Left Atrial Decompression by Percutaneous Left Atrial Venting Cannula Insertion during Venoarterial Extracorporeal Membrane Oxygenation Support

  • Patients with venoarterial extracorporeal membrane oxygenation (ECMO) frequently suffer from pulmonary edema due to left ventricular dysfunction that accompanies left heart dilatation, which is caused by left atrial hypertension. The problem can be resolved by left atrium (LA) decompression. We performed a successful percutaneous LA decompression with an atrial septostomy and placement of an LA venting cannula in a 38-month-old child treated with venoarterial ECMO for acute myocarditis.

Enhancement of Ca2+ Current Does Not Regulate the Speed of Depolarization-induced Ca2+ Propagation Wave in Rat Atrial Myocytes

  • In atrial myocytes, lacking t-tubules, $Ca^{2+}$ current ($I_{Ca}$)-initiated $Ca^{2+}$ release at the peripheral junctional sites propagates into the interior of the cell by diffusion of $Ca^{2+}$. We have previously reported that time of activation of the central sites is independent of $I_{Ca}$. In the present study we have probed the effects of Bay K 8644 on $Ca^{2+}$ propagation wave to the center of the myocyte using rapid 2-D confocal $Ca^{2+}$ imaging in the rat atrial myocytes. Enhancement of $I_{Ca}$ by Bay K 8644 accelerated the rate of peripheral $Ca^{2+}$ release, but did not affect the speed of propagation of central release. In contrast, enhancement of $I_{Ca}$ by intracellular cAMP reduced the magnitude of peripheral and central $Ca^{2+}$ transients, but significantly accelerated the speed of central $Ca^{2+}$ release. Our data suggest that the speed of central $Ca^{2+}$ propagation triggered by $I_{Ca}$ is not regulated by the magnitude of either $I_{Ca}$ or local cytosolic $Ca^{2+}$ releases.

심방이뇨호르몬의 분비조절에 있어서 superoxide anion의 영향 (Effect of superoxide anion in the regulation of artrial natriuretic peptide (ANP) secretion)

  • Atrial Natriuretic Peptide(ANP) is a hormone with potent natriuretic, diuretic and relaxing properties of vascular smooth muscle. Specific chemical modulator responsible for the ANP secretion has not yet been found. Although atrial stretch of stretch-release is to be a major stimulus for the secretion of ANP, the precise mechano-molecular transduction mechanism responsible for its evoked secretion remains to be elucidated. It is interested to clarify the effect of superoxide anion in the stretch-induced ANP secretion. In order to investigate the effectg of $H_2O_2$ in the regulation of ANP secretion, a perfused model of left atrium of rats was used. The results obtained were as follows; 1. The ANP secretion and the extracellular fluid(ECF) translocation were accentuated by the effect of repetitive atrial distension-reduction volume at atrial pressure($4cmH_2O$). 2. The dilution curve showed to be in parallel between pure atriopeptin III (AP III) and perfusated buffer. 3. $H_2O_2(5{\times}10^{-4}M)$ accenturated a strectch-release induced increase of the ANP secretion. The amount of released ANP was significantly(p<0.01) increased. These results suggest that the superoxide anion may be involved in the regulatory mechanism of mechanically activated ANP release.

신생아에서 발생한 다소성심방빈맥 (Multifocal Atrial Tachycardia in a Newborn)

  • 다소성 심방 빈맥은 신생아에서 드물게 발생하며 진단하기에 어려워 심방 조동으로 오인되기도 한다. 치료가 어려우나 생후 1년 안에 자연적으로 사라지는 경우도 있다. 불규칙한 심방 빈맥을 가진 신생아에게 직접 제작한 경식도 전극을 이용한 심전도를 시행하여 다소성 심방 빈맥을 진단하였다. Propranolol은 치료에 효과적이지 않았으나 digoxin과 sotalol 투여 후 환자는 점차 동리듬으로 회복되었다. 신생아에서 경식도 전극을 이용하여 다소성 심방 빈맥을 진단하였던 1례를 보고한다.

Initial Experience with Total Thoracoscopic Ablation

  • Background: Recently, a hybrid surgical-electrophysiological (EP) approach for confirming ablation lines in patients with atrial fibrillation (AF) was suggested. The aim of this approach was to overcome the limitations of current surgery- and catheter-based techniques to yield better outcomes. Methods: Ten consecutive patients with AF underwent total thoracoscopic ablation (TTA) following transvenous catheter EP ablation (residual gap and cavotricuspid isthmus [CTI] ablation). Holter monitoring was performed 6 months postoperatively. Results: Ten patients (90% with persistent AF) underwent successful hybrid procedures, and there was no in-hospital mortality. An EP study was performed in 8 patients and showed that successful antral ablation in all pulmonary veins was achieved in 7 of them. The median follow-up duration was 7.63 months (range, 6.7 to 11.6 months). Nine patients underwent Holter monitoring 6 months postoperatively, and the results indicated an underlying sinus rhythm without AF, atrial flutter, or atrial tachycardia lasting more than 30 seconds in all of the patients. There was no recurrence of AF during follow-up. Conclusion: A hybrid approach that consists of TTA followed by transvenous catheter EP ablation (residual gap and CTI ablation) yielded excellent outcomes in our patient population. A hybrid approach should be considered in patients with a high risk of AF recurrence.

  • Rehmannia Radix is a thick tuberous root about 3-6 cm diameter. The tuberous root is repared for medicine. The material in the fresh state is shengdihuang (生地黃). The material in the dried state is gandihuang (乾地黃). Shudihuang (熱地黃) is made by taking juicy roots, washing them in millet wine, steaming on a willow frame in a percelain vessel, drying, and resteaming and redrying nine times. The aim of this experiments was to investigate the effect of Rehmannia Radix Water extracts on the plasma renin activity and plasma levels of aldosterone and atrial natriuretic peptide (ANP) in rats. The results of study were as follows: Plasma renin activity was not changed after administration of Rehmannia Radix water extracts. Plasma levels of aldosterone was decreased significantly after administration of Shudihuang (熱地黃) water extracts. Plasma levels of atrial natriuretic peptide was increased significantly after administration of Shudihuang (熱地黃) water extracts.

황금(黃芩)의 심장(心臟)에 대한 약리작용(藥理作用) (Pharmacological action of extracts of Scutellaria baicalensis on Cardiovascular System)

  • The adrenergic blocking activity and refractory period of cardiac muscle on isolated rabbit atria were measured after administration of Scutellaria. In rabbits and cats the antiarrhythmic action of Scutellaria on atrial and ventricular arrhythmias produced by epinephrine or ouabain was examined and also compared with that of propranolol and quinidine. The alcoholic extract of Scutellaria produced a marked decrease in heart rate and contractile amplitude of the isolated rabbit atria. Pretreatment with Scutellaria rendered the atria to fail to respond to epinephrine, indicating that this crude drug possesses an adrenergic blocking activity. The extract produced a marked prolongation of the refractory period of atrial muscle. The extract effectively abolished the spontaneous arrhythmia occurring in the isolated rabbit atria. As propranolol and quinidine it also suppressed the atrial arrhythmia induced by ouabain. The extract prevented, as propranolol and quinidine, the induction of ventricular arrhythmia arising from excessive dose of epinephrine in anesthetized rabbits and cats. With regard to the ventricular arrhythmia induced by a continuous infusion of ouabain, the alcoholic extract of Scutellaria exerted some suppressive effect in anesthetized rabbits but no effect on cats. From the above results, it may be concluded that Scutellaria is effective against atrial and ventricular arrhythmias. The antiarrhythmic effects of this drug may be the result of adrenergic beta receptor blocking and cardiac depressive activities including prolongation of the refractory period of cardiac muscle.

심장질환 치료를 위한 체내삽입형 저전력 Pacemaker에 관한 연구 (Implantable low-power Pacemaker for Heart Disease Therapy)

  • 본 연구는 체내 이식형 Pacemaker를 연구하면서 심장 질환을 Therapy 해 주는 방법에 대해 저전력 및 성능향상에 중점을 두고 연구 및 실험을 하였다. 우선적으로 심장의 심박동을 연산량이 적은 Peak_detection에서 체크하여 전력소모를 줄이고 나오는 각 심실 및 심방의 Interval을 Disease_episode 에서 받는다. 여기서 5가지 심실 및 심방에 관한 질환들 (VF : Ventricular Fibrillation, VT : Ventricular Tachycardia, FVT : Fast Ventricular Tachycardia, FAT_AF : Fast Atrial Tachycardia/Atrial Fibrillation, AT_AF : Atrial Tachycardia AT_AF : Atrial Fibrillation)을 판별한 후 각 병증에 맞는 Therapy 값을 출력하게 하였다. 그 외에 남아있는 병증에 대해서도 Therapy가 저전력 및 성능향상 되도록 설계하였다. 기존에 적용되어 있는 Detection 기법에서는 각각의 병증에 대해서 각 Detection이 있어 VF와 VT 사이에 있는 FVT와 같은 병증을 치료할 때 FVT 같은 경우에는 VF와 VT사이에 있는 질병이기 때문에 FVT_VF 및 FVT_VT와 같이 각각의 Detection을 두어 전력 소모가 있었다. 심장에서는 여러 질병이 한번에 나을 수 없다는 것에 착안하여 (심박동 Interval에 의해 질병이 판단되므로) 다른 병증이지만 같은 진단 기준을 쓰는 Detection을 통합함으로써 하나의 모듈로 구성하여 Gate수를 줄이고 저전력을 구현하였다. 또한 병증을 판별하는 진단 기준 모듈 중 Onset_Criterion 재설계하여 좀더 성능 향상에 중점을 두었다.

Right to Left Atrial Septum Defect Compounded with Patent Ductus Arteriosus in a Maltese Dog

  • A nine-month-old intact female Maltese dog (body weight: 1.5 kg) presented with frequent syncopal episodes (1-2/day), exercise intolerance, and dyspnea. Cardiomegaly with marked dilation of right and left atria along with ascites was indicated on radiography. Tricuspid regurgitant jets (4.36 m/s), pulmonary regurgitant jets (3.4 m/s), left-to-right shunting flow at the ductus arteriosus, and an atrial septal defect were observed on echocardiography. A bubble study with agitated saline found right-to-left shunting at the interatrial septum. Based on diagnostic studies, the dog was diagnosed with right-to-left atrial septum defect (ASD) compounded with patent ductus arteriosus (PDA). The treatment was directed to reduce the pulmonary hypertension, improve cardiac performance, and lower fluid retention. Despite medical treatment for 2 months, the dog died of sudden cardiac arrest. Herein, we describe a very rare case of right-to-left ASD compounded with PDA.

  • Valvular heart disease is one of the main current of cardiac problems and has many problems must be solved by sequelae and complications, etc. Rheumatic Mitral Stenosis is mainly attacked by rheumatic fever and developed by process of treatment of mitral valve or formation of trace. The purpose of this study is to examine the efficacy of oriental treatment for Cerebral Infarction with Rheumatic Mitral Stenosis. At the time of visiting ER, he was shown the symptoms of dull mentality, Rt. hemiplegia, global aphasia, dysphagia, chest discomfort, insomnia, dyspnea, etc, It showed that Atrial fibrillation in EKG monitoring, Atrial fibrillation, Rheumatic Mitral Stenosis, Ejection-Fraction slope 60% in Cardiac echography, Lt. atrial hypertrophy, Rt. atrial hypertrophy, Rt. ventricular hypertrophy with pulmonary congestion in chest X-ray. From the view of oriental diagnostic criteria. We classified the patient's clinical conditions and treated accordingly. As a result of treatment, symptoms were markedly improved and he was discharged. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.
