson correlation coefficient. Results of the study were as follows: "H.C Group" means the group of patient with cancer who received hospice care. "Non H.C Group" means the group of patient with cancer who did not receive hospice care. 1. There is a difference between H.C Group and Non H.C Group in term of the number of physical symptoms, subjective degree of pain sensation and pain control, subjective beliefs in physical cure, emotional reaction, help of present emotional and spiritual care from other personal, needs of emotional and spiritual care in future, selection of treatment method by patients and personal religious orientation. 2. The comparison of H.C Group and Non H.C Group 1) There is no difference in wishes for physical therapy between two groups(p=.522). Among Non H.C Group, a group, who didn't receive traditional therapy and herb medicine was higher than a group who received these in degree of belief that the traditional therapy and herb medicine can cure their disease, and this result was higher in comparison to H.C Group(p=.025, p=.050). 2) Non H.C Group was higher than H.C Group in degree of emotional reaction(p=.050). H.C Group was higher than Non H.C Group in denial and acceptant stage among 5 different emotional stages before dying described by Kubler Ross, especially among the patient who had disease more than 13 months(p=.0069, p=.0198). 3) Non H.C Group was higher than H. C Group in demanding more emotional and spiritual care to doctor, nurse, family and pastor(p=. 010). 4) Non H.C Group was higher than H.C Group in demanding more emotional and spiritual care to each individual of doctor, nurse and family (p=.0110, p=.0029, P=. 0053). 5) H.C Group was higher th2.n Non H.C Group in degree of intrinsic behavior orientation and intrinsic belief orientation of personal religious orientation(p=.034, p=.026). 6) In H.C Group and Non H.C Group, the degree of emotional demanding of christians was significantly higher than non christians to doctor, nurse, family and pastor(p=. 000, p=.035). 7) In H.C Group there were significant positive correlations as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and: the degree of intrinsic behavior orientation in personal religious orientation(r=. 5512, p=.000). (2) Between the degree of emotional demandings to doctor, nurse. family & pastor and the degree of intrinsic belief orientation in personal religious orientation(r=.4795, p=.000). (3) Between the degree of intrinsic behavior orientation and the degree of intrinsic: belief orientation in personal religious orientation(r=.8986, p=.000). (4) Between the degree of extrinsic religious orientation and the degree of consensus religious orientation in personal religious orientation (r=. 2640, p=.015). In H.C. Group there were significant negative correlations as following; (1) Between the degree of intrinsic behavior orientation and extrinsic religious orientation in personal religious orientation (r=-.4218, p=.000). (2) Between the degree or intrinsic behavior orientation and consensus religious orientation in personal religious orientation(r=-. 4597, p=.000). (3) Between the degree of intrinsic belief orientations and the degree of extrinsic religious orientation in personal religious orientation(r=-.4388, p=.000). (4) Between the degree of intrinsic belief orientation and the degree of consensus religious orientation in personal religious orientation(r=-. 5424, p=.000). 8) In Non H.C Group there were significant positive correlation as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of intrinsic behavior orientation in personal religious orientation(r= .3566, p=.007). (2) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of intrinsic belief orientation in personal religious orientation(r=.3430, p=.010). (3) Between the degree of intrinsic behavior orientation and the degree of intrinsic belief orientation in personal religious orientation(r=.9723, p=.000). In Non H.C Group there were significant negative correlation as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of extrinsic religious orientation in personal religious orientation(r= -.2862, p=.027). (2) Between the degree of intrinsic behavior orientation and the degree of extrinsic religious orientation in personal religious orientation(r=-. 5083, p=.000). • When phenylation of styrene was carried out in the presence of Pd(OAc)2 and PPh3 in benzene, trans-stilbene was obtained in good yield (566%) with high selectivity (98%) under mild condition (55 ℃, 50 psi O2, 20 h). Since trans-stilbene could be produced not only from benzene but also from phenyl group of PPh3 by migration of its phenyl group to Pd, the competitiveness of benzene and the migratory aptitude of aryl group of triarylphosphine toward styrene has been investigated with various phosphines (PR3: P(p-C6H4CH3)3, P(p-C6H4OCH3)3, P(p-C6H4F)3, P(p-C6H4Cl)3, P(C6H5)3, P(C6H11)3, P(OC4H9n)3, P(CH2C6H5)3 and P(C6F5)3). The yield and selectivity toward trans-stilbene are increased as the basicity of the phosphines increases. The composition of arylated olefin from arylphosphine, in turn, increases as the electronegativity of the substituent on the aryl group of arylphosphines increases.

  • Background: This study had been carried out with 18 ischemic stroke patients as its object for about eight months from October, 2006 to May, 2007 in order to observe the recovery of motor function and the change of important blood factors according to the different quantitative exercises. Methods: Subjects were assigned randomly either experimental group (n=19) or the control group (n=19), when the study began the halfway on this study dropout 20 patients, and final subjects remained experimental group's 9 patients and control group's 9 patients. Both groups received thermotherapy and functional electrical stimulation (FES), also taken different quantitative exercise therapy (experimental group 180 minutes, control group 80 minutes). Subjects were assessed for upper and lower extremities motor function Fugl-Meyer Scale; FMS), blood test (white blood count; WBC, low density lipoprotein -cholesterol; LDL-C, high density lipoprotein-cholesterol; HDL-C, Troponin) during pretest, after 2 months, after 3 months. Results: The results of this study were as follows; 1. FMS has no statistically significant difference with intergroup(p>.05). But there was a statistically significant difference with each groups (p<.05). 2. WBC has no statistically significant difference with intergroup (p>.05). But there was a statistically significant difference in control group (p<.05), without experimental group (p>.05). 3. LDL-C has no statistically significant difference with intergroup (p>.05). But there was a statistically significant difference in control group (p<.05), without experimental group (p>.05). 4. HDL-C has no statistically significant difference with intergroup (p<.05). But there was a statistically significant difference with each groups (p>.05). 5. Troponin Ⅰ has no statistically significant difference with intergroup (p>.05). Also there was no statistically significant difference with each groups (p>.05). Conclusion: These findings suggest that different quantitative exercises has no effect on FMS, LDL-C, HDL-C, WBC, Troponin Ⅰ with ischemic stroke patients. But the treatment period that there's less correlation between the recovery of motor function and the different quantitative exercise, also less correlation between the change of important blood factors and the different quantitative exercises with ischemic stroke patients.

  • The principal objective of this study was to assess the effects of eating habits and health-related lifestyle on blood pressure, $\gamma$-Glutamic acid Peptide Transferase ($\gamma$-GPT), glucose and HDL-Cholesterol (HDL-C). All subjects (261 male, 252 female) were from the Iksan area of Korea, and were at least 50 years of age. The mean systolic blood pressure (SBP), diastolic blood pressure (DBP) and HDL-C for all the subjects was 131.3 mmHg, 78.5 mmHg, and 43.1 mg/dl, respectively. The HDL-C of the $50{\sim}64$ year-old group was higher than that of the over-75-year-old group. The "regular breakfast" group evidenced a lower SBP, $\gamma$-GPT, and higher HDL-C than the "seldom breakfast" group (p<0.001, p<0.001, p<0.01). SBP in the "snacking everyday" group was higher than that of the "seldom snacking" group (p<0.001). As for the frequency of using alcohol, SBP and $\gamma$-GPT for the group using alcohol everyday were higher than those of the non-drinking group (p<0.001, p<0.001), SBP and DBP were higher and $\gamma$-GPT was lower in the group that regularly drank more than 4 glasses of Soju than in the non-drinking group (p<0.001, p<0.05, p<0.001). SBP, DBP, and $\gamma$-GPT for the "heavy smoker" group were higher than those of the non-smoker group (p<0.01, p<0.01, p<0.05). The HDL-C was lower in the "heavy smoker" group than in the "non-smoker" group (p<0.05). The SBP with exercise was as follows: Group 1 ($0.022{\sim}0.073\;kcal/min/kg$) was lower than that of Group 3 ($0.144{\sim}0.161\;kcal/min/kg$) and Group 4 (0.161 kcal/min/kg) (p<0.001). To conclude: advancing age, snacking, and frequent alcohol consumption increased blood pressure; the lowest blood pressure was detected in the group that ate breakfast everyday and in the group that engaged in more frequent exercise; Moreover, $\gamma$-GPT was higher and HDL-C was lower in the smokers' group than in the non-smokers' group. Considering the results of this study, there appears to be an urgent need to instruct aging adults about eating breakfast everyday, reducing smoking, using less or no alcohol, and getting proper and regular exercise.

  • Objective: This study was conducted to investigate the effects of Aconiti Tuber on the plasma IL-6 and $TNF-{\alpha}$ level in mice stimulated by intracerebroventricular(I.C.V.) Injection of Lipopolysaccharide(LPS). Method: 6 mice were assigned to each of the normal group, the control group, and the experimental group. In the normal group only saline was administered intragastrically, and in the control group LPS was injected intracerebro-ventricularly 1 hr after intragastric administration of saline. In the experimental groups (Aconiti Tuber 0.5g/kg, Aconiti Tuber 1.0g/kg, Aconiti Tuber 3.0g/kg) each sample was administered intragastrically to mice 1 hr prior to the stimulation by LPS I.C.V. Injection. To measure the plasma IL-6 and $TNF-{\alpha}$ level of mice, their blood samples were collected from retro-orbital plexus, immediately centrifuged at $4^{\circ}C$, and plasma was removed and stored frozen at $-83^{\circ}C$ for later determination of plasma IL-6 and $TNF-{\alpha}$. Result : 1. LPS I.C.V. Injection increased plasma IL-6 level significantly in a dose-dependent manner compared with normal group(P<0.01). The plasma IL-6 concentration reached a significant maximal level about 1 hr after LPS I.C.V. Injection(P<0.001). LPS I.C.V. Injection increased plasma $TNF-{\alpha}$ level significantly in a dose-dependent manner(P<0.05). The plasma $TNF-{\alpha}$ concentration reached a significant maximal level about 1 hr after LPS I.C.V. Injection(P<0.001). 2. Sample A group to which Aconiti Tuber(0.5g/kg) was administered intragastrically 1 hr prior to the stimulation by LPS I.C.V. Injection showed insignificant lower plasma IL-6 level in 1 hr than control group(P<0.0635), and sample B group (Aconiti Tuber 1.0g/kg) showed significant lower plasma IL-6 level in 1 hr than control group(P〈0.05), and sample C group (Aconiti Tuber 3.0g/kg) showed significant lower IL-6 plasma level in 1 hr than control group(P<0.01). 3. sample A group to which Aconiti Tuber(0.5g/kg) was administered intragastrically 1 hr prior to the stimulation by LPS I.C.V. Injection showed insignificant lower plasma $TNF-{\alpha}$ level in 1 hr than control group(P>0.05), and Both sample B(1.0g/kg) and sample C(3.0g/kg) groups showed significant lower $TNF-{\alpha}$ plasma level in 1 hr than control group(P<0.01). These data revealed that Aconiti Tuber might have the anti inflammatory effect by reducing the plasma IL-6 and $TNF-{\alpha}$ level in a dose dependent manner in mice LPS I.C.V. Injection.

  • The present study explored the influence of temporomandibular disorders(TMD) with anterior open bite on swallowing. Fifteen subjects with anterior open bite(group A), 9 subjects with anterior open bite and TMD(group B), and 24 subjects without malocclusion or TMD symptoms (group C) were included. BioPAK system(Bioresearch Inc., Milwaukee, WI, USA) was used to record the muscle activities of anterior temporal, masseter, sternocleidomastoid(SCM) and anterior digastric muscles during maximum clenching and swallowing. Positional change of the mandible during swallowing was also recorded using the same system. The obtained results were as follows: 1. Group A, B, and C did not show significant differences each other in the muscle activity of resting position in most of head and neck muscles. 2. Group B showed significantly lower muscle activity in maximum clenching than group C in anterior temporal(p<0.01), masseter(p<0.05), SCM(p<0.05) and digastric muscles(p<0.05). 3. Group A showed significantly lower muscle activity during swallowing than group C in anterior temporal and masseter muscles(p<0.01). Group B showed significantly lower muscle activity during swallowing than group C in anterior temporal(p<0.01), masseter(p<0.01), and SCM muscles(p<0.05). 4. Group A and B showed increased positional change of the mandible during swallowing measured from vertical, anteroposterior and lateral aspects, and prolonged swallowing(p<0.05). 5. After given instructions for normal swallowing pattern, group A and B showed increased muscle activity during swallowing in anterior temporal, masseter and SCM muscles(p<0.01). Positional change of the mandible and time elapsed for swallowing also decreased after the instruction(p<0.01). 6. Occlusal splint did not change the muscle activity during swallowing. However, vertical change(p<0.01) and swallowing time(p<0.05) were decreased with splint.

  • This study compared intake of vitamins and antioxidant nutritional status of smokers and nonsmokers, and the effect of supplementation of vitamin C and vitamin E on antioxidant system of smokers and nonsmokers. Subjects were 30 smokers and 30 non-smokers of male university students. They were divided into groups of 10 subjects each one with supplementation for 4 weeks, to investigate the effects of supplementation. Smokers were divided into vitamin C supplement group, vitamin E supplement group and vitamin C and vitamin E combination supplement group, and so were nonsmokers. The supplementation of vitamin C was 500mg and vitamin E was 400IU. There was no significant difference of antioxidant vitamin intakes between smokers and non-smokers, and plasma concentration of vitamin C in smokers was lower than non-smokers (p<0.05). Blood pressure was higher in smokers (p<0.05), with no difference in blood glucose levels, methemoglobin and TBARS, but SOD activity was significantly higher in non-smokers (p<0.001). Vitamin C supplementation resulted in a significant decrease of diastolic blood pressure (p<0.01), systolic blood pressure (p<0.001) and methemoglobin (p<0.001) in smokers. Also a significant decrease of diastolic blood pressure (p<0.05), systolic blood pressure (p<0.05), blood glucose (p<0.05), methemoglobin (p<0.001) and TBARS (p<0.05), with significant increase of SOD activity (p<0.001) were found in vitamin E supplement group. In vitamin C and vitamin E combination supplement group, there was a significant decrease of diastolic blood pressure (p<0.05), methemoglobin (p<0.01) and TBARS (p<0.05). In non-smokers, methemoglobins (p<0.001) of vitamin C supplement group and vitamin E supplement group decreased, and diastolic pressure (p<0.05), methemoglobin (p<0.01) and TBARS (p<0.05) significantly decreased in vitamin C and vitamin E combination supplement group. These results indicated better efficacy of antioxidant supplementation in smokers than in nonsmokers, suggesting that the supplementation of vitamin C and vitamin E might decrease the oxidative stress and various risk factors of smoking-related diseases.
