Dental needle foreign body in the neck: a case report

  • Foreign body (FB) ingestion is commonly seen in the ear nose and throat (ENT) field, with different presentations and sequelae. FBs can arrest in the upper aerodigestive tract or continue further down into either the airway tract to the bronchus or the digestive tract to the intestines. The pathway of an FB depends on the size and shape of the FB and how sharp its edges are. Since the 20th century, the use of disposable stainless-steel needles in the oral cavity has proven to be an effective and safe method for performing various intraoral procedures like dental infiltration or a root canal wash. Complications from their use are rare. Generally, dental needle breakages are caused by patients biting the needle, incorrect injection techniques, or inadequate preventative measures. The sudden movement of a patient during a procedure is one of the most common causes of breakage. Occasionally, needles are swallowed during dental procedures such as a root canal. Here, we report a case of a patient that swallowed a broken needle during a dental procedure. A few days later, the patient presented with neck pain, swelling, and a FB sensation. When the patient presented, she claimed that her symptoms had onset after consuming a meal containing duck meat. Initially, the patient was diagnosed as having ingested a duck bone. However, intraoperatively, the FB was discovered to be an injection needle that had migrated from the throat to the neck.

MALT Lymphoma of Ocular Adnexa: A Case Report (안구 부속기의 점막연관 림프조직형 림프종의 증례보고)

  • Purpose: Lymphoma originated from mucosa associated lymphoid tissue(MALT) is most common in gastrointestinal system, and rarely found in salivary gland, thyroid, bronchus or orbit. We experienced a case of MALT lymphoma which was originated from conjunctiva and involving lower eyelid without metastasis. Methods: A 40-year-old man suffered palpable mass on right lower eyelid without pain. Orbital computed tomographic and ultrasonographic findings showed a conical mass($1.9{\times}1.2{\times}0.9cm$ size) inside lower eyelid. The mass was completely excised under local anesthesia and histopathological examination was followed. Results: Microscopic finding showed a multiple follicular colonization. In the follicle, small lymphocytes and plasma cells differentiated to centrocyte-like cell, monocyte B cell, plasma cell were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. After excision, SPECT, abdominal CT was carried out and there were no evidence of extraorbital disease. Conclusion: Biopsy and pathological examination should be performed in patients who complain palpable mass on lower eyelid because of possibility of MALT lymphoma. Although MALT lymphoma is rarely metastasized, it is necessary to evaluate the extraorbital involvement using SPECT or other radiologic exams. For detecting extraorbital involvement, periodic follow-up examination is need.

  • Postpneumonectomy syndrome is a rare and delayed complication of left pneumonectomy in most patients with normal mediastinal vascular anatomy. This syndrome is characterized by dyspnea and recurrent pulmonary infection in the remaining right lung that typically occur within the first postoperative year. The condition is believed to be secondary to postsurgical changes that include a marked shift of the mediastinum to the left, clockwise rotation of the heart and great vessels, and herniation of the right lung into the left anterior thorax. These changes lead to compression of the trachea or right main bronchus among the thoracic spine and the right pulmonary artery. We report a case of postpneumonectomy syndrome in 15 year-old girl that followed by left pneumonectomy for bronchiectasis 6 years ago. We have inserted an expandable prosthesis in the left thoracic cavity posterior to the heart. After implantation of an expandable prosthesis, an anatomic reposition of the shifted mediastinum was achieved, which resulted in instantaneous and sustained relief. The post-operative course was uneventful and the patient was followed in OPD from after discharge to now.

Photodynamic Therapy for Neoplasms in Intrathoracic Constructed Stomach (식도암 수술 후 흉곽 내 위장에 발생한 원발성 위암에 대한 광역동치료 -1예 보고 -)

  • Photodynamic therapy (PDT) is a local, endoscopically controlled nonoperative therapeutic technique based on selective sensitization of mucosal, malignant and precancerous lesions of the esophagus, trachea and bronchus prior to light-induced tissue destruction in the department of thoracic and cardiovascular surgery. PDT is effective and safe for palliative treatment of neoplasms in the stomach, esophagus, and lung. But skin phototoxicity is unsatisfactory, therefore optimization of management of post-PDT is necessary for preventing phototoxic side effects of skin. Careful patient education in photoprotection techniques, close patient follow-up, early dermatologic referral and medical treatment are recommended. We performed PDT in a patient with intrathoracic constructed stomach. We report this case with a brief review of literatures, therefore.

Clinical Value of Bronchoscopic Examination in the Diagnosis of Cancer of the Lung (기관지경검사의 폐암진단에 관한 임상적 의의)

  • Two-hundred and eighteen patients were taken bronchoscopy to evaluate the value of bronchoscopic diagnosis in the lung cancer patient at the Department of Thoracic Surgery of the Kyung-pook University Hospital. Ninety-eight eases of these patients were studied for possible lung cancer by bronchoscopic examination, biopsy and bronchial cytologv. The results were obtained as follows: Two-hundred and eighteen patients were taken bronchoscopy to evaluate the value of bronchoscopic diagnosis in the lung cancer patient at the Department of Thoracic Surgery of the Kyung-pook University Hospital. Ninety-eight eases of these patients were studied for possible lung cancer by bronchoscopic examination, biopsy and bronchial cytologv. The results were obtained as follows: 1) Forty-five (55.5%) of 81 cases impressed as definitive, and six (35. 3%) of 17 cases impressed as probable lung cancer were subsequently proved to have lung cancer. This represents a total of 51 cases (52. 0%) of 98 patients. 2) Twenty-six (51.0%) of 51 patients were proved by bronchoscopic biopsy and cytology. 3) The positive rates of bronchoscopic biopsy were higher in the lesions on tile intermediate bronchus, carina and main stem bronchi by bronchoscopy, and also in the hilar than in peripheral lesions by X-ray finding and in histologically undifferentiated cell carcinoma than in adenocarcinoma. 4) The positive diagnosis rates of bronchoscopic aspirate were not correlated to the locations of cancer lesions. 5) Thirty- nine percent of 51 case3 were visualized definitive tumor mass and obstructive lesions under bronchoscopy. 6) Patient of lung cancer varied in age from 27 to 77 years, with highest concentration lying in the 5th decade (43.0%). Forty-seven patients were men and 4 were women giving a proportion of men to women, 12: 1 7) Sixty-five percent of proved lung cancer patients were diagnosed as operable cases. Fifty-three percent of explored patients were resected. These results conclude that bronchoscopy is of a considerable value as a diagnostic procedure in these lung cancer patients.

Preparation of In Vivo Rat Lung Model for Ischemia-Reperfusion Injury (허혈 재관류 손상 실험의 쥐 생체 모델 작성)

  • Ischemia reperfusion injury occurs in various diseases. The role of oxygen free radicals in IR injury of the lung has been spotlighted and many studies have been performed. In this study, we tried to prepare a stable rat lung model for IR injury, focusing on surrounding conditions as hilar stripped left lung, clamped left pulmonary artery and bronchus,and declamped after determined period was passed, and right main pulmonary aretery was clamped. Arterial blood gas analyes were performed at 1, 10, 20, 30, minutes after reperfusion. Before clamping, PaO2 was 95 to 120 mmHg in all animals. There were six groups; Group I : temperature 15o C, and 120 minutes clamping, Group II: 20 oC, and 120 minutes clamping, Group III : 25 oC, and 120 minutes clamping, Group IV : 15oC, 90 minutes clamping, Group V : 20 oC, 90 minutes clamping,Group VI: 20 oC, 75 minutes clamping. Each groups contained 10 Sprague Dayley rats. The humidity was maintained 100 % as circulation imerged isotonic Hartmann`s solution of the pleural cavity. In group IV, V, and VI, PaO2 decreased significantly in all animals immediately after reperfusion, but 43 % survived till 10 minutes after reperfusion, it was 74.0$\pm$5.7, 73.3$\pm$10.8,and 88.2$\pm$17.7 mmHg. Pulmonary edema was observed histologically in 2/10 animals in group IV, 6/10 in group V , 3/10 in group VI, 9/10 in group I, and the other lungs showed all edema. We established a stable model by setting ischemic time,and temperature, between 75 to 90 minutes,15 to 20o C, and isotemperature Hartmann`s solution immersion of the pleural cavity.

A Case of Hodgkin's Disease as Endobronchial Presentation (기관지내 종양으로 발견된 호지킨병 1예)

  • Although mediastinal involvement by Hodgkin's disease is frequent, the initial presentation of the lymphoma by an endobronchial lesion is rare. A 23-year-old man was admitted with progressive dyspnea. Initial chest roentgenogram showed atelectasis of right middle and lower lobe. Fiberoptic bronchoscopy showed a polypoid mass obstructing the right mainstem bronchus. The mass lesion was histologically confirmed as Hodgkin's disease, nodular sclerosis type. He received systemic chemotherapy and improved rapidly.

  • Chronic cough is one of the most common respiratory symptoms, especially in children. And it can be the sale presenting manifestation of bronchial asthma. Although most coughs are self limiting, chronic cough often proves to be a frustrating problem. It is commonly defined as a persistent or recurrent cough exceeding 3weeks duration. The post nasal syndrome has been determined to be the most common cause of chronic cough, followed by asthma, chronic bronchitis, gastroesophageal reflux and bronchiectasis. This study was performed at both City-Oriental Medicine Hospital and Pundang Cha Oriental Medicine Hospital from January,1,1998 to November 31,1998, and 114 children with chronic cough persisting for longer than 3 weeks were evaluated. We investigated the clinical findings and evaluated the etiology in children with chronic cough syndrome including: type of cough (with or with out sputum and daily onset) and associated signs & symptoms. The results were as follows: The most common cause of chronic cough was asthma with sinusitis (27.2%); The second and third were post nasal drip syndrome(22.8%) and bronchial hypereactivitic cough(14.9%). The other causes included asthma, paranasal sinusitis, bronchitis and rhinitis. Therefore, in the diagnostic and therapeutic approach to this symptom, it should be considered that the cause of this type of cough is either bronchus and pulmonary disease or that associated with another problem, especially sinusitis, gastroesophageal reflux and allergic disease. Also, in infants and toddlers, congenital abnormaly should be considered.

Primary Tracheal Tumor C (원발성 기관 종양)

  • Background: Tumors of the trachea are rare despite their histologic similarity to tumors of the main stem bronchus and lung. Materials and methods: Fourteen patients with tracheal tumor underwent surgical, radiational, or laser photocoagulation therapy from March 1981 to July 1996. Nine patients were malignant and five patients were benign. The most common malignant tumor was adenoid cystic carcinoma. Results: Age ranged from 10 to 65 years with mean age of 45.9 years. Most tumors were located middle and lower one-third of trachea. Surgery was done through collar incision, or collar incision with vertical partial sternal division, or left posterolateral thoracotomy, or sternal division with laryngeal release. Two patients died after operation, because of the disruption of anastomosis and airway obstruction,and laryngeal edema after suprahyoid release. Only one patient died after 8 month of diagnosis. The other patients were doing well during the follow-up period.

Surgical Treatment of Bronchial Restenosis Occuring After Insertion of Self-Expandable Metalic Stent in Patients with Bronchial Stenosis -2 Cases Reports- (기관지협착환자에서 기관지내 팽창성 급속 스텐트 삽입후 재발한 기관지협착 치험 2례)

  • Since the insertion of self expandable metalic stent[SEMS has became popular method for hollow organ stenosis, many attempts for further apply the stent to airway stenosis as an simple procedure has been made, but intrabronchial migration of stent or occurrence of inflammatory granuloma around stent develop occasionally and sometimes it worsen bronchial stenosis further more. This report describes 2 case of surgically treated bronchial restenosis in whom intrabronchial stent were applied for release of bronchial stenosis. Our surgical option was pneumonectomy and bronchoplasty with sleeve right middle and upper lobectomy respectively. During the operation we found the SEMSs were tightly impacted in restenotic bronchial lumen with overgrowth of granulation tissues. The bronchial obstructions occupied more than 90% of lumens in both cases, and needed much complicated procedure to be relieved. Therefore, even though the insertion of SEMS remains as a prcedure determined by the physician`s preference, it has to be considered prudently that the use of SEMS can cause severe restenosis and the surgeon has more difficulties in performing segmental resection of restenotic bronchus in patient with SEMS previously inserted. Throughout these experiences we can conclude that the insertion of SEMS must be performed only in very selected cases of bronchial stenosis.
