The Spread of Contrast media in Splanchnic Nerve Block (내장신경차단시 조영제확산에 관한 연구)

  • Splanchnic nerve block is performed to relieve intractable upper abdominal pain caused by carcinoma of the pancreas, stomach, liver, or colon; and upper abdominal metastasis of tumors having more distant origins. We have performed splanchnic nerve blocks under control of X-ray fluoroscopy, for all cases of alcohol splanchnic nerve block at $L_1$ vertebra, to determine both the position of the needle tips and the spread of contrast media. During the period from December 1987 to August 1988, this method was used in 40 cases of malignancy at Severance Hospital and we clinically evaluated the location of the needle tip and the spread of contrast media. The results were as follows: 1) Our method was a retrocrural approach, the splanchnic nerve block, in all cases. 2) Most of the inserted needle points were located in the upper and anterolateral part of the $L_1$ vertebra on the antero posterior roentagenogram and in the upper quarter anteriorly on the lateral roentgenogram. 3) There was no specific relationship between the location of the needle and the spread of the contrast media. 4) The contrast media was spread around the needle and then upward along the anterior margin of the vertebral body in most of the cases. 5) Pain relief was obtained immediately in 37 cases (92.5%), but in 3 cases only after a second splanchnic nerve block.

Preliminary study on contrast flow analysis of thoracic transforaminal epidural block

  • Background: The thoracic transforaminal epidural block (TTFEB) is usually performed to treat herpes zoster or postherpetic neuralgia (PHN). Especially, multiple segmental involvements and approximate contrast medium spread range, according to volume, help to choose the proper drug volume in the transforaminal epidural block. This study investigated the contrast medium spread patterns of 1-ml to 3-ml TTFEBs. Methods: A total of 26 patients with herpes zoster or PHN were enrolled in this study. All participants received 1 ml, 2 ml, or 3 ml of contrast medium. Results were divided into Groups A, B and C based on the volume (1, 2, or 3 ml), with n = 26 for each group. After the injection of contrast medium, the spread levels were estimated in both the lateral and anteroposterior (AP) images using fluoroscopy. Results: The cephalad spread of contrast medium in the lateral image as expressed by the median (interquartile range) was 2.00 levels (1.00-2.00) for Group A, 2.50 (2.00-3.00) for Group B, and 3.00 (2.00-4.00) for Group C. The caudal spread level of contrast medium was 1.00 (1.00-2.00) for Group A, 2.00 (2.00-3.00) for Group B, and 2.00 (2.00-3.00) for Group C. There was ventral and dorsal spread of the 3-ml contrast medium injection in 88% (23/26) of cases in the lateral image. Conclusions: Injection of 3 ml of contrast medium through the foramina spread 6 levels in a cephalocaudal direction. Spread patterns revealed a cephalad preference. TTFEB resulted in dorsal and ventral spread in a high percentage of cases. This procedure may be useful for transferring drugs to the dorsal and ventral roots.

The Spread of Contrast Media in Celiac Plexus Block (복강신경총 차단시의 조영제 확산)

  • Celiac plexus block is performed to relieve intractable upper abdominal cancer pain. Generally, celiac plexus blocks have been performed under control of X-ray fluoroscopy to determine the position of the needle tip and the spread of contrast media. During the period from March 1992 to February 1994, we have performed 21 cases of neurolytic celiac plexus block to alleviate pain of intra-abdominal malignancy. We retrospectively evaluated the location of the needle tip and the spread of contrast media. P-A views of simple abdomen demonstrated the locations of the needle tip: 66.7% of the left needle tips were in upper 3/1 of L1 (6 cases) and $T_{12}-L_1$ interspace (8 cases), 50% of the right needle tips were in upper 1/3 of $L_1$, (6 cases) and $T_{12}-L_1$ interspace (4 cases). Contrast media from the right needle spread upward to middle 1/3 of $T_{10}$ (5 cases) and middle 1/3 of $T_{11}$ (5 cases), downward to middle 1/3 of $L_1$ (6 cases) and lower 1/3 of $L_1$ (3 cases). Contrast media from the left needle were spread upward to middle 1/3 of $T_{10}$ (5 cases) and evenly to other spaces, downward to middle 1/3 of $L_1$, (4 cases) and $L_1-L_2$ interspace (6 cases). We analyze the spread of contrast media according to distance from needle tip by authors score system. Contrast media of right needle spread upward 6.1 and downward 3.4, that of left needle spread upward 6.5 and downward 3.7.

  • In this paper, in order to reduce the power consumption of LCD TV, we present a two-dimensional dimming algorithm for LED BLU(Backlight Unit) using light spread function and a pragmatic implementation method. In conventional two dimensional dimming algorithms, LED BLU is divided into MxN blocks and proper backlight luminance for each block is computed according to input image and used for dimming control. But, LED light of each block is diffused and affects the neighboring blocks so that luminance of each block becomes larger than that of intent. In the proposed algorithm, dimming control values are reduced by the amount of quantity affected by the neighboring blocks using light spread function, resulting in additional reduction of power consumption.

  • MANET transmits data by hop-by-hop method because it is composed of mobile nodes without support of any infrastructure. Its structure is very similar to a block chain. However, it is exposed to various threats such as data tampering or destruction by malicious nodes because of transmission method. So, ensuring the integrity of transmitted data is an important complement to MANET. In this paper, we propose a method to apply the block chain technique in order to protect the reliability value of the nodes consisting the network from malicious nodes. For this, hierarchical structure of a cluster type is used. Only cluster head stores the reliability information of the nodes in a block and then, this can be spread. In addition, we applied block generation difficulty automatic setting technique using the number of nodes selecting cluster head and the reliability of cluster head to prevent the spread of wrong blocks. This can prevent block generation and spread by malicious nodes. The superior performance of the proposed technique can be verified by comparing experiments with the SAODV technique.

  • This paper presents adaptive spread-spectrum watermarking scheme on the H.264/AVC video. The H.264/AVC has become popular video coding since it's compression performance has been improved greatly. That is why there is not enough space for embedding watermark using the spread-spectrum method. To overcome this problem, we propose selective block embedding scheme. The blocks which don't contain non-zero DCT (Discrete Cosine Transform) coefficients are selected by value. So we have enough space to embed watermarks. Experimental result shows that proposed scheme improves recovery rates of watermark about 12% and reduces distortion of watermarked video.

  • In this paper, we propose an efficient watermarking algorithm using block indexing. The proposed algorithm is a novel blind watermarking scheme using the indexed watermark value based on the spread spectrum method. The watermark insertion is allocated into index value of each block after dividing original image into sub-blocks. The watermark embedded in mappinged with index values of blocks, And the mappinged blocks convert to DCT and then the PN sequence embedded to middle frequency band. Consequently the watermark is expressed by index value of sub-blocks. The watermark extracted from the correlation of between PN sequence and watermarked image. Experimental results demonstrate that the watermarked image has a good quality in terms of imperceptibility and is robust against various attacks.

  • In this paper, we propose an algorithm of block synchronization that uses data withoug synchronization preambles. Block synchronization systems is implemented using the DSP chip employing the proposed algorithm. The data spread of the DFT blocks is proportional to the offset of DFT block and this information is used to achieve the block synchronization in the receiver. The initial bleock synchronization and the clock synchronization between transmitter and receiver are achieved using the early-late removal of the guard interval. The hardware implmentation is carried out using the DSP chip TM320C30 to verify the proposed block synchronization algorithm with the data rate 1200bps. The DSP chip calculates the spread of the 128 complex FFT in the receiver with the system clock 30MHz. It is believed that the proposed synchronization algorithm can be used in the design of OFDM block synchronization with the high processing DSP chip.

  • In this study, FDS fire simulation experiments and measured wind speed by applying the exterior installation portion for blocking the spread of the fire was investigated. As a result, aluminum composite panels installed in the lower and the upper part of the panel to remove all the lower side, and then the maximum wind speed 0.24 m/s and the upper side 0.58 m/s were measured. In the FDS, the measured wind speed difference air currents are approximately 3.7 times in 12 seconds, the occurrence of 17 seconds early moment wind 2.2 m/s was measured from. Before and after the fire occurred in early of the air velocity about 39 seconds was 3.5 times difference. Such air currents caused by the temperature of the building but also by the building height was found. Turbulent flame of fire by expanding the vertical extent of damage become greatly important factor. Therefore, through the exterior installation portion of the block that can delay the spread of fire is expected that this should be taken.
