  • In case of the hearing impairment with tinnitus and increased sound sensitivity, it is known that the patients tend to appeal the psychologically oriented social handicap rather than communication disability. The audiologist who is responsible for such patients in aural rehabilitation should pay special attention to the counseling techniques including tinnitus retain therapy (TRT), ear protector, noise generator, or specific acoustic training based on close cooperation and rapport. And then the audiologist should try to lessen their reaction to the tinnitus by using a hearing aid. This therapies tries to focus not a. total approach but a treatment to lessen the severity of tinnitus. This paper as a case report that a unilateral sharply slopped sensorineural hearing impaired person with tinnitus and increased sound sensitivity by using four channel digital signal processing (DSP) hearing aid with programming increment at low level (PILL).

가청범위압축방식 보청기의 청각학적 이득에 관한 연구 (Audiological Benefits in Dynamic Range Compression Hearing Aids)

  • Hearing aids play an very important role in aural rehabilitation for hearing impaired person who could not be medically treated. Especially, hearing aids with dynamic range compression (level dependent frequency response: LDFR) method concentrated dynamic output range of receiver into the narrowing dynamic range of sensorineural hearing impaired person. Thus, if aided hearing threshold level is improved and uncomfortable loudness level rises, then the users of hearing aids are favored with extended dynamic range. This study aims to evaluate audiological benefits in dynamic range compression hearing aids. To achieve this, pure tone and speech audiometry were examined to 15 children and 3 adults with narrow dynamic range moderate-to-severe binaural sensorineural hearing loss.

  • 학령 전기 아동은 옹알이 단계부터 8세까지 모국어의 음운 체계를 습득하고 정상적으로 산출한다. 이 시기 청력손실은 외부 소리 자극을 수용하는데 어려움이 따른다. 청각적 수용의 문제는 언어 습득 및 발달을 지체시키고, 의사소통 장애를 일으킬 수 있다. 이외에도 학습, 사회성, 정서 발달 등에도 영향을 준다. 청력 손실은 조기 선별과 진단 및 중재가 중요하다. 그러나 그 정도가 낮거나 특정 음역에 국한된 경우 발견이 어려울 수 있고, 발견하더라도 재활 중재로 연결되기 어려운 경우가 많다. 이 연구는 학령 전기 아동에서 나타난 경도 및 중등고도 대칭성 고음급추형 감각신경성 난청의 사례를 청각학적 평가 결과 해석과 함께 의사소통 관련 문제를 분석하고, 청각 언어병리학적 재활 방향 등을 고찰하고자 한다.

  • The human baby appears to be born with preexistent knowledge of language. This specialized neural structure in the brain awaits auditory experience with language to trigger it into functioning. The auditory-linked acquisition of language is a time-locked function related to early maturational periods in the infant's life. The longer auditory language stimulation is delayed, the less efficient the language facility will be. It is for these reasons that it is urgent to fight the hearing problems of children with all the skill, knowledge and insights of which we are capable, the so called 'rehabilitative process'. An understanding of the timetable and the origin of prenatal to early life development of auditory mechanism will help in planning the aural rehabilitation. Further interests and studies are needed to establish the systematic structure of rehabilitative audiology.

학령전 말더듬아동과 어머니 간 상호작용 시 끼어들기 특성 종단적 분석 (A longitudinal analysis on interruption in preschool children who stutter during interactions with their mothers)

  • 본 연구는 학령전 말더듬아동과 어머니의 자연스러운 상호작용에서 나타나는 아동과 어머니의 끼어들기 특성을 분석하고 아동의 비유창성과의 관련성을 종단적으로 살펴보고자 하였다. 연구대상은 만 2-5세 말더듬아동과 어머니 6쌍, 일반아동과 어머니 6쌍을 대상으로 하였다. 자연스러운 상호작용 상황에서 나타난 발화를 총 두 차례(평가 시점, 평가 후 12개월 시점) 수집하여 아동의 ND(normal disfluency) 빈도와 AD(abnormal disfluency) 빈도, 아동과 어머니의 끼어들기 빈도 및 끼어들기 시 동시에 말한 시간을 분석하였다. 연구 결과, (1) 평가시점에 말더듬아동 집단과 일반아동 집단 간에 비유창성 빈도에서 유의한 차이를 보였다. (2) 평가 시점과 평가 후 12개월 시점에 말더듬아동 집단과 일반아동 집단 간의 끼어들기 빈도와 끼어들기 시 동시에 말한 시간에 유의한 차이가 없었다. 그러나 말더듬아동 집단은 평가 시점보다 평가 후 12개월 시점에 끼어들기 빈도가 유의하게 증가하였다. (3) 평가 시점에 말더듬아동 어머니 집단과 일반아동 어머니 집단 간 끼어들기 빈도에서 유의한 차이가 나타났으나 끼어들기 시 동시에 말한 시간에 유의한 차이가 없었다. (4) 말더듬아동·어머니 집단에서 시점별(평가 시점, 평가 후 12개월 시점) 아동의 비유창성 빈도와 아동과 부모의 끼어들기 특성 간 상관관계를 살펴본 결과, 평가 시점과 평가 후 12개월 시점에서 아동의 ND 및 AD 빈도와 상관관계를 보인 요인은 없었다. 이러한 결과는 아동과 부모의 상호작용 시 나타나는 끼어들기 특성을 대화 상대방의 발화를 방해하는 부정적인 상호작용의 증거로 해석하여 말더듬아동·부모 상호작용 치료 시 끼어들기 행동에 대한 조언이 적절한지에 대해 고려해볼 필요가 있음을 시사한다.

편측 인공와우 이식자의 보청기 사용 (Use of Hearing Aids in Unilateral Cochlear Implantee)

  • The cochlear implantation(CI) as an useful tool for aural rehabilitation in bilateral severe to profound hearing impairment. However, CI prefer to usually one ear in spite of bilateral hearing impaired. because of the various characteristics of hearing loss, the hearing conservation for the future possibility, and socioeconomic condition of hearing impaired person and their families. The unilateral CI has limitations such as a directional loss, a difficult speech understanding in noise and a neural plasticity. These limitations will be overcome by hearing aid(HA) which is familiar with hearing impairer. but HA fitting for bimodal-binaural hearing are difficult because the difference output characteristic of HA and CI. This study will be confirm realities of use of HA in unilateral cochlear implantee. For this goal, 25(m:f=10:15) child participated who are used to HA for 1 to 17 months. We had telephone interviews with their mother about use of HA, change of auditory performance and own voice. As the results, hearing threshold levels of unimplanted ear, the use of a appropriate HA, implanted and aided hearing threshold level(HTL) are must be considered for successful biomodal-binaural hearing. Especially, implanted and aided HTL should be very useful parameter for a prediction of HA effect and a criterion of selection for bilateral cochlear implantation.

A Review of Contemporary Teleaudiology: Literature Review, Technology, and Considerations for Practicing

  • The scope of teleaudiology has been noted with telehealth due to Coronavirus disease (COVID-19) recently. As the notion has been around us for more than 20 years ever since 1999, it is necessary to perceive the knowledge accurately and prepare for the successful implementation of it. Therefore, the literature review including screening and diagnostic audiometry, cochlear implants and hearing aids, and aural rehabilitation, telecommunications technology regarding several fields of teleaudiology, and considerations for practicing were identified. Although overall internet-based audiological services showed benefits in terms of outcome and accessibility, uncertainties of cost-effectiveness, the optimal level of support, and a need for further studies of many aspects for teleaudiology has arisen. In the view of technology, the store-and-forward (asynchronous/hybrid) and a real-time (synchronous) methods were introduced with one applied and nine registered patents recorded from 2004 to 2020 for the invention of teleaudiology in the United States. Also, 10 checklists were suggested for planning teleaudiology practice from prior experience in hosting the teleaudiology program. Conclusively, it is hoped that this review sheds light on recognizing and improving the existing teleaudiology services and helps overcome the challenges faced in the era of pandemic and untact world to come.

학령기용 음소지각검사 표준화를 위한 기초연구: 청각장애아동을 대상으로 (A preliminary study on standardization of phoneme perception test for school-aged children : Focused on hearing impaired children)

  • 본 연구는 학령기용 음소지각검사(Phoneme Perception Test for School-Aged Children, PPT-S)를 활용하여 보청기와 인공와우를 착용하고 있는 청각장애아동의 자음지각능력과 오류를 분석하고, 청각장애아동에 대한 검사 목록의 적합성을 확인하고자 하였다. 연구결과, 청각장애아동들은 초성에 비해 종성을 지각하는 데 어려움을 더 느끼는 것으로 나타났다. 목표어와 보기어의 조음방법과 조음위치가 유사한 PPT-S의 어려운형을 쉬운형보다 더 어렵게 느꼈다. 초성은 기식음에 대한 오답률이 더 높았고, 종성의 경우 'ㄷ'와 'ㅁ'에 대한 오답률이 상대적으로 더 높았다. 검사자의 성별에 따른 정답률에 유의미한 차이는 나타나지 않았다. 이상의 결과는 청각장애아동 대상 음소지각검사의 표준화와 청능재활 전·의 중재효과를 평가하기 위한 기초자료로 유용하게 사용될 수 있을 것이다.

A Needs Analysis of Educational Content for Overseas Job Applicants in the Digital Bio-health Industry

  • Background: The globalization of the healthcare industry and the increasing demand for skilled professionals in the global healthcare industry have opened up opportunities for specialized biotech healthcare professionals to seek overseas employment and career advancement. Objects: This study aimed to develop educational content essential for the overseas employment of digital bio-health professionals. Methods: A survey was conducted among 196 participants. Google Forms (Google) were utilized to create and administer the survey, employing purposive sampling, a non-probability sampling method. Data analysis was performed using IBM SPSS 25.0 (IBM Co.), including Cronbach's α and independent sample t-tests to assess significant differences. Results: About half of college students are interested in overseas employment and international careers, while the other half had not. The most common reason for wanting to work or go overseas was "foreign experience will be useful for future activities in Korea." Students who had experience taking courses from the Bio-health Convergence Open Sharing University preferred overseas programs more than those who did not have that experience. In terms of the degree of desire for overseas education courses provided by universities, contents related to human health were the highest, followed by bio-health big data. Conclusion: Many students wanted to work and go overseas if there is sufficient support from the university. The findings in this study suggest that universities are necessary to play an important role in supporting students' aspirations to work or go overseas by providing language education, education and training programs, information on overseas jobs, and mentoring programs.
