Relationship between Food Behavior and Level of Attention Deficit Hyperactivity Disorder in Elementary School Students in Suwon-si, Korea (수원시내 일부 초등학생의 식행동과 주의력결핍 과잉행동장애 수준과의 관계 연구)

  • The purpose of this study was to investigate the correlation between food behavior and Attention Deficit Hyperactivity Disorder (ADHD) score in elementary students in Suwon-si, Gyeonggi-do. Two hundreds and thirty two students (113 male and 119 female) in grades 4~6 participated. We assessed food behavior by using Nutrition Quotient (NQ). NQ was examined by an NQ questionnaire, which consisted of 19 food behavior checklist items. Their items were grouped into five categories: balance, diversity, moderation, regularity, and practice. All data were statistically analyzed by SPSS 18.0. Among the five factors, NQ for food behavior and balance factor showed the lowest score, whereas that of diversity factor showed the highest score. The final Nutrition Quotient (NQ) score weighted on such five factors was 62.59 points. The average level of ADHD by Conners-Wells Adolescent Self-Report Scale (Short Form) [CASS(S)] was 17.02 points out of a total of 81 points. Six students (2.2%) who scored more than 41 points were classified as ADHD risk. By gender, male students (19.76 points) showed a higher CASS(S) score than female students (14.41 points) did. There was a significant negative correlation between NQ and CASS(S) score (r=-0.445, P<0.001). Multiple regression determined the effects of moderation ($Exp({\beta})=-0.193$, P<0.01) and practice ($Exp({\beta})=-0.345$, P<0.001) on CASS(S) score. In conclusion, distinctive nutritional education is needed for students with a high level of ADHD to help their understanding considering their different levels of attention.

Clinical Effects of Korean Red Ginseng on Attention Deficit Hyperactivity Disorder in Children: An Observational Study

  • We investigated the clinical effects of Korean red ginseng on attention deficit hyperactivity disorder (ADHD) in children. Eighteen subjects aged between 6 and 14 diagnosed with ADHD based on the Diagnostic and Statistical Manual of Mental Disorders, fourth edition diagnosis criteria were enrolled. Korean red ginseng (Panax ginseng) at 1,000 mg b.i.d. was administered to the subjects for 8 weeks. Eighteen children completed the questionnaire and clinical assessment by visiting the hospital at baseline, then in the first, fourth and eighth weeks. Clinical assessments were performed by using the ADHD Diagnostic System (ADS, a computerized attention assessment program), the abbreviated Conners' rating scale in addition to psychiatric interviews. After 8 weeks, significant differences were found in the omisssion errors of ADS ($78.56{\pm}43.33$ at baseline, $55.17{\pm}21.44$ at 8 weeks, p<0.023), Conners ADHD Rating Scale ($13.78{\pm}6.32$ at baseline, $9.50{\pm}4.80$ at 8 weeks, p<0.042) and Spielberger State Anxiety Scale ($30.94{\pm}6.25$ at baseline, $28.83{\pm}6.23$ at 8 weeks, p<0.024). In the Korean Personality Inventory for Children, a significant reduction of score was found in the physical development scale ($56.44{\pm}9.63$ at baseline, $50.94{\pm}8.91$ at 8 weeks, p<0.017) and social dysfunction scale ($56.33{\pm}6.82$ at baseline, $51.94{\pm}7.13$ at 8 weeks, p<0.025). These results suggest that Korean red ginseng may be effective in improving inattentiveness in ADHD children, but it remains uncertain if it improves the general severity of ADHD, depression, anxiety personality and behavioral changes.

Clinical and Neuropsychological Factors Associated with Treatment Response and Adverse Events of Atomoxetine in Children with Attention-Deficit/Hyperactivity Disorder

  • Objectives: The objective of this study was to investigate clinical and neuropsychological factors associated with treatment response and adverse events of atomoxetine in children with attention-deficit/hyperactivity disorder (ADHD) in Korea. Methods: Children with ADHD were recruited at the Department of Psychiatry of Asan Medical Center from April 2015 to April 2018. Diagnoses of ADHD and comorbid psychiatric disorders were confirmed with the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version. The subjects were subsequently treated with atomoxetine for 12 weeks and illness severity was scored using the ADHD Rating Scale, Clinical Global Impression-Severity scale (CGI-S) and/or Improvement scale (CGI-I), at pre- and post-treatment. They also completed the Advanced Test of Attention (ATA), while their caregivers completed the Korean Personality Rating Scale for Children (KPRC) at pre- and post-treatment. Independent t-test, Fisher's exact test, ${\chi}^2$ test, mixed between-within analysis of variance and correlation analysis were used for statistical analysis. Results: Sixty-five children with ADHD (mean age: $7.9{\pm}1.4years$, 57 boys) were enrolled, of which, 33 (50.8%) were treatment responders. Scores on the social dysfunction subscale of the KPRC (p=0.021) and commission errors on the visual ATA (p=0.036) at baseline were higher in treatment non-responders than in responders; however, the statistical significances disappeared after adjusting for multiple comparisons. Mood changes were also observed in 13 subjects (20.0%), and three of them discontinued atomoxetine due to this. Additionally, atomoxetine-emergent mood change was observed more frequently in girls (p=0.006), while the intelligence quotient (p=0.040) was higher in those subjects with mood changes than in those without. Conclusion: The results of our study suggest that clinical and neuropsychological factors could be associated with treatment response or adverse events of atomoxetine in children with ADHD. Further long-term studies with larger samples are needed.

  • There has been a growing interest in sleep problems associated with attention deficit hyperactivity disorder (ADHD). In this paper, we reviewed the current literature on the underlying sleep problems associated with ADHD. Sleep problems are very common in children with ADHD. Patients with ADHD may have sleep difficulties including difficulty falling asleep, frequent night awakening, increased tiredness upon waking. Children with ADHD are associated to restless legs syndrome, periodic limb movement and sleep-disordered breathing. Screening for other psychiatric comorbidities and the side effects of medications, such as psychostimulants, is also necessary when considering sleep problems in ADHD. Sleep problems can cause a negative impact on the quality of life and emotional well-being both of children with ADHD and their parents. Many evidences suggest that assessment of sleep difficulties should be included in evaluating the patient of ADHD.

  • This study examines the effect of group-counseling programs for parents of children with Attention-Deficit/Hyperactivity Disorder (ADHD) on parenting stress, parenting competence, and symptoms related to ADHD. For this study, 9 parents of children with ADHD were selected as an experimental group that participated in a group-counseling program and another 9 parents of children with ADHD for a control group that participated in a group of free conversation without a program. The program consisted of 9 sessions (a 70 minute meeting per session) with 2 sessions a week. Data were analyzed by a paired t-test and independent samples t-test, using SPSS program packages. Results showed significant before and after differences in the degree of parenting competence and that Parenting competence was improved with the experimental group.

  • The purpose of the this study was to examine the relationship between mother-child interaction play assessment of children with Attention Deficit Hyperactivity Disorder(ADHD) and delinquency through observational method. The subjects were 30children(normal: 15, ADHD: 15) between 4-6 elementary school grade. Instruments used Parent-Child Interaction Assessment(P-CIPA), Inventory of Delinquency-Oriented Behavior and Korea Children Behavior Check List(K-CBCL). Results showed that following : 1) ADHD affect the delinquency-oriented behavior; 2) there were significant differences in mother-child interaction play assessment between normal children group and ADHD children group; and 3) there was a significant relationship between parent-child interaction play assessment and delinquency.

Modafinil for the Treatment of Attention Deficit Hyperactivity Disorder: A Systematic Review (Modafinil의 주의력결핍 과잉행동장애 치료효과에 대한 체계적 문헌 고찰)

  • 주의력결핍 과잉행동장애는 미국에서 약 200만 명의 어린이에게 나타나는 심각한 만성 신경행동학적 장애이다. 주의력결핍 과잉행동장애 치료의 중심은 약물요법이다. 그러나 현재 환자의 약 30%는 주의력결핍 과잉행동장애를 적응증으로 갖도록 허가된 의약품들을 사용해도 적절한 치료효과를 얻지 못하고 있는 상황이다. Modafinil은 methylphenidate나 amphetamines와 같은 각성제와는 다른 약리학적 기전으로 중추신경계를 항진시킨다. 본 연구는 modafinil의 주의력결핍 과잉행동장애 치료효과에 대한 최신 지견을 얻고자, 1990년부터 2010년 3월까지 MEDLINE에 등재된 논문을 ADHD와 Modafinil이라는 MeSH terms로 검색하여 추출한 자료 중에서 대조군이 사용된 무작위 배정 및 이중맹검 임상연구 사례만을 선별하여 임상적 유용성을 평가하였다. 현재 modafinil은 주의력결핍 과잉행동 장애 치료제로 허가된 의약품은 아니지만 최근 여러 국가에서 시행된 연구들은 modafinil이 안전하고 효과적인 주의력결핍 과잉행동장애 치료제일 수 있다는 결과를 보여주고 있다.

Machine Learning-Based EEG Classification for Assisting the Diagnosis of ADHD in Children (아동의 ADHD 진단 보조를 위한 기계 학습 기반의 뇌전도 분류)

  • Attention Deficit Hyperactivity Disorder (ADHD) is one of the most common neurological disorders in children. The diagnosis of ADHD in children is based on the interviews and observation reports of parents or teachers who have stayed with them. Since this approach cannot avoid long observation time and the bias of observers, another approach based on Electroencephalography(EEG) is emerging. The goal of this study is to develop an assistive tool for diagnosing ADHD by EEG classification. This study explores the frequency bands of EEG and extracts the implied features in them by using the proposed CNN. The CNN architecture has three Convolution-MaxPooling blocks and two fully connected layers. As a result of the experiment, the 30-60 Hz gamma band showed dominant characteristics in identifying EEG, and when other frequency bands were added to the gamma band, the EEG classification performance was improved. They also show that the proposed CNN is effective in detecting ADHD in children.

Dietary Factors Associated with Attention Deficit Hyperactivity Disorder (ADHD) in School-aged Children (학동기 어린이 주의력결핍 과잉행동장애에서 식이요인의 역할 규명)

  • Objectives: An association between dietary patterns and mental health in children has been suggested in a series of studies, yet detailed analyses of dietary patterns and their effects on ADHD (attention deficit hyperactivity disorder) are limited. Methods: We included 4569 children who had dietary intake data as part of the CHEER (Children's Health and Environmental Research) study conducted nationwide from 2005 to 2010. We assessed ADHD (Attention Deficit Hyperactivity Disorder) by the DuPaul's ADHD Rating Scales and dietary intake by a semi-quantitative food frequency questionnaire. Using intake data, we constructed five dietary patterns: "Plant foods & fish," "Sweets," "Meat & fish," "Fruits & dairy products," and "Wheat based." Results: The overall proportion of ADHD was 12.3%. Boys (17.8%) showed a higher rate of ADHD than girls (6.5%). The total intake of calories (85 kcal) and plant fat (2g) in the ADHD group was significantly higher than that of the normal group. ADHD was significantly negatively associated with dietary habits such as having breakfast and meal frequency, and positively associated with eating speed, unbalanced diet, overeating, and rice consumption. Regarding dietary patterns, the "Sweets" category was relevant to high ADHD risk (OR 1.59, 95% CI: 1.18, 2.15 for Q5 vs. Q1) in a linear relationship. An inverse, non-linear association was found between "Fruits & dairy products" and ADHD (OR 0.55, 95% CI: 0.39, 0.76 for Q4 vs. Q1). Conclusions: Our study confirms both positive and negative associations between diet and ADHD in elementary school age children. Moreover, linear or nonlinear associations between diet and ADHD draw attention to the possible threshold role of nutrients. Further studies may consider characteristics of diet in more detail to develop better intervention or management in terms of diet and health.

An Open-Label Study of OROS-Methylphenidate for Neuropsychological Changes in Children and Adolescents with Attention-Deficit Hyperactivity Disorder (주의력 결핍 과잉 행동 환자에서 OROS-Methylphenidate 투여 후 신경심리학적 변화에 대한 개방 연구)

  • Objectives : Previous studies reported that attention-deficit hyperactivity disorder (ADHD) resulted from a deficit of selective attention and sustained attention. In this study, we assessed the result of methylphenidate-induced changes of the cerebral frontal executive functions in patients with ADHD. Methods : The subjects in this study consisted of 16 ADHD patients whose age ranged from 7 to 12. We used ADHD Diagnostic System (ADS) for the attention improvement, and the Stroop Test for the executive function response to pharmacotherapy with MPH. Results : After pharmacotherapy with methylphenidate for 12 weeks, the study group showed improvement in the clinical aspects through Clinical Global Impression-Severity, ADHD-rating scale and Inattention/Overactivity With Aggression Conner's Parents Rating Scale. In the ADS test, only in auditory task there was a decrease of both the response time and the standard deviation of the response time significantly. In the Stroop Test, there was a decrease in the word task, color task and color-word task significantly. Conclusion : Our results show that psychostimulant medication improves neuropsychological function, including the cerebral frontal executive function. This study implies that we have to consider the improvement of executive function, as well as attention when evaluating the efficacy of treatment.
