One case of Second Degree Burn Wound (2도 화상 치험 1례)

  • Burn is a dermal injury mainly caused by heat. It occurs by flame, boiling water or liquid, steam, sparks, chemicals, radiation, and electricity. Burn is classified as four grades depending on how deep into the skin a person is burned. As the deep second-degree burns possibly leave scars, it can have various physical, psychological, and aesthetical effects with seriousness on the patient with a burn. This report is on the 50-year-old female patient who had a burn with boiling water and a mixture condition of superficial and deep second degree burn. The Korean medical treatments such as Herbal acupuncture, Herbal medication, Acupuncture, Herbal-ointment were used for 17 days to treat effectively the patient with a mixture of superficial and deep second degree burn. It is difficult to find out the report that treats a burn with Korean medical treatment. If more clinical treatments as in this report are proved to be effective, we are able to expect that Korean medical treatment should be a good treatment in burn.

Development of New Biodegradable CHITULOSE film from Composite of Chitin and Cellulose (Chitin과 Cellulose 복합체로부터 새로운 생분해성 CHITULOSE film의 개발)

  • A Chitulose film was made by dissolving chitin and cellulose in a dimethylacetamide/LiCl solvent system This film was completely degraded in 20 days in soil and penetrated by soil microorganisms in 16-26 hours. A permeability analysis of the film showed that water permeability was in the range of 0-187.5 $1/m^2$/day, depending upon the ratio of chitin to cellulose and decreased with increasing a cellulose content in the Chitulose film The permeabilities of organic compounds, $Na^+$ and oxygen change with the composition of the Chitulose file The film was tested with a burned 2~3degree rat for exploring its use as artificial skin. The rat was completely cured within 31 days without inflammation.

Effect of Microcurrent Electrostimulation on the Burn of Rats (미세전류 전기자극이 흰쥐의 화상에 미치는 영향)

  • Purpose: This study aims to examine effect of microcurrent electrostimulation on burn healing by electric intensity and of which the electric intensity on the acute burn being cured with microcurrent electrostimulation therapy. Methods: 28 Sprague Dawley Rats is classified into a control group of 8 rats, an experimental group I of 10 rats and an experimental group II of 10 rats. The control group is not cured, the experimental group I is exposed to 10 Hz, and $100{\mu}A$ with microcurrent electrostimulation, and the experimental group II is exposed to 10 Hz, $300{\mu}A$ for 15 minutes a day. The next day, 2th, 4th, and 6th day after rats is burned. Result: There are not significant differences of length change of the burn cure between the control group, the experimental group I, and the experimental group II by a period. However, systematically hair follicle cell on the 2th day and epidermal cell on the 6th day turn up in the experimental group I, and the experimental group II. Inquiry: Nancy(1994) did not obtain the desired result when the skin of a pig is exposed to 0.1 Hz, and $100{\mu}A$ for wound healing. In the result of the study, when burn length is measured on the 2th, 4th, and 6th to see the length change of acute burn, there is not significant differences among 3 groups. Conclusion: Statistically, there is not significant differences of the length change between 3 groups. However, systematically the burn is cured faster in the experimental group I, and the experimental group II than in the contrast group.

The Effect of Jerksukji-san and Tangnisodok-eum on burn (적석지산(赤石脂散)과 탁리소독음(托裏消毒飮)의 화상치료(火傷治療)효과)

  • Objectives : The present study was conducted to determine whether Jerksukji-san and Tangnisodok-eum have an effect on treatment of burn. Methods : 24 male rats were randomly divided into 4 groups. Burns were created by scalding on the skin surface. Immediately after being burned, the rats of group 1 were applied with silver sulfadiazine ointment topically on the burn wound. The rats of group 2 were applied with Jerksukji-san ointment topically and administrated Tangnisodok-eum orally. The rats of group 3 were applied with Jerksukji-san ointment topically. The rats of group 4 were cleaned with saline solution. All treatments were applied twice a day. After 3 and 7 days the inflammatory cellularinfiltration and epidermal gap were measured. Data were analyzed by one-way ANOVA and Kruskal-Wallis test. Results : The rats of groups 1 and 2 showed a statistically narrow epithelial gap compared with those of groups 3 and 4 on the third and on seventh days (p<0.05). Group 3 showed a statistically narrow epithelial gap compared with group 4 on the third and seventh days (p<0.05). The decrease of inflammatory cellularinfiltration observed in group 2 was bigger than in the other groups (group 2>1>3>4) on the third and seventh days. Conclusions : These results demonstrate that Tangnisodok-eum and Jerksukji-san ointment are effective to treat burn, and it is necessary for clinical study to better ascertain this method.

A Study for Farmers to Reduce Work Load on the Different Working Conditions (part II) - Cultivating Welsh Onion in the Summer Ground - (농민의 작업환경별 노동부담경감방안에 관한 연구(II) - 여름철 노지에서 대파재배 작업을 중심으로 -)

  • In this study we tried to give a decision on propriety of working conditions, to present ideas on reducing work loads, and to grope for efficiency of agricultural works. For this we examined the actual working conditions of cultivating welsh onion in the summer ground. And we improved harmful factors that affect farmer's health by considering results of previous study and farmer's subjective sensation. And we measured. compared, and analyzed the farmer's work loads before and after improvement. The results of this study are as follows ; 1. According to examine the actual working conditions of cultivating welsh onion in the summer ground, farmers have experienced physical and mental chronic fatigue on the basis of farmer's appel to eye - fatigue and sun - burned skin on hot working environment including excessive ultraviolet rays, the rough ground condition, inconsistent arrangement of working stand and sorter, heavy - weared habits, and unsuitable working posture. 2. When we improved harmful factors that affect farmer's health, conformed the effects on important work efficiency index such as heart rate, electromyovolume, body temperature, and microclimate inside clothing and work loads were decreased by eliminating the hillock and obstacles of ground, decreasing the clothing weight, using proper clothing appliances such as hat and sunglasses, controlling height of working stand and sorter suitably, improving the working postures and methods as using assistant appliances, alloting the working time and sequence effectively and presenting the light gymnastic exercises and rest for fatigue restoration.

Reconstruction of post-burn anterior neck contractures using a butterfly design free anterolateral thigh perforator flap

  • Anterior neck burns represent a major reconstructive challenge due to severe sequalae including restriction in movement and poor aesthetic outcomes. Common treatment options include skin grafting with/without dermal matrices, and loco-regional and distant free flap transfers with/without prior tissue expansion. Such variation in technique is largely influenced by the extent of burn injury requiring resurfacing. In order to optimize like-for-like reconstruction of the anterior neck, use of wide, thin and long flaps such as the anterolateral thigh (ALT) perforator flap have been reported with promising results. Of note, some patients have a tendency towards severe scar contractures, which may be contributed by the greater extent of inflammation during wound healing. We report our experience at 4 years' followup after secondary reconstruction of severe, anterior neck burn contractures in two patients by harvesting the ALT flap with a butterfly design. This technique provides adequate wound resurfacing of the burned neck and surrounding areas, and provides good neck extensibility by addressing both anterior and lateral aspects of the scar defect simultaneously. Such a flap design reduces tension on wound edges and thus, the risk of contracture recurrence in what remains a particularly challenging type of burn reconstruction.

An Outer Membrane Protein Preparation as a Vaccine against Pseudomonas aeruginosa Infection

  • We developed a simple and efficient method to prepare a Pseudomonas vaccine of outer membrane (OM) proteins free from lipopolysaccharide (LPS). A three step purification process including extraction, ultrafiltration and ultracentrifugation effectively removed LPS from the OM protein fraction. Approximately 2 mg of the OM proteins was obtained from 1 g of wet cell. LPS contaminant in the vaccine preparation was less than 0.003% (w/w) of protein and protease activity was not detectable. To achieve a wide range of protection, OM proteins prepared from four attenuated P. aeruginosa strains were mixed in equal amounts and used as a vaccine, which elicited in rabbits a high titer of antibody reactive to all of the seven Fisher types. The antisera from the immunized rabbit had a strong reactivity to vaccine proteins larger than 25 kDa. In a burned mouse infection model, immunization with the vaccine significantly enhanced bacterial clearance in the Pseudomonas infected skin. The vaccination also provided mice an excellent protection against Pseudomonas infection (11, 16). Data on antigenicity, mutagenicity, acute, subacute toxicity and pharmacological tests confirmed the safety of the vaccine (1, 3, 10, 12, 17). These data demonstrate that this method can be applied to manufacture a bacterial vaccine of OM proteins with safety and prophylactic efficacy at a practical low cost.

Therapeutic Effect of Hydrocolloid Membrane Containing Liriope platyphylla Extracts on the Burn Wounds of SD Rats (맥문동 혼합 하이드로콜로이드막의 제조 및 화상치료 효능평가)

  • A variety of previous pharmacological studies have suggested Liriope platyphylla (L. platyphylla) may exert beneficial biological effects on inflammation, diabetes, neurodegenerative disorder, obesity, constipation, and atopic dermatitis. In addition, hydrocolloid membranes (HCMs) have attracted attention in dermatological care, including in the treatment of scleroderma skin ulcers, cutaneous ulcers, permanent tympanic membrane perforations, pressure sores, and decubitus ulcers in the elderly. To investigate the therapeutic effects of HCM containing an aqueous extract of L. platyphylla (HCM-LP) on second-degree burn wounds, their physico-chemical properties were analyzed and the therapeutic effects were observed in SD rats after treatment with HCM-LP for 14 days. Significant declines in tensile strength (38.4%) and absorptiveness (46.3%), as well as an increase in surface roughness (38.1%) were detected in HCM-LP compared with that of HCM. In SD rats with burned skin, the wound diameter was shorter in the HCM-LP treated group than in the GZ group on post-surgical day 14, while the significant improvements in scar tissue reduction, epithelium regeneration, angiogenesis, and extracellular matrix deposition were observed in the HCM-LP-treated group during all experimental periods. Overall, these results suggest HCM-LP may accelerate the process of healing the burn injury skin of SD rats through the regulation of angiogenesis and connective tissue formation.

The Role of Protein Kinase C in the Cardiac Injury Induced by Skin Burn (피부화상으로 유도된 심근손상에서 Protein Kinase C의 역할)

  • The aim of the present study was to assess the role of protein kinase C (PKC) in the development of cardiac injury following scald burn. Sprague-Dawley rats were induced a scald burn a 15% total body surface area. Phorbol 12-myristate 13-acetate (PMA, 2 mg/kg) and bisindolylmaleimide (BIS, 0.05 mg/kg) were immediately administered i.p. after burn injury. 5 h and 24 h later, heart was removed and examined biochemical assay, ultrastructural changes and stereological analysis. The activity of serum aspartate aminotransferase was significantly increased at 5h (p<0.01) and 5h+BIS (p<0.001) after burn compared with that of control. The activity of serum creatinine was significantly decreased in PMA-treated groups after burn compared with postburn 5 h. PMA caused a decrease in MPO activity and induced wavy fibers in cardiac myocytes at postburn 5 and 24h. BIS induced contraction band, separation of intercalated disk and abnormal mitochondria in cardiac myocytes at postburn 5 and 24h. In stereological analysis, treatment of rats with PMA increased volume density of myofibril and mitochondria compared with postburn 5 and 24h. Our data suggest that the activation of PKC in scald burned heart decreases inflammation and protects the myocardium.

  • In Chinese medicine, the activities of removing and solving the inner cause are called the regimen in preventing the disease and the good condition of mind emphasizes us to practice ourselves the doctrine of regimen of the mind to promote the health and long life of the living body, in other words, it means we should have clean mind, less desire, less thought, less agony and widely open our emotion and will, and it also means the raising of good nature, the moral cultivation, and we can be healthy when we live by good virtus as the root of regiem. The meaning of the Qi has been expanded more and more since its origination throught the process of practice and perception of man. 1) Matter is changed to the smoke when burned, the energy of water becomes the rain in the sky and the rain raises all living creatures by making them wet. Throught these changes of circulation, men could realize the energy to be the common and original matter of forming all the creatures. 2) The direction of showing the breathing of men in and out has been expended. 3) It was widely understood as the meaning of showing vitality of men, and it was the original root of chinese medicine. 4) It was expended to be showing spirit of nature such as the sun, moon, star, sky, land, mountain and the moral spirit of men as peacefullness. By the original meaning of the word of energy expanded to the wider side of sky, land, men and things, nature, society, man and the moral spirit could get the unified basis of the matter. As the above, the word of Qi has been used in wider meaning at this time from the past. In other words, all things in the universe come into being and extinct by the chang of motion of the Qi and it is recognized to be the living activity in human body. The Qi-kong based on this energy and the motion applied to our daily life are very extensive (Lee, Hye - lung, 1997). Here are the summaries of the effect of the Qi-kong ; 1) The physical constitution is strenthened, physiological function is prosperous and we are free from various geriatric disease and psychosomatic disease not to mention the cold and indigestion. 2) It maintains the clear spirit, elevates intelligence, strengthens the spiritual power and demonstrates the potentiality at the amximum. 3) It maintain beautiful figure and clean skin never losing the charm but full of energy and vitality. 4) It keeps the balanced body never suffering from the unbalance of the bones, various and neuralgia. 5) It maintain the spiritual comfort, the natural posture in everything and real happiness (Lee, Hye-Jung, 1997 : Suh, Yong Kyu, 1989). Chinese medicine mentions the motion by Qi with Qi - kong. But the motion does not require special method in Western medicine. I t is the only way of living of our ancestor indaily life. It is maintaining the healthy lifr by training the Qi, and it is the motion of being with the nature with the open mind in breathing rather than artificially restricting our daily life, it is the motion of the body.
