Curcumin Inhibits Osteoclastogenesis by Decreasing Receptor Activator of Nuclear Factor-κB Ligand (RANKL) in Bone Marrow Stromal Cells

  • Curcumin (diferuloylmethane), a pigment derived from turmeric, has anti-oxidant and anti-inflammatory activities. Accumulating evidence points to a biochemical link between increased oxidative stress and reduced bone density. Osteoclast formation was evaluated in co-cultures of bone marrow stromal cells (BMSC) and whole bone marrow cells (BMC). Expression of receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL) was analyzed at the mRNA and protein levels. Exposure to curcumin led to dose-dependent suppression of osteoclastogenesis in the co-culture system, and to reduced expression of RANKL in $IL-1{\alpha}$-stimulated BMSCs. Addition of RANKL abolished the inhibition of osteoclastogenesis by curcumin, whereas the addition of prostaglandin $E_2$ ($PGE_2$) did not. The decreased osteoclastogenesis induced by curcumin may reduce bone loss and be of potential benefit in preventing and/or attenuating osteoporosis.

항과립구 항체 골수스캔을 이용한 다발성 골수종 병변의 평가: 단순골X-선점사 및 골스캔과의 비교 (Bone Marrow Scintigraphy with Antigranulocyte Antibody in Multiple Myeloma: Comparison with Simple Radiography and Bone Scintigraphy)

  • 목적: 단순골X-선검사와 골스캔은 다발성 골수종 병변의 조기검출에 많은 제한점을 가진다. 본 연구는 다발성 골수종에서 방사능 표지 항백혈구항체를 이용한 골수스캔의 유용성을 평가하기 위해 실시되었다. 대상 및 방법: 다발성 골수종 환자 22례의 경우 남녀비는 2.1:1이었고 중간연령은 57세였으며, Stage II는 3례, Stage III는 19례였다. 골수스캔은 $^{99m}Tc$으로 표지된 항백혈구 항체를 이용하여 전신상을 촬영하였고, 그 결과를 단순골X-선검사 및 골스캔과 부위별로 비교하였다. 결과: 단순골X-선검사는 14명(64%), 골스캔에서는 11명(50%)의 환자에서 병변을 검출한 반면, 골수스캔에서는 19명(86%)의 환자에서 국소결손병변을 검출하였다. 골수스캔은 Stage II에서는 33%의 환자(1/3)에서 병변을 검출하였고, Stage III에서는 90%의 환자(17/19)에서 병변을 검출하였다. 골수스캔에서 골수확장의 소견은 68% (15/22)에서 관찰되었다. 전체환자 22명에서 124개의 국소결손병변이 단순골X-선 검사, 골스캔, 골수스캔에서 검출되었는데, 단순골X-선검사는 58개 병변을, 골스캔은 40개 병변을 검출한 반면 골수스캔을 통해서는 92개 병변을 검출하였고, 단순골X-선검사나 골스캔에서는 검출하지 못한 국소결손병변을 51개나 더 검출할 수 있었으며, 이는 특히 새로운 흉요추 국소결손병변의 검출에 도움이 되었다. 결론: 골수스캔은 단순골X-선검사나 골스캔보다 높은 검출율을 보였고, 흉요추병변의 검출에 많은 도움을 줄 수 있는 것으로 생각된다. 또한 골수스캔은 진행한 병기에서 더 높은 검출율을 보여주어 골수스캔소견과 임상적 병기간에 상관성을 가지는 것으로 생각된다. 다발성 골수종에서의 골수스캔은 새로운 병변의 검출뿐만 아니라 병기정도의 평가에 도움이 될 것으로 판단된다.

로지스틱 回歸分析을 이용한 癌의 骨髓轉移에 대한 判定基準 決定 (On the decision rule of bone marrow metatasis of cancer using logistic regression analysis)

  • 癌 환자에서 癌의 骨髓轉移 여부를 판정하는 것은 임상적으로 매우 중요하다. 癌의 骨髓轉移에 대한 說明變數 을 찾기 위하여 癌의 骨髓轉移가 있는 ? 60 例와 公水轉移가 없는 환자 41 例를 대상으로 로지스틱 回歸分析을 시도하였다. 上記 資料는 1977년 1월부터 1985년 12월까지 연세대학교 의과대학 부속 세브란스 병원의 기록을 後向的으로(retrospectively) 조사하여 수집되었다. 가장 적합도가 높은 로지스틱 回歸分析의 說明變數를 기초로 하여 임상적으로 적용이 편리한 癌의 骨髓轉移에 대한 判定基準을 構成하였고, 이러한 判定基準의 銳敏度(sensitivity)와 特異度(specificity)도 계산되었다.

Tuberculosis-associated hemophagocytic lymphohistiocytosis in adolescent diagnosed by polymerase chain reaction

  • We present a case of tuberculosis-associated hemophagocytic lymphohistiocytosis in a 14-year-old girl. The patient presented with weight loss, malaise, fatigue, prolonged fever, and generalized lymphadenopathy. Laboratory investigation revealed pancytopenia (white blood cells, $2,020cells/{\mu}L$; hemoglobin, 10.2 g/dL; platelets, $52,000cells/{\mu}L$), hypertriglyceridemia (229 mg/dL), and hyperferritinemia (1,420 ng/mL). Bone marrow biopsy showed a hypocellular bone marrow with a large numbers of histiocytes and marked hemophagocytosis; based on these findings, she was diagnosed with hemophagocytic lymphohistiocytosis. Polymerase chain reaction (PCR) with both the bone marrow aspiration and sputum samples revealed the presence of Mycobacterium tuberculosis. Antitubercular therapy with immune modulation therapy including dexamethasone and intravenous immunoglobulin was initiated. The results of all laboratory tests including bone marrow biopsy and PCR with both the bone marrow aspiration and sputum samples were normalized after treatment. Thus, early bone marrow biopsy and the use of techniques such as PCR can avoid delays in diagnosis and improve the survival rates of patients with tuberculosis-associated hemophagocytic lymphohistiocytosis.

Propranolol attenuates calorie restriction- and high calorie diet-induced bone marrow adiposity

  • We investigated the effects of ${\beta}$-adrenergic activation on bone marrow adiposity and on adipogenic differentiation of bone marrow mesenchymal stem cells (BMSCs). C57BL/6 mice were subjected to a control (CON), high calorie (HIGH) or low calorie (LOW) diet for 12 weeks. In each group, mice were treated with vehicle (VEH) or propranolol. The number of adipocytes per area bone marrow was increased in LOWVEH and HIGHVEH mice compared with CONVEH mice, which was attenuated by propranolol. Isoproterenol increased lipid droplet accumulation and adipogenic marker gene expression in 3T3-L1 preadipocytes and mouse BMSCs, which were blocked by propranolol. Conditioned medium obtained from MC3T3-E1 osteoblasts suppressed adipogenic differentiation of 3T3-L1 cells, which was significantly attenuated by treatment of MC3T3-E1 cells with isoproterenol. These data suggest that ${\beta}$-adrenergic activation enhances bone marrow adipogenesis via direct stimulation of BMSCs adipogenesis and indirect inhibition of osteoblast anti-adipogenic potential.

A Study of Gastrointestinal and Bone Marrow Adverse Events after Administration of Various Chemotherapeutic Agents in Canine Malignant Tumor Patients

  • The purpose of this present study was to objectively evaluate gastrointestinal and bone marrow AEs after administration of various chemotherapeutic agents in canines with malignant tumors, using the Veterinary Cooperative Oncology Group-Common Terminology Criteria for Adverse Events (VCOG-CTCAE), which includes descriptive terminology used for adverse events (AEs) reported in dogs and cats. The medical records of 42 dogs with malignant tumor that underwent chemotherapy were reviewed retrospectively. There were no significant differences in the prevalence of gastrointestinal AEs among the 5 chemotherapeutic agents (vincristine, cyclophosphamide, doxorubicin, lomistine, and carboplatin). The prevalence of bone marrow AEs was significantly higher after administration of lomustine than after administration of vincristine or doxorubicin. Grade 1 AEs of the gastrointestinal tract and bone marrow were most often observed after administration of various chemotherapeutic agents. Delayed and cumulative myelosuppression of lomustine in some dogs receiving regular blood examination were identified. The findings of this study will help predict possible gastrointestinal and bone marrow AEs due to the use of chemotherapeutic agents to treat canines with malignant tumors.

Focal Bone Marrow Lesions: A Complication of Ultrasound Diathermy

  • Ultrasound diathermy is widely used for the treatment of musculoskeletal disorders and other soft tissue injuries. Its use as a therapeutic modality is believed to be safe, with very few reported complications. Here, we report two patients who developed focal bone marrow abnormalities after receiving ultrasound diathermy. Both patients' magnetic resonance (MR) evaluations revealed linear subchondral bone lesions of the superolateral humeral head similar to those in osteonecrosis. The patients' symptoms subsequently improved, and available follow-up MR evaluation revealed near complete resolution of bone lesions. These findings suggest that ultrasound diathermy, and its interaction with bone tissue through thermal mechanisms, can cause focal bone marrow abnormalities. Furthermore, the bone marrow abnormalities seem to be transient, resolving upon cessation of ultrasound diathermy, therefore osteonecrosis should be differentiated from this temporal lesion.

혈액 종양 질환에서 항과립구항체 골수 스캔을 이용한 중심 골수와 말초 골수 분포의 분석 (Central and Peripheral Distribution of Bone Marrow on Bone Marrow Scintigraphy with Antigranulocytic Antibody in Hematologic Malignancy)

  • 목적: 혈액 종양 질환을 가진 한자들에서 과립구에 대한 단일클론항체를 이용한 골수 스캔을 시행하여 혈액 종양 질환의 종류에 따라서 말초 골수의 분포를 등급화하고 중심성 골수의 정량적 섭취율을 구하였으며 또한 이들 상호간의 관련성을 조사하였다. 대상 및 방법: 혈액 종양 환자 중에서 골수 조직검사를 시행하여 진단이 확진되고 Tc-99m antigranulocyte monoclonal mouse antibody BW 250/183를 이용한 골수 스캔을 시행한 환자 50명을 대상으로하였다. 환자는 4군으로서 급성 골수성 백혈병 환자 11명, 급성 임파구성 백혈병 환자 12명, 임파선종 환자 15명, 골수이형성증후군 환자 12명이었다. 골수 스캔의 분석은 말초 골수 분포는 골수 확장 소견에 따라 4개의 등급으로 분류하였고, 중심성 골수 기능은 장골과 천골에 관심영역을 그려 섭취율을 계산하였다. 결과: 말초 골수 확장의 정도는 전체 환자 50명 중에서 1등급에 4명, 2등급에 27명, 3등급에 15명, 4등급에 4명 이었다. 말초 골수 확장의 정도는 골수이형성증후군과 임파구성 백혈병에서 3, 4등급이 58% (7/12)로 심하였고 임파종에서는 1, 2등급이 93% (14/15)로 가장 경하였으며 질환별로 특이적인 분포는 보이지 않았다. 중심성 골수의 섭취율은 4가지 질환 중 골수이형성증후군에서 $8.5{\pm}4.0$으로 가장 높았고, 급성 임파구성 백혈병에서 $5.9{\pm}3.6$으로 가장 낮았으나 4군 간에 통계적으로 유의한 차이는 없었다(p>0.05). 말초 골수 화장 정도에 따른 중심 골수의 정량화는 4가지 등급간에는 섭취율에 있어서 통계적으로도 유의한 차이가 있었으나(p=0.003), 개개의 환자에서 말초 골수 확장 정도와 중심 골수의 정량값의 상관관계는 거의 없었다(r=0.05). 결론: 혈액 종양 질환에서 골수 스캔은 말초 골수의 분포와 중심 골수의 기능에 있어서 질병 특이적인 소견은 보이지 않았으며, 골수의 변화는 말초 골수와 중심 골수에 서로 상관 관계없이 변화를 초래하므로 한 가지 방법으로 보다는 두 가지 방법으로 평가할 필요가 있다.

A ramus cortical bone harvesting technique without bone marrow invasion

  • Autogenous bone grafts from the mandibular ramus are a known source of inadequate bone volume scenarios of the residual alveolar ridge. However, the conventional block-type harvesting technique cannot prevent bone marrow invasion, which can cause postoperative complications such as pain, swelling, and inferior alveolar nerve injury. This study aims to suggest a complication-free harvesting technique and present the results of bone grafting and donor sites. One patient received two dental implants with a complication-free harvesting technique that involves creation of ditching holes with a 1 mm round bur. Sagittal, coronal, and axial osteotomies produced grid-type cortical squares using a micro-saw and a round bur to confirm the cortical thickness. The grid-type cortical bone was harvested from the occlusal aspect, and the harvesting was extended through an additional osteotomy on the exposed and remaining cortical bone to prevent bone marrow invasion. The patient did not suffer postoperative severe pain, swelling, or numbness. After 15 months, the harvested site exhibited new cortical bone lining, and the grafted area had healed to a cortico-cancellous complex with functional loading of the implants. Our technique, grid-type cortical bone harvesting without bone marrow invasion, allowed application of autogenous bone without bone marrow invasion to achieve acceptable bone healing of the dental implants and to regenerate the harvested cortical bone.

A magnetic resonance imaging study on changes in rat mandibular bone marrow and pulp tissue after high-dose irradiation

  • Purpose: This study was designed to evaluate whether magnetic resonance imaging (MRI) is appropriate for detecting early changes in the mandibular bone marrow and pulp tissue of rats after high-dose irradiation. Materials and Methods: The right mandibles of Sprague-Dawley rats were irradiated with 10 Gy (Group 1, n=5) and 20 Gy (Group 2, n=5). Five non-irradiated animals were used as controls. The MR images of rat mandibles were obtained before irradiation and once a week until week 4 after irradiation. From the MR images, the signal intensity (SI) of the mandibular bone marrow and pulp tissue of the incisor was interpreted. The MR images were compared with the histopathologic findings. Results: The SI of the mandibular bone marrow had decreased on T2-weighted MR images. There was little difference between Groups 1 and 2. The SI of the irradiated groups appeared to be lower than that of the control group. The histopathologic findings showed that the trabecular bone in the irradiated group had increased. The SI of the irradiated pulp tissue had decreased on T2-weighted MR images. However, the SI of the MR images in Group 2 was high in the atrophic pulp of the incisor apex at week 2 after irradiation. Conclusion: These patterns seen on MRI in rat bone marrow and pulp tissue were consistent with histopathologic findings. They may be useful to assess radiogenic sclerotic changes in rat mandibular bone marrow.
