오메가 3 지방산이 뇌혈류 및 혈관저항에 미치는 영향: 예비연구 (The Effect of Omega-3 Fatty Acid Supplementation on Cerebral Blood Flow and Vascular Resistance: A Preliminary Study)

  • Background: The effects of omega-3 polyunsaturated fatty acids (PUFAs) on cerebral vessels have not been clarified until now. Thus we investigated the efficacy of omega-3 PUFAs supplementation on cerebral blood flow velocity and vascular resistance via transcranial doppler (TCD). Methods: Consecutive twenty patients (13 male and 7 female) with at least 1 cerebrovascular risk factor or a known cerebrovascular disease were enrolled. Patients were treated with omega-3 PUFAs (1 g, two times per day) for 12 weeks. Cerebral blood flow velocity, resistance index, and pulsatile index were checked before and after 12 weeks of treatment using TCD. Results: The change of resistance index in right MCA (from $0.58{\pm}0.07$ to $0.55{\pm}0.07$, p = 0.042) and left PCA (from $0.56{\pm}0.07$ to $0.53{\pm}0.06$, p = 0.037) showed significant improvement after 12 weeks of omega-3 PUFAs treatment. The changes in other vessels, however, failed to show any significant changes compared to the baseline. Conclusions: Omega-3 PUFAs treatment showed feasible efficacies for cerebral vascular resistances in this open label trial. To confirm these results, larger samples of patients and longer period of follow-up is warranted.

박동 혈액 순환 모의 시스템에서 시간 동기화된 혈압 및 혈액의 초음파 영상 측정 및 주기적 분석 (Time-synchronized measurement and cyclic analysis of ultrasound imaging from blood with blood pressure in the mock pulsatile blood circulation system)

  • 뇌혈관 질환의 발생 및 진행 기작을 이해하고 그 질환의 조기진단과 진행예측을 위해서는 경동맥 분지에서의 혈류역학 정보가 매우 중요하다. 본 논문에서는 정상인 경동맥 분지 탄성 모형 혈관과 생체 외 돼지혈액을 이용하여 모의박동 혈액 순환 시스템을 구축하여 혈류를 조절하면서 혈관과 혈액의 초음파 영상을 내부 압력과 시간 동기화하여 측정하였다. 박동 펌프의 박동률이 분당 20회, 40회, 60회(r/min)일 때의 초음파 영상의 에코 값, 혈류속도, 혈관 벽의 움직임, 혈압을 펌프의 5주기 동안 평균하여 한 주기의 데이터를 추출하였다. 결과로 박동률이 20 r/min, 40 r/min, 60 r/min일때 수축기 최고 혈류 속도는 각각 20 cm/s, 25 cm/s, 40 cm/s, 혈압 차는 각각 30 mmHg, 70 mmHg, 85 mmHg, 동맥벽은 각각 0.05 mm, 0.15 mm, 0.25 mm로 확장 하였다. 에코의 주기적 변화는 혈류속도와 압력과는 시간 지연이 있었으며 20 r/min에서는 변화량이 최소였다. 이러한 시간 동기화된 인자들의 주기적 변화는 전산혈류역학 실험의 정확한 입력정보와 검증을 위한 중요한 정보이며 경동맥 협착 질환의 발생 및 진행 기작을 밝히는데도 유용한 정보를 제공할 것이다.

  • 기존의 Cuff 기반의 혈압(Blood Pressure)측정 방법은 연속적인 실시간 혈압측정에는 한계를 갖는다. 이러한 이유로 ECG와 PPG 센서 신호를 상호융합한 다양한 혈압추정이 이루어졌다. 그러나 PPG 중심에 측정기법은 AC 노이즈, 작은 맥동, 비박동 등의 많은 문제를 지니고 있다. 본 논문은 ECG와 PPG 관계에 발생하는 맥파전달시간(PTT)과 맥파속도(PWV)를 이용하여 혈압을 추론기법이다. 신호 피크를 이용하는 HRF(Height Ratio Features)에 비해, 본 제안방식은 ECG, PPG의 최고점 혹은 최저점을 사용한 시차를 이용해 추정하기 때문에 PPG 센싱 시그널의 오류에도 안정적인 추출이 가능한 장점이 있다. 본 논문에서 제안 방법을 이용하여 25만 회의 혈압측정의 결과 ±28.5%의 정확도를 갖는 혈압 추정기법을 제시할 수 있었다.

두 개의 광전용적맥파 기반의 수축기 혈압과 이완기 혈압 추정 융합 알고리즘 모델 분석 (Analysis of the Convergence Algorithm Model for Estimating Systolic and Diastolic Blood Pressure Based on Two Photoplethysmography)

  • 최근 만성질환자 건강관리의 목적으로 혈압측정에 대한 접근성을 높이는 제품 연구가 지속적으로 이루어지고 있다. 기존 연구에서는 심전도(ECG)와 광전용전맥파(PPG)를 분석하여 수축기혈압과 이완기 혈압을 산출하는 방식을 사용하고 있다. 주 과제는 정확도와 재현성을 위한 분석 알고리즘 개발이다. 본 연구에서는 초소형 혈압측정장치를 개발하는 단계에서 장치의 크기를 줄이고 측정방법도 간단히 하는 동시에 알고리즘도 두 개의 PPG만을 이용하여 최고혈압(SBP)을 추출하고 이에 따른 최저혈압(DBP)을 구하고자 하였다. 이를 위해 두 개의 PPG에서 얻은 측정값과 SBP, DBP 관계를 통계적으로 추적하여 상호관계를 분석하였다. PPG의 차이 값인 DF_P는 SBP와 반비례 관계가 있으며, DBP와는 비례적 관계가 성립되어 알고리즘에 의해 혈압값을 유추할 수 있으며, SBP를 통해 DBP를 추적할 수 있다.

가압훈련의 혈류 압박 정도에 따른 복합근 활동전위의 변화 (Changes in Compound Muscle Action Potential Depending on Pressure Level of Blood Flow During KAATSU Training)

  • Purpose: In recent years, there has been increasing interest in using blood flow-restricted exercise (BFRE) or KAATSU training. The KAATSU training method, which partially restricts arterial inflow and fully restricts venous outflow in the working musculature during exercise at reduced exercise intensities, has been proven to result in substantial increases in both muscle hypertrophy and strength. The purpose of this study was to investigate the proper level of pressure for KAATSU training using compound muscle action potential (CMAP) analysis. Methods: Twenty-two healthy adults voluntarily participated in this study. CMAP was conducted by measuring the terminal latency and amplitude using a motor nerve conduction velocity test. For reference-line, supramaximal electrical stimulation was applied to the median nerves of the participants to obtain CMAP for the abductor pollicis brevis. For baseline, the intensity of the electrical stimulation was decreased to a level at which the CMAP amplitude was about a third of the CMAP amplitude obtained by the supramaximal electrical stimulation. The pressure levels for the KAATSU were set as a systolic blood pressure (strong pressure), the median values of systolic and diastolic blood pressure (intermediate pressure), and diastolic blood pressure (weak pressure). In the KAATSU condition, CMAP was performed under the same conditions as baseline after low-intensity thumb abduction exercises were performed at the subjects' own pace for one minute. Results: As the pressure increased, the CMAP amplitude was significantly increased, signifying that more muscle fibers were recruited. Conclusion: This study found that KAATSU training recruited more muscle fibers than low-intensity exercise without the restriction of blood flow.

The effect of oral glucose tolerance testing on changes in arterial stiffness and blood pressure in elderly women with hypertension and relationships between the stage of diabetes and physical fitness levels

  • [Purpose] The purpose of this study was to assess changes in blood glucose level, blood pressure, and arterial stiffness after a 75 g oral glucose tolerance test (OGTT) in elderly women aged over 65 years with hypertension and either normal glycemic control, impaired fasting glucose tolerance, or diabetes mellitus. We also wished to investigate the relationship between stages of diabetes and physical fitness. [Methods] A total of 24 elderly women with hypertension were assigned to a control group (CON; n=7), impaired fasting glucose group (IFG; n=9), and diabetes mellitus group (DM; n=8). In each group, blood glucose level, brachial ankle pulse wave velocity (PWV), and blood pressure were measured at baseline as well as 60 and 120 minutes after a 75 g OGTT. Physical fitness factors such as hand grip strength, balance test, 4 m gait speed test, chair stand test, short physical performance battery, and 6-minute walking test were subsequently assessed. [Results] In all three groups, blood glucose levels were significantly increased at 60 and 120 minutes after a 75 g OGTT. In the DM group, blood glucose levels were significantly higher before and after a 75 g OGTT than in the CON group. In the CON group, PWV was significantly increased at 60 minutes after a 75 g OGTT; however, there were no changes in other groups after glucose ingestion. In the CON group, systolic and diastolic blood pressures were significantly decreased at 60 and 120 minutes after a 75 g OGTT compared to baseline. However, there was no change in blood pressure after ingestion in the DM group. The IFG group had greater grip strength than the CON group; however, there were no differences in other variables between the groups. [Conclusion] After a 75 g OGTT, elderly women with hypertension and diabetes maintain higher blood glucose levels compared to those with hypertension alone. Unlike elderly women with hypertension alone, those with hypertension and diabetes did not show changes in arterial stiffness and blood pressure after a 75 g OGTT. Therefore, elderly women with hypertension and diabetes may not be able to control their blood vessels following a 75 g OGTT due to impaired vascular endothelial function. Moreover, there was no association between diabetes stage and physical fitness in elderly women with hypertension.

Changes of Cerebral Metabolism and the Related Factors during Cardiac Surgery

  • The effect of cardiopulmonary bypass (CPB) on cerebral physiology during heart surgery remains incompletely understood. This study was carried out to investigate changes of cerebral metabolism and the association between the changes and clinical factors during heart surgery. Seventy adult patients (n=70) scheduled for elective cardiac surgery were participated in the present study. Middle cerebral artery blood flow velocity (V$_{MCA}$), cerebral arteriovenous oxygen content difference (C(a-v)O$_2$), cerebral oxygen extraction (COE), and modified cerebral metabolic rate for oxygen (MCMRO$_2$) were measured during six phases of the operation; Pre-CPB, CPB-10 min, Rewarm-1 (nasopharyngeal temperature 34$^{\circ}C$), Rewarm-2 (nasopharyngeal temperature 37$^{\circ}C$), CPB-off, and Post-OP (at skin closure after CPB-off). Each relationship of age, arterial blood gas parameters, or other variables to V$_{MCA}2$, C(a-v)O$_2$, COE, or MCMRO$_2$ was evaluated. V$_{MCA}$ increased (P<0.0001) whereas C(a-v)O$_2$ decreased (P<0.01) throughout the five phases of the operation compared to Pre-CPB value (control). COE diminished at CPB-10, Rewarm-1, and CPB-off (P<0.05) while MCMRO$_2$ reduced at CPB-10 and Rewarm-1 (P<0.05) compared to Pre-CPB value. Positive correlation was found between age and cerebral metabolic parameters (V$_{MCA}$, C(a-v)O$_2$, COE, or MCMRO$_2$) during CPB (range r=0.24 to 0.38, p<0.05). Four cerebral metabolic parameters had partially negative or positive correlation with arterial blood gas parameters and other variables (arterial blood pH, $O_2$ tension, $O_2$ content, $CO_2$ tension, blood pressure, blood flow, temperature, or hematocrit) during the operation. In conclusion, CPB led to marked alterations of cerebral metabolism and age, pH, and $CO_2$ tension profoundly influenced the changes during cardiac surgery.

협착이 있는 탄성혈관을 흐르는 혈액의 유동특성에 관한 수치해석적 연구 (A Numerical Analysis on the Hemodynamic Characteristics in Elastic Blood Vessel with Stenosis)

  • 심혈관계에서 자주 발생하는 죽상경화증과 혈전의 발생 및 성장에 관한 복잡한 기전을 이해하기 위하여 뇌의 혈액공급을 담당하는 경동맥을 2차원 축대칭으로 모사하여 수치해석하였다. 박동유동 상태에서 경동맥 내에 25%. 50%, 75%의 협착이 형성된 경우에 대하여 혈관내의 속도분포 및 혈류역학적 벽 파라미터들이 고찰되었다. 혈액은 뉴턴유체 및 전단변형률에 따라 점성이 변화하는 비뉴턴유체로 간주되었으며 비뉴턴모델로는 혈액과 유사한 점성치를 나타내는 Carraeu-Yasuda 모델이 적용되었다. 해석 결과 혈관내벽에 작용하는 벽전단응력은 협착이 커질수록 크게 증가하였으며 비뉴턴유체보다 뉴턴유체의 경우에서 벽전단응력이 크게 평가되었다. 벽전단응력 진동지표(OSI)에 의해 시간평균 재부착점이 예측되었는데 비뉴턴유체보다 뉴턴유체의 경우에서 협착 영역으로부터 멀리 떨어진 곳에서 관찰되었다. 시간평균 벽전단응력구배(WSSG)도 협착이 큰 경우에 상당히 크게 나타났는데 비뉴턴유체보다 뉴턴 유체의 경우에 더 큰 값이 나타났다.

시판 요가용 브라탑 착용 시 50~60대 여성의 의복압 및 혈류와 주관적 반응 (Clothing Pressure, Blood Flow, and Subjective Sensations of Women in Their 50s and 60s When Wearing a Commercial Yoga Bra Top)

  • This study analyzed clothing pressure, blood flow, and subjective sensations among women in their 50s and 60s while wearing commercially available yoga bra tops (YBTs). These findings could then serve as basic data for designing YBT for women in this age range. Clothing pressure and blood flow reactions were measured in seven participants wearing four commercially available YBTs. Subjective sensations of pressure and comfort were also evaluated. The calculated data were analyzed using the SPSS 24.0 statistical program. The results suggested that YBT4 exerted the lowest clothing pressure, while YBT2 and YBT3 exerted relatively higher clothing pressures. Participants' blood flow decreased while wearing YBTs compared to the control garment. The decrease of 47.3% when the participants wore YBT2 was especially significant. The results confirmed that the blood velocity rate was physiologically lower while wearing all four YBTs than when wearing the control garment. Results for subjective pressure and overall comfort confirmed that YBT2 and YBT3 exerted relatively high pressure, while YBT4 did not exert pressure.

TCD를 이용한 정상군과 중풍원인질환군의 혈류측정에 관한 비교연구 (Case-control Study : Cerebral Blood Flow as Measured by TCD in Patients with Risk Factors for Stroke)

  • Objectives : The purpose of this study was to evaluate the blood flow using doppler ultrasound of the MCA, ACA, PCA, BA, ICA in the patients with risk factor for stroke. Methods : 110 patients with risk factor for stroke were selected who had hypertension, diabetes mellitus, hyperlipidemia, or heart disease, as well as 89 healthy adults who did not have any symptoms of those diseases. To evaluate the blood flow, the Vs and Vm of the MCA, ACA, PCA, BA, and ICA in the two groups were measured. Result : In normal healthy adults, subjects showed a decrease in the Vs and Vm: with advancing in age, there was a significant difference in the Vs of MCA, PCA, BA, and ICA. There was a significant difference in the Vm of MCA, PCA, BA, and ICA. In normal healthy adults, females showed high velocities in all examined vessels. There was a significant difference in the Vs of BA. There was a significant difference in the Vm of BA, and ICA. There was a decrease in the Vs, and Vm of all examined vessels of patients with risk factors for stroke in comparison with normal healthy adults. There was no significant difference in the Vs. There was a significant difference in the Vm of MCA, ACA, and PCA. Results were the same between patients aged under 50 and total patients in the Vs and Vm of examined vessels. There were differences between patients aged over 50 and total patients in the Vs of MCA, PCA, BA and for patients aged over 50; patients with risk factor for stroke who had hypertension, diabetes mellitus, hyperlipidemia, or heat disease has higher Vs than healthy adults. Conclusions : There was a significant difference in the blood flow velocity between patients with risk factors for stroke and healthy adults. However, this result was different from results of comparison of TCD findings between patients and healthy individuals by age. Therefore, more detailed studies about aged patients are needed.
