학령전기 아토피피부염 아동 어머니의 스트레스 경험 (Experiences of Stress among Mothers of Preschoolers with Atopic Dermatitis)

  • Purpose: The purpose of this study was to explore and describe the meaning and essence of experiences of stress in mothers of preschoolers with atopic dermatitis. Methods: This study employed a qualitative research design. Seven mothers of preschoolers with atopic dermatitis were interviewed and the data were analyzed using Giorgi's phenomenological method. Results: Five main themes and 12 theme clusters emerged. The 5 main themes were 1) the mother's enduring heartache from the conflict that arise among family members, 2) the complete loss of daily happiness, 3) the steady stream of emotional fluctuations, 4) struggling to bear the heavy financial burden, and, 5) a life of hope accompanied by stress. Conclusion: The results of this study provide an in-depth understanding of stress among mothers of preschoolers with atopic dermatitis. These results can be used in the development of nursing interventions to provide psychological and emotional support for mothers and family members.

학령전기와 학령기 아동에서의 아토피 피부염 유병률과 위험인자 (Prevalence and Risk Factors for Atopic Dermatitis in Pre-school and School Aged Children)

  • Purpose: This study was done to estimate the prevalence of atopic dermatitis (AD) and its risk factors for AD in children living in the community. Method: Random samples of 10,236 were selected from 43 kindergarten (1,418) and 57 elementary (8,718) students in K city. Data from 1,079 (kindergarten children) and 7,271 (elementary) students were used in the final analysis. The Korean-translated modified version of the International Study of Asthma and Allergies in Childhood (ISSAC) questionnaire was used in this cross-sectional survey. Parents answered the survey questionnaire. Results: The lifetime and last 12-month prevalence of AD were 40.15%; 30.86% in kindergarten children and 33.56%; 25.37% in elementary children. The lifetime and last 12-month prevalence of AD diagnosis were 40.8%; 18.68% in kindergarten children and 34.36%; 12.63% in elementary children. The lifetime prevalence of AD treatment was 25.93% in kindergarten children and 22.07% in elementary children. Multivariate logistic regression analysis revealed that risk factors for AD were age, allergic disease, age of house, carpets, pets. Conclusion: The study suggests that prevalence of AD has increased among children in the community. These data can be used to manage possible risk factors that are predictors of childhood AD.

  • Atopic dermatitis is a condition that makes skin red and itchy. It is common in children but can occur at any age. Atopic dermatitis is chronic and tends to flare periodically. The pathogenesis of the disease has not yet been clearly elucidated but genetic predisposition, immunological dysfunction, and environmental factors are presumed to be involved in the pathogenesis. In general, it is difficult to cure, but as time passes, most of them heal naturally and the symptoms disappear, but the symptoms continue to recur. So, the basic treatment is to relieve the pruritus and prevent it from reoccurring. Treatment involves avoiding things that make the condition worse, daily bathing with application of moisturizing cream afterwards, applying steroid creams when flares occur, and medications to relieve itching sensation. Steroid pills or creams based on calcineurin inhibitors may occasionally be used if other measures are not effective. When examining a pilot with atopic dermatitis, the dermatitis condition, the treatment being used, and the side effects of the medications should be considered. This case involves an otherwise healthy applicant for the 1st class medical certification who has had atopic dermatitis.

소아.청소년에서 아토피 피부염과 주의력결핍과잉행동장애와의 관계 (Association between Atopic Dermatitis and Attention Deficit Hyperactivity Disorder Symptoms in Korean Children)

  • 연구목적 : 본 연구에서는 소아청소년들의 아토피 피부염과 주의력결핍과잉행동장애 (Attention Deficit Hyperactivity Disorder : ADHD)의 관계에 대하여 알아보고, 더 나아가서 아토피 피부염 환자를 MAST 양성 및 음성의 아토피 피부염 환자로 분류하여 ADHD와의 관계에 대하여 알아보고자 하였다. 방법: 본 연구에는 아토피 피부염 아동 69명과 정상 대조군 42명이 참여하였다. 피부과 전문의가 아토피 피부염에 대하여 평가하고 중증도를 측정하였으며, 아토피 피부염 아동을 대상으로 MAST 검사를 시행하였다. 부모용 ADHD 평가척도로 환자군 및 대조군의 ADHD 증상을 평가하였다. 결과: 아토피 피부염 환자군에서 정상 대조군에 비해 ADHD 평가척도의 총점, 주의력결핍 및 과잉행동/충동성 증상의 점수가 유의하게 높은 것으로 나타났다(t=3.35, p<0.01 ; t=6.41, p<0.01 ; t=3.93, p<0.01). ADHD로 정의된 아토피 피부염 환자군 중 과잉행동/충동성 군에서 주의력결핍 군에 비해 중등도의 아토피 피부염 환자의 수가 더 많은 것으로 나타났다. $({\chi}^2=4.74,\;p=0.04)$. MAST 검사에서 양성 반응을 보인 아토피 피부염 환자들에서 MAST 음성 반응을 보인 환자들에 비해 ADHD의 증상이 더 높은 비율로 나타났다. $({\chi}^2=9.82,\;p<0.01)$.

보육교사의 아토피 피부염 아동 관리 현황과 아토피 피부염에 대한 지식 및 실천 (Current Status in Management of Children with Atopic Dermatitis and Knowledge and Practice of Preschool Teachers)

  • 목적 보육교사를 대상으로 보육시설에서 아토피 피부염 아동을 관리하고 있는 현황과 보육교사의 지식수준 및 실천 정도를 확인하고 현재의 관리 현황이 보육교사의 지식과 실천에 어떻게 영향을 주는지 파악하는 데에 있다. 방법 대상자는 G광역시 1개구 소재의 보육시설에서 근무하는 보육교사 107명이다. 인구학적 특성, 아토피 피부염 아동의 관리 현황, 그리고 지식수준과 실천 정도(Park, 2011)로 구성된 설문지를 이용하여 자료를 수집하였고 서술적 분석, 일원분산분석 및 사후검정 그리고 상관관계로 분석하였다. 결과 대상자의 대부분이 아토피 피부염 아동을 지도할 때 단순히 부모의 요구에 따른다고 답했다. 절반이상은 아토피 피부염 아동 지도에 대한 지식 부족으로 어려움을 겪고 있었으며 교육과 관리 지침의 제공을 희망하였다. 지식은 75% 수준이었고, 하위영역 중 증상 및 징후에 대한 지식 점수가 가장 높았고 관리에 대한 지식 점수가 가장 낮았다. 실천은 84.3%정도였고, 4개 하위영역 중 음식 관리점수가 가장 높았고 의복 및 침구 관리점수가 가장 낮았다. 그러나 지식과 실천 사이의 통계적으로 유의한 상관관계는 보여주지 않았다(r=.141, p =.149). 결론 대상자는 아토피 피부염 아동을 지도할 때 부모의 요구에 반응하는 소극적 태도를 보였고 지식과 실천은 충분하지 못한 수준이었다. 그들은 지식부족의 어려움을 호소하면서 지침의 제공과 교육을 원한다는 것을 확인할 수 있었다. 따라서 보육교사의 아토피 피부염에 대한 지식과 구체적 실천 방법을 포함한 교육프로그램의 개발이 필요하며 다양한 방법으로 접근할 수 있는 전략을 수립할 필요가 있다.

충남 일부지역 아토피 피부염 초등학생의 식습관과 영양소 섭취 실태 조사 (Dietary Habits and Nutrient Intakes of Elementary School Students with Atopic Dermatitis in Chungnam)

  • The purpose of this study was to investigate the dietary behaviors, eating habits, and nutrient intakes of elementary students with atopic dermatitis. Dietary conditions during infancy, dietary regularity, dietary behaviors, eating habits, and daily nutrient intakes were surveyed and compared between children with (WAD) and without (WOAD) atopic dermatitis using the 24-hr recall method. The ratio of children who responded 'I eat regular meals' was significantly lower in the WAD compared to WOAD group (76.4% vs. 81.7%, p<0.05). The ratio of children who answered 'I eat processed foods or fast foods more than once a week' (p<0.01) and 'I take nutritional supplements' (p<0.001) was significantly higher in the WAD compared to WOAD group. The scores for 'I eat milk and its products everyday' (1.4 vs. 1.5, p<0.05), 'I eat dishes fried or stirred with oil more than twice a week' (2.0 vs. 2.1, p<0.05), and 'I eat snacks such as ice cream, cake, snacks, carbonated beverages more than twice a week' (1.9 vs. 2.0, p<0.05) were significantly lower in the WAD group than in the WOAD group. Daily energy intakes were 1,859.1 kcal and 1,829.5 kcal in WAD and WOAD children, respectively, with no significant difference. However, daily intakes of fat (p<0.05), phosphorus (p<0.05), and vitamin $B_2$ (p<0.05) in WAD children were significantly higher than in WOAD children. To sum up, elementary students with atopic dermatitis had irregular eating habits and higher intake frequency of processed or fast foods, milk and its products, dishes fried or stirred with oil, and snacks compared to normal children. Dietary guidance and nutritional education are required to improve irregular eating habits and induce adequate nutrient intakes in children with atopic dermatitis.

서울 지역을 대상으로 모집된 유.소아기 아토피피부염 환아의 삶의 질과 경제적 비용 부담에 관한 조사 (The Burden of Atopic Dermatitis on Children and Their Families : Quality of Life and Financial Impact in Seoul Area)

  • Objective : Atopic dermatitis (AD) in children may profoundly affect the quality of life (QOL), and also cause financial burden, to the families of those suffering from this ailment. The aim of our study was to examine the quality of life and the financial burden of atopic dermatitis in children and their families to evaluate this relationship with the degree of AD. Methods : 37 infant and child atopic dermatitis patients were included and evaluated using the SCORing of Atopic Dermatitis (SCORAD) Index and Eczema Area and Severity Index (EASI). Patients and carers were asked to fill in the questionnaires about their quality of life and financial costs during the past year. Data about sleep disturbance and pruritus were also obtained. Pearson's correlation was used for statistical analysis. Results : 1. The mean score of Children's Dermatology Life Quality Index (CDLQI) was $10.52{\pm}4.82$, Infants' Dermatologic Quality of Life (IDQOL) was $8.21{\pm}3.95$. 2. The mean score of Family Dermatology Life Quality Index (FDLQI) was $13.30{\pm}5.72$, Dermatitis Family Impact (DFI) was $12.5{\pm}4.98$. 3. By analyzing the questionnaire, the monthly average cost was determined to be 730,800 won for each patient : the direct cost was 283,500 won, and the indirect cost was 447,300 won. 4. By analyzing the correlation between the severity of AD and QOL, subjective SCORAD were significantly and positively correlated with QOL(IDQOL, FDLQI, DFI, CDLQI). 5. By analyzing the correlation between the severity of AD and any economic impact, EASI were significantly and positively correlated with the direct cost. Conclusion : The above results show that the QOL of the patients and carers is significantly related to their disease severity. Atopic dermatitis patients pay an average of 730,800 won a month, and the economic impact on the patients is significantly related to their disease severity. The CDLQI, IDQOL, FDLQI and DFImay potentially be of value to help in the appropriate management of AD and can be used as an added measurement in clinical trials involving AD management.

유아의 성별 문제행동과 또래상호작용 및 부모 요인과의 관계: 아토피피부염 유아를 중심으로 (Relationships among Behavior Problem, Peer Interaction, and Parental Factors in Young Boys and Girls with Atopic Dermatitis)

  • The present study examined differences in children's behavior problems and peer interaction, and parental factors including self-esteem, depression and childrearing stress between atopic dermatitis(AD) and normal(non-AD) children in each gender, and analyzed the effects of the parental factors on the two variables of AD children. 165 AD and 1176 non-AD children of age 5 were selected from the 6th year data of the Panel Study on Korean Children. The results from t-test and multiple regression are as follows. First, only AD boys showed a higher level of behavior problems than non-AD boys, but peer interaction showed no difference between AD and non-AD groups. Compared with non-AD children's parents, mothers of AD children showed a higher stress level and mothers of AD boys had a higher depression level. But there were no differences in maternal self-esteem and paternal factors between the groups. Second, AD boys' behavior problems were related to maternal factors and paternal stress, and some characteristics of AD girls' behavior problems were related with some parental variables. But peer interaction of AD children had no relation with parental factors. Third, behavior problems were influenced by maternal stress in AD boys and influenced by maternal depression and parental self-esteem in AD girls.

가미청심련자탕(加味淸心蓮子湯)의 아토피 피부염 치료에 대한 임상고찰 (A Clinical Study on the Treatment of Atopic Dermatitis by Using Kami-chungsimyeunjatang(加味淸心蓮子湯))

  • Objectives The purpose of this study is to investigate the clinical effect of Kami-chungsimyeunjatang on atopic dermatitis and to survey the general characteristics in children with atopic dermatitis. Methods 30 patients suffering from atopic dermatitis were treated with water extract of Kami-chungsimyeunjatang and the clinical evaluation were made by SCORAD index system before and after treatment. We also investigated some characteristics of improvements by using questionnaire. Results 1. Among 30 patients diagnosed as atopic dermatitis, 18 of the people were male(60%) and 12 of the people were female(40%). 2. Among 30 patients diagnosed as atopic dermatitis, 24 of them have family history of allergic disease(80%) and 18 of them have past history of allergic disease(60%). 3. The distribution of nursing method, 13 of them (44%) had powdered milk, 10(33%) of them had both breast milk and powdered milk, 7 of them (23%) had only breast milk. 4. 20 patients(67%) who have atopic dermatitis were less than 1 year old and 6 of them (20%) were between 1 or 2 years old. The initial lesion of 22(74%) of the patients were face and neck. 5. 9 patients(30%) of them answered that the symptoms are getting worse especially during the winter while 8 patients(25%)of them said summer. 6. The mean period of treatment value was $4.6{\pm}2.20$ months. 7. After treatment, there was statistically significant decrease(p<0.05) of extent, intensity, subjective symptoms and total SCORAD index score(p=0.001). 8. After treatment, 28(93%) of the patients were improved. Conclusions Considering the above results, we speculate that Kami-chungsimyeunjatang is effective in the treatment of atopic dermatitis, and further studies are needed with more clinical cases of hematological evaluation.

Effect of processed foods on serum levels of eosinophil cationic protein among children with atopic dermatitis

  • The prevalence of atopic dermatitis (AD) in school-age children has increased in industrialized countries. As diet is one of the main factors provoking AD, some studies have suggested that food additives in processed foods could function as pseudoallergens, which comprise the non-immunoglobulin E-mediated reaction. Eosinophil cationic protein (ECP) is an eosinophil granule protein released during allergic reactions to food allergens in patients with AD. Thus, serum ECP levels may be a useful indicator of ongoing inflammatory processes in patients with AD. The purpose of this study was to investigate the effect of consuming MSG in processed foods on serum ECP levels among children with AD. This study was performed with 13 patients with AD (age, 7-11 years) who had a normal range of total IgE levels (< 300 IU/ml). All participants ate normal diets during the first week. Then, six patients were allocated to a processed food-restricted group (PRDG) and seven patients were in a general diet group (GDG). During the second week, children in the PRDG and their parents were asked to avoid eating all processed foods. On the third week, children in the PRDG were allowed all foods, as were the children in the GDG throughout the 3-week period. The subjects were asked to complete a dietary record during the trial period. Children with AD who received the dietary restriction showed decreased consumption of MSG and decreased serum ECP levels and an improved SCORing score on the atopic dermatitis index (P < 0.05). No differences in serum ECP levels or MSG consumption were observed in the GDG. Serum total IgE levels were not changed in either group. In conclusion, a reduction in MSG intake by restricting processed food consumption may lead to a decrease in serum ECP levels in children with AD and improve AD symptoms.
