불면증의 형상의학적 고찰 (Treatment of Insomnia in Hyungsang Medicine)

  • Insomnia describes not only a state of a lack of sleep but also a causative factor for both psychological and physical disorders. Most people tend to underestimate the effects of insomnia on psychological problems, stroke, Sogal or other life-threatening conditions. insomnia may occur when 1) the flow of Jung(精), Ki(氣), Shin(神) and Hyul(血) is not smooth; 2) the five Jang organs and six Bu organs are in disharmony; 3) Shin(身) and Hyung(形) are inconsistently developed; 4) Young and Wi(衛) do not work in concert; 5) and other factors such as age or exposure to Cold are involved. I observed my patients suffering from insomnia according to Hyungsang medicine, which is based on 'Dongyuibogam'(東醫寶鑑). Insomnia due to the problems with Jung, Ki. Shin and Hyul was found in the Shin type(神科) and the Ki type(氣科). When it comes to the organs, sleep disorder significantly correlates to weakened Spleen, Liver and Kidneys. In terms of the inconsistent development of Shin and Hyung, patients who were shortsighted or experienced surgery showed insomnia. Patients exposed to Cold showed false type agitation. With respect to age, while children showed a weakened Gallbladder, the aged had deficient Blood. Sleep disorder caused by disharmony of Young and Wi occurs at dusk or dawn.

"동천오지(洞天奧旨)"에 관한 소고(小考) (A Study on "Dongchunohji(洞天奧旨)")

  • This book was completed in the 33rd year of Gang-hui-gapsul(康熙 甲戌) in the Cheong(淸) dynasty(1694), and was first inscribed in the 55th year of Geonryung(乾隆). In this first edition named 'Daechudangbon(大雅堂本)', his descendant Jinbonghui(陳鳳輝) wrote the postscript. There are also other editions such as 'Ga-gyeong-ganchwihyeondangbon(嘉慶間聚賢堂本)', 'Wimundanggeonsangbon(緯文堂巾箱本)' and 'Guangseo-ganseonseongdangbon(光緒間善成堂本)' Second, this book was called "Oe-gwabirok(外科秘錄)" and used the pen name 'Gibaekchunsasojeon(岐伯天師所傳)'. There are 16 volumes in total. The beginning of volume one, is a drawing of the 14 meridians. Volumes 1$\sim$4 are on the symptoms and treatment of abscesses and sores and ulcers[癰疽瘡瘍]. Volumes 5$\sim$13 are on surgery, dermatology and 156 diseases such as wounds by contusion, sharp objects and insects and beasts[跌撲, 金刃, 蟲獸傷]. Volumes 14$\sim$16 list the internal treatments, external medicine, acupuncture and moxibustion and surgery of sores and ulcers. Third, the book stressed early detection and treatment of diseases, emphasized inner resolving[內消] being cautious about using medicine, and further deepened the syndrome differentiation and treatment[辨證施治] of sores and ulcers[瘡瘍] by dividing fire toxin(火毒) into Eumhwa and Yanghwa(陰火/陽火). Also, it established the cause of sores and ulcers development as the deficiency of Gi(氣) and blood[血] and focused especially on the liver and kidney's involvement in the process. It also asserted that the pathogen[邪] is the tip[標], so one should eliminate and reinforce[攻補] to balance it out. The sore and ulcer surgery[瘡瘍外科] part is mostly based on detoxifying[解毒] and resolving[消散]. Although the meridians were mentioned, they were not emphasized, while moxibustion treatment of sores and ulcers[瘡瘍] were thoroughly analyzed. Also, of all 550 formulas in this book, 90% are experience-based which frequently use Geum-eunhwa(金銀花), Pogong-yeong(蒲公英) and Jahwajijeong(紫花地丁). The usages are unique, and the formulas have good adaptability. The symptoms of the diseases are explained first, followed by according main and sub treatments.

한의학의 정신 생리와 병리에 대한 소고 -황제내경의 오신, 칠정을 중심으로- (A Study on Psychophysiology and Psychopathology of Korean Medicine -Focus on Emotion and Thought in Huangdineijing)

  • Objectives: The purpose of this study is to understand the theories about the psychophysiology and psychopathology of Korean Medicine. Methods: The concepts and functions of spirit (神), soul (魂魄), five spirits (五神), and seven emotions (七情) occurring in the Huangdineijing have been interpreted, and the correlation between thought and emotion considered. Results and Conclusions: (1) Spirit (神) refers to the source and discipline of vital activity and mental activity. (2) With soul (魂魄), ethereal soul (魂) manifests itself as the mental process, and corporeal soul (魄) as the physical sensory interaction, such as the nervous system. (3) In the five spirits, ethereal soul (魂) is the recognition process of drawing out memories. Spirit (神) is the process of creative thought. Cognition (意) is the ability to recognize and integrate information. Corporeal soul (魄) is the process of selecting what is important and choosing it. Will (志) is the process of storing memories. Ethereal soul (魂) and corporeal soul (魄) of the five spirits (五神) and soul (魂魄) use the same characters, but the meaning differs. Also, spirit (神) and the spirit (神) in five spirits (五神) are the same character, but, because the meaning is different, they need to be interpreted according to the context. (4) Heart (心), Cognition (意), Will (志), Thought (思), Consideration (慮), and Wisdom(智) are all cognitive processes, like perception, recognition, and thinking. (5) Psychopathology is when excessive emotion affects the five viscera, harming the Energy (氣) and Blood (血) and eventually affecting the five spirits, which causes problems in thinking. Therefore, for healthy mental functioning, not only must the emotions be regulated so that they do not become excessive, but the five viscera also need to be kept healthy.

북미춘포(北尾春圃)의 신간동기(腎間動氣) 복진법(腹診法)에 대한 고찰(考察) (A Study on the Abdominal Diagnostic Method of Kitao Shunpo about the Movement Qi between kidneys)

  • Objectives : In this study, the Abdominal Diagnostic Method of Kitao Shunpo(北尾春圃) about Movement Qi between kidneys in Sanghauidam(桑韓醫談) and Jeonggisinron(精氣神論) are investigated for the purpose of understanding the definite technique of the method and the position in the context of abdominal diagnosis in Japan. Methods : The materials related to the abdominal diagnosis to Movement Qi between kidneys were selected from Sanghauidam and Jeonggisinron written by Kitao Shunpo, and analyzed to know the characteristics of the method. Results : Kitao Shunpo suggested the theoretical background of the Abdominal Diagnostic Method that Original Qi(元氣) is made from Original Essence(元精) when a person is born, and mentality(神) and body essence(精), qi(氣) and blood(血) are formed from those. Movement Qi between kidneys(腎間動氣), that is to say innate Original Qi can be diagnosed in abdominal region by the method in Japanese Medicine. Kitao Shunpo decided deficiency and excess of Original Qi and prognosis in clinical treatment by the method, and suggested the standard pulse condition of the Movement Qi between kidneys. Conclusions : After the theory about Movement Qi between kidneys appeared in Nanjing(難經), many doctors have not made attention to diagnose that for a long time, until Kitao Shunpo with other Japanese doctors established the theoretical background of the Abdominal Diagnostic Method, and applied the theory to clinical treatment.

담음에 관한 형상의학적 고찰 (Review on Phlegm in Hyungsang medicine)

  • Phlegm is defined as the static fluid in excess in one or more parts of the body. It is not only a pathologic product but also a pathogen itself. However, as it says Phlegm is another title for the body fluid. phlegm can be physiological. In Hyungsang medicine. since the Hyungsang which an individual shows determines his or her illness. the principle to treat phlegm also varies according to the individual's Hyungsang. Thus, the author reviewed ‘Jisan’s Special Lectures for Clinicians’ and summarized the concepts and diagnostics of phlegm, and treatment and regimens by Hyungsang medicine. The concepts of phlegm : Phlegm is not only a pathologic product of disharmony of Jung(精), Ki(氣), Shin(神) and Hyul(血) but also a driving force to mature and transform these constituents. Phlegm is another designation for the Fluid. Phlegm can act as an alternative substance or buffering agent. If we describe our body as the habitat of worms. phlegm could be the inhabitant. Diagnostics of phlegm : The infraorbital areas have a dark-gray or blackish coloring. A man who is feminine or a woman who is masculine tends to develop phlegm. One of the major signs of phlegm is pain in Chungwan(中脘). The color of the skin does not change. Treatment of phlegm by Hyungsang medicine. For the Jung type(精科) and the Hyul type(血科), phlegm treatment is to supply Jung and Blood or eliminate Damp-phlegm. The man needs tonifying herbs and the woman needs herbs which promote the flow of Ki(氣). Children and the aged are. tegardless of the nature of disease, to be warmed and supplemented. For the heavy man. treatment is to supplement Ki and eliminate Damp; for the slim man. treatment is to supplement Yin(陰) and purge Heat.

왕륜(王綸)의 삼기론 및 그 관련 논의들에 대한 고찰 (A Study of Wang Lun (王綸)'s Assertion on Ginseng and Astragali Radix and Its Related Arguments)

  • Objectives : The purpose of this study was to investigate Wang Lun (王綸)'s assertion on Ginseng and Astragali Radix and its related arguments that had shown critical or advocative point of views about Wang lun's theory. It was expected that this study would help understand the history of arguments on the usage of Ginseng and Astragali Radix that had been triggered by Wang lun's assertion. Methods : In the process of investigation, this study first looked into the assertions of Wang Lun and the representative arguments about it, which includes Yu Tuan, Wang Ji (汪機), Sun Yikui (孫一奎). And secondly, this study also examined the other following additional arguments that had shown the conclusional aspects of this disputes. Results & Conclusions : Wang Lun (王綸)'s assertion on Ginseng and Astragali Radix proclaimed that in the symptoms of fevers in lung or lack of blood (血) the use of Ginseng and Astragali Radix must be prohibited. This assertion later faced severe criticism by opposers like Yu Tuan, Wang Ji. These opposers asserted that Yang (陽) has the ability of creating Yin (陰), and Ginseng and Astragali Radix could be the right drugs to the symptoms by lack of Yin. But a few approvers like Sun Yikui (孫一奎) had tried to advocate Wang Lun's assertion and casted the question of true meaning of Wang Lun. And In the other following arguments, it could be said that the influence of Wang Lun's assertion had gradually waned but could have been reciprocally accepted in the opposite point of view.

괸당, 정낭(錠木), 묘(墓)의 신문(神門)과 유전자(RNA)의 접목 (The Hyper Connection of The Heredity Gene(RNA) and The Goendang with Jong Nang/Tomb Gate)

  • 죽음의 문화는 삶의 문화의 반쪽이라는 의미에서 상보적(相補的)이다. 3개의 이승 정낭과 2개의 묘(墓)의 저승 신문(神門)은 올레길 공간체로 연결되어 있다. 그 공간체에는 삶과 죽음사이의 상생(相生)과 상극(相剋)이 공존하는 상보성(相補性)(complementarity) 원리가 제주 문화(文化)에 숨어있다. 대(對)와 대(待)이다. 즉 반대되는 것은 서로 보완적이다란 말이 "(Contraria Sunt Complementa 라틴어)" 서로 대립하면서도 서로 의존하는 관계로 서로가 서로를 품은 관계를 뜻한다. 정낭은 통신 원리로 사용될 뿐 아니라 인체의 RNA Codon에 기본 원리로 사용된다. 또한, 묘의 사각형 산담 귓돌과 한국의 태극과 괘(卦), 유전자(RNA)의 괘(卦), 연결고리의 유사성 Pattern을 들 수 있다. 제주에는 흑용만리 곡선밭담과 사각형 산담이 들판에 펼쳐있다. 제주에서 돌담은 괸담(Stone Networks)으로 연결되고, 괸담의 관습상 발음이 되는 괸당은 친족(Relative Family Networks)로 연결된다. 조상의 명당 묘와 자손들 관계는 영혼적으로 동기감응(同氣感應: Soul Synchronizing the Ancestor to Offspring)이 되어 발복(發福: Change in Future)이 된다고 믿고, 육체적인 피(血)인 유전인자가 자식들에게 직접 전수된다.   • This thesis is a study composed of eight chapters from 9. to 16. of the Maek Kyoung(脈經) Vol. VIII. ; the symptom & the pulse of Jaundice(黃疸), Malarial Disease(?疾). Carbuncle(癰腫) Intestines Carbuncle(腸癰), & c. It is as follows : Chapter 9 refers to the symptom, pulse, treatment and prognosis of Jaundice(黃疸) and Malarial Disease(?疾). Chapter 10 refers to the cause, symptom, pulse and treatment of Cardialgia(胸痺), Cardiagra(心痛) and Nephric Accumulation(賁豚). Chapter 11 refers to the symptom, pulse and treatment of Abdominal Fuliness(腹滿), Cold Mounting(寒疝) and Abiding Food(宿食). Chapter 12 refers to the symptom and pulse of Accumulation and Mass of the Five Viscera(五臟積聚). Chapter 13 refers to the cause, pathogenesis, symptom, pulse, treatment and prognosis of Terror and Palpitation due to Fright(驚悸), Hematemesis(吐血), Nasal Hemorrhage(?血), Metrorrhagia(下血) and Extravasated Blood(瘀血). Chapter 14 refers to the cause, pathogenesis, symptom, pulse and treatment of Vomiting(嘔吐), Hiccough(?) and Diarrhea(下利). Chapter 15 refers to the cause, pathogenesis, symptom, pulse and treatment of Atrophy of Lung(肺?), Pulmonary Abscess(肺癰), Lung-distention(咳逆上氣) and Phlegm(痰飮). Chapter 16 refers to the cause, pathogenesis, symptom, pulse. treatment and prognosis of Carbuncle(癰腫), Intestines Carbuncle(腸癰), Wound(金瘡) and Acute Eczema(侵淫瘡). There have been abundant investigations in China. But we couldn't find a clear result yet, and they were written in archaic texts and colloquial Chinese, therefore it is needed to be translated into Korean. And there was only one inaccurate translation with insufficient annotation. So I hope this study will be useful to develope Oriental Medical Diagnostics.

  • 1. 형상의학(形象醫學)에서는 사람을 얼굴형태에 따라 정(精) 기(氣) 신(神) 혈(血)과로 이목구비(耳目口鼻)의 기능에 따라 어(魚) 조(鳥) 주(走) 갑류(甲類)로 분류하며, 이러한 분류에 따른 장부(臟腑)의 특성을 사암침(舍巖鍼) 운용에 활용하면 임상에서 활용도가 높다. 2. 어류(魚類)는 수(水)의 기운(氣運)이 많아 수체(水體)라고도 하며 신장(腎臟)이 발달하여 신장(腎臟)과 관련된 병이 오기 쉽다. 그 본치(本治)가 보정보기(補精補氣)이며 주로 신정격(腎正格)을 운용하고 신양허쇠(腎陽虛衰)인 경우 신열격(腎熱格), 신음허(腎陰虛)인 경우 신한격(腎寒格)을 쓰며 정혈(精血)의 휴손(虧損)이 심한 경우 간정격(肝正格)도 운용할 수 있다. 3. 조류(鳥類)는 화(火)의 기운(氣運)을 많이 받아 화체(火體)라고도 하며 심장(心臟)이 발달하여 심장병(心臟病) 신경성 질환이 잘 온다. 그 본치(本治)가 자음강화(滋陰降火)며 심장(心臟)의 음혈(陰血)을 보(補)하고 화(火)를 내리는 심한격(心寒格)을 위주로 하여, 심화(心火)를 사(瀉)하면서 음혈(陰血)을 보해주고 신지(神志)를 안정시키는 심승격(心勝格), 심기(心氣)가 부족한 경우 심정학(心正格), 심담(心膽)이 모두 허(虛)한 경우 담정격(膽正格)을 운용할 수 있다. 4. 주류(走類)의 목(木)의 기운(氣運)이 많아 목체(木體)라고도 하며 간(肝)이 발달(發達)하여 간(肝)과 관련된 병(病)이 잘 온다. 그 본치(本治)가 청열사습(靑熱瀉濕), 자혈양근(滋血養筋)이므로 간정격(肝正格)을 주로 운용하고, 간화(肝火)가 동(動)하거나 간실증(肝實證)이 나타난 경우 간한격(肝寒格)이나 간승격(肝勝格)을, 간기(肝氣)가 항진(亢進)으로 인해 비기(脾氣)가 허(虛)해져 있는 경우 비정격(脾正格)을, 습열이 너무 성(盛)한 경우 대장정격(大場正格)을 운용할 수 있다. 5. 갑류(甲類)는 금(金)의 기운(氣運)이 많아 금체(金體)라고도 하며 폐(肺)가 발달하여 폐(肺)와 관련된 병이 잘 온다. 그 본치(本治)가 해울소담(解鬱消痰)이므로 폐정격(肺正格)을 통해 보폐순기(補肺順氣)하며 울증(鬱症)이 심한 경우 폐승격(肺勝格)으로 통해 소담(消痰)시켜주고 기울방(氣鬱方)으로 해울(解鬱)하기도 한다. 6. 정과(精科)는 그 특성상 정(精)의 누설(漏泄)에 의한 증상 및 정부족(精不足), 양허증상(陽虛證狀)과 정(精)의 과도한 응집(凝集)에 의한 습열(濕熱)이 기본 병리이며 기본처방은 신기(腎氣)를 강화하는 신정격(腎正格)을 중심으로 하여, 신열격(腎熱格) 신한격(腎寒格) 등을 변증에 따라 운용하며 습열(濕熱)이 성한 경우 대장정격(大腸正格) 비승격(脾勝格) 등 습열(濕熱)을 다스리는 처방과 비정격(脾正格), 습담방(濕痰方) 등 습담(濕痰)을 다스리는 처방이 운용될 수 있다. 7. 기과(氣科)의 기본 병리(病理)는 기울(氣鬱), 기체(氣滯)에 의한 구기(九氣), 칠기(七氣), 중기(中氣), 매핵기(梅核氣), 불면증(不眠症) 등 신경성 질환이 많고 특히 여자의 경우 손발과 하복부가 차고 대소변이 안좋으며 징가(癥痂), 현벽(痃癖) 등의 질환이 많다. 또한 지나친 발산(發散)으로 인하여 기허증(氣虛證)이 나타나기도 한다. 따라서 기본처방은 기울방(氣鬱方), 담음방(痰飮方), 담현방(痰眩方), 기수방(氣嗽方), 폐승격(肺勝格), 삼초정격(三焦正格) 등이며, 기허증(氣虛證)이 나타나는 경우 폐정격(肺正格)을 사용할 수 있다. 8. 신과(神科)의 기본병리는 칠정울결(七情鬱結)이나 담화(痰火), 화성음허(火盛陰虛)이며 대표적인 증상은 경계(驚悸), 정충(怔忡), 건망(健忘), 불면(不眠), 전간(癲癎), 전광(癲狂) 등이다. 따라서 기본처방은 심한격(心寒格), 심승격(心勝格), 심정격(心正格), 담정격(膽正格), 비한격(脾寒格)을 중심으로 열담방(熱痰方), 군화방(君火方), 상화방(相火方), 화울방(火鬱方) 등을 사용할 수 있다. 9. 혈과(血科)의 기본병리는 어혈(瘀血) 및 출혈이며 대표적인 증상은 구규출혈(九竅出血)과 어혈(瘀血), 혈허증(血虛證)이다. 따라서 기본처방은 간정격(肝正格), 손혈방(損血方), 심한격(心寒格) 비한격(脾寒格) 심비한격(心脾寒格), 소장정격(小腸正格), 소장한격(小腸寒格), 어혈방(瘀血方), 뉵혈방(衄血方) 등을 사용할 수 있다.

치(齒)에 대(對)한 문헌적(文獻的) 고찰(考察) (A Literature Study of the Teeth)

  • The purpose of this study was to investigate the relationship between the teeth and Zhang-Fu(臟腑), dental diseases, and the hygiene of the mouth through the literature of oriental medicine. First, the relatonship between the teeth and Zhang-Fu is reviewed as follows: The teeth are influenced by Shen(腎) because they are the end of bone and Biao(標) of Shen. Gingiva is related to Wei(胃) and Da-Chang(大腸) because it is passed by Yangming-Channel(陽明經). The growth and nutrition of teeth depends on Shen. The pathological condition of Shen causes the gingival atrophy, the loose of teeth, dedentition due to aging, withering of teeth, and tartar: whereas the pathological condition of Wei and Da-Chang causes toothache, gingivitis, inflamed gums, bad breath, and gingival hemorrhage. Second, the causes and therapies of dental diseases through the literature can be summarized as follows: The major causes of toothache are the pathogenic condition of wind-heat and wind-cold, the heat syndrome of Wei, the damp-heat of intestine, flaring-up of fire of deficiency type, rotten tooth, etc... The principal causes of dedentition and the shaking and loose of teeth are the deficiency of Shen, and the rest of causes are the damp-heat of Yangming. Gingival atrophy is caused by the deficiency of Shen, whereas the gingival hemorrhage comes from the factors in the pathogenic factor of wind-heat of Yangming-Channel, the heat syndrome of stomach, and the deficiency of Shen. The causes of grinding of teeth during sleeping are stomach-heat, and the delayed dentition and the withering result from the deficiency of Shen-Jing.(腎精) The principal therapies of toothache are removing wind and heat, clearing away heat and prompting diuresis, clearing away the stomach-heat, replenishing vital essence to tonify the Shen, relieving superficial syndrome by wind-cold, and alleviating pain by destroying parasites. For the prescription of the principal therapies, there are Xijio Dihuang Tang, Jiajian Ganlu Yin, Qufeng Wan, Qingwei San, Tiaowei Chenggi Tang Shengong Wan, Liangge San Qingwei Tang Yunu Jian, Liuwei Dihuang Wan Zuogui Yin Bawei Wan Wanshao Dan, Xixin San Badou Wan Gianghuo Fuzi Tang, Jiuzi Tang Badou Wan, etc... The therapies of dedentition and the shaking and loose of teeth are replenishing vital essence to tonify the Shen, and warming and recuperating the Shen-Yang: as the prescription, there are Liuwei Dihuang Wana Zuogui Yin, and Bawei Wan Anshen Wan Wanshao Dan Yougui Wan etc... The therapies of gingival hemorrhage are clearing away the stomach-heat, replenishing vital essence to tonify the Shen, warming and recuperating the Shen-Yang(腎陽), and moisturing and purging intence heat with the prescription of Tiaowei Chenggi Tang Xijiao Dihuang Tang, Liuwei Dihuang Wan Zuogui Yin, Bawei Wan Anshen Wan, and Yunu Jian.
