A Case of Primary Endobronchial Leiomyoma Diagnosed During Treatment of Pneumonia (폐렴치료 중 발견된 원발성 기관지내 평활근종 1예)

  • A 44 year-old nonsmoker female presented to our hospital with persistent cough with sputum for three months. She had been diagnosed as pneumonia and treated with antibiotics in a local hospital but was transferred to our hospital because of no symptomatic improvement. We performed flexible bronchoscopy and found an ovoid and smooth surfaced mass obstructing the bronchus of right middle lobe. The biopsy specimen revealed a benign tumor composed of spindle cells without dysplasia consistent with leiomyoma. There was no evidence of leiomyoma in uterus. The right middle lobectomy and bronchoplasty were performed and the patient was healthy twelve months later. Primary endobronchial leiomyoma is a rare benign tumor of the lung. Herein we report a case of primary endobronchial leiomyoma diagnosed during treatment of pneumonia.

Surgical Treatment for Primary Pulmonary Paraganglioma - A case report - (폐에 발생한 원발성 부신경절종의 수술치험 - 1예 보고 -)

  • We describe the case of primary pulmonary paraganglioma in a 37-year-old woman who presented recurrent, severe cough. Computed tomography revealed a lobulated inhomogeneous enhanced mass with endobronchial protruding lesion suspected to be lung neoplasm, located in the upper lobe of the left lung. Bronchoscopic biopsy showed chronic inflammation with granulation tissue which was not in accord with the radiologic findings. Subsequently, a left lower sleeve lobectomy was peformed. Histological analysis of the resected tumor proved to be compatible with pulmonary paraganglioma. Primary pulmonary paragangliomas are very uncommon tumors. So we report this case with literature review.

  • Endobronchial leiomyoma is extremely rare and accounts for less than 2% of benign tumors of the lower respiratory tract. Leiomyomas are predominantly found in the young and the middle aged : of the average age being 35 years for bronchial and lung parenchymal lesions and 40.6 years for tracheal lesions. The symptom depends on the location of the tumor, its size, and changes in the lung distal to the lesion. A 37-year-old woman was admitted to our hospital complaining of coughing. Bronchoscopy revealed complete obstruction of the right main bonchus at the carina by an oval-shaped, nonulcerative, smooth, and pinkish-tan tumor with a broad margin and extended to the left main bronchus. A biopsy was performed and showed a benign spindle cell tumor. A right pneumonectomy was performed because of chronic infection, and the lung could not expanded during aeration. The histological diagnosis of the resected specimen was leiomyoma. The postoperative course was uneventful.

Videothoracoscopic Treatment of Spontaneous Pneumothorax - A Prospective Study of 30 Patients - (비디오 흉강경을 이용한 자연기흉의 수술치료)

  • 30 patients with spontaneous pneumothorax underwent videothoracoscopic treatment between March and July 1992. The patients ranged in age from 16 years to 62 years (mean age, 30.4 years) and the incidence according to age group was highest as 50 % in the adolescence between 21 and 30 years old. The indications of the therapeutic videothoracoscopy for spontaneous pneumothorax were recurrence (30.8%), persistent air leak (30.8%), visible blebs on the chest X-ray (20.4%), tension pneumothorax (15.4%), and bilaterality (2.6%). Intraoperative scopic findings were as follows; blebs (87.1%), pleural adhesion (45.2%), and pleural effusion (22.6%). The operation was performed under general anesthesia with one lung intubation guided by flexible fiberoptic bronchoscopy. Procedures included bleb and/or wedge resection, tetracycline pleurodesis with mechanical abrasion, and parietal pleurectomy. Successful treatment was obtained in 66.7% (20/30) and the mean postoperative hospital stay of the successful cases was 5 days. Videothoracoscopy also provided the benefits of lesser postoperative pain, rapid recovery, short hospitalization, and smaller scar of wound by reduced trauma on access. The total 13 postoperative complications were occured in 10 patients, which showed somewhat higher rate than that of other reports because of lack of experiences in the earlier period, however, it had decreased progressively as experiences were accumulated and instruments were improved in the later period. The operative mortality was absent. Conclusively, videothoracoscopy is a new interesting modality of surgical treatment of spontaneous pneumothorax and also can be extensively applicable in the diagnosis and treatment of other thoracic surgery.

  • A 42-year-old female was admitted to our hospital complaining of a dyspnea. Chest X-ray showed left atelectasis. A mass was detected in left main bronchus by computed tomography and bronchoscopy. The mass was diagnosed as a endobronchial leiomyoma by biopsy exam. After open thoracotomy and bronchotomy, mass removal was done and middle lobe was ventilated normally. Aberrant arterial supply from descending aorta to left lower lobe of the lung was detected and left lower lobectomy was done. The lower lobe of the left lung was pathologically diagnosed as intralobar pulmonary sequestration. Herein we report a rare coexistent case of endobronchial leiomyoma and intralobar pulmonary sequestration.

  • The finding of a tracheal penetrating injury that's caused by a foreign body is rare in adulthood. A 42-year-old man had experienced penetrating trauma due to a glass fragment 10 years ago. He presented with blood tinged sputum and dyspnea on exertion, and this had developed 1 year previously. Chest CT scan and bronchoscopy revealed a foreign body crossing the tracheal lumen and the object arose from outside of the trachea; this was all associated with airway edema. We removed the foreign body, which was a 5cm length of glass fragment, and we repaired the tracheal defect using a simple primary suture. The postoperative course of the patient was uneventful and he is now being followed up at the outpatient department; he has had no additional symptoms.

Clinical Manifestations of Spontaneous Pneumomediastinum

  • Background: Spontaneous pneumomediastinum (SPM) is an uncommon disorder with only a few reported clinical studies. The goals of this study were to investigate the clinical manifestations and the natural course of S PM, as w ell as examine the current available treatment options for SPM. Methods: We retrospectively reviewed 91 patients diagnosed with SPM between January 2008 and June 2015. Results: The mean age of the patients was $22.7{\pm}13.2years$, and 67 (73.6%) were male. Chest pain (58, 37.2%) was the predominant symptom. The most frequent precipitating factor before developing SPM was a cough (15.4%), but the majority of patients (51, 56.0%) had no precipitating factors. Chest X-ray was diagnostic in 44 patients (48.4%), and chest computed tomography (CT) showed mediastinal air in all cases. Esophagography (10, 11.0%), esophagoduodenoscopy (1, 1.1%), and bronchoscopy (5, 5.5%) were performed selectively due to clinical suspicion, but no abnormal findings that implicated organ injury were documented. Twelve patients (13.2%) were discharged after a visit to the emergency room, and the others were admitted and received conservative treatment. The mean length of hospital stay was $3.0{\pm}1.6days$. There were no complications related to SPM except for recurrence in 2 patients (2.2%). Conclusion: SPM responds well to conservative treatment and follows a benign natural course. Hospitalization and aggressive treatment can be performed in selective cases.

A Case of Leiomyoma of the trachea (기관에 발생한 평활근종 1예)

  • Leiomyoma of the trachea is a very rare tumor, and the tracheal tumor is frequently misdiagnosed and treated as bronchial asthma. We report here a case of leiomyoma of the trachea which was successfully resected. A 40-year-old woman has sufferred from a repeated episode of asthmatic attack for two years. Intensive therapy for asthma had no beneficial effect on her respiratory symptoms, and wheezing and stridor did not disappear. Chest CT and bronchoscopy revealed a pedunculated mass on the trachea just above the carina. Wedge resection of the trachea and right main stem bronchus including the tumor and end-to-end anastomosis was performed. The result of the pathologic examination of the tumor was tracheal leiomyoma. Her postoperative course was uneventful and postoperative bronchoscopic findings showed clear healing of the anastomosis site.

  • Bacillus Calmette-Guerin(BCG) has been widely used for the prophylaxis of superficial bladder tumor recurrence and for the treatment of bladder carcinoma in situ. More than 95% of patients who receive BCG instillation tolerate the treatment well and side reactions have been reported in less than 5% of patients. Most side effects are minor and self-limiting. However, a rare occurrence of severe systemic reactions have been reported. Among the severe systemic reactions, hypersensitivity pneumonitis should be considered in patients with pneumonic complications after BCG instillation in cases where the culture for mycobacteria is negative in the sputwn, brochoalveolar lavage and blood specimen. In addition, a fiberoptic bronchoscopy with transbronchial lung biopsy demonstrates a fibrosis of the alveolar septums, where there is and an increased lymphocyte count with out tuberculous inflammatory changes, the and CD4 : CD8 ratio is increased and no symptomatic response to antituberculosis chemotherapy is observed. Here we report a 68 years old man with interstitial pneumonitis following intravesical BCG instillation.

Clinical Utility of CT-Based Bronchial Aspirate TB-PCR for the Rapid Diagnosis of Pleural Tuberculosis

  • Background: Thoracoscopic pleural biopsy is often required for rapid and confirmative diagnosis in patients with suspected pleural tuberculosis (PL-TB). However, this method is more invasive and costly than its alternatives. Therefore, we evaluated the clinical utility of the chest computed tomography (CT)-based bronchial aspirate (BA) TB-polymerase chain reaction (PCR) test in such patients. Methods: Bronchoscopic evaluation was performed in 54 patients with presumptive PL-TB through diagnostic thoracentesis but without a positive result of sputum acid-fast bacilli (AFB) smear, pleural fluid AFB smear, or pleural fluid TB-PCR test. Diagnostic yields of BA were evaluated according to the characteristics of parenchymal lesions on chest CT. Results: Chest radiograph and CT revealed parenchymal lesions in 25 (46%) and 40 (74%) of 54 patients, respectively. In cases with an absence of parenchymal lesions on chest CT, the bronchoscopic approach had no diagnostic benefit. BA TB-PCR test was positive in 21 out of 22 (95%) patients with early-positive results. Among BA results from 20 (37%) patients with patchy consolidative CT findings, eight (40%) were AFB smear-positive, 18 (90%) were TB-PCR-positive, and 19 (95%) were culture-positive. Conclusion: The BA TB-PCR test seems to be a satisfactory diagnostic modality in patients with suspected PL-TB and patchy consolidative CT findings. For rapid and confirmative diagnosis in these patients, the bronchoscopic approach with TB-PCR may be preferable to the thoracoscopy.
