Study of Intravenous Single-Dose Toxicity Test of Bufonis venonum Pharmacopuncture in Sprague-Dawley Rats

  • Objectives: Bufonis venonum (BV) is toad venom and is the dried, white secretions of the auricular and the skin glands of toads. This study was performed to evaluate the toxicity of intravenous injection of Bufonis venonum pharmacopuncture (BVP) through a single-dose test with sprague-dawley (SD) rats. Methods: Twenty male and 20 female 6-week-old SD rats were injected intravenously in the caudal vein with BVP or normal saline. The animals were divided into four groups with five female and five male rats per group: the control group injected with normal saline, the low-dosage group injected with 0.1 mL/animal of BVP, the medium-dosage group injected with 0.5 mL/animal of BVP and the high-dosage group injected with 1.0 mL/animal of BVP. We performed clinical observations every day and body weight measurements on days 3, 7 and 14 after the injection. We also conducted hematology, serum biochemistry, and histological observations immediately after the observation period. Results: No mortalities were observed in any experimental group. Paleness occurred in the medium- and the high-dosage groups, and congestion on tails was observed in females in the medium- and the high-dosage groups. No significant changes in weight, hematology, serum biochemistry, and histological observations that could be attributed to the intravenous injection of BVP were observed in any experimental group. Conclusion: The lethal dose of intravenously-administered BVP in SD rats is over 1.0 mL/animal.

  • Objectives: Bufonis venonum (BV) is the dried white secretions of the auricular and skin glands of the toads Bufo bufo gargarizans or Bufo melanosticus Schneider. This study was performed to evaluate the toxicity of intramuscularly-administered Bufonis venonum pharmacopuncture (BVP) and to calculate its approximate lethality through a single-dose test with Sprague-Dawley (SD) rats. All injections were in the left thighs of the rats. After administration, we conducted clinical observations everyday and body weight measurements on days 3, 7 and 14 after the injection. We also carried out hematology, serum biochemistry, and histological observations on day 15 after treatment. Results: No mortalities were observed in any experimental group. No significant changes in weight, hematology, serum biochemistry, and histological observations that could be attributed to the intramuscular injection of BVP were observed in any experimental group. Conclusion: Lethal dose of BVP administered via intramuscular injection in SD rats is over 0.5 mL/animal.

A Case Study on a Patient with Non-alcoholic Fatty Liver Disease(NAFLD) Improved on Sonography by Oriental Obesity Therapy (한방비만치료 후 초음파 영상에서 비알코올성 지방간의 호전을 보인 환자 치험 1례)

  • Objectives The purpose of this study was to investigate the clinical effect of oriental obesity therapy on obese patient with non-alcoholic fatty liver disease(NAFLD). Methods The patient was treated from November 17, 2008 to May 15, 2009 for obesity treatment. And the patient had intensive care through admission treatment from November 17, 2008 to November 29, 2008. Electrolipolysis, auricular acupuncture, herbal medicine, low calorie diet and aerobic exercise were done during the treatment period. Results His body Weight decreased from 102.8kg to 82.7kg. The BMI decreased from 35.2kg/m$^2$ to 28.9kg/m$^2$. NAFLD effectively improved on sonography form grade II${\sim}$III to grade I${\sim}$II after oriental obesity therapy, AST decreased from 65 to 19. ALT decreased from 152 to 18. Oriental obesity therapy could be effective to treat obese patient with NAFLD. Conclusions The obese patient with NAFLD improved on sonography by oriental obesity therapy.

A Case Study on A Non-alcoholic fatty liver disease(NAFLD) Patient Improved by The Oriental Obesity Therapy Program (한방 비만 치료 후 간기능 개선을 보인 비알코올성 지방간 환자 1례 보고)

  • Objectives The purpose of this study was to investigate the clinical effect of oriental medical obesity therapy on obese patient with non-alcoholic fatty liver disease. We supposed to treat patient 21 years old that had non-alcoholic fatty liver with oriental medical obesity therapy. Methods The patient was treated from the 19th of July to the 22th of August for obesity treatment. And the patient had intensive care through admission treatment from the 19th of July to the 30th of July. Herbal medicine, low calorie and low cholesterol diet, electrolipolysis, auricular acupuncture, aerobic exercise was done during the treatment period. Results His body weight decreased from 107.10㎏ to 98.85㎏. The BMI decreased from 36.2㎏/$m^2$ to 34.5㎏/$m^2$. Abdominal visceral fat tissue decreased from 190.80$cm^2$ to 145.05$cm^2$. AST was decreased from 113 to 45. ALT was decreased from 145 to 40. r-GT was decreased from 123 to 51. Oriental obesity therapy could be effective to treat obese patient with non-alcoholic fatty liver disease. Conclusions The obese patient with non-alcoholic fatty liver disease had improved liver function after oriental obesity treatment.

Effects of Non-pharmacological Interventions on Primary Insomnia in Adults Aged 55 and Above: A Meta-analysis (수면장애가 있는 중장년 환자에게 적용한 비약물적 중재의 효과: 메타분석)

  • Purpose: This study was performed to evaluate the effects of non-pharmacological interventions on sleep disturbance amongst adults aged 55 and above. Methods: PubMed, Cochrane Library, EMBASE, CINAHL and several Korean databases were searched. The main search strategy combined terms including non-pharmacological interventions and presence of insomnia. Non-pharmacological interventions included cognitive behavioral therapy, auricular acupuncture, aromatherapy, and emotional freedom techniques. Methodological quality was assessed using Cochrane's Risk of Bias for randomized studies and Risk of Bias Assessment tool for non randomized studies. Data were analyzed by the RevMan 5.3 program of Cochrane Library. Results: Sixteen clinical trials met the inclusion criteria with a total of 962 participants. Non-pharmacological interventions was conducted for a mean of 5.5 weeks, 7.7 sessions, and an average of 70 minutes per session. The effects of non-pharmacological interventions on sleep quality (ES=-1.18), sleep efficiency (ES=-1.14), sleep onset latency (ES=-0.88), awakening time after sleep onset (ES=-0.87), and sleep belief (ES=-0.71) were significant, and their effect sizes were ranged from moderate to large. However, the effects on total sleep time and insomnia severity were not significant. Conclusion: The findings of the current study suggest that non-pharmacological interventions have a positive impact on attitudes and beliefs about sleep, sleep quality, sleep duration, and sleep efficiency. Therefore, the findings of the study provide an evidence to incorporate various non-pharmacological interventions into nursing practice to improve both sleep quality and quantity in patients with insomnia.

Review of Korean Clinical Studies on Treatment for Childhood Obesity (소아비만의 치료에 대한 국내 임상연구 동향)

  • Objectives The purpose of this study is to investigate Korean clinical studies on treatment of childhood obesity and to propose for better treatment options for childhood obesity. Methods Based on RISS, KISS, OASIS, KMbase with the keyword 'childhood obesity', 'child obesity', 'obese child', 'overweight child', total of 21 randomized controlled trials (RCTs), 11 single clinical studies, 3 chart reviews and 3 case reports have been found, and were analyzed. Results and Conclusion 1. Some study subjects were classified based on their sex - only men or women in a group or sometimes both. Another way of classify the study subjects were based on their age - preschooler, primary school students, middle school students, and high school students. The diagnostic criteria of the subjects were BMI (body mass index), Body fat percentage, Obesity index, Weight and unknown criteria. 2. The treatments used in 38 studies were exercise alone, education alone, both exercise and education or herbal therapy. The 7 studies that used herbal therapy as part of their study intervention had a study group with herbal intervention only and the other 2 study group with herbal treatment in addition to exercise and education. 3. 9 studies included herb medicines, electroacupuncture, auricular acupuncture, cupping, aroma massage, infrared light, and abdomen pad. Herb medications used in 5 studies included Chegameuiin-tang 2 kinds, Sobieum, Biman-tnag, and I-razin. 4. The methods of assessment used in 38 studies were classified by 7 categories and anthropometry parameters which is the basic methods are used in all studies 5. Almost studies (29 studies in all 38 studies) were using anthropometric parameters results on significantly effectiveness of childhood obesity. 6. More studies are needed to prove true effectives from various treatments, especially herbal therapy for childhood obesity.

  • Pain is the most common symptom causing patients to seek physical therapy. Auriculotherapy has shown to be effective in pain management as well as in treating a wide variety of disorders. The pupose of this study were 1) to describe the historical, theoretical background and the effects of Auriculotherapy 2) to identify the applied method in clinic and 3) to encorage of the application in physical therapy.

  • The authors are using Qi-diagnosis (integrated diagnosis by bio-energy) that is a method of diagnosis and treatment. We applied Qi-diagnosis to the main study to lay the foundation and framework for research and education about the Korean Medicine. The authors try to describe systemically and specifically the Qi-diagnosis that the authors are using in clinical diagnosis ane treatment so that anyone can use it. The authors have been able to grasp the flow of human bio-energy through years of training. It has had many effects by applying the Qi-diagnosis to patients. The steps of the bio-energy flow have become objective. And the authors have been applied to acupuncture, herbal medicine, moxibustion, bruising treatment and anthrax anesthesia in clinical through the Qi-diagnosis. Also, it is applied to the life management of patients. It is applied to arts such as music therapy and art therapy. The deeper the depth of the Qi-diagnosis, the greater the opportunity to utilize the Qi-diagnosis. The Qi-diagnosis is the origin of the korean medicine. It was able to make diagnosis and treatment correct and to establish clues that the medical problems would be solved through the Qi-diagnosis. In order to do so, the diagnostician must be able to feel the auricular flow of the body accurately and objectively. In addition, he must have a comprehensive understanding of the overall framework of medicine. As a result, diagnosis and treatment of the patient as well as general problems of the patient can be identified and advised, so comprehensive treatment is possible. And it is not only a specific person can do it, but it is a diagnostic method that anyone can take if they take the basic steps step by step.

A Case Study on Effect of Herbal Diet an Male Abdominal Obese Patient (남성 복부비만 환자의 치험 1례)

  • We experienced a male patient who was diagnosed as very severe abdominal obesity. From the 3rd of January to the 14th of April we applied herbal medication(Tiganyiyiren-tang), very low calorie diet, aerobic exercise, behavioral therapy, electrolipolysis, infra-red therapy, auricular acupuncture, and aqua-massage to him. And we examined the changes of his height, weight, body composition, body size, liver functional test, abdominal CT finding. The food production of this year has decreased by 5 percent. His weight decreased by 28.2%, PIBW by 28.5%, BMI by 28.2%, PBF by 22.7%, and WHR by 15.9%. The circumference of upper extremity decreased by 25.7%, the circumference of the muscle of upper extremity by 18.2%, the circumference of chest by 20.5%, the circumference of abdomen by 29.5%, the circumference of hip by 17.4%, and the circumference of thigh by 15.9%. In the liver functional test GOT decreased from 42IU/l to 361U/I, GPT from 1211U/l to 48IU/I. Total cholesterol from 211mg/dl to 152mg/dl, and Trigliceride from 192mg/dl to 81 mg/dl. In the abdominal CT finding the amount of fat in the abdominal cavity decreased from $9732mm^2$ to $3639mm^2$.

The Efficacy of Low Frequency Electroacupuncture Therapy for Weight Loss in Obese Korean Women (저주파 전침 위주의 한방비만치료의 체중감량 효과 및 관련 인자에 관한 연구)

  • Purpose: We investigated the efficacy of electroacupuncture therapy in obese Korean women in order to make basic guidelines for the use of oriental medicine in treating obesity. Methods: Forty women were divided into two groups and each was treated 12 times: 1) one group was treated using electroacupuncture (EA) involving auricular acupuncture and physical treatment, without exercise (EA group, n=10), and 2) the other group was treated using electroacupuncture and exercise (EA plus exercise group, n=30). The EA was applied to subcutaneous fat tissue manually. Body weight was evaluated every visit and body composition was checked at the $1^{st}$ and $12^{th}$ visits. Statistical comparisons were made using SPSS13.0. We compared the weight loss, body mass index (BMI), and body composition before and after treatment using the pairedt-test. The correlations between weight loss and BMI and age were examined. Results: After the $12^{th}$ treatment, both groups showed significant reductions in body weight, BMI, body fat, and body fat percentage. The weight loss averaged 4.58${\pm}$3.87% in the EA group and 4.69${\pm}$3.10% in the EA plus Exercise group. The reduction in skeletal muscle was significant in the EA group, but not significant in the EA plus Exercise group. The speed of the weight loss was correlated with age using Pearson's correlation coefficient. Based on BMI, normal and obese subjects in the EA plus Exercise group had significant reductions in body weight, BMI, body fat, and body fat percentage; the obese subjects also had a significant reduction in skeletal muscle. Conclusion: Combined EA therapy may be effective for weight reduction. Age and BMI were related to weight loss using EA therapy.
