주변성 법랑모세포종의 증례 (Peripheral ameloblastoma: A case report)

  • Peripheral ameloblastoma is an extremely rare odontogenic soft tissue tumor with histologic characteristics similar to those of the intraosseous ameloblastoma. It appears in the gingiva and oral mucosa. And it usually does not show any bone involvement on radiographs, except for saucer shaped erosion of underlying alveolar bone. Recurrence is considered uncommon. We report a case of peripheral ameloblastoma with bone involvement. Histologically it presented with follicles and nest of tumor cells with palisading pattern. And radiographs showed the typical saucer shaped alveolar bone erosion at the distal area of right mandibular third molar. At 6-month follow-up after operation, no local recurrence was noted.

비골이식술로 치료한 요골 원위부의 거대세포종 - 증례 보고 - (Giant Cell Tumor of the Distal Radius Treated with the Proximal Fibular Graft - A Case Report -)

  • Giant cell tumor was described by Sir Astley Cooper in 1818. This tumor is considered to be a benign tumor but has problems of recurrence and metastatic change after treatment. Methods of operative treatment of this tumor have included currettage, currettage and bone graft, excision, resection, excision and graft and amputation. We experienced a case of giant cell tumor which involved the distal part of right radius and treated by wide excision and fibular graft. The postoperative courses have been satisfactory because of no recurrence or malignant change. After 6 years and 1 month follow up, the patient was able to return to daily life without any problem.

  • Malignant peripheral nerve sheath tumors (MPNSTs) usually arise in soft tissues; they are rarely found in the bone. This paper reports a case of MPNST in the foot and ankle joint involving the distal tibia, talus, calcaneus, navicular, medial intermediate, and lateral cuneiform, cuboid, and 2nd to 4th metatarsal bone. Palliative treatment was performed. The authors encountered a patient with intraosseous MPNST of the midfoot who presented with nonspecific clinical and radiologic findings. This case shows that a high index of suspicion and a histopathology examination, including immunohistochemistry, will be necessary for an accurate diagnosis.

우측 관골체부에 발생한 골연골종의 치험례 (A case report of Osteochondroma Occuring on the Right Zygoma body)

  • Purpose: An osteochondroma, also known as osteocartilaginous exostosis, is a common bone tumor, but rarely occurs in the facial bone, especially in the zygoma body. Because most of the craniofacial bones develop from intramembranous ossification, osteochondromas are relatively infrequent in mesenchymal bones of the head and neck. The osteochondroma of the facial bone is a slow growing, painless mass, causes facial asymmetry. In spine and other extremities, it rarely changes malignant, but untill now, there is no evidence of malignant change in facial bone. We herein describe a rare case of osteochondroma occuring on zygoma body with review of the literature. Method: A 50 - year - old male has painless, slowly growing mass on a right cheek for several years. For a diagnosis, CT and whole body bone scan were done and a diagnosis, osteochondroma was made. The tumor was removed with osteotome under general anesthesia. Result: Radiography showed a well - defined calcified mass attatched to the anterior aspect of the right zygoma body. And pathologic exam showed degenerative chondocyte and cancellous bone. As a result, these appearance is that of an osteochondroma. Conclusion: An osteochondroma is a common bone tumor, but rarely occurs in the facial bone. To the authors knowldege, this is the first case of osteochondroma occuring on zygoma in korea, body. For this case, we reviewed literature related to this topic.

Analysis of the Expression and Regulation of PD-1 Protein on the Surface of Myeloid-Derived Suppressor Cells (MDSCs)

  • Myeloid-derived suppressor cells (MDSCs) that are able to suppress T cell function are a heterogeneous cell population frequently observed in cancer, infection, and autoimmune disease. Immune checkpoint molecules, such as programmed death 1 (PD-1) expressed on T cells and its ligand (PD-L1) expressed on tumor cells or antigen-presenting cells, have received extensive attention in the past decade due to the dramatic effects of their inhibitors in patients with various types of cancer. In the present study, we investigated the expression of PD-1 on MDSCs in bone marrow, spleen, and tumor tissue derived from breast tumor-bearing mice. Our studies demonstrate that PD-1 expression is markedly increased in tumor-infiltrating MDSCs compared to expression in bone marrow and spleens and that it can be induced by LPS that is able to mediate $NF-{\kappa}B$ signaling. Moreover, expression of PD-L1 and CD80 on $PD-1^+$ MDSCs was higher than on $PD-1^-$ MDSCs and proliferation of MDSCs in a tumor microenvironment was more strongly induced in $PD-1^+$ MDSCs than in $PD-1^-$ MDSCs. Although we could not characterize the inducer of PD-1 expression derived from cancer cells, our findings indicate that the study on the mechanism of PD-1 induction in MDSCs is important and necessary for the control of MDSC activity; our results suggest that $PD-1^+$ MDSCs in a tumor microenvironment may induce tumor development and relapse through the modulation of their proliferation and suppressive molecules.

흉벽 연부조직에 발생한 전이성 거대세포종 - 1예 보고 - (A Metastatic Giant Cell Tumor of the Soft Tissue of the Thoracic Wall - A case report -)

  • 골조직에 발생하는 거대세포종은 국소적으로는 양성이지만 재발률이 매우 높고, 아주 드물게 원격전이를 일으킬 수 있다. 우측 요골에 발생한 거대세포종이 치료 후 재발되어, 요골을 제거한 후, 다시 동측 흉벽 연부조직에 전이된 29세의 남자환자를 보고하는 바이다. 종양은 흉벽의 골조직과는 연관이 없었다. 수술은 주위 연부조직과 함께 광범위 절제하였으며, 조직학적 검사상 악성 종양을 시사하는 소견은 없었다.

전두골에 발생한 호산구성육아종의 치험례 (A Case Report of Eosinophilic Granuloma in the Frontal Bone)

  • Purpose : Eosinophilic granuloma is a rare benign tumor that is characterized histologically by the presence of destructive granulomas containing numerous Langerhans, cells. The most common presentation of eosinophilic granuloma is a painful, immobile scalp mass in the frontal and parietal bones occurring predominantly in children and adolescents or young adults. We report a representative case of eosinophilic granuloma. Methods : A 16-year-old woman complained of an enlarging fixed scalp mass without pain and tenderness which measured $3{\times}4.5cm$ at the frontal area, which had been found incidentally 2 months before. Plain skull x-ray showed a punched-out bone lesion. Computed tomography and magnetic resonance imaging showed a non-enhancing osteolytic lesion. The tumor and surrounding bony edges were completely removed via a bicoronal approach. The bony defect was reconstructed with bone cement. Results : The tumor was involved frontal bone and dura mater. We confirmed the tumor by the documentation of Birbeck's granules by electron microscopy. There is no evidence of local recurrence during postoperative 1.5 years. Conclusion : The present case shows the characteristic feature of frontal bone involvment of the eosinophilic granuloma. The prognosis of eosinophilic granuloma depend on age at diagnosis and number of bones involved. We consider that best choice of treatment for eosinophilic granuloma is surgical excision.

Tumor necrosis factor-α에 의한 골수 유래 중간엽 줄기세포의 골세포로의 분화 촉진에서 JNK의 역할 (Tumor Necrosis factor-α Promotes Osteogenesis of Human Bone Marrow-derived Mesenchymal Stem Cells through JNK-dependent Pathway)

  • Tumor necrosis $factor-{\alpha}\;(TNF-{\alpha})$는 염증성 골질환에서의 골조직의 손실과 밀접한 관련이 있다. 본 연구에서는 인체 골수 유래 중간엽 줄기세포의 골세포로의 분화과정에 대한 $TNF-{\alpha}$의 영향을 조사하였다. $TNF-{\alpha}$는 골수 유래 중간엽 줄기세포의 골세포로의 분화를 나타내는 표시인 세포외 무기질 축적과 alkaline phosphatase의 발현의 증가를 일으켰으며 2ng/ml의 농도에서 최대의 증가를 나타내었다. $TNF-{\alpha}$에 의한 골세포로의 분화는 $NF_kB$의 저해제에 의해서는 영향받지 않았으나 JNK 특이 저해제인 SP600125에 의해 완벽하게 억제되었다. 이는 $TNF-{\alpha}$에 의한 골수 유래 중간엽 줄기세포의 골세포로의 분화과정에 JNK가 중요한 역할을 한다는 것을 제시한다.
