  • Radiofrequency ablation is an effective treatment for atrial fibrillation. Pulmonary vein stenosis/occlusion is one of its rare complications. Herein, the case of a 50-year-old man with hemoptysis and migratory pulmonary infiltrations after transcatheter radiofrequency ablation for atrial fibrillation is presented. Initially, pneumonia, interstitial pulmonary disease, or lung cancer was suspected, but wedge resection revealed hemorrhagic infiltrations. Chest computed tomography pulmonary angiography detected no left superior pulmonary vein due to its total occlusion, and left upper lobectomy was performed. Post-ablation pulmonary vein occlusion must be strongly suspected in cases of migratory pulmonary infiltrations and/or hemoptysis.

  • Sinus venosus type atrial septal defect is commonly associated with partial anomalous pulmonary venous return[PAPVR . Ideal surgical repair of sinus venosus ASD with PAPVR demands complete closure of septal defect with redirection of the anomalous pulmonary venous return to the left atrium without obstructing the superior vena cava[SVC or the anomalous pulmonary vein and without injury of sinoatrial node and residual shunt. In our two patients, the closure of sinus venosus ASD and correction of PAPVR could be accomplished by simple direct sutures without using a patch or flap. Both patients had a good outcome.

  • From July 1966 to July 1983, sixteen atrial myxomas in fifteen patients were seen at Severance Hospital. Fifteen of the sixteen myxomas were located in the left atrium and one in the right atrium. All the cases except three were correctly diagnosed preoperatively. Presently, M-mode and two-dimensional echocardiography are utilized as safe, reliable, and noninvasive imaging modalities. Echocardiography provided an accurate diagnosis in twelve cases since November 1977. In all cases, myxoma were excised successfully. On patient had reoperation and mitral valve replacement on postoperative first day due to persist mitral regurgitation after excision of left atrial myxoma. One patient had recurrence requiring reoperation 37 months after primary operation. Follow up results of each patient were excellent.

  • For the purpose of clinical analysis of secundum atrial septal defect, the patients with atrial septal defect operated on during the period from Aug. 1959 to Aug. 1983 were analysed. The age of the patients was ranged from 3 months to 60 years and there were 132 males [45.3%] patients and 145 female patients [54.6%] in this series. The most frequent type was oval type defect in 219 [86.9%]. The PAPVC were present in 7 patients of high defect group. Clinical analysis revealed 53 patients [19.9%] were asymptomatic. The QP/QS above than 3.61 were 45 patients and less than 1.5 were 24 patients. The pulmonary artery pressure was 10 to 100 mmHg. The majority of the defects were closed in primary suture and closed with a Dacron patch in 19 patients. The postoperative complications occurred in 35 patients [13.1%] and the most frequent complication was wound problem which was present in 10 patients. The hospital mortality rate was 1.6%.

  • Cardiac atrium is now well-known as an endocrine organ which secretes atrial natriuretic peptide (AMP), participating in the regulation of body fluid and blood pressure. ANP is released mainly from cardiac muscle cells in response to various physiological and pathological conditions to induce atrial stretch. Ca$\^$2+/ may be one of the most important factors affecting ANP secretion even though controversy still persists. The aim of the present study is to investigate the effect of lysophosphatidylcholines (LPCs) and moxonidine on atrial hemodynamics and ANP secretion in hypertrophied atria. LPC is an endogenous phospholipid released from cell membrane during ischemia, and moxonidine is a imidazoline 1 (Il) receptor agonoist.

  • A 18-year-old female underwent surgical correction of tricuspid atresia and complete transposition of great arteries combined with atrial 8 ventricular septal defect and pulmonary stenosis. After the transection of main pulmonary artery just above the pulmonic valve, proximal portion of main pulmonary artery was closed with running suture and distal portion of main pulmonary artery anastomosed with right atrial appendage without valve insertion. Atrial septal defect was closed with running suture. Postoperative course was uneventful and she discharged on 18th postoperative day. Her condition is in very good until present. Modified Fontan`s operation without valve placement [in the condition of low pulmonary vascular resistance and good left ventricular function] may has a good result.

  • Till June 1988, We experienced 3 cases of single atrium at Dept. of Thoracic k Cardiovascular Surgery of National Medical Center. Case 1 was 26 years old female, and had single atrium associated with partial cleft in mitral anterior leaflet and PAPVC. Atrial septation with Dacron patch and MVR [I-S, 31 mm] were done, followed by TAP, De Vega. Postoperative course was good, OPD follow-up for about 4 years with normal physical activity and ordinary working. Case 2 was 4 years old female with single atrium alone. Atrial septation with pericardial patch. Good postoperative course and OPD follow-up for about 2 years with appropriate physical growth. Case 3 was 22 years old female, single atrium with complete cleft in the anterior and posterior mitral leaflet, and septal tricuspid cleft were identified. Atrial septation with polystan bovine pericardial patch and repair of mitral anterior cleft. Postoperatively, residual regurgitation of mitral and tricuspid valve, but tolerable in some limitation of physical activity.

  • We at Keimyung University Dongsan Medical Center experienced simultaneous repair of pectus excavatum and secundum atrial septal defect We used resection deformed perichondrium, raising sternum at right angle to secure good operative field for open heart surgery. Mechanical ventilation was applied which could be weaned on postoperative 2 hours. The hospital course was uneventful without any other sequale. The patient was discharged on postoperative day 6.

  • A 75-year-old man was admitted to the hospital because of a pericardial effusion. After 3 L of blood-stained pericardial fluid was drained, clinical examination together with echocardiography and chest computed tomography showed a tumor in the right atrium. At operation a pedunculated vascular tumor was found with a broad base which was embedded in the atrial wall and extended into the pericardium.A wide resection was performed resulting in a large defect of the right atrial wall. The defect was reconstructed with a pericardial patch. The patient did well postoperatively, but bloody pleural effusion developed later, presumably because of pulmonary metastasis. The patient died 2 months after surgery as a result of respiratory failure.

  • The percutaneous transcatheter closure of secundum atrial septal defect has recently become an increasingly widespread alternative to surgical closure in many centers. Although immediate, short, and intermediate term results of percutaneous transcatheter septal closure are promising, the procedure is not free from inherent complications that could be lethal. We report a case of device embolization necessitating emergent surgical retrieval.
