Amitraz 중독 후 혼수 상태를 보인 환자 1례 (A Case of a Patient with Stuporous Mentality and Hypotension after Amitraz Ingestion)

  • Amitraz is a formamidine-derived insecticide and acaricide which is commonly used throughout the world. Amitraz intoxication is mediated through ${\alpha}_2$ adrenergic receptor agonist effects, similar to those of the ${\alpha}_2$ agonist clonidine. We report a case of a patient who experienced coma and hypotension after amitraz ingestion. A 37-year-old woman visited the ER with symptoms of vomiting and altered mental state. She had ingested a mouthful of liquid amitraz concentrate (12.5%), which rapidly led to vomiting, hypotension, bradycardia, hyperglycemia, and mental stupor. Supportive treatment, including mechanical ventilation and administration of inotropics, resulted in full recovery within four days.

저(低)Calcium혈증(血症)에 관한 심전도학적(心電圖學的) 연구(硏究) (Electrocardiographical Studies on Hypocalcemia in Goats)

  • Experimental hypocalcemia was induced in normal goats by intravenous infusion with various concentration of $Na_2$-EDTA solution. 1. Progressive depression of reflexes and body temperature, paresis, and cardiac arrest were observed in two groups infused with 4% and 8% EDTA solution, whereas paresis and cardiac arrest were not observed in 3% EDTA group. 2. The patterns of electrocardiogram were changed according to the decrease of plasma Ca level in all of 7 goats. When plasma Ca decreased to 6mg per 100ml or below, sinus bradycardia or tachycardia and prolongation of QT interval were showed, while atrioventricular block was noted in case which the plasma Ca level decreased to 4mg per 100ml or below. Fibrillation and cardiac arrest were evident in case which the plasma Ca level decreased to as low as 3mg per 100ml.

Propafenone 음독 후 발생한 치명적인 부정맥 1례 - 증례 보고- (A Case of Fatal Arrhythmia after Propafenone Overdose)

  • A 36-year-old female was transferred to our emergency medical center with decreased mental status after a 6.0 g propafenone overdose because of domestic disturbance. She had no previous history of epilepsy, diabetes mellitus, hypertension or psychiatric illness. Before presenting to our center, gastrointestinal decontamination, charcoal administration, and endotracheal intubation due to bradycardia and generalized seizure had been performed. Soon after hospital arrival, at 5 h after ingestion, she collapsed into shock and fatal arrhythmia. We successfully resuscitated the patient with amiodarone, sodium bicarbonate, a large volume of normal saline, calcium, and ventilator care. At 23 h after ingestion, she was fully recovered and had no subjective signs or symptoms. To our knowledge, this is the first case report of intentional propafenone overdose in Korea, which we report with reviews of the previous literature.

Profound trigeminocardiac reflex from lingual nerve stimulation: a case report

  • Trigeminocardiac reflex (TCR) is a well-known brainstem reflex that manifests as hypotension, bradycardia, dysrhythmia, and asystole when stimulation is applied to a branch of the trigeminal nerve. Most commonly associated with ophthalmic, orbital, and neurologic surgeries, mandibular division and oral cavity variants occur far less frequently. Here, we describe a case of asystolic TCR elicited by lingual nerve stimulation. This case highlights the role of specific anesthetic medications in modulating this phenomenon and reinforces the need for early recognition and clear communication in case of its occurrence. Anesthesia providers must consider discontinuing or avoiding certain medications when clinically appropriate, even during low TCR-risk procedures.

가토의 Quabain-Induced Arrhythmia에 미치는 Carbamzepine의 효과 (Effect of Carbamazepine on the Ouabain-Induced Arrhythmia in Rabbits)

  • 가토의 Ouabain유발 부정맥에 미치는 Carbamazepine의 영향을 검색한 결과 다음과 같은 결론을 얻었다. 1. Ouabain을 지속적으로 정맥 주사하여 64+$8.8{\mu}g/kg$이 투여되었을 때 부정맥이 발생 했으며, 이 양을 부정맥 유발 가능용량으로 정했다. 2. Ouabain $64{\mu}g/kg$을 단회 정맥 주사했을 때 발생한 부정맥은 약 7~9분간 지속된 후 모든 예에서 자연 소실되었고, 정상 심박동으로 회복된 지 20분 후 다시 동량의 Ouabain을 정맥 주사했을 때 모든 예에서 다시 나타났다. 3. 부정맥 유발 용량($64{\mu}g/kg$)의 Ouabain을 단회 정맥 주사한 후 부정맥이 나타난 것을 관찰 즉시 Carbamazepine을 투여한 결과 즉시 정상 신박동으로 환원되었으며 어느 정도 지속된 후 모든 예에서 부정맥이 발생했으나 즉시 동량의 Carbamazepine 투여로 다시 정상 심박동으로 환원되었다. 한편, Carbamazepine의 양이 증가되면서 항 부정백 작용의 기간은 길어졌으나 항 부정맥 작용없이 사망한 예가 많아졌다. 4. Carbamazepine을 단독 투여 해 본 결과 그 양이 증가함에 따라 심한 서맥, A-V block, 심방 세동 등이 나타나면서 심장이 정지함을 볼 수 있었다. 이상의 실험 결과로 미루어 Carbamazepine은 Ouabain의 독작용에 의한 심한 부정맥을 일시적으로 억제할 수 있으며, 보다 대량에서는 그 항 부정맥 작용이 보다 오래 지속할 수 있으나 Carbamazepine 자체의 심장에 대한 부작용이 발현될 위험이 존재한다고 생각된다.

개에서 Medetomidine-Ketamine-Butorphanol과 Xylazine-Ketamine-Butorphanol의 마취 효과 및 심폐에 미치는 영향 (Anesthetic and Cardiorespiratory Effects of Medetomidine-Ketamine-Butorphanol and Xylazine-Ketamine-Butorphanol in Dogs)

  • Medetomidine과 xylazine은 수의 임상에서 많이 이용되고 있는 마취약물이다. Medetomidine-ketamine-butorphanol과 xylazine-ketamine-butorphanol의 개에 미치는 마취효과와 심폐 효과를 비교 연구하였다. Medetomidine-ketamine-butorphanol를 MKB군으로 xylazine-ketamine-butorphanol를 XKB군으로 분류하고 각각 5두의 개를 4주간의 휴약기간을 거쳐 실험하였다. 각각의 군에서 마취효과 점수는 자세와 진통에 대한 반응을 통해 평가하였고, 마취 시간은 XKB군 ($87.4{\pm}18.9$분)이 MKB군 ($62.6{\pm}15.0$분)에 비하여 유의성 있게 높은 시간의 결과를 보였다. 하지만, 기타 심폐 효과에서는 두 군 사이에 통계적으로 유의성 있는 차이를 보이지 않았다. 하지만 MKB군에서는 서맥을 보였으므로, 마취 모니터링 시 주의하여야 한다. 만약 서맥의 위험이 있는 환자라면 MKB 합제는 선택을 안 하는 것이 요구된다. 결론적으로, XKB군에서 더 긴 마취시간과 더 적은 부작용을 보였으며, MKB 합제 약물은 XKB군에 비하여 특별한 장점이 없으므로 XKB 합제가 더욱 유용한 것으로 판단하였다. 하지만 XKB군과 MKB군 모두에서 적절한 체온유지와 산소공급이 필요하다.

혈관미주 신경성 실신 소아에서 기립경사 검사 동안에 뇌혈류 변화 (Change of Cerebral Blood Flow during Tilt Tests in Children with Vasovagal Syncope)

  • 목 적 : 실신은 소아에서 가장 흔한 증상이지만 기전은 잘 알려져 있지 않다. 증가된 미주신경의 활동과 감소된 교감신경의 자극이 저혈압, 서맥을 일으키고 마침내는 의식 소실을 초래한다. 이 연구의 목적은 혈관미주 신경성 실신으로 진단된 소아에서 기립경사 검사를 하는 동안 뇌 도플러 초음파를 실시하여 실신 전후의 뇌혈류 속도와 혈압, 심박동수 변화를 측정하고자 하였다. 방 법 : 실신의 과거력을 갖고 있는 64명의 소아를 대상으로 혈압을 10분간 안정 후와 15분 동안 기립 후에 측정하였고, 기립 경사 검사는 $80^{\circ}$에서 45분간 시행하였다. $80^{\circ}$ 각도의 경사 테이블에서 소아를 5분 간격으로 45분 동안 혈압, 심박동수, 산소 포화도를 측정하였다. 기립경사 검사를 하는 동안 실신이나 실신 전 증상이 나타난 소아에서는 검사를 중단하였다. 12-유도 심전도, 뇌파 검사, 심장 초음파를 시행하였다. 기립경사 검사에서 양성으로 나온 10명의 소아에서 Acouson 128XP/10 model 초음파 기기를 이용하여 중 대뇌동맥 혈류 속도, pulsatility 지수, time velocity integral, 혈압, 심박동수, 산소포화도 등을 측정하였다. 결 과 : 기립경사 검사에서 31.3%(20/64)의 양성률을 보였다. 검사 동안에 혈압과 맥박은 감소되지 않았고 뇌혈류 속도와 integral은 유의하게 감소되었다. 결 론: 뇌혈류 속도는 혈압이나 심박동수가 감소하기 전에 미리 감소하므로 실신 전 증상을 미리 예측할 수 있다. 뇌혈류의 자율 조절의 장애가 혈관미주 신경성 실신의 병인론에 중요한 역할을 하리라 생각된다.

Cardiac Complications Associated with Eating Disorders in Children: A Multicenter Retrospective Study

  • Purpose: Eating disorders often result in somatic complications, including cardiac abnormalities. Cardiac abnormalities may involve any part of the heart, including the cardiac conduction system, and can lead to sudden cardiac death. The current study aimed to evaluate the incidence of cardiac complications in pediatric patients with eating disorders and their associated factors. Methods: We retrospectively analyzed patients aged 10-18 years who were diagnosed with DSM-V (Diagnostic and Statistical Manual of Mental Disorder-V) eating disorders and underwent electrocardiography (ECG) and/or echocardiography between January 2015 and May 2020. Results: In total, 127 patients were included, of whom 113 (89.0%) were female. The median body mass index (BMI) was 15.05±3.69 kg/m2. Overall, 74 patients (58.3%) had ECG abnormalities, with sinus bradycardia being the most common abnormality (91.9%). Patients with ECG abnormalities had significantly lower BMI (14.35±2.78 kg/m2 vs. 16.06± 4.55 kg/m2, p<0.001) than patients without ECG abnormalities, as well as lower phosphorus and higher cholesterol levels. Among the 46 patients who underwent echocardiographic evaluation, 23 (50.0%) had echocardiographic abnormalities, with pericardial effusion being the most common (60.9%). The median left ventricular mass (LVM) and ejection fraction were 67.97±21.25 g and 66.91±28.76%, respectively. LVM and BMI showed a positive correlation (r=0.604, p<0.001). After weight gain, the amount of pericardial effusion was reduced in 3 patients, and 30 patients presented with normal ECG findings. Conclusion: Cardiac abnormalities are relatively frequent in patients with eating disorders. Physicians should focus on this somatic complication and careful monitoring is required.

Safety and efficacy of target controlled infusion administration of propofol and remifentanil for moderate sedation in non-hospital dental practice

  • Background: Fearful and anxious patients who find dental treatment intolerable without sedative and analgesic support may benefit from moderate sedation. Target controlled infusion (TCI) pumps are superior to bolus injection in maintaining low plasma and effect-site concentration variability, resulting in stable, steady-state drug concentrations. We evaluated the safety and efficacy of moderate sedation with remifentanil and propofol using TCI pumps in non-hospital dental settings. Methods: A prospective chart review was conducted on 101 patients sedated with propofol and remifentanil using TCI pumps. The charts were completed at two oral surgeons and one general dentist's office over 6 months. Hypoxia, hypotension, bradycardia, and over-sedation were considered adverse events and were collected using Tracking and Reporting Outcomes of Procedural Sedation (TROOPS). Furthermore, patient recovery time, sedation length, drug dose, and patient satisfaction questionnaires were used to measure sedation effectiveness. Results: Of the 101 reviewed sedation charts, 54 were of men, and 47 were of women. The mean age of the patients was 40.5 ±18.7 years, and their mean BMI was 25.6 ± 4.4. The patients did not experience hypoxia, bradycardia, and hypotension during the 4694 min of sedation. The average minimum Mean Arterial Pressure (MAP) and heartbeats were 75.1 mmHg and 60.4 bpm, respectively. 98% of patients agreed that the sedation technique met their needs in reducing their anxiety, and 99% agreed that they were satisfied with the sedation 24 hours later. The average sedation time was 46.9 ± 55.6 min, and the average recovery time was 12.4 ± 4.4 min. Remifentanil and propofol had mean initial effect-site concentration doses of 0.96 µ/.ml and 1.0 ng/ml respectively. The overall total amount of drug administered was significantly higher in longer sedation procedures compared to shorter ones, while the infusion rate decreased as the procedural stimulus decreased. Conclusion: According to the results of this study, no patients experienced adverse events during sedation, and all patients were kept at a moderate sedation level for a wide range of sedation times and differing procedures. The results showed that TCI pumps are safe and effective for administering propofol and remifentanil for moderate sedation in dentistry.

Safe Sedation and Hypnosis using Dexmedetomidine for Minimally Invasive Spine Surgery in a Prone Position

  • Dexmedetomidine, an imidazoline compound, is a highly selective ${\alpha}_2$-adrenoceptor agonist with sympatholytic, sedative, amnestic, and analgesic properties. In order to minimize the patients' pain and anxiety during minimally invasive spine surgery (MISS) when compared to conventional surgery under general anesthesia, an adequate conscious sedation (CS) or monitored anesthetic care (MAC) should be provided. Commonly used intravenous sedatives and hypnotics, such as midazolam and propofol, are not suitable for operations in a prone position due to undesired respiratory depression. Dexmedetomidine converges on an endogenous non-rapid eye movement (NREM) sleep-promoting pathway to exert its sedative effects. The great merit of dexmedetomidine for CS or MAC is the ability of the operator to recognize nerve damage during percutaneous endoscopic lumbar discectomy, a representative MISS. However, there are 2 shortcomings for dexmedetomidine in MISS: hypotension/bradycardia and delayed emergence. Its hypotension/bradycardiac effects can be prevented by ketamine intraoperatively. Using atipamezole (an ${\alpha}_2$-adrenoceptor antagonist) might allow doctors to control the rate of recovery from procedural sedation in the future. MAC, with other analgesics such as ketorolac and opioids, creates ideal conditions for MISS. In conclusion, dexmedetomidine provides a favorable surgical condition in patients receiving MISS in a prone position due to its unique properties of conscious sedation followed by unconscious hypnosis with analgesia. However, no respiratory depression occurs based on the dexmedetomidine-related endogenous sleep pathways involves the inhibition of the locus coeruleus in the pons, which facilitates VLPO firing in the anterior hypothalamus.
