Effects of Dysphagia Treatment Applied to Infants with Pierre Robin Syndrome - Single Subject Research Design

  • Pierre Robin syndrome is characterized by micrognathia, glossoptosis, and cleft palate. Infants with Pierre Robin syndrome causes feeding difficulty, upper airway obstruction, and other symptoms. This study aims to examine the effects of applying dysphagia treatment to infants with Pierre Robin syndrome. The study participant was an infant who was born four weeks premature and referred for dysphagia treatment approximately 100 days after birth. At the initial assessment, the infant showed oral sensory sensitivity, a high level of facial and masticatory muscle tension, and a low stability of the chin and cheeks with almost no normal "sucking-swallowing-breathing" pattern. We set the baseline period and intervention period using the AB design. During the baseline period, non-nutritive sucking training using a rubber nipple was conducted without implementing an oral stimulation intervention. During the intervention period, non-nutritive sucking training and an oral stimulation intervention were performed. After the intervention period, the infant's daily oral intake and oral intake per time significantly increased compared to that during the baseline period. We observed that the oral intake time of the infant decreased during the intervention period compared to that in the baseline period, which indicated an improvement in control over the chin, tongue, and lip movements, a change in muscular tension, and stabilization of the "sucking-swallowing-breathing" pattern. We provided dysphagia treatment before breastfeeding, it was positive effects such as normal development of the infant, transition from tube feeding to bottle feeding, and enhancement of overall oral motor function.

균일류 하에서 작업하는 근로자의 노출농도 예측 (Prediction of Worker's Exposure in a Uniform Freestream)

  • In industrial field, there are several operations where a horizontal unidirectional airflow is used to control airborne contaminants. When a worker is immersed in a uniform freestream, a recirculating airflow can be created downstream of the worker by the phenomenon of boundary layer seperation. If the contaminant source and the breathing zone are within this near-wake region, high exposure may occur. The investigation for the effect of contaminant source location on worker exposure was performed by using CFD(Computational Fluid Dynamics). The airflow field was numerically calculated by assuming a steady flow and using the standard $k-{\varepsilon}$ turbulence model. As the results were compared with experimental data, the applicability of CFD was successfully verified. Subsequently, the breathing zone concentrations of the worker were predicted and compared with experimental data. The effects of contaminant density and turbulence intensity of freestream on worker exposure were evaluated.

호흡이 Ballet Pour de Bra 동작의 부드러움에 주는 영향 (Does the Control of Breathing Help a Dancer to Perform a Smoother Ballet Pour de Bra?)

  • The purpose of this study was to investigate the effects that breathing, thoracic and abdominal, had on the smoothness while performing ballet pour de bra. Five skilled ballet dancers(age: $24{\pm}1$, height: $163.4{\pm}2.88$, weight: $44.4{\pm}1.34$) with experience of over 10 years participated in this study. Each participant performed the ballet movement three times with abdominal respiration and with thoracic respiration. The kinematic data was recorded at 60 Hz with three digital cameras (Sony VX-2100). The pour de bra movement consists of two phases, up and down. The up phase is defined as the movement from the en bas through the en avant to the en haut. The down phase is defined as the movement from the en haut through the $\grave{a}$ la seconde to the en bas. During these two phases the Jerk Cost (JC) factor was calculated for the shoulder, elbow and wrist to quantify the smoothness. The group who performed the movement while abdominal respiration had a lower JC factor and so it was concluded that while abdominal respiration the smoothness of the movement was increased as opposed to the thoracic respiration.

Musculoskeletal Model for Assessing Firefighters' Internal Forces and Occupational Musculoskeletal Disorders During Self-Contained Breathing Apparatus Carriage

  • Background: Firefighters are required to carry self-contained breathing apparatus (SCBA), which increases the risk of musculoskeletal disorders. This study assessed the newly recruited firefighters' internal forces and potential musculoskeletal disorders when carrying SCBA. The effects of SCBA strap lengths were also evaluated. Methods: Kinematic parameters of twelve male subjects running in a control condition with no SCBA equipped and three varying-strapped SCBAs were measured using 3D inertial motion capture. Subsequently, motion data and predicted ground reaction force were inputted for subject-specific musculoskeletal modeling to estimate joint and muscle forces. Results: The knee was exposed to the highest internal force when carrying SCBA, followed by the rectus femoris and hip, while the shoulder had the lowest force compared to the no-SCBA condition. Our model also revealed that adjusting SCBA straps length was an efficient strategy to influence the force that occurred at the lumbar spine, hip, and knee regions. Grey relation analysis indicated that the deviation of the center of mass, step length, and knee flexion-extension angle could be used as the predictor of musculoskeletal disorders. Conclusion: The finding suggested that the training of the newly recruits focuses on the coordinated movement of muscle and joints in the lower limb. The strap lengths around 98-105 cm were also recommended. The findings are expected to provide injury interventions to enhance the occupational health and safety of the newly recruited firefighters.

마음챙김 호흡과 신체이완이 여성노인의 주의집중력에 미치는 영향 분석 (Analysis on the Effects and Influences of Mindfulness Based Breathing and Body Scan for Elderly Women's Attentive Concentration)

  • 본 연구는 마음챙김 호흡과 신체이완이 여성노인의 주의집중력에 어떤 영향을 미치는 지를 분석하기 위하여 객관적이고 정량적인 2-channel system EEG(electroencephalogram: EEG)를 통해 분석하였다. 연구 대상은 김포시에 거주하는 65세이상 여성노인 60명으로 그중 30명은 실험집단으로, 30명은 통제집단으로 선정하였다. 프로그램 실시 전과 후에 EEG를 측정하였다. 연구기간은 2013년 3월에서 2013년 7월까지 이며, 프로그램은 주 1회, 회기 당 60분씩, 총 16주 동안 실시하였다. 연구 결과는 다음과 같다. 실험집단에서 주의지수 우뇌(69.61/70.85, p<.024), 주의지수 비율 좌뇌(4.22/3.75, p<.037), 주의지수 비율 우뇌(4.29/3.70, p<.019)로 나타났다. 주의지수는 주의집중력 등 각성상태를 나타내는 지수이다. 따라서 마음챙김 호흡과 신체이완이 여성 노인의 주의집중력을 향상시킬 수 있는 실천적인 방법으로서 효용과 가치가 있다는 것을 뇌과학적으로 분석한 것에 의의가 있다.

인체기관지내의 입자펄스 확산 실험 (Dispersion of Particle Pulse in Human Lung Airway)

  • 세기관지의 구조와 이상유무의 신속 정확한 측정을 위해 세계적으로 활발히 연구되고 있는 에어로졸볼러스기법을 확립하기 위하여, 호흡과정 중 흡기에 공급된 입자펄스가 호흡의 진행에 따라 인체기관지내에서 대류분산된 결과로 호기중 입자농도의 증가된 분포폭을 측정하는 장치를 개발하고 이를 이용한 검증실험을 수행하였다. 개발된 장치는 인체에 무해한 sebacic acid를 이용하여 평균입경이 1 $\mu$m이고 기하표준편차가 1.2정도인 임자를 $10^{9}$개/cc 정도의 고농도로 발생하고, 솔레노이드밸브를 이용하여 호흡중의 원하는 시점에 정확히 짧은 펄스형태로 입자를 공급하며, 레이저광산란을 이용하여 흡기와 호기중의 입자농도의 시간변화를 측정한다. 흡기중의 농도분포는 매우 매끈한 분포를 보임으로써 발생장치와 측정장치의 정강작동은 잘 검증되었다. 농도의 시간변화 측정치로부터 계산된 분산도를 1차원 대류확산방정식 이론치 및 외국에서 보고된 실험결과와 비교한 격과, 입자도달깊이에 따른 분산도의 변화에 대한 정성적인 실험결과는 이론치와 상당히 잘 일치하고 기존의 결과보다도 견과의 산포가 개선되었다. 다만 아직도 산포가 상당히 남아있어 개선의 여지를 가지고 있으며, 외국 결과와의 분산도의 타이는 신체조건의 타이 등으로 파악되었다. 산포도의 개선을 위해서는 호흡시에, 특히 최고농도점 부근에서, 호흡유량이 일정하게 유지되어야만 불규칙 이차파형의 형성을 방지하여 측정결과의 해석에 오차를 줄일 수 있음을 확인하였다.

호흡패턴교정이 요추불안정성자의 흉·복부 사용률에 미치는 효과 (Changes in Thoracoabdominal Usage Rate after Respiratory Pattern Correction in Patients with Lumbar Instability)

  • 많은 요추 불안정성 환자들은 호흡 기능 감소와 호흡 패턴 변화로 인해 몸통 움직임 패턴의 변화를 나타낸다. 본 연구는 요추 불안정성 환자의 호흡 패턴 교정 운동 후 안정 및 노력성 호흡 동안 3개의 흉·복부 둘레 선 (겨드랑이, xiphoid junction, 10번째 갈비뼈)에 대한 사용률 변화를 조사하기 위해 실행하였다. 실험군 15명은 호흡 패턴 교정 운동을, 대조군 15명은 요추 안정화 운동을 실시했다. 중재 전, 두군 모두 안정 호흡과 노력성 호흡 동안 3개의 흉·복부 라인 간의 사용률에서 유의한 차이를 보였다(p<.05). 중재 후, 실험군은 안정 호흡 동안에 3 라인간 사용률 차이가 없는(p>.05) 변화를 나타내었고 노력성 호흡 동안에 두 라인 간 사용률의 변화를 나타내었다(p<.05). 호흡 패턴 교정 운동은 호흡 패턴의 회복을 통해 몸통 운동 패턴을 개선하였다. 따라서 우리는 호흡 패턴 교정이 요추 불안정성 환자를 위한 재활-프로그램의 재교육 및 강화 과정에 적용될 수 있음을 제안한다.

지하철 객실의 이산화탄소 제어방안 연구 (Study on control of carbon dioxide in subway car)

  • High concentration of carbon dioxide in subway cabin may cause various adverse effects on the passengers. Because carbon dioxide is produced by breathing of passengers, carbon dioxide concentration at rush-hours is extremely high. In most cases, the ventilation is not that easy for subway cars due to the high concentration of dust in tunnels. In this study, more practical way to control carbon dioxide in subway car was suggested. The decrease of carbon dioxide concentration was observed when the system was applied to a real subway car. The regenerative method for maintenance-free carbon dioxide control system was also suggested.

화재시뮬레이션을 이용한 터널 내 화재시 제연설비 필요성 검토 (A Study on the necessity of smoke control system in the tunnel fire using fire simulation)

  • In this study, fire simualtion was performed to examine the necessity of smoke control system in the tunnel fire. The heat release rate was set to 5 MW and 20MW, and the visibility was measured at 1.8 m, which is the breathing limit, when there is no jet fan. Through this, it was confirmed that 5 MW did not affect the visibility even without the jet fan, and in the case of 20 MW, a jet fan was required to secure the visibility. The visibility was measured at the same location by installing the jet fan, and the simulation was performed by reducing the design volume flow rate of 8.5 m3/s by 80% and 50%, respectively. As a result, it was confirmed that sufficient visibility was secured when the design flow rate and 80% were reduced.

The Effect of Neuromuscular Electrical Stimulation on Pulmonary Function, Gait Ability, and Quality of Life in Patients with Chronic Obstructive Pulmonary Disease

  • Purpose: This study aims to investigate the changes in pulmonary function, gait ability, and quality of life when NMES is applied along with CBE and to provide basic clinical data to be used in pulmonary rehabilitation guidelines for patients with COPD to treat patients with severe COPD. Methods: For this study, CBE and NMES of quadriceps femoris on both sides were applied to the experimental group (n=10), and only CBE was applied to the control group (n=10). For a pre-test, a 6-minute walk test was performed, and pulmonary function and health-related quality of life were measured. Moreover, an exercise program was applied to each group for 30 minutes per session, 5 times a week, for 6 weeks. After that, a post-test was conducted the same way as the pre-test. Results: In the within-group comparison, there were significant differences in forced expiratory volume in one second, 6-minute walk test and health-related quality of life between the experimental group and the control group (p<0.01)(p<0.05). In the between-group comparison, the experimental group showed an increase in forced expiratory volume in one second and 6-minute walk test (p<0.05) and showed a decline in health-related quality of life (p<0.05). Conclusion: The 6-weeks NMES program improved health-related quality in patients with severe COPD by increasing expiratory volume by reinforcing the function of quadriceps femoris. This finding implies that NMES could be an alternative mode for improving physical functions of patients with severe COPD, who cannot participate in a breathing exercise program or are reluctant to participate.
