Could Clinical Pathways Improve the Quality of Care in Patients with Gastrointestinal Cancer? A Meta-analysis

  • This meta-analysis was performed to assess the implementation effects of clinical pathways in patients with gastrointestinal cancer. A comprehensive search was conducted in the Cochrane Library, PubMed, EMBASE, Web of Science and Chinese Biomedical Literature Database (from inception to May 2014). Selection of studies, assessing risk of bias and extracting data were performed by two reviewers independently. Outcomes were analyzed by fixed-effects and random-effects model meta-analysis and reported as mean difference (MD), standardized mean difference (SMD) and odds ratio (OR) with 95% confidence intervals (CI). The Jadad methodological approach was used to assess the quality of included studies and the meta-analysis was conducted with RevMan 5.1 software. Nine citations (eight trials) involving 642 patients were included. The aggregate results showed that a shorter average length of stay [MD = -4.0; 95% CI (-5.1, -2.8); P < 0.00001] was observed with the clinical pathways as compared with the usual care. A reduction in inpatient expenditure [SMD = -1.5; 95% CI (-2.3, -0.7); P = 0.0001] was also associated with clinical pathways, along with higher patient satisfaction [OR = 4.9; 95% CI (2.2, 10.6); P < 0.0001]. Clinical pathways could improve the quality of care in patients with gastrointestinal cancer, as evidenced by a significant reduction in average length of stay, a decrease in inpatient expenditure and an improvement in patient satisfaction. Therefore, indicators and mechanisms within clinical pathways should be a focus in the future.

High Expression of Bcl-2 Protein Predicts Favorable Outcome in Non-small Cell Lung Cancer: Evidence from a Systematic Review and Meta-analysis

  • Background: The prognostic value of Bcl-2 protein expression in non-small cell lung cancer (NSCLC) is under debate. We therefore systematically reviewed the evidence for Bcl-2 protein effects on NSCLC survival to elucidate this issue. Materials and Methods: An electronic search in Pubmed and Embase complemented by manual searches in article references were conducted to identify eligible studies to evaluate the association between Bcl-2 protein expression and overall survival (OS) as well as disease free survival (DFS) of NSCLC patients. Combined hazard ratios (HRs) with corresponding 95% confidence intervals (95%CIs) were pooled using the random-effects model. Results: A total of 50 trials (including 52 cohorts) encompassing 7,765 patients were pooled in the meta-analysis regarding Bcl-2 expression and OS of NSCLC patients. High expression of Bcl-2 protein had a favorable impact (HR=0.76, 95%CI=0.67-0.86). In the group of Bcl-2 expression and DFS, 11 studies including 2,634 patients were included. The synthesized result indicated high expression of Bcl-2 protein might predict good DFS (HR=0.85, 95%CI=0.75-0.95). Conclusions: Our present meta-analysis demonstrated favorable prognostic values of Bcl-2 expression in patients with NSCLC. Further prospective trails are welcomed to validate the utility of assessing Bcl-2 in NSCLC patient management.

The Relationship between Children's Information Processing and Basic Learning Abilities (유아의 정보처리능력과 기초학습능력 간 관계)

  • The purpose of this study was to examine the relationship between children's information processing ability and basic learning abilities. To collect the data, two tests were given to 99 children. The Korean K-ABC(Moon & Byun, 1997) and Pictorial Basic Learning Abilities for Children(Kim, 2011) were used to examine the relationship between children's information processing and basic learning abilities. The collected data were analyzed by correlation analysis and multiple regression analysis. According to the results of this study, there was a significant positive correlation between information processing(sequential processing, simultaneous processing) and basic learning abilities including reading, writing, and basic mathematics. And information processing significantly affected basic learning abilities. Namely, simultaneous processing explained 22% of basic learning abilities and by adding sequential processing, the explanation was increased to 25%. In conclusion, the results of this study suggest various implications about children's basic learning abilities. These implications will help teachers and parents to understand their children's learning.

  • The purpose of this study is to provide basic information for elderly women's clothing design and to develop dress forms that can reflect the characteristics of their bodies. Three hundered twenty subjects, between the ages of 60 .approx. 85, were chosen and the data were collected from anthropometric data. Principal component analysis which is one of the basic methods in factor analysis was applied to the interpretation of anthropometric data. As a result of analysis, the data are able to be decided into appropriate group.

  • Fatigue strength analysis is one of the most important themes of ship structure design, as fatigue damages are reported on ship structures even now. But these need basic research workes which will take time. The others are the problem to apply fatigue strength analysis in design and have to be investigated in parallel with basic researches. The one of major items in the latter is the critical damage factor to define with S-N curve for fatigue strength analysis of ship structure design.

Analysis of 'Basic Information Technology' Textbooks in Technical High Schools (공업계 고등학교 '정보 기술 기초' 교과서의 분석)

  • In this paper, both content analysis and Romey analysis were employed to analyze 'Basic Information Technology' textbooks in technical high schools. Not only was the content analysis employed to study the organization and contents of the textbooks, but also the Romey analysis was applied so as to determine inquisitive tendency on four factors - text, data, activity and evaluation. The results from content analysis showed that the total number of pages, reading material, the number of concepts introduced in each section, and the use of application software demonstrated a discrepancy among textbooks. Also, Romey analysis showed that while the section 'text' was written with relatively low inquiry tendency for its authoritative tone, two textbooks showed high inquisitive tendency for section 'activity'. Finally, based on the obtained results, the ways to improve 'Basic Information Technology' textbooks were proposed.

Examination of Influences of Elementary Schoolers' Basic Psychological Needs in Physical Education Classes on School Happiness and Physical Lifestyles (초등학생의 체육수업 기본심리욕구가 학교행복감과 신체적 라이프스타일에 미치는 영향)

  • The purpose of this study is to examine how basic psychological needs of elementary school physical education classes influence school happiness and physically active lifestyles. Questionnaires were distributed to 300 elementary schoolers in five elementary schools in I city. After excluding 22 incomplete questionnaires, a total of 278 questionnaires were used for data analysis. For data analysis, descriptive analysis, exploratory factor analysis, correlation analysis, and multiple regression analysis were used. Correlation analysis revealed that the competence and relatedness of basic psychological needs are positively correlated with all the subsets of school happiness and moderate-vigorous physical activity. Multiple regression showed that the competence of basic psychological needs positively predicted self-respect and relationships with teachers of school happiness and also predicted moderate-vigorous physical activity. Multiple regression also showed that the relatedness of basic psychological needs positively predicted all the subsets of school happiness. The findings indicate the importance of basic psychological needs in physical education classes on school happiness and physically active lifestyles. Thorough this research, it is hoped that the values of physical education classes in elementary schools increase.
