Objectives : Hwanglyeonhaedok-tang(黃連解毒湯) is a representative herbal formula with "clear heat(淸熱)" effects. The aim of this study was to evaluate the effects of Hwanglyeonhaedok-tang-based intranasal herbal ointment Biyeom-go based on the cold and heat pattern identification questionnaire(CHPIQ). Methods : We performed a subgroup analysis of the previously published prospective observational study. A total of 58 patients with rhinitis were administered Biyeom-go for 4 weeks, and its effects on the Total Nasal Symptom Score(TNSS), Mini Rhinoconjunctivitis Quality of Life Questionnaire(Mini-RQLQ) score, and nasal endoscopy index score were analyzed based on CHPIQ. Results : Among the 58 patients, the heat and non-heat patterns were shown by 39 and 19 patients, respectively, while the cold and non-cold patterns were shown by 46 and 12 patients, respectively. The change in TNSS from baseline negatively correlated with the heat pattern score(p=0.011). Improvement in TNSS was greater in the heat pattern group than in the non-heat pattern group, with a borderline significant difference(p=0.07). Mini-RQLQ and nasal endoscopy index scores tended to be lower in the heat pattern group than in the non-heat pattern group, but without a statistically significant difference. Conclusion : The findings indicate that CHPIQ is a useful tool for the diagnostic and prognostic evaluation of patients with rhinitis. This study provides fundamental evidence of the close association between the cold-heat pattern in patients with rhinitis and the treatment effects of Biyeom-go.

Objective: We performed small-scale mutation and large genomic rearrangement (LGR) analysis of BRCA1/2 in ovarian cancer patients to determine the prevalence and the characteristics of the mutations. Methods: All ovarian cancer patients who visited a single institution between September 2015 and April 2017 were included. Sanger sequencing, multiplex ligation-dependent probe amplification (MLPA), and long-range polymerase chain reaction (PCR) were performed to comprehensively study BRCA1/2. The genetic risk models BRCAPRO, Myriad, and BOADICEA were used to evaluate the mutation analysis. Results: In total, 131 patients were enrolled. Of the 131 patients, Sanger sequencing identified 16 different BRCA1/2 small-scale mutations in 20 patients (15.3%). Two novel nonsense mutations were detected in 2 patients with a serous borderline tumor and a large-cell neuroendocrine carcinoma. MLPA analysis of BRCA1/2 in Sanger-negative patients revealed 2 LGRs. The LGRs accounted for 14.3% of all identified BRCA1 mutations, and the prevalence of LGRs identified in this study was 1.8% in 111 Sanger-negative patients. The genetic risk models showed statistically significant differences between mutation carriers and non-carriers. The 2 patients with LGRs had at least one blood relative with breast or ovarian cancer. Conclusion: Twenty-two (16.8%) of the unselected ovarian cancer patients had BRCA1/2 mutations that were detected through comprehensive BRCA1/2 genetic testing. Ovarian cancer patients with Sanger-negative results should be considered for LGR detection if they have one blood relative with breast or ovarian cancer. The detection of more BRCA1/2 mutations in patients is important for efforts to provide targeted therapy to ovarian cancer patients.

Over the North East Asia, extreme anomalous precipitation were observed in 2013 and 2014. During 2013 summer the precipitation was found to be higher (two standard deviation) than the climatological mean of the region; whereas during 2014, which was a borderline El Ni?o year, precipitation was found to be lower (one standard deviation). To understand the differences of these two anomalous years the Global/Regional Integrated Model system (GRIMs) has been used. The study found that low landsurface temperature and high sea-surface temperature over ocean caused a smaller land-sea contrast of surface temperature between East Asia and North West Pacific Ocean in 2014, which could have caused an eastward shift of mean monsoon circulation in that year compared to the circulation in 2013. Due to a change in the lower level circulation and wind field over East Asia the evaporation and moisture transport patterns became very different in those two years. In 2013, this study found high latent heat flux over Eastern China, which implies an increased surface evaporation over that region, and the moisture transported to the north by the mean monsoon circulation; whereas, there was no correlated transport of moisture to the North East Asia during 2014. The precipitable water over North East Asia has a stronger correlation with the latent heat flux over southern land region than that from Ocean region in the eastern side in both the years. A new approach is proposed to estimate the sub-grid scale hydrometeors from GRIMs, overestimated in the existing model.

Background: Medically unexplained symptoms (MUSs) are common in primary care. Atpresent, there are no proven, comprehensive treatments available in primary care forpatients with MUSs. However, MUS has parallels with "subhealth" or Mibyeong from tradi-tional East-Asian medicine, and thus, Mibyeong interventions could be effective in treatingMUS. Unfortunately, studies on Mibyeong and its intervention methods are relatively rare.Methods: We administered a web-based survey to 17,279 Korean medicine (KM) practitionersregistered with the Association of Korean Medicine. The response rate was 4.9% (n = 849).Based on the responses received, we assessed how much they agreed with concepts relatedto Mibyeong on a 7-point scale from "do not agree" to "strongly agree." Respondents werealso asked to indicate how frequently they encountered various subtypes and patterns ofMibyeong, and how frequently they use listed intervention methods.Results: Data from 818 respondents were analyzed after excluding those with no clinicalexperience. On average, respondents were male general practitioners aged between 30 yearsand 49 years, working or living in metropolitan areas such as Seoul, Incheon, and Gyeonggi-do. Responses did not differ by demographics. Respondents generally thought that Mibyeongreferred to subjective or borderline findings without certain disease, and that Mibyeong hasvarious subtypes and patterns. Subtypes included fatigue, pain, and digestion problems; pat-terns were either deficiencies (e.g., qi, blood, and yin deficiency) or stagnations (e.g., liver qidepression and qi stagnation). Decoction was the most frequently used type of interventionfor Mibyeong of all items listed, followed by acupuncture and moxibustion. Patient educa-tion was also recommended, suggesting healthy eating, promoting healthy environment,and exercise.Conclusion: We were able to provide preliminary results on KM practitioners' recognition ofand interventions for Mibyeong, but further research is needed to develop a detailed defi-nition of Mibyeong and its myriad subtypes and patterns, and evaluations of the efficacy ofMibyeong interventions.
Background: PTSD (Post-traumatic stress disorder, PTSD) had a great impact on health care workers during the COVID-19 (Corona Virus Disease 2019, COVID-19). Better knowledge of the prevalence of PTSD and its risk factors is a major public health problem. This study was conducted to assess the prevalence and important risk factors of PTSD among medical staff during the COVID-19. Methods: The databases were searched for studies published during the COVID-19, and a PRISMA (preferred reporting items for systematic review and meta-analysis) compliant systematic review (PROSPERO-CRD 42021278970) was carried out to identify articles from multiple databases reporting the prevalence of PTSD outcomes among medical staff. Proportion random effect analysis, I2 statistic, quality assessment, subgroup analysis, and sensitivity analysis were carried out. Results: A total of 28 cross-sectional studies and the PTSD results of doctors and nurses were summarized from 14 and 27 studies: the prevalences were 31% (95% CI [confidence interval, CI]: 21%-40%) and 38% (95% CI: 30%-45%) in doctors and nurses, respectively. The results also showed seven risks (p < 0.05): long working hours, isolation wards, COVID-19 symptoms, nurses, women, fear of infection, and pre-existing mental illness. Two factors were of borderline significance: higher professional titles and married. Conclusion: Health care workers have a higher prevalence of PTSD during COVID-19. Health departments should provide targeted preventive measures for medical staff away from PTSD.
Objectives The purpose of this study is to propose a method to more specifically identify Sasang constitutional risk factors of metabolic syndromes by adjusting the cut-off value of Korea Sasang Constitutional Diagnostic Questionnaire (KS-15). Methods Data of 1997 participants in Korean medicine Daejeon Citizen Cohort study (KDCC) were analyzed. Metabolic syndrome was defined according to the NCEP-ATP III, lifestyle information, and hematologic information including KS-15 and demographic characteristics were used as covariates. Results The 179 subjects with metabolic syndrome accounted for 9.0% of the total. As a result of determining the Sasang constitution for the KS-15 response based on the cut-off values (approximate 0.33), 0.5, and 0.6 of the constitutional score, when performed at the 0.6 cut-off model, the odds ratio of TE was 2.46 which showed a statistically significantly higher risk than the borderline group. For the accuracy of the model and the Area under the curve (AUC), the model accuracy based on the original cut-off of the KS-15 was 0.902 and AUC was 0.737. The accuracy of the model with cut-off of 0.5 and with of 0.6 were 0.904 and 0.902, respectively, and the AUCs were 0.687 and 0.741, respectively. Conclusion In this study, we confirmed that it is effective to increase the cut-off value of KS-15 to 0.6 in the metabolic syndrome risk model. It is expected that this could increase the accuracy of identifying high-risk groups for metabolic syndrome.
Objective: This study aimed to evaluate whether human chorionic gonadotropin (hCG) therapy is beneficial for improving semen parameters and clinical hypogonadism symptoms in hypogonadic oligozoospermic or severe oligozoospermic men with low or borderline testosterone levels. Methods: A weekly dose of 250 ㎍ (equivalent to approximately 6,500 IU) of hCG was administered subcutaneously for 3-6 months to 56 hypogonadic oligozoospermic or severe oligozoospermic men. Semen, biochemical, and genetic analyses were performed before the start of treatment followed by analyzing semen parameters every 3 months after the start of therapy. We grouped participants into responders and non-responders depending on positive changes in semen parameters. Results: Out of 56 men, 47 (83.93%) responded, while 9 (16.07%) did not. Upon statistical analysis, it was found that age did not affect the overall outcomes (p=0.292); however, men with higher body mass index (BMI; 28.09±3.48 kg/m2) showed better outcomes than those with low BMI (25.33±3.06 kg/m2) (p=0.042). The duration of therapy (in months) was higher in non-responders than in responders (p=0.020). We found significant improvements in sperm concentration (p=0.006) and count (p=0.005) after 3 months of therapy. Sperm motility and progressive motility were also found to be higher in responders, but did not show statistically significant changes. Conclusion: We conclude that hCG therapy can be beneficial in men with hypogonadic oligozoospermia or severe oligozoospermia.
Objectives: The aim of this study was to analyze public and researcher interests in suicide and related illnesses and acupuncture and acupressure treatment using Google Trends and some electronic databases. Methods: Search results for keywords "suicide," "acupuncture," "acupressure," and several illnesses related to suicide were analyzed in Google Trends from January 2004 to June 2023. Illnesses included anxiety, depression (including major depressive disorder), schizophrenia, bipolar disorder, post- traumatic stress disorder (PTSD), eating disorder (including anorexia nervosa and bulimia nervosa), substance use disorder, autism spectrum disorder, personality disorder (including borderline person- ality disorder), and chronic pain. Search results were extracted using relative search volume (RSV) scores between 0 and 100. Search terms were also searched in online databases, including PubMed, CNKI, and OASIS, to estimate the number of related studies, and descriptive analysis was conducted. Results: Google Trends analysis showed a strong positive correlation between the RSVs of "suicide and depression," "acupuncture and chronic pain," and "acupressure and PTSD." The electronic database search results produced numerous studies published on "suicide and depression," "acupuncture and depression," and "acupressure and anxiety." High interest in "suicide and depression," "acupuncture and chronic pain," and "acupressure and anxiety" was seen among the public and researchers. Interest in "suicide and chronic pain," "acupuncture and eating disorder," and "acupressure and PTSD" was higher in the public than among researchers, while "anxiety and suicide" and "anxiety and acu- puncture" showed opposite trends. Conclusions: The results of this research enable an understanding of public and researcher interest in suicide, acupuncture, acupressure, and suicide-related illnesses. The results also provide a basis for fu- ture research and examining public health implications in Korean medicine.
Purpose: Although the term "transition to motherhood" is commonly used in research, the concept is not clear. This study, hence, was conducted to clarify the concept of "transition to motherhood." Methods: The concept analysis framework developed by Walker and Avant is used to analyze the concept of transition to motherhood. Results: Transition to motherhood is defined as the physical, psychological, social, and relational (mother-baby relationship/interpersonal relationship) changes that happen to a woman after pregnancy and delivery of a baby. The attributes of the transition to motherhood include: 1) adapting to physical changes after pregnancy and childbirth; 2) experiencing various psychological changes; 3) changing of her social perception from being a woman to someone's mother; and 4) forming and developing a relationship with the newborn, adjusting priorities, and redefining the relationship between family and others. Meeting the newborn is regarded as an antecedent of the transition to motherhood. Redefining identity and physical image, ensuring mother's well-being, maternal attachment, and confidence in the maternal role are regarded as consequences of the transition to motherhood. The concept was clarified by the presentation of model, borderline, and contrary cases. Conclusion: The significance of this study lies in the clarification of the concept of transition to motherhood and defining its attributes. It is recommended that tools be developed to measure transition to motherhood based on the results of this study. Furthermore, nurses and midwives can use study findings to better understand the concept of transition to motherhood in providing care and support to mothers who experience it.
Since Plath killed herself in 1963, the theme of death has become one of the central motifs and allusions in her work. The biographical emphasis continues to blur the boundary between the artistic world and the material world. While approaching Plath's work from the perspective of personal experience, the objective of this paper is not to suggest that we encounter Plath's personal voice and emotions directly in her work. Rather, I emphasize how Plath's work of mourning is substantiated in the act of writing. Plath protects herself from the unnamable or the existential void by writing poems. She shows the way in which art or writing enables the subject to confront traumatic memory. While the death drive propels Plath towards destruction, artistic formation serves to alleviate her psychic crisis. What I shall suggest in the paper is how works of art lead the melancholic subject to challenge traumatic events. Plath herself suggests the therapeutic power of language. Plath's hostility toward women as well as men situates her work nearer to the Kristevan psychoanalytic theory which examines depressive anxieties intrinsically linked to the loss of maternal objects. Kristeva's particular focus on the concept of "death-bearing mother" or the unnamable offers a fruitful reading of the representation of infantile fantasies, sexuality, anger, and ambivalence toward lost loved object which clearly dominates most of Plath's poems. Kristeva elaborates mourning and melancholia through the framework of signification and it is of especial relevance in deciphering the recurring death drive and melancholic rage in Plath's work. Melancholic subjects in Plath's work are characterized by an amorphous state, occupying a borderline state regulated by the death drive.
