24개월 이하의 자녀를 가진 여성의 모유수유 교육 경험에 관한 연구 (A Study on the Experience of Breastfeeding Education for Women with Children 24 Months of Age and Younger)

  • This study was conducted to investigate and improve experiences regarding breastfeeding education. Questionnaires were distributed to 185 women with children 24 months of age and younger. The period of breastfeeding was about 7.2 months and showed significant differences (p<0.01) according to women's age. About 56% of the respondents participated in breastfeeding education. Women with 3 or more children showed the highest rates of participation compared to the women in other groups(p<0.05). The reason for nonparticipation showed significant differences (p<0.05) according to women's occupation. The biggest reason for nonparticipation was 'lack of time' for housewives and 'no information' for working women. The subjects of education were benefits(20%) and techniques(19%) of breastfeeding, breast care before and after delivery(15%), nutrition care for breastfeeding mothers(11%), etc. The teaching method used most frequently was lecturing(32%). Around 81% of women were not satisfied with their breastfeeding education. The most useful subject was breastfeeding techniques(59%). Thirty percent of women pointed out that 'assessing mother's milk quantity' was the subject which women wanted to learn but were not taught enough about. Around 76% of women wanted more education and preferred personalized education such as home visitation(35%) and personal counselling(27%). Desired education methods showed significant differences(p<0.05) according to women's occupations. 'Home visitation' was the most preferred method for housewives and 'personal counselling' for working women. These results showed breastfeeding education was not helpful for practical life. Therefore, breastfeeding education should reflect women's individual needs to heighten its effectiveness.

모유수유실천 예측요인 (A Study of Predictors of Breastfeeding)

  • Purpose: The purpose of this study was to determine rate of breastfeeding practice at 3 months and to examine predictors for breastfeeding practice. Method: Study subjects were 114 mothers who gave birth at one hospital in Busan area. Data regarding breastfeeding practice and potentially related factors to breastfeeding were collected through a self-administered questionaire at 1 weeks and a telephone survey at 3 months. Logistic regression was used to identifiy significant the predictors on breastfeeding practice. Result: Rate of breastfeeding practice was 46.5% at three months of postpartum. The significant predictors for performing breastfeeding were lower the experience of breastfeeding trouble problem(OR=.88 compared with breastfeeding women, 95% CI; 78-.99), higher first breastfeeding satisfaction (OR=1.81 compared with non-breastfeeding women, 95% CI; 1.08-3.32), higher breastfeeding confidence(OR=1.84 compared with non-breastfeeding women, 95% CI; 1.08-3.32). Conclusion: The findings suggest the necessity of support program for mothers during the positive postpartum period to provide correct information about breastfeeding knowledge and attitude and to teach problem-solving skills for any breastfeeding problems for highere rate of breastfeeding practice

Association between breastfeeding and breast, thyroid, and cervical cancer among Korean adult women based on the Korean Genome and Epidemiology Study: a cohort study

  • Purpose: The purpose of this study was to explore the association between breastfeeding and the prevalence of breast, thyroid, and cervical cancer among Korean adult women. Methods: The study was a secondary analysis of data from the Korea Genome and Epidemiology Study. The final samples were 113,944 Korean women among 173,205 urban-based cohort participants collected between 2004 and 2013 for adults aged forty and over. To determine the association between female cancers and breastfeeding experience, the number of childbirth, and total breastfeeding duration, logistic regression analysis was done. The demographic characteristics, health behavior, and female history were adjusted. Results: The prevalence of breast cancer was 1.37 times higher in the non-breastfeeding group than in the breastfeeding group. Compared to having breastfed for more than 36 months, the prevalence of thyroid cancer was 1.68 times higher at breastfeeding for 13 to 36 months, 1.67 times higher at breastfeeding for 6 to 12 months, and 2.06 times higher at breastfeeding less than 6 months. Also, the prevalence of cervical cancer was 1.54 times higher at breastfeeding for 13 to 36 months, compared to breastfeeding for more than 36 months. Conclusion: The study found that breastfeeding experience and a longer breastfeeding duration are associated with reduced risk of breast, thyroid, and cervical cancer in Korean women. It can be used as a basis for encouraging breastfeeding, and suggests further research on modifiable factors that reduce cancer risks.

임신성 당뇨 산모의 모유수유 이행 경험 (Breastfeeding experiences of women with gestational diabetes)

  • Purpose: This study aimed to explore the lived experiences of breastfeeding women with gestational diabetes and to understand the meaning of breastfeeding for them and its encompassing context. Methods: Qualitative data were collected by interviewing 15 mothers with gestational diabetes. The transcript data from 5 focus group interviews and 2 individual interviews were analyzed using thematic analysis. Results: A core theme and 10 sub-themes emerged. The core theme that emerged was "the maternal commitment enabled for a limited time a way to compensate for guilt." Because mothers had to control their blood sugar levels before childbirth, they were worried about whether it would harm the newborn baby and thus searched for dietary information. After the babies were born and separated from their mothers, they were free from gestational diabetes. Mothers then felt guilty about their unhealthy bodies when they were pregnant. This regret provides a motivational context for breastfeeding. However, the mothers realized that breastfeeding requires commitment and dedication and that they need breastfeeding support, such as being provided practical help. Conclusion: The results showed that the breastfeeding intention of women with gestational diabetes might originate from their feelings of guilt. Therefore, an integrated breastfeeding program that combines the empathetic support of women with similar experiences and that shares practical information from experts should be implemented in a structured and systematic way.

모유수유 실천 및 수유기간에 영향을 미치는 요인 (Factors Affecting Breastfeeding Rate and Duration)

  • Objectives: The purpose of this study is to examine the factors affecting the rate and duration of breastfeeding. Methods: We analyzed the data from the year 2000 Korea National Fertility Survey that was collected through direct interviews. In particular, the mothers who delivered their last child and the child was under 1 year of age from January 1998 to June 2000 (N=1,066) were analyzed via a logistic model to assess the factors affecting the breastfeeding rate. Among the study subjects, those who had initiated breastfeeding (N=740) were analyzed through Cox's proportional hazard model to evaluate the factors affecting the duration of breastfeeding. Results: The multivariate logistic model showed that the delivery type and the baby's birth-weight have a statistically significant influence on the breastfeeding rate. Women who delivered their babies through Cesarean section were less likely than others to breastfeed. In contrast, the women whose babies weighed 2.5Kg or more were more likely than others to breastfeed. The results obtained from the survival analysis are as follows: the higher the mother's education level, the shorter is the breastfeeding duration. The mother's work status played a significant role in the early termination of breastfeeding. Women aged 35 or older showed a longer breastfeeding duration than the younger age groups, whereas the maternal age was not a significant factor in affecting whether or not a mother would breastfeed. Conclusions: Reducing the cases of operative delivery (Cesarean section) and low weight births, enlightening young and highly educated women on breastfeeding and improving the environment for breastfeeding on the job are important strategies to encourage women to breastfeed.

일 지역 여성의 임신기와 수유기 칼슘섭취 양상 (Women's Calcium Intake during Late Pregnancy and Breastfeeding)

  • Purpose: The purpose of the study was to identify the amount of calcium intake during late pregnancy and breastfeeding and compare the differences in the amount of calcium by subjects' characteristics. Methods: With a descriptive survey design, 121 Korean breastfeeding women were recruited in a community setting. The list of foods and drinks with calcium extracted from the study of Song and So (2007) was used to measure the amount of calcium intake during pregnancy and lactation. The subjects' characteristics were collected to identify differences in the amount of calcium intake. Results: Mean age of the women was 31 years old. Amount of calcium intake was 568mg per day during late pregnancy and 431mg per day during breastfeeding. These amounts were quite lower than a recommended dose that adult women should consume a daily minimum of 1,000mg of calcium. Women who had higher household income and attending experience for nutritional education took more calcium during breastfeeding and late pregnancy, respectively. Conclusion: More than a half of the breast-feeding women do not consume the recommended dose of calcium. Education for proper calcium consumption during pregnancy and lactation should be an additional subject in the prenatal and breastfeeding education program for women's bone health.

모유수유 교육 프로그램 개발을 위한 여성들의 수유양상 기초조사 -수유관련 지식 수준 및 모유수유율- (A Survey on the Feeding Practices of Women for the Development of a Breastfeeding Education Program - Breastfeeding Knowledge and Breastfeeding Rates -)

  • A survey was carried out to investigate the breastfeeding knowledge, breastfeeding rates ultra related factors among women in Seoul and the Chungbuk area, in order to obtain baseline data for the development of an education program on breastfeeding. Subjects included 671 lactating mothers who visited public health or pediatric centers, between December, 1999 and February, 2000 and were interviewed using a questionnaire. The results are as follows: In the breastfeeding knowledge, most of the subjects (93.4%) know correctly that 'breastfeeding is better for a baby's emotional development than formula feeding,'whereas only half of the subjects (51.7%) answered correctly the question on whether 'formula is more nutritious than breastmilk.'The breastfeeding rates were about 57%, 40%, 33% and 12%, respectively for baby's aged 1, 3, 6, and 12 months. These results showed that the breastfeeding rate rapidly decreased with the baby's age. Lower breastfeeding rates were associated with high educational level (college graduate and above), high income, mothers'employment, no experience of attending breastfeeding education programs, and no planning on breastfeeding prior to pregnancy. There was a significant relationship between the level of knowledge and breastfeeding practice; the higher the knowledge score, the higher the breastfeeding rate. In conclusion, an education program should be developed and offered for each low breastfeeding rate group, in order to promote breastfeeding.

The extended Theory of Planned Behavior in explaining exclusive breastfeeding intention and behavior among women in Kelantan, Malaysia

  • BACKGROUND/OBJECTIVES: The purpose of this study is to utilize an extended Theory of Planned Behavior in identifying predictors of exclusive breastfeeding intention and behavior among women in Kelantan, Malaysia. SUBJECTS/METHODS: A prospective cohort study was conducted, recruiting pregnant womenthrough two-stage cluster sampling. Their exclusive breastfeeding intention, attitude, perceived norm, perceived behavioral control and past behavior were obtained at baseline through interviewer-guided questionnaire. At one month after delivery, another interview was conducted to determine the two additional variables in the extended theory, which were their postpartum support and breastfeeding difficulty. The behavior, which was the actual duration of exclusive breastfeeding, was obtained from the second follow-up at six months. Pearson correlation and two hierarchical regression analyses were conducted. RESULTS: A total of 200 women completed the study follow-up. Their median intended exclusive breastfeeding duration was 4.0 (IQR 5) months, and the median actual duration was 1.0 (IQR 4) month. The Theory of Planned Behavior explained 51.0% of the variance in intention, with perceived behavioral control and attitude were the significant predictors. It also explained 10.0% of the variance in behavior, but the addition of postpartum support and breastfeeding difficulty increased the amount of explained variance in behavior by 6.0%. The significant predictors of exclusive breastfeeding behavior were intention, postpartum support and breastfeeding difficulty. CONCLUSION: The extended Theory of Planned Behaviorhad a good predictive ability in explaining exclusive breastfeedingintention and behavior. The women's intention to practice exclusive breastfeeding may be improved by improving their perceived behavioral control and attitude. Providing correct postpartum support and skills to handle breastfeeding difficulties after delivery will improve their exclusive breastfeeding behavior.

The Effect of Attitudes Toward Breastfeeding in Public on Breastfeeding Rates and Duration: Results from South Korea

  • Background: Attitudes toward breastfeeding in public are one potential barrier to optimal breastfeeding rates and durations. Method: Questions about breastfeeding experience and attitudes toward breastfeeding in public were asked in face-to-face interviews as part of the Korean Academic Multimode Open Survey (KAMOS), May-July, 2017. The response rate was 65.8% (2000 respondents nationwide). Results: A majority of Koreans disagreed (1 or 2 on a 4-point scale) with the statement "Women should not breastfeed their child in open, public places" (53.9%) and agreed (3 or 4 on the 4-part Likert scale) with the statements "I do not feel uncomfortable seeing women breastfeed their child in open, public places" (64.0%) and "Breastfeeding a baby, instead of letting the baby cry, in public places is better for other people" (71.8%). However, despite these generally positive attitudes, the majority also said that they would not breastfeed in public (57.4% of women) or, in the case of men, would not want a close female relative to do so (63.8% of men). Breastfeeding in public was positively correlated with the duration of breastfeeding. People were more positive about breastfeeding in public if they: were parents; did not use formula and breastfeeding a similar amount; had children who had been breastfed in public; were older; were Buddhists rather than Christians. An attempt was made to compare attitudes toward breastfeeding in public and breastfeeding durations internationally, but was inconclusive due to not perfectly comparable data. Conclusion: Our results may be useful in planning public health campaigns in South Korea or future attempts at international comparisons to better understand and address the effect of public opinion regarding breastfeeding in public on breastfeeding rates and durations.

모유수유 임파워먼트 프로그램이 초산모의 모유수유 자기효능감, 적응 및 지속에 미치는 효과 (Effects of Breastfeeding Empowerment Program on Breastfeeding Self-efficacy, Adaptation and Continuation in Primiparous Women)

  • Purpose: The purpose of this study was to develop a breastfeeding empowerment program and to investigate the effects of the breastfeeding empowerment program on self-efficacy, adaptation and continuation of breastfeeding for primiparous women. Methods: The 5 session breastfeeding empowerment program was developed and a non-equivalent control group non-synchronized quasi-experiment design was used. Fifty-five participants were assigned to either the experimental group (n=27) or the control group (n=28). Effects were tested using repeated measures ANOVA and $x^2-test$. Results: Scores for self-efficacy, adaptation and continuation of breastfeeding of in the experimental group after program were significantly higher than 1week, 4weeks, 8weeks scores in control group. Conclusion: The effects of the breastfeeding empowerment program for elevating self-efficacy, adaptation and continuation of breastfeeding in primiparous women were validated. Therefore, this program can be recommended for vigorous use in clinical practice.
