천혜의 항생제: 항균펩티드 (Natural Antibiotics: Antimicrobial Peptides)

  • Antimicrobial Peptides are natural antibiotics evolved by many plants, invertebrate, and vertebrate to defend against the microbial infection. Antimicrobial peptides show a broad-spectrum antimicrobial activity with little opportunity for the development of resistance since they target microbial membranes that distinguish microbes from enkaryotic cells. The oral cavity is constantly exposed to microbial challenges and antimicrobial peptides play an important role in managing the oral health. With the increase of resistant micro-organisms to conventional antibiotics, antimicrobial peptides are attracting interests as novel antibiotics. In this review, the characteristics of antimicrobial of antimicrobial peptides including the classification, mechanism of action, resistance, and expression in the oral cavity have been discussed in the prospects of application to oral disease.

Trends in the use of antibiotics among Korean children

  • Inappropriate antibiotic use is the most important factor causing increased bacterial resistance to antibiotics, thus affecting patient outcomes. Multidrug-resistant bacteria have become a serious public health threat, causing significant morbidity and mortality worldwide. In Korea, the burden of antibiotic-resistant bacteria has become an important public health issue. There is increasing evidence of overuse and misuse of antibiotics in Korea, as observed in cohorts with large sample sizes. Antibiotic use among children should receive particular attention because of the frequency of community-associated infections among this population and the elevated risk of transmission. Recent studies from Korea have demonstrated that the use of broad-spectrum antibiotics, either for inpatient or outpatient treatment, has increased among many age groups, especially children. In this review, we aim to describe the patterns of antibiotic prescription and evaluate recent trends in antibiotic use among children. Coordinated efforts toward communication and education in order to address misunderstandings regarding antibiotic use, involving interprofessional antimicrobial stewardship programs, are required in the near future.

Pseudomonas aeruginosa KGM-100이 생산하는 항생물질의 특성 및 구조 (Characterization and Structural Dtercination of an Antifungal Compound Produced by Pseudomonas aeruginosa KGM-100)

  • During the screening of antifungal antibiotics from microbial metabolites, we selected Pseudomonas aeruginosa KGM-100 showing powerful antagonistic activity against various phytopathogenic fungi. Antibiotics KGM-100A and KGM-100B were purified from the culture broth of Pseudomonas aeruginosa KGM-100 by diaion HP-20 column chromatography, ethyl acetate extraction, silica gel column chromatography, preparative TLC and recrystallization. KGM-100A which was recrystallized in MeOH showed antimicrobial activities against a broad spectrum of fungi and bacteria. Physico-chemical properties of KGM-100A were determined and identified to be phenazine-l-carboxylic acid by UV, IR, $^{1}$H-NMR, $^{13}$C-NMR, mass spectrum, and elemental analyses.

Removal of a synthetic broad-spectrum antimicrobial agent, triclosan, in wastewater treatment systems: A short review

  • Contaminants of emerging concern (CECs) including endocrine disrupting compounds (EDCs) and pharmaceuticals and personal care product chemicals (PPCPs) have recently received more attention because of their occurrence in water bodies and harmful impacts on human health and aquatic organisms. Triclosan is widely used as a synthetic broad-spectrum antimicrobial agent due to its antimicrobial efficacy. However, triclosan detected in aquatic environment has been recently considered as one of CECs, because of the potential for endocrine disruption, the formation of toxic by-products and the development of cross-resistance to antibiotics in aquatic environment. This comprehensive review focuses on the regulations, toxicology, fate and transport, occurrence and removal efficiency of triclosan. Overall, this review aims to provide better understanding of triclosan and insight into application of biological treatment process as an efficient method for triclosan removal.

  • 3-[(1-Methyl-1H-tetrazol-5-y)thiomethyl] is one of the most important side chains in cephaloporins. This side chain which occures in numerous antibacterial agents, suc as cefamandole, latamoxef, cefoperazone, and cefmenoxime, contributes remarkable potency and broad spectrum. For the continuous work of study on the development of broad spectrum cephalosporins, we become interested in the effect of structural modification of the substituent at the N-1 position of mercaptoterazole toward biological activity because our recent work had demonstraated that the arylsubstituted mercaptotetrazoles (Fig. 1) had favorable effect on the activity against gram-positive bacteria. The main focus of this report was to investigate the relationship between activity and functional groups in the mercaptotetrazole.

Establishment of an Axenic Culture of Microcystin-Producing Microcystis aeruginosa Isolated from a Korean Reservoir

  • In order to establish an axenic (bacteria-free) culture of Microcystis aeruginosa NIER 10039 isolated from a Korean reservoir, the culture was subjected to sequential treatment, including ultrasonication, washing, and addition of antibiotics. Three broad-spectrum antibiotics, namely, kanamycin, ampicillin, and imipenem, were applied separately in that order. Axenicity of the culture was confirmed by cultivation on bacterial media and observation under epifluorescence and scanning electron microscopes. We are the first to establish an axenic culture of a Microcystis strain isolated from Korean reservoirs and can be used in physiological and molecular studies to control toxic Microcystis blooms.

Clostridium sp.가 생산하는 항생물질 KG-1167Adhk KG-1167B의 최적 생산조건 (The Optimum Culture Conditions for the Production of Antibiotics KG-1167A & KG-1167B Produced by Clostridium sp.)

  • 본 실험에서는 Clostridium sp. KH-1167의 항생물질 생산을 위한 최적 배양조건에 대하여 고찰하였다. PY 배지를 기초배지로 사용하였을 때 탄소원으로는 1의 glucose가 가장 효과적이었으며, 질소원으로는 casamino acid가 비교적 양호한 것으로 나타났다. Salt에 있어서는 여러 salt를 복합적으로 첨가하였을 때 활성이 최대로 나타났다. 배지초기 pH 및 배양온도에 대한 최적 조건은 각각 pH5와 $30^{\circ}C$로 나타났으며 108시간 배양하였을 때 항생물질의 생산이 최대로 나타났다. 이러한 배양조건 최적화 실험결과 KH-1167의 항생물질 생산능력은 처음 분리에 사용되었던 배지에서 보다 450정도 증가되었다.

Uterine Prolapse in a Korean Black Goat

  • A postpartum total uterine prolapse in a 2-years old Korean black goat was presented. This paper reports the clinical management of the case. The everted uterus was carefully assessed and gross debris were gently removed and disinfected with dilute povidone iodine solution. The prolapsed uterus was replaced with epidural anesthesia and retention suture was placed on the vulva. Broad spectrum antibiotics, calcium gluconate, and supportive therapy were administered. The patient made complete recovery.

심실중격결손증 수술후 발생한 Aspergillus 심내막염 (Aspergillus Endocarditis after Open Herat Surgery (VSD Closure))

  • A 15-year-old boy having a small VSD was readmitted with clinical manifestations of acute endocarditis and aortic regurgitation one month after open heart surgery. In spite of vigorous treatment with broad-spectrum antibiotics, high fever persisted. Pseudomonas aeruginosa was isolated just one time among several blood cultures. Progressive pulmonary infarction due to embolization. Progressive congestive heart failure and D.I.C. caused patient`s death. Aspergillus was found in autopsy specimen.

의약분업 이후 의원 및 약국에서의 항생제 사용 실태 (Patterns of Antibiotic Usage in Clinics and Pharmacy after Separation of Dispensary from Medical Practice)

  • This study aimed to investigate the prescription of antibiotics in clinics, and evaluate their usage appropriateness after the 2000 Korean separation of dispensary from medical practice. A retrospective study was performed on the antibiotic use for 4 years from August 2000 to July 2004 in three clinics (general, internal medicine and ear-nose-and-throat (ENT) clinics). Moreover, prescription of antibiotics for acute upper respiratory infection (AURI), concomitant drugs, duplicate antibiotics and patient adherence were assessed for 260 patients experienced AURI in a pharmacy. The prescription rates of antibiotics amongst the whole prescription decreased annually during the study period, but those in ENT clinic still constituted more than 90%. The usage of penicillins declined, but that of broad spectrum antibiotics such as amoxicillin/clavulanic acid and 1st/2nd generation cephalosporins increased. Moreover, the categories of antibiotics for the same indication were different among the clinics. For patients with AURI, the more antibiotics were prescribed as its missing days and days under its therapeutic dose increased. The drug interactions with concomitant drugs decreased annually, but the use of duplicate antibiotics was similar across the period. Potential inappropriate antibiotic use was common after the Korean policy, so the observation of pharmacists needs in addition to the patients and practitioners' attention.
