치근접촉이 마이크로 임플란트 인접골 응력에 미치는 영향에 대한 유한요소해석 (Finite element analysis of peri-implant bone stresses induced by root contact of orthodontic microimplant)

  • 마이크로 임플란트 시술의 중요한 위험요인 중 하나로 치근접촉 문제가 있으나, 관련 연구는 결과 분석에 치중되어 있고, 치근접촉이 마이크로 임플란트 안정성 상실로 이어지는 기전에 대한 연구는 아직 미흡한 것으로 보인다. 이에, 본 연구에서는 생역학적 측면에서 그 영향을 분석하였다. Absoanchor 마이크로 임플란트(SH1312-7, Dentos Inc., Daegu, Korea) 첨부가 치근에 접촉되어 있을 때, 저작압 전달에 의한 마이크로 임플란트 변위가 인접골에 가하는 압축응력을 축대칭 유한요소모델을 사용하여 계산하였다. 요소별 응력이 해면골의 최대압축강도나, 치밀골의 비정상 골개조 임계능력을 넘을 경우 해당 요소를 순차적으로 해석모델에서 제거하며 실행한 6단계해석의 결과, 마이크로 임플란트에 인접한 해면골의 전체적인 파절과 과부하에 따른 치밀골의 비정상 골개조가 임플란트 지지력 상실에 주 요인이 될 것으로 평가되었다. 치밀골의 과부하 영역은 초기에는 치밀골판의 하부에 존재하였으나 상부로 확장되었고, 응력 재분포로 인한 감소효과 없이 양성 되먹임(positive feedback)으로 결국 치밀골 전 두께로 확대됨을 관찰하였다. 본 연구를 통해 치근접촉된 마이크로 임플란트가 인접골을 훼손시켜 안정성 상실로 이어지는 과정을 모사할 수 있었으며, 이로부터 치근접촉에 따른 마이크로 임플란트의 불량한 예후에 대한 생역학적 측면의 원인을 파악할 수 있었다.

생체 내 경골의 피질골에서 시간역전 램파의 군속도와 피질골 특성 사이의 상관관계 (Correlations between the group velocity of time-reversed Lamb waves and cortical bone properties in tibial cortical bone in vivo)

  • 노화 및 골다공증으로 인한 경골 또는 요골과 같은 긴 뼈의 외부 껍질을 구성하는 피질골의 골강도 변화는 골절의 위험인자로 알려져 있다. 본 연구에서는 시간역전법을 이용하여 생체 내 경골의 피질골에서 발생된 시간역전 램파의 군속도를 측정하고, 군속도가 피질골의 골강도와 밀접한 관련이 있는 cortical bone thickness(cTh) 및 cortical bone mineral density(cBMD)와 갖는 상관관계를 조사하였다. 7명의 실험대상자의 우측 경골에서 측정된 시간역전 램파의 군속도는 cTh와 r = 0.90(p < 0.0001)의 매우 높은 상관관계를 보이며, cBMD와는 r = 0.69(p < 0.0001)의 상대적으로 낮은 상관관계를 보이는 것으로 나타났다. 본 생체 내 연구의 한계점은 단지 7명의 건강한 성인으로 구성된 정상군에 대하여 시간역전 램파의 군속도가 측정되었다는 것이다. 향후 다수의 건강한 성인 및 골다공증 환자로 각각 구성된 정상군 및 골다공증군에 대한 후속연구를 통하여 시간역전 램파의 임상적 유용성이 입증된다면 골다공증 진단을 위한 정량적 초음파 기술의 신뢰도를 향상시킬 수 있을 것으로 기대된다. 또한 시간역전 램파의 군속도를 측정하기 위한 골격 부위를 경골뿐만 아니라 대퇴골 또는 요골까지 확장시킬 필요가 있다.

The Efficacy of Transverse Fixation and Early Exercise in the Treatment of Fourth Metacarpal Bone Fractures

  • Background Several techniques have been designed to treat fifth metacarpal fractures reported to be effective. However, these methods cannot be easily applied to the fourth metacarpal due to its central anatomical position. In this study, we sought to analyze the functional outcomes of patients who underwent transverse pinning for a fourth metacarpal bone fracture. Methods A total of 21 patients were selected and their charts were retrospectively reviewed. After fracture reduction, two transverse Kirchner wires were first inserted from the fifth metacarpal to the third metacarpal transversely at the distal part of the fractured bone, and then another two wires were inserted at the proximal part of the fractured bone. The splint was removed approximately one week postoperatively and the Kirchner wires were removed four to five weeks postoperatively. Patients started active and passive exercise one week after the operation. Pain visual analog scores, total active and passive motion, and the active and passive range of motion of the metacarpophalangeal joint and grip strength were evaluated. Results Dorsal angulation improved from a preoperative value of $44.2^{\circ}$ to a postoperative value of $5.9^{\circ}$. Six weeks after surgery, functional recovery parameters, such as range of motion and grip strength, had improved to 98% of the function of the normal side. No major complication was observed. Conclusions We suggest that the transverse pinning of fourth metacarpal bone fractures is an effective treatment option that is less invasive than other procedures, easy to perform, requires no secondary surgery, minimizes joint and soft tissue injury, and allows early mobilization.

Antiosteoporotic activity of ‘Dae-Bo-Won-Chun’ in the ovariectomized rats

  • The preventive effect of herbal formulation, ‘Dae-Bo-Won-Chun’(DBWC), on the progress of bone loss induced by ovariectomy (OVX) was studied in rats. From light microseopic analyses, a porous or erosive appearances were observed on the surface of trabecular bone of tibia in ovariectomized rats. Whereas those of the same bone in sham rats were composed of fine particles. The trabecular bone area and trabecular thickness in ovariectomized rats decreased by 50% from those in sham-operated rats, these decreases were completely inhibited by administration of DBWC at concentration of 10mg/kg per day for 7 weeks. The mechanical strength in femur neck was decreased by ovariectomy, and this was significantly suppressed by the administration of DBWC. Serum phosphorus, alkaline phosphatase, and thyroxine levels in ovariectomized rats were increased compared to those in sham-operated rats, and increases were completely inhibited by the administration of DBWC. These results strongly suggest that DBWC is effective in preventing the development of bone loss induced by ovariectomy in rats.

  • Osteoporosis is a common disorder characterized by reduced bone mineral density, deterioration of the microarchitecture of bone tissue and increased risk of fracture. The aim of treatment of osteoporosis is to maintain and, ideally, to restore bone strength safely. In recent years the role of polypeptide growth factors in bone metabolism has begun to appear. It has been proposed that alterations in the expression or production of growth factor can modulate the proliferation and activity of bone forming cells. Thus, the role of structurally diverse peptides for the management and diagnosis of osteoporosis has attracted the attention of many investigators. This paper reviews numerous findings concerning the use of polypeptides, hormones, and growth factors, for the management of osteoporosis. Many of the compounds mentioned here are experimental prototypes of new therapeutic classes. Though it is unlikely that some of the compounds may ever be used clinically, development of safe and efficacious agents in each class will define the future course of therapy for osteoporosis.

랫드에서 1α,25 dihydroxycholecalciferol의 골다공증성 골절 치유효과 (Therapeutic effects of 1α,25 dihydroxycholecalciferol on osteoporotic fracture in a rat model)

  • Osteoporosis is defined as a decrease in bone mass that leads to an increased risk of fracture. The therapeutic effect of $1{\alpha}$,25 dihydroxycholecalciferol, the hormonal form of vitamin $D_3$ that mediates calcium translation in intestine and bone, on the healing process of fracture has still been controversial. These studies were designed to understand the healing process of normal fibular fracture, the osteoporotic changes after ovariectomy, and the therapeutic effects of $1{\alpha}$,25 dihydroxycholecalciferol on the osteoporotic fracture in rats. The simple transverse fractures of rat fibulae were produced with a rotating diamond saw. The changes of the biochemical and mechanical indices of rats were investigated. The mechanical study based on bending test revealed the healing of the fibular fracture in the 5th week after simple transverse fracture. The osteoporosis impaired more the healing of osteoporotic fibular fracture than normal non-osteoporotic fibular fracture. The healing process of osteoporotic fracture was facilitated by the treatment with $1{\alpha}$,25 dihydroxycholecalciferol, however, was delayed more than the healing process of normal fracture. The bone strength based on the bending test also confirmed this tendency. The bone strengths in the 5th week after fracture of normal bone, osteoporotic bone, and $1{\alpha}$,25 dihydroxycholecalciferol-treated osteoporotic bone were 75%, 41%, and 67%, respectively, in comparison with those of intact bone. In conclusion, $1{\alpha}$,25 dihydroxycholecalciferol was effective in promoting the osteoporotic fracture healing.

Fabrication of Artificial Bone through the Imitation of human bone

  • In this work, HAp-(t-ZrO2) ceramic composites of biomimic artificial bone were fabricated by multi-extrusion process in order to replace nature bone. HAp-(t-ZrO2) and graphite powders were mixed separately with ethylene vinyl acetate (EVA) and stearic acid using shear mixer. Extruded HAp-(t-ZrO2) filaments and carbon filaments were arranged in the die to fabricate the first pass filament. The first pass filaments were arranged in the same die with a central carbon core for making the space for spongy bone. Burning out and sintering processes were performed to remove the binder and lubricant. The microstructure channel diameter was researched around $300{\mu}m$. Microstructure analysis was carried out by OM, SEM, and $\mu$-CT. Compressive strength was investigated for the artificial bone. Some preliminary bio-compatibility test was evaluated.

척추성형술에서 PMMA 주입에 의한 망상골의 압축강도 변화 연구 (The Study of Changes in Compressive Strength of Trabecular Bone with PMMA Injection in Vertebroplasty)

  • 골다공증에 의한 척추체의 압박골절은 뼈의 파단성 질병중에서 가장 흔하게 나타나는 증상이다 최근에 척추압박골절에 대한 의학적 치료방법으로 각광받고 있는 척추성형술은 골강화제를 주입함으로써 척추체를 강화하고 변형을 보정하며. 환자의 고통을 줄여준다. 그러나 종전 연구에서는 골강화제가 주입된 후 척추체의 기계적 특성 강화에 대한 연구가 전무했으므로, 본 연구에서는 PMMA의 물성을 바탕으로 돼지와 사체로부터 시편을 얻어 겉보기 밀도에 대한 순수 망상골과 PMMA가 주입된 망상골의 영률을 측정하였다. 겉보기밀도에 따른 영률은 순수망상골의 경우 멱급수의 형태를 가지며. PMMA가 주입된 망상골의 경우 선형적인 형태를 가졌다.

개에서 Poly-L-lactic-acid 이식물의 생분해성과 생체적합성에 관한 연구 (Study of Bio-absorbability and Bio-compatibility of Poly-L-lactic-acid Implant in Dogs)

  • Bioabsorbable devices have been utilized and experimented in many aspects of orthopaedic surgery. Depending upon their constituent polymers, these materials can be tailored to provide sufficient rigidity to allow bone healing, retain mechanical strength for certain period of time, and then eventually begin to undergo degradation. The objective of this study was to estimate extent in which Poly-L-latic acid (PLLA) implants had bioabsorbability and biocompatibility with bone and soft tissue in dogs and also to develop bioabsorbable, biocompatible materials with the appropriate strength and degradation characteristics to allow for regular clinical use for treating orthopedic problems in humans as well as animals. Eighteen dogs were used as experimental animals and were inserted two types of PLLA implants. PLLA rods were inserted into subcutaneous tissue of back or the abdomen wall. And the rods were tested for material properties including viscosity, molecular weight, melting point, melting temperature, crystallinity, flexural strength, and flexural modulus over time. PLLA screws were inserted through cortical bone into bone marrow in the femur of the dogs and stainless steel screw was inserted in the same femur. Radiographs were taken after surgery to observe locations of screw. Histological variations including cortical bone response, muscular response, bone marrow response were analyzed over the time for 62weeks. The physical properties of PLLA rods had delicate balances between mechanical, thermal and viscoelastic factors. PLLA screws did not induce any harmful effects and clinical complications on bone and soft tissue for degradation period. These results suggest that PLLA implants could be suitable for clinical use.

일개 농촌지역 주민의 체구성, 운동 및 체력상태와 골밀도와의 관계 (A Study on the Relationship between Body Composition, Exercise Status, Fitness Status and Bone Mineral Density in Some Rural Residents)

  • 본 논문은 일개 농촌주민들을 대상으로 신체구성, 운동(과거 운동여부), 체력상태, 골밀도를 조사하고, 골밀도와의 관련된 요인을 파악하여 농촌 주민들의 골다공증 예방을 위한 기초자료를 제시하고자 143명을 대상으로 연구하였고, 결과로서 조사대상자들 연령이 증가할수록, 교육수준이 낮을수록 골밀도가 낮았으며(p<0.05), 만성질환이 있는 경우는 없는 경우보다 골밀도가 낮았다(p<0.05). 그리고, 체중 BMI 체지방량 제지방량이 많이 나가는 경우가 낮은 경우보다 골밀도가 유의하게 높았으며(p<0.01), 과거 운동을 했던 경우와 기초 대사량이 높은 경우, 근육량이 많은 경우 골밀도가 높았다(p<0.01). 또한, 악력 윗몸일으키기 팔굽혀펴기 등 체력이 좋은 경우 골밀도가 높았으며(p<0.01), 골밀도 수치를 종속변수로 한 다중 회귀분석결과 연령, 만성질환 유무, 과거 운동유무가 유의한 변수로 나타났다.
