폐암환자에서 종격동 방사선조사 후에 발생하는 림프구 감소증 (Lymphopenia after Mediastinal Irradiation in Lung Cancer)

  • 목적 : 방사선치료가 세포매개면역을 저하시킨다는 보고들이 있었으나 이를 방사선치료 부위 안의 혈류와 관련해서 고려되지는 않고 있다. 본 연구는 폐암의 방사선치료 후에 발생하는 백혈구 감소, 특히 림프구의 감소를 후향적으로 분석하고, 면역억제의 한 기전으로서 방사선치료 부위 안의 많은 혈액량과 동적인 혈류가 관련될 수 있는지를 알아보고자 한다. 대상 및 방법 : 34명의 폐암환자 들을 대상으로 하였으며, 방사선치료 단독군(RT군)이 10명, 방사선치료와 화학요법 병용군(RT/CT군) 16명, 화학요법 단독군(CT군)이 8명이었다. 말초혈액의 전체 백혈구수와 림프구수, 전체 백혈구수 중 림프구수의 비율(림프구 율)을 방사선치료 전중후에, 화학요법 전후에 측정하였는데 방사선치료 중에는 매 주마다 검사를 하였고, 화학요법과 방사선치료 후에는 좀 더 긴 간격으로 하였다. 환자의 특성과 병변의 특성은 세 군간에 통계적으로 유의한 차이를 보이지 않아다. 치료방법에 있어서도 RT군과 RT/CT군 사이에 총 방사선량, 분할조사선량, 치료면적, 치료용적은 통계학적으로 유의한 차이는 없었다. 방사선치료를 시행한 군(RT군, RT/CT군)과 방사선치료를 시행하지 않은 군(CT군)의 평균 추적기간은 각각 6개월과 8개월이었다. 결과 : 치료 후 전체 백혈구수의 변화는 다른 치료군 간에 의미 있는 차이는 보이지 않았다. 방사선치료를 시행한 RT군과 RT/CT군은 방사선치료 후 훨씬 낮은 림프구수와 림프구 율을 보였다. 치료 전 림프구수와 최종 림프구수 사이의 감소는 방사선치료를 시행한 군과 시행하지 않은 군간에 통계학적으로 유의한 차이를 보였다(p=0.044). 또한 림프구 율에 있어서도 치료 전과 최종 림프구 율 사이의 감소는 방사선치료를 시행한 군과 시행하지 않은 군간에 통계학적으로 유의한 차이를 보였다(p=0.037). 결론 : 림프구 감소증은 화학요법만을 시행한 경우보다 방사선치료를 시행한 경우에 더 현저하였다. 그리고 이러한 림프구 감소증이 폐암의 흉부 방사선치료 후 면역감소의 한 원인이 될 수도 있겠다. 저 선량의 방사선조사 후 림프구의 아포토시스 유도를 보여준 이전의 몇몇 연구들을 함께 생각해 볼 때, 조심스럽게 제안하고 싶은 것은 혹시 본 연구에서 보이는 림프구 감소증이 방사선조사야 내의 많은 혈액량이나 역동적인 혈류와 약간의 관련성이 있을 수도 있다는 것이다.

Nodal tumor response according to the count of peripheral blood lymphocyte subpopulations during preoperative chemoradiotherapy in locally advanced rectal cancer

  • Purpose: The objective of this prospective study was to evaluate the relationship between the circulating lymphocyte subpopulation counts during preoperative chemoradiotherapy (CRT) and tumor response in locally advanced rectal cancer. Materials and Methods: From August 2015 to June 2016, 10 patients treated with preoperative CRT followed by surgery were enrolled. Patients received conventional fractionated radiotherapy (50.4 Gy) with fluorouracil-based chemotherapy. Surgical resection was performed at 4 to 8 weeks after the completion of preoperative CRT. The absolute blood lymphocyte subpopulation was obtained prior to and after 4 weeks of CRT. We analyzed the association between a tumor response and change in the lymphocyte subpopulation during CRT. Results: Among 10 patients, 2 (20%) had evidence of pathologic complete response. In 8 patients with clinically node positive, 4 (50%) had nodal tumor response. All lymphocyte subpopulation counts at 4 weeks after CRT were significantly lower than those observed during pretreatment (p < 0.01). A high decrease in natural killer (NK) cell, count during CRT (baseline cell count - cell count at 4 weeks) was associated with node down staging (p = 0.034). Conclusion: Our results suggest that the change of lymphocyte subset to preoperative CRT may be a predictive factor for tumor response in rectal cancer.

ABO Blood Group, Epstein-Barr virus Infection and Prognosis of Patients with Non-metastatic Nasopharyngeal Carcinoma

  • Background: A prior study showed blood type A/AB to be associated with an increased risk of nasopharyngeal carcinoma (NPC) compared to subjects with blood type O. However, the relationship between ABO blood groups and prognosis of NPC patients is still questionable. In addition, whether Epstein-Barr virus (EBV) infection is associated with prognosis of NPC patients with different ABO blood groups is unclear. Materials and Methods: We conducted univariate and multivariable Cox regression analyses based on a consecutive cohort of 1,601 patients to investigate the above issues. Results: There was no significant difference in overall survival (OS) between different ABO blood groups (p=0.629), neither between A vs. non-A blood groups (p=0.895) nor AB vs. non-AB blood group (p=0.309) in univariate analyses and after adjusting for other factors. Interaction tests revealed that high immunoglobulin A against Epstein-Barr virus viral capsid antigen (VcA-IgA) level was associated with a favorable prognosis in male patients with UICC stage II disease who had an A blood type (p=0.008), compared with those with non-A blood type. In addition, male patients with an A blood group with a high blood lymphocyte level showeda tendency towards better survival in UICC stage III (p=0.096). Conclusions: ABO blood group status is not associated with the prognosis of patients with NPC. Additionally, blood group A male NPC patients with high VcA-IgA level or high blood lymphocyte counts might be correlated with a favorable prognosis in UICC stage II or III, respectively.

Physiological Profile of Growing Rats: Effects of Cage Type and Cage Density

  • This experiment was conducted to examine the effects of cage type (CT) and cage density (CD) on physiological variables in growing rats. Sprague Dawley rats (n=108) weighing an average of 46 g were housed in metallic cage with woodchip bedding (MCWB), metallic cage with wire mesh (MCWM), and plastic shoebox with woodchip bedding (PCWB) separately by sex at normal ($160-cm^2/rat$, ND) and high ($80-cm^2/rat$, HD) CD from 3 to 10 wks of age. All cages were in dimension of $24{\times}40{\times}20$ cm ($W{\times}D{\times}H$). At the end of the experiment, blood samples were collected and 6 rats from each cage were sacrificed. No death was observed among rats at ND, whereas mortality rate at HD was 22.3% for males and 13.9% for females. Heart weight was affected by CT. Doubling CD caused 23, 11.8, 17.9, 8.6, 6.9, and 16.4% decreases in BW and weights of heart, liver, kidney, testis, and ovary, respectively. Except for adrenal gland, other organs for males were heavier than for females. Liver weight of males and females responded differently to CT and CD. Comparing with females, males had 7.3 and 5.2% heavier and 9.9% lighter liver weights in MCWB, MCWM, and PCWB, respectively. As CD doubled, liver weight for males and females decreased by 22.4 and 13.1%, respectively. Mean adrenal gland weight increased by 8.4% and decreased by 9.7% for males and females, respectively, with doubling CD. CT affected glucose, TG, Ca, and ALP levels. However, CD did not alter blood chemistry. Rats housed in metallic cages had greater neutrophil count and neutrophil:lymphocyte ratio than rats housed in plastic cages. Doubling CD caused a 24.2% increase in lymphocyte count. There were CT by CD, CT by sex, and CD by sex interaction effects on lymphocyte count. Doubling CD caused 0.1% decrease and 49.8 and 26.7% increases in lymphocyte count for rats housed in MCWB, MCWM, and PCWB, respectively. Comparing with females, lymphocyte count for males housed in MCWB, MCWM, and PCWB had 8.9 and 12.9% greater and 30.3% less lymphocyte counts, respectively. Lymphocyte count decreased by 4.12% for males, whereas it increased by 61.0% for females as CD doubled. Doubling CD resulted in 2.5 and 2.3% increases in erythrocyte count and hematocrit value. These data suggest that animals perform better in metallic cages than in plastic cages and that cage density had pronounceable effects on physiological parameters in a cage type and sex dependent-manner.

Lack of Prognostic Value of Blood Parameters in Patients Receiving Adjuvant Radiotherapy for Breast Cancer

  • Aim: To determine prognostic value of blood parameters on overall and progression-free survival in cases received adjuvant radiotherapy and chemotherapy with diagnosis of stage I-III breast cancer. Materials and Methods: We retrospectively reviewed files of 350 patients with non-metastatic breast cancer who were treated in the Radiation Oncology Department of Kayseri Teaching Hospital between 2005 and 2010. Pretreatment white blood cell (WBC), neutrophil, monocyte, basophil and eosinophil counts, and the neutrophil/lymphocyte ratio (NLR) and platelet lymphocyte ratio (PLR) were recorded. The relationship between clinicopathological findings and blood parameters was assessed. Results: Overall, 344 women and 6 men were recruited. Median age was $55.3{\pm}0.3$ years (range: 22-86). Of the cases, 243 (61.4%) received radiotherapy while 329 (94.3%), received chemotherapy and 215 (61.4%) received hormone therapy. Mean overall survival (OS) and progression-free survival (PFS) was 84.4 and 78.8 months, respectively. During follow-up, 48 patients died due to either disease-related or non-related causes. Local recurrence was detected in 14 cases, while distant metastasis was noted in 45 cases. In univariate analysis, age, pathology, perinodal invasion were significantly associated with overall survival, whereas gender, stage and hormone therapy were significantly associated with progression-free survival. In multivariate analysis, histopathological diagnosis (OR: 0.3; 95%: 0.1-0.7; p=0.006) and perinodal invasion (OR: 0.1; 95% CI: 0.1-1.3; p=0.026) were significantly associated with overall survival, whereas tumor stage (OR: 2.1; 95% CI: 0.0-0.7; p=0.014) and hormone therapy (OR: 2.1; 95%: 1.2-3.8; p=0.010) were significantly associated with progression-free survival. Conclusions: It was found that serum inflammatory markers including WBC, neutrophil, lymphocyte and monocyte counts, and NLR and PLR had no effect on prognosis in patients with breast cancer who underwent surgery and received adjuvant radiotherapy and chemotherapy.

삼출건비탕(蔘朮健脾湯)이 흰쥐의 면역반응(免疫反應)에 미치는 영향(影響) (Effects of Samchulkunbitang on the Immunosuppression Induced by Methotrexate in Rats)

  • Introduction The effects of Samchulkunbitang on the immunosuppression induced by methotrexate in rats were investigated in this experument. The multiple parameters of immunity assessed in each rats includes the rate of body weight loss, weight changes in thymus, spleen and axillary lymphnode. The number of lymphocytes and CD4+ T cell count in the blood, thymus, spleen and axillary lymphnode were also measured. Methodology Male Sprague-Dawley rats were chosen as an experiment object and were divided into 4 groups by a random selection. Each group consisted of 6 rats. The normal group didn't receive any treatment. The control group was administered with methotrexate for 5 days. The sample Ⅰ group was administered with Samchulkunbitang for 5 days. The sample Ⅱ group was administered with both Samchulkunbitang and methotrexate for 5 days. The dosage of medication was 2cc/day, 1cc given at 10 AM and another 1cc given at 5 PM. Results The rate of body weight loss was significantly increased in the Sample Ⅰ and Sample Ⅱ group. The weight of thymus and spleen were significantly increased in Sample Ⅰ and Sample Ⅱ groups While the weight of axillary lymphnode did not show much increase. No significant differences were measured among the experimental groups. Blood lymphocyte count, blood CD4+ T cell count, spleen lymphocyte count, axillary lymphnode lymphocyte and CD4+ T cell count were significantly increased in Sample Ⅰ and Sample Ⅱ groups while spleen CD4+ T cell count did not show much increase. No significant differences were measured among the experimental groups. Conclusion As one can witness from the above results, administration of Samchulkunbitang played potent role in increasing immune system among the rats treated with methotrexate which induces immunosuppression. Overall increase of lymphocyte count and CD4+ T cell count in the sample groups with Samchulkunbitang effectively proves its ability to boost the immune system.

  • The cellularity and composition of the spleen lymph node thymus and peripheral blood and tempo of regeneration were studied at various time points after syngeneic bone marrow transplantation(BMT) in C3H/Hen mice. Significant depression of absolute lymphocyte count was noted on week 1 after lethal whole-body irradiation and BMT. In comparison to the lymph node thymus and spleen had an rapid regeneration of cellularity. The distinct cell populations($CD4^+,\;CD8^+,\;CD28^+,\;B220^+) have determined in the lymphoid tissue of mice subjected to irradiation. The relative representation of these subpopulations was significantly different from that in nonirradiated control. $CD4^+\;and\;CD8^+$ cells were present in very low numbers whereas the $B220^+$ cells reached more than normal range at 2 weeks after BMT. The number of $CD4^+$ cells returned to normal relatively soon than $CD8^+$ cell. At week 4 after BMT, the cellularity and composition of spleen lymph node and peripheral blood lymphocyte reached about 50% of the normal range therefore we can choose this time point for the other tests of immune function after BMT.

젖소에 있어서 원유 중 체세포수, 스트레스 및 면역물질에 대한 환경효과와 유전모수 추정 (Estimation of environmental effects and genetic parameters for somatic cell score, stress and immunological traits in Holstein cattle)

  • 본 시험은 유질에 영향을 미치는 체세포수에 대하여 영향을 미칠 것으로 생각되는 면역물질에 대한 환경효과와 유전모수를 추정하고자 축산연구소에서 사육하고 있는 착유우를 공시하여 이들의 혈액과 우유 시료를 채취하여 조사 분석하였다. 분만산차는 체세포수와 호중구에 영향을 미치는 것으로 나타났고(p<0.01), 비유기는 체세포수와 IgG, 호중구에 영향을 미치는 것으로 나타났다. 하지만 코티솔과 림프구는 산차와 유기에 의하여 영향을 받지 않는 것으로 나타났다. 체세포수의 산차간 비교에서 1, 2, 3 산차간에는 차이가 없었으나 4산차 이상에서는 높아지는 경향이 있어 3산차 이하와 비교 할 시에 유의적인 차이가 있었고 유기별 비교에서는 비유초기(90일 이하)와 비유후기(181일 이후)가 비유중기(91~180) 보다 높게 나타났다. 코티솔은 체세포수의 산차 성적과 마찬가지로 3산차 이하와 4산차 이상 간에 차이가 있었고 비유기별 성적에서는 높아지는 것처럼 보이나 차이는 없었다. 면역물질인 IgG는 1산차와 4산차 이상의 비교에서만 차이가 있었고 유기에서는 비유 초기가 비유 중기와 후기에 비하여 높은 결과를 보였고 림프구는 1산차와 4산차 이상 간에 차이가 있었으나 유기간에는 차이가 없었다. 호중구는 산차가 증가할수록 감소하는 경향으로 2산차 이하와 3산차 이상간에 차이가 있었으며 비유중기 까지는 차이가 없었고 비유 후기에는 높게 나타났다. 체세포수의 유전력은 낮은 수준이지만 유질 개량을 위하여 의미 있을 정도로 높게 추정되었으며 코티솔, 림프구와 부의 높은 유전상관을 보였다. 코티솔의 유전력은 비교적 낮은 수준이었고 림프구, 호중구간의 정의 상관관계를 보였다. IgG의 유전력은 중 정도를 보였으며 림프구와 호중구와는 높은 부의 유전상관을 보였다. 림프구는 높은 유전력을 보였고 호중구와도 높은 유전적 상관을 보였다. 호중구는 체세포수와 비슷한 수준의 유전력을 나타내었다. 이상의 연구 결과에서 코티솔, IgG, 림프구, 호중구가 체세포수와 유전적으로 높은 상관관계를 가지고 있어 우유 품질 개선에 중요한 형질임을 알 수 있었다. 또한 농가의 사육규모가 증가하고 두 당 생산성이 더욱 증가함에 따라 생산성과 질병을 동시에 고려하는 젖소개량 관련 연구가 수행되어야 할 것으로 사료된다.

폐결핵 환자의 말초 혈액 및 기관지 폐포세척액내의 임파구 아형과 세포성 매개면역과의 관계 (The Relationship between Cell-mediated Immunity and Subtypes of Lymphocyte in BAL Fluid and Peripheral Blood in Patients with Pulmonary Tuberculosis)

  • 연구배경 : Mycobacteria 감염에 의한 폐결핵은 세포매개성 면역 반응이 관계한다고 알려져 있는 바, 먼저 Mycobacteria가 흡입되면 T 임파구가 활성화되어 세포매개성 면역 반응을 나타낸다. 결핵환자에서 tuberculin-purified protein derivative에 대한 피부 면역반응은 주로 세포매개성 면역반응에 의해 이루어짐으로 활동성 폐결핵 환자에서는 양성반응을 나타내어야하나 활동성 폐결핵 환자에서도 음성반응(anergy)을 나타내는 경우가 있다. 저자들은 폐결핵 환자에서 말초혈액 및 기관지 폐포세액내의 임파구의 세포조성과 이들이 피부반응에 미치는 영향을 조사함으로써 활동성 폐결핵 환자에서의 PPD에 대한 피부반응 검사상 음성으로 나타나는 이유를 알아보고자 하였다. 방법 : 11명의 정상대조군, 20명의 활동성 폐결핵 환자를 대상으로 PPD에 대한 피부 반응 검사 및 말초 혈액 및 기관지 폐포세척액의 임파구의 아형을 분석하였다. 결과 : 1) 기관지 폐포세척액내의 임파구는 환자군에서 정상대조군에 비하여 유의하게 증가되어 있었고($25.2{\pm}4.8$ vs $6.6{\pm}1.3%$, p<0.01), 단구세포(monocyte)는 환자군에서 정상 대조군에 비하여 유의하게 감소되어 있었다($69.6{\pm}5.7$ vs $89.2{\pm}1.4%$, p<0.05). 2) PPD에 대한 양성 반응을 보인 환자군과 음성 반응을 보인 환자군 사이의 기관지 폐포세척 세포 조성의 비는 차이가 없었다. 3) 환자군에서 기관지 폐포세척액 내의 $CD3^+$, $CD4^+$ 임파구의 조성비는 말초혈액보다 유의하게 증가되어 있었고($CD3^+$: $76.56{\pm}2.18$ vs $57.59{\pm}2.17%$, p<0.001; $CD4^+$: $51.24{\pm}2.33$ vs $35.20{\pm}2.32%$, p<0.005), $CD3^+IL-2R^+$, $CD3^+HLA-DR^+$ 임파구의 조성비도 말초혈액보다 증가되어 있었다($CD3^+IL-2R^+$:$2.41{\pm}0.57$ vs $0.93{\pm}0.16%$, p<0.005; $CD3^+$ HLA-$DR^+$: $16.92{\pm}3.89$ vs $3.94{\pm}0.70%$, p<0.005). 4) 환자군중 PPD에 대한 양성반응을 보인 군과 음성반응을 보인 군 사이에는 기관지 폐포세척액내의 임파구의 조성의 차이 및 수에서 차이가 없었다. 5) 환자군에서 PPD에 대한 피부반응과 기관지 폐포세척 세포의 조성비 사이에는 모두 상관관계가 없었으며, PPD에 대한 피부반응과 기관지 폐포세척액 및 말초 혈액내의 임파구아형의 조성과도 상관관계가 없었다. 결론 : 결론적으로 기관지 폐포세척액내의 염증 세포와 세포매개성 면역 반응과는 직접적인 관계가 없음을 알수 있었으며, 활동성 폐결핵 환자에서의 지연성 피부 반응의 저하는 임파구의 구획에 의한 결과라는 것을 배제할 수 없었다.

Relationship Between Prognosis and Neutrophil: Lymphocyte and Platelet:Lymphocyte Ratios in Patients with Malignant Pleural Mesotheliomas

  • Background: It has been demonstrated that neutrophil:lymphocyte (NLR) and platelet:lymphocyte (PLR) ratios are associated with prognosis in cancer patients. The aim of this study was to investigate whether pretreatment white blood cell (WBC), neutrophil, lymphocyte, monocyte, platelet, basophil and eosinophil counts, LDH level, NLR and PLR are associated with prognosis in patients with malignant pleural mesothelioma (MPM). Materials and Methods: We retrospectively reviewed files of 50 patients who were managed with a diagnosis of MPM between 2005 and 2010. Demographic and clinical characteristics, treatments, response to treatment and prognostic factors were evaluated, along with relationships between pretreatment blood parameters and prognosis. Results: Overall, 38 men and 12 women were included to the study. Mean age was $61.5{\pm}9.4$ years (range: 39-83 years). There was advanced disease in 86% (n=43) and the histological type was epithelial mesothelioma in the majority (82%). Of the cases, 17 (34%) received radiotherapy, while 42 cases underwent first- and second-line chemotherapy, with cisplatin plus pemetrexed as the most commonly used regimen. In the assessment after therapy, it was found that there was complete response in 4 cases (8%), partial response in 10 cases (20%), stable disease in 17 cases (34%) and progression in 19 cases (38%). Median follow-up was 10 months (range: 10 day-30 months). Median overall survival was found to be 20.7 months while median progression-free survival as 10 months. In univariate and multivariate analyses, it was found that factors significantly affecting overall survival included stage (p=0.030), response to treatment (p=0.026) and monocyte count (p=0.004), while factors affecting disease-free survival included NLR (p=0.018), response to treatment (p=0.001), and PLR score (p=0.003). Conclusions: Overall and disease-free survival was found to be better in cases with a WBC count<8.000, platelet count<300,000, and low NLR and PLR scores in malignant pleural mesothelioma.
