Bio-Conjugated Polycaprolactone Membranes: A Novel Wound Dressing

  • Background The combination of polycaprolactone and hyaluronic acid creates an ideal environment for wound healing. Hyaluronic acid maintains a moist wound environment and accelerates the in-growth of granulation tissue. Polycaprolactone has excellent mechanical strength, limits inflammation and is biocompatible. This study evaluates the safety and efficacy of bio-conjugated polycaprolactone membranes (BPM) as a wound dressing. Methods 16 New Zealand white rabbits were sedated and local anaesthesia was administered. Two $3.0{\times}3.0cm$ full-thickness wounds were created on the dorsum of each rabbit, between the lowest rib and the pelvic bone. The wounds were dressed with either BPM (n=12) or Mepitel (n=12) (control), a polyamide-silicon wound dressing. These were evaluated macroscopically on the 7th, 14th, 21st, and 28th postoperative days for granulation, re-epithelialization, infection, and wound size, and histologically for epidermal and dermal regeneration. Results Both groups showed a comparable extent of granulation and re-epithelialization. No signs of infection were observed. There was no significant difference (P>0.05) in wound size between the two groups. BPM (n=6): $8.33cm^2$, $4.90cm^2$, $3.12cm^2$, $1.84cm^2$; Mepitel (n=6): $10.29cm^2$, $5.53cm^2$, $3.63cm^2$, $2.02cm^2$; at the 7th, 14th, 21st, and 28th postoperative days. The extents of epidermal and dermal regeneration were comparable between the two groups. Conclusions BPM is comparable to Mepitel as a safe and efficacious wound dressing.

Immunomodulatory activities of ethanolic extract of Drynariae Rhizoma (골쇄보(骨碎補) ethanol 추출물의 면역 조절 작용에 관한 연구)

  • In the traditional Chinese medicine, Drynariae Rhizoma (DR) has been reported as a good enhancer for bone healing. DR, a plant widely used in the traditional medicinal systems of Korea, has been reported to possess antiviral, antibacterial and anti-inflammatory activities. Modulation of immune response to alleviate disease has been of interest for a long time. Plant extracts have been widely investigated for possible immunomodulatory properties. Thus, I have evaluated the anticellular and immunomodulatory properties of ethanolic extract of DR. DR extract inhibited proliferation of mitogen (phytohaemagglutinin; PHA) and antigen (purified protein derivative; PPD)-stimulated human peripheral blood mononuclear cells (PBMCs). In addition, DR inhibited growth of several cell lines of mouse and human origin. It also inhibited production of nitric oxide (NO), interleukin-2 (IL-2) and tumor necrosis $factor-{\alpha}\;(TNF-{\alpha})$. Intracytoplasmic $interferon-{\gamma}\;(IFN-{\gamma})$ and expression of cell surface markers, CD16 and HLA-DR, on human PBMC, were not affected on treatment with DR but CD25 expression was down regulated. This study demonstrates the antiproliferative and immunosuppressive potential of ethanolic extract of DR in vitro.

Clinical Investigation of Desmoid Tumors (유건종(類腱腫)의 임상적 고찰)

  • A desmoid tumor is a locally aggressive growth of connective tissue origin which infiltrates the surrounding tissue and has a marked tendency for recurrence. And so it was also called as an aggressive fibromatosis, musculofascial fibromatosis or fibrosarcoma etc. Thirteen cases of desmoid tumor was treated since 1980, and their retrospective study was done with 79 months of follow-up after initial surgical excision. The female was involved in 12 cases(one male) with the age ranged from 7-50 years, average 28 years, and seven patients in third decade. A slowly growing mass was excised on average 4 months after first notice of the mass, but their margins are not demarcated clearly in most cases. Wide excision in 12 cases was done, but wide excision and saphenous vein graft was performed in one case because of invasion of posterior tibial artery by tumor mass. The tumor was found on extraabdominal region in 8 cases(61.5%) but 5 cases in abdominal wall(38.5%). The recurrence rate was high(6/13, 46.2%), and 11 times in 6 patients were recurred(average 1.8 times), within 27 months of initial excision. Six cases of recurrence were treated with wide excision again in 3 cases, wide excision combined with radiotherapy(4,000-6,000cGy) in 4 cases and wide excision with chemotherapy in one case. During the follow-up for average 21 months after treatment, no recurrences are found. Tumor remission periods without recurrence are average 67 months in all, and 11 years in longest case. Histologically it was very mimic with fibrosarcoma but could be differentiated with Trichrome stain, and their findings are not changed after recurrence.

Microbiology and Antibiotic Selection for Diabetic Foot Infections (감염된 당뇨병성 족부 병변의 균주 조사 및 항생제 선택)

  • Purpose: To help the empirical antibiotics selection in diabetic foot infection patients, we investigated prevalence of microorganisms and their antibiotics sensitivity results. Materials and Methods: Patients who came to our clinics to treat diabetic foot infections with deep ulceration and were followed up more than 6 months until complete recovery were adopted. From March 2006 to June 2009, there were 140 patients who corresponded with such a inclusion criteria. Wound cultures were done by deep tissue or bone debris at first visit to our clinics. Microorganisms which was documented by wound culture and most susceptible antibiotics by minimum inhibitory concentrations were surveyed retrospectively. Results: Microorganisms were confirmed in 113 cases (80.7%). In the other 27 cases (19.3%), there were no cultured microorganisms. In bacterial growth group, there were 72 cases (63.7%) of gram-positive bacteria and 41 cases (36.3%) of gram-negative bacteria. All of them were aerobic microorganisms and there were no anaerobic microorganisms. Methicillin-sensitive staphylococcus aureus was the most common pathogen and accounted for 35 cases (31.0%). As other common pathogens, there were Enterobacter cloacae (11 cases, 9.7%), pseudomonas aeruginosa (10 cases, 8.8%), Methicillin-resistant staphylococcus aureus (10 cases, 8.8%) and enterococcus faecalis (6 cases, 5.3%), and so on. Common susceptible antibiotics in gram positive microorganism were vancomycin (60 cases, 83.3%), teicoplanin (60 cases, 83.3%), nitrofurantoin (60 cases, 83.3%) and ciprofloxacin (53 cases, 73.6%). In gram negative ones, common susceptible antibiotics were imipenem (35 cases, 85.3%), piperacillin/tazobactam (33 cases, 80.5%) and gentamicin (31 cases, 75.6%). Conclusion: Methicillin-sensitive Staphylococcus aureus in gram positive and enterobacter cloacae in gram negative was the most common pathogen in each group. Ciprofloxacin and gentamicin might be adaptable as a first-line empirical antibiotics in infected diabetic foot patients.

Root zone environments in two cropping system within a year for Kyoho grapes (포도 '거봉'의 2기작 재배에서 근권환경 특성)

  • This study was performed to investigate the behaviour of root zone environments under the control of soil temperature and tension of soil moisture near the root Bone of 'Kyoho' grapes tree grown on restricted root zone system in plastic greenhouse. Maximum diurnal air temperature inside plastic greenhouse ranged between 25.1 and 32.7$^{\circ}C$, and the average of nocturnal air temperature inside plastic greenhouse maintained at 18$^{\circ}C$ in winter season. Also the minimum diurnal relative humidity ranged between 50 and 55%, and the maximum nocturnal relative humidity ranged between 84 to 87%. At a depth of 15cm from soil surface, the average soil temperature maintained at 25.6$^{\circ}C$ for under-ground heating, and appeared to 17.4$^{\circ}C$ for unheated condition. Although the tension of soil moisture just after irrigation sharply decreased to pF 1.5, the tension of soil moisture at the depth of 15cm maintained at pF 2.0~2.2. It is suggested that the tension of soil moisture at the depth of 15cm might be used as the standard for the determination of irrigation set point. Effective drainage system is needed to prevent the spindly and succulent growth of vine trees grown in restricted root zone system.

IL-12 Production and Subsequent Natural Killer Cell Activation by Necrotic Tumor Cell-loaded Dendritic Cells in Therapeutic Vaccinations

  • Background: Immunization of dendritic cells (DCs) pulsed with tumor antigen can activate tumor-specific cytotoxic T lymphocytes (CTL) that are responsible for protection and regression. In this study, we examined whether the uptake of necrotic tumor cells could modulate DC phenotypes and whether the immunization of necrotic tumor cell-loaded DCs could elicit efficient tumor specific immune responses followed by a regression of established tumor burdens. Methods: We prepared necrotic tumor cell-pulsed DCs for the therapeutic vaccination and investigated their phenotypic characteristics, the immune responses induced by these DCs, and therapeutic vaccine efficacy against colon carcinoma in vivo. Several parameters including phagocytosis of tumor cells, surface antigen expression, chemokine receptor expression, IL-12 production, and NK as well as CTL activation were assessed to characterize the immune response. Results: DCs derived from mouse bone marrow efficiently phagocytosed necrotic tumor cells and after the uptake, they produced remarkably increased levels of IL-12. A decreased CCR1 and increased CCR7 expression on DCs was also observed after the tumor uptake, suggesting that antigen uptake could induce DC maturation. Furthermore, co-culturing of DCs with NK cells in vitro enhanced IL-12 production in DCs and IFN-${\gamma}$ production in NK cells, which was significantly dependent on IL-12 production and cell-to-cell contact. Immunization of necrotic tumor cell-loaded DCs induced cytotoxic T lymphocytes as well as NK activation, and protected mice against subsequent tumor challenge. In addition, intratumoral or contra-lateral immunization of these DCs not only inhibited the growth of established tumors, but also eradicated tumors in more than 60% of tumor-bearing mice. Conclusion: Our data indicate that production of IL-12, chemokine receptor expression and NK as well as CTL activation may serve as major parameters in assessing the effect of tumor cell-pulsed DC vaccine. Therefore, DCs loaded with necrotic tumor cells offer a rational strategy to treat tumors and eventually lead to prolonged survival.

  • Unerupted maxillary anterior teeth is not a common case, However it may present practitioners with management problem. The cause of impaction is considered to be multifactorial, and local cause is the most common. These impacted teeth require surgical intervention, removal, transplantation, or surgical exposure, with or without orthodontic traction to align the malpositioned tooth. The preferred option is surgical exposure and orthodontic correction. Surgical intervention and orthodontic correction should not be delayed to avoid unnecessary difficulties in aligning the tooth in the arch. Surgical exposure should be performed with the intent of providing sufficient attached gingiva rather than simply uncovering the crown, which results in only alveolar mucosal attachment. Attached gingiva is essential to secure the gingival tissues to the adjacent teeth at the dentogingival junction. Thus preventing loss of periodontal tissues as a result of the pull of the surrounding soft tissues and facial muscles. Labially impacted maxillary anterior teeth uncovered with an apically positioned flap technique have more un- esthetic sequelae than those uncovered with a closed-eruption technique. In the case of severly displaced impacted teeth, autotransplantation ensures preservation of the alveolar bone and will facilitate future placement of an osseointegrated implant once growth has ceased or if ankylosis/resorption of the transplant occurs.

Dental Management in a Patient with Infantile Osteopetrosis : A Case Report with a 7-Year follow-up (유아기형 골화석증 환자의 치과적 관리 : 7년간의 경과 관찰)

  • Osteopetrosis is characterized by impaired osteoclast function and increased bone density. Infantile osteopetrosis is a severe form of the disease and has characteristics such as diffusely sclerotic skeleton, pancytopenia, cranial nerve entrapment, infection susceptibility, and abnormal craniofacial appearance. Patients with infantile osteopetrosis often experience developmental delay, and may have a short life span. A 14-month-old girl with osteopetrosis presented to the department of pediatric dentistry. Incipient caries on deciduous incisors were observed. The patient revisited 4 years of age. Besides medical problems, oral complications such as growth retardation, narrow upper arch, crowding, dental caries, and abnormal tooth development were observed. After consultation with her pediatrician, dental treatments were performed on the deciduous molars under sedation after a prophylactic antibiotic injection. At a periodic follow-up, multiple deciduous teeth were treated and extracted, and oralrehabilitation with a removable partial denture was initiated. Patient with osteopetrosis are highly susceptible to infection because of their compromised immune system and problems associated with wound healing that lead to osteomyelitis or sepsis development. Active participation in dental care for sugar intake management and proper oral hygiene are obligatory.

TREATMENT OF IMPACTED MANDIBULAR FIRST MOLAR BY SURGICAL EXPOSURE : A CASE REPORT (매복된 하악 제1대구치의 외과적 노출술을 이용한 치험례)

  • The first molar is important for mastication and also it plays roles to formation of vertical occlusion and growth of jaw bone after mixed dentition. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The options of treatment plans are as follows; observation, surgical exposure, orthodontic traction, surgical relocation and extraction. Surgical exposure could be considered as a basic treatment plan. For surgical exposure it is important to maintain patent channel between the crown and the normal eruptive path into the oral cavity, many techniques including cementation of a celluloid crown, packing with zinc oxide-eugenol surgical pack are used. In these cases, we could observe spontaneous eruption of mandibular first molar using surgical exposure with or without removal of odontoma. Also we could obtain the main patency effectively and conveniently by using surgical pack and translucent retainer.

Full mouth rehabilitation of the patient with severely worn dentition using monolithic zirconia prosthesis: A clinical report (치아 마모 환자에서 단일 구조 지르코니아를 이용한 완전 구강 회복 증례)

  • Excessive occlusal wear causes loss of tooth structure, occlusal plane disharmony, impaired function and esthetic problems. Although the decrease of occlusal vertical dimension may be compensated by the growth of alveolar bone and tooth eruption, minimal increase of occlusal vertical dimension may be required for esthetics and retention of prosthesis. In this case, a 44-year-old male patient visited Seoul National University Dental Hospital with chief complaint of severe tooth wear and shade disharmony. Based on assessment of diagnostic wax-up, 3 mm increase of occlusal vertical dimension was determined. Removable occlusal splint and interim prosthesis was used to ascertain patient's comfort and adaptation. After the adaptation period, definitive prosthesis fabricated with full-contour monolithic zirconia were delivered and the patient was recommended to wear a nightguard device for prosthesis protection. This report presents a case of full mouth rehabilitation with the elevation of patient's occlusal vertical height, resulting in satisfactory esthetics and functions.
