Evaluating the Predictive Validity for the New Pressure Sores Risk Assessment Scale (수정욕창위험 사정도구의 예측타당도 평가)

  • Purpose: The purpose of this study was To exam the skin and pressure sore and To evaluate the predictive validity for the new pressure sores risk assessment scale. Method: There was finally 211 neurosurgery subject admitted in Chung-Ang Univ. Hospital from Nov, 11, 2002 to Feb, 11, 2003. Data was collected three times per week from 48-72hr after admission until incidence of pressure sores or discharge or die. Inclusion criteria were; (1) no pressure sores at admittance, (2) at least 3 times assessment, (3) adults older than 16yrs, (4) patients consent to participate in study. Result: 1. 34 case of 211 developed pressure sores(11.6%). 2. The coccyx area was the most common occurrence site of pressure sores. 3. At the cutoff point 23 of sensitivity 100%, specificity 76.3% was higher in 2003 than specificity 63.8% at the cutoff point 26 of sencitivity 100% in 1991. 4. "Moisture" of subscale for pressure sores risk factor was the strongest predictor. Conclusion: This study shows that the New Pressure Sores Risk Assessment Scale still predict the risk of developing pressure sores in neurosurgical subject.

Development of Fall Assessment Scale in Adult Inpatients (한국형 성인 환자 낙상위험 사정도구 개발)

  • Purpose: The purpose of this study was to develop a valid Adult Fall Assessment Scale Korean version (FAS-K) for inpatients in general hospitals in Korea. Methods: This a 14 item FAS-K was developed based on a review of national and international research and expert opinions in related field. Interrater reliability of FAS-K was analyzed using the ICC (Intraclass Correlation Coefficients). To examine the validity of FAS-K, a CVI (Content Validity Index), the Pearson correlation coefficients between FAS-K and two widely used scales, the Morse Fall Scale (MFS) and the Johns Hopkins Hospital Fall Risk Assessment Tool (JHFRAT), and the Mann-Whitney test were used. Results: The ICC was .958. The CVI ranged from .70 to 1.00. The FAS-K was significantly associated with the MFS (r=.74, p<.001) and with the JHFRAT (r=.87, p<.001), verifying the concurrent validity of FAS-K. As for the construct validity of FAS-K, significant differences between the fall and non-fall groups were found not only in the MFS (Z=-3.23, p=.001) but also in the FAS-K (Z=-3.10, p=.002). Conclusion: The findings indicate that the FAS-K is a reliable fall risk assessment tool with high validity.

Screening and Assessment Tools for Measuring Delirium in Patients with Cancer in Hospice and Palliative Care: A Systematic Review

  • Purpose: This study reviewed screening and assessment tools that are used to measure delirium in patients with cancer in hospice and palliative care settings and examined their psychometric properties. Methods: Four databases were searched for studies using related search terms (delirium, tools, palliative care, cancer, and others). The inclusion criteria were a) studies that included screening/assessment tools for measuring delirium in cancer patients receiving hospice/palliative care, and b) studies published in English or Korean. The exclusion criteria were a) studies that were conducted in an intensive care setting, and b) case studies, qualitative studies, systematic reviews, or meta-analyses. Results: Out of the 81 studies identified, only 10 examined the psychometric properties of tools for measuring delirium, and 8 tools were ultimately identified. The psychometric properties of the Memorial Delirium Assessment Scale (MDAS) were the most frequently examined (n=5), and the MDAS showed good reliability, concurrent validity, and diagnostic accuracy. The Delirium Rating Scale had good reliability and diagnostic accuracy. The Delirium Rating Scale-Revised 98 also showed good reliability and structural validity, but its diagnostic performance was not examined in hospice/palliative care settings. The Nursing Delirium Screening Scale showed relatively low diagnostic accuracy. Conclusion: The MDAS showed evidence of being a valid assessment tool for assessing delirium in patients with cancer in palliative care. Few studies examined the diagnostic performance of delirium tools. Therefore, further studies are needed to examine the diagnostic performance of screening/assessment tools for the optimal detection of delirium in patients with cancer in hospice/palliative care.

A Pilot Study for Developing an Assessment Scale for the Effect of Herbal Medicine in Healthy Children: Open-Label Study with Gami-Jiwhangtang

  • Objective : While the demand for herbal medicine has increased continuously, scientific data attesting to pharmacological activity are still insufficient. One important reason, especially in child patients, is the shortage of standardized instruments for clinical research. This study was designed to develop a scale to assess the effect of herbal medicine in children. Methods : The authors chose Gami-jiwhangtang (GJT) as a standard formulation and developed a scale, Bahn's Drug Evaluation Scale (BaDES), for this experiment. Forty-two healthy children, 7 and 8 years old, living in Seoul, Korea, volunteered to use GJT. The experimental group received GJT for 6 weeks, whereas the control group received no medicine. The children's mothers in both groups completed the BaDES on the sixth and twelfth week after GJT was commenced. Results : The experimental group showed a significant improvement in overall physical condition and gastrointestinal function as compared with the control group. Conclusion : These results suggest that BaDES may be a useful assessment tool for measuring the effect of herbal medicine.

Development a multidimensional assessment scale for work-family spillover in working couples with children (유자녀 맞벌이 부부의 다차원적 일-가족 전이 척도 개발)

  • The purpose of this study was to develop and validate a scale for the multidimensional assessment of work-family spillover in working couples with children. The subjects of this study were working wives and husbands with children in Daegu. Two surveys were administered, eliciting 243 respondents and 227 respondents, respectively. Then a series of tests were run to analyze the data, including item-to-total correlation, Cramer's V coefficients, item discrimination, Cronbach's ${\alpha}$, and exploratory and confirmatory factor analysis. The results yielded that, first, a preliminary 102 items were developed on the basis of the literature review, and this was narrowed down to 72 items selected with the help of experts. Through preliminary, primary and secondary survey analysis, the scale for the multidimensional assessment of work-family spillover was developed and validated, with a 36 items scale, 18 items for work to family spillover and 18 items for family to work spillover. Furthermore, the scale constructed three spillover processes - time interference, energy depletion, and psychological distraction - and consisted of four different domains of family life into which spillover occurs - namely marital relationship, parent-child relationship, leisure, and housework. The internal reliability was evaluated to show a confidence range of .85 ~.92 for each factor.

A Basic Study on the Development of a Grading Scale of Discourse Competence in Korean Speaking Assessment -Focusing on the Scale of 'REFUSAL' Task (한국어 말하기 평가에서 '담화 능력' 등급 기술을 위한 기초 연구 -'부탁'에 대한 '거절하기' 과제를 중심으로-)

  • Most grading scales of Korean language proficiency tests are based on existing grading scales that are not empirically verified. The purpose of this study is to develop an empirically verified scale descriptor. The 'Performance data-driven approach' that is suggested by Fulcher (1987) was used to develop the detailed description of characteristics for each level of performance. This study is focused on the functional phase of speech samples analysis (coding data) to create explanatory categories of discourse skills into which individual observations of speech phenomena can be scored. The speech samples that were collected through this study demonstrated stages of speech that can be a foundation of a grading scale. The data used in the study was collected from 23 native speakers of Korean. Speech samples were recorded from simulated speaking tests using the 'REFUSAL' task, and transcribed for analysis. The transcript was analyzed using discourse analysis. The result showed that the 'REFUSAL' task needs to go through four functional phases in actual communication. Furthermore, this study found specific and detailed explanatory categories of discourse competence based on the actual native speaker's speech data. Such findings are expected to contribute to the development of more valid and reliable speaking assessment.

A Preliminary Study for Developing a Child and Adolescent Functional Assessment Scale in the Mental Health Service (정신보건시스템 내에서의 아동청소년 기능평가척도 개발을 위한 예비연구)

  • Objectives : The aim of this study was to develop an assessment tool for measurement of children's functioning in the mental health service. We conducted a preliminary study to develop a sensitive and inclusive scale focused on the functional changes of children rather than just focusing on their symptoms or screening. Methods : The Child and Adolescent Functional Assessment Scale and the Korean-Child Behavior Checklist (K-CBCL) were both administered to 134 parents of children with emotional or behavioral problems who participated in the Aizone program and also to 186 parents of elementary school children in the Seoul metropolitan area as a control group. SPSS version 12.0 was used for statistical analysis. Results : Most of the reliability coefficients were over 0.70 except those of the conduct behavior items, which showed relatively high internal consistency. The corrected item-total correlations ranged from .411 to 758, except those of conduct behavior items and special measure items. In a concurrent validity test with K-CBCL, the total behavior problem score of K-CBCL was highly correlated with the total score of the Child and Adolescent Functional Assesment Scale (r=.610). For the construct validity, confirmatory factor analysis was performed for each of four areas, including behavior (at home/school), achievement, peer relationship, and emotion. Conclusion : The results showed that the scale was statistically reliable and valid, except for conduct behavior items. This study was conducted only for parents with elementary children. An adolescents group should be included in future studies.

The Potential Assessment and Creation Programming of Biotopes in Small and Medium City in Korea (우리나라 중소도시 비오톱 공간의 조성방안)

  • This study suggests the methods of the assessment and creation of biotopes in small and medium sized cities, in Korea. For this purpose, Chonju city was selected to classify and asses the biotope types. Moreover, relevant legislations to conserve and enhance urban biotopes were examined. The results of this study are as follows: 1) There were two approaches to asses the potential of urban biotopes in medium-sized cities. One was the urban scale evaluation for urban green spaces and the other was the biotope scale evaluation for the classification and evaluation of biotopes. 2) The urban scale evaluation was developed through overlaping analysis of landuse and vegetation factors. This study also included the conception of watershed. In this conception, three watersheds in Chonju city were characterized. According to these characteristics, individual programs for conservation and enhancement of urban green space were suggested. 3) For the biotope scale evaluation, Selected site was inclusively mapped and field investigation actually was carried. There were total 9 types of bitopes. Especially landuse was appeared in various evaluation items were vegetation structure, area of green space, condition of vegetation and vegetation profile. Mt, Gonji and Dukjin park, Chonbuk national University and fields were evaluated highly I the potential. 4) The biotope programs were based on the results of assessment and physical characteristics of biotopes. The uniform and simple levels on vegetation must be modified with various levels of vegetation structure and vernacular plants. And the physical characteristics like Points, Corridors and Patches can be organized by the conception of biotope networking theory. 5) The proper legislative environment was the clue elements for the biotope programs. Until now, only five types of parks and two types of green space are defined and the minimum size of green space has been proposed by the law. So, it is necessary to enlarge the conception of green space in legislation and improve the quality of green space by amending the related regulations. This study has limitation because it was selected only in Chonju. Through the continuous studies, we need to apply this other small and medium sized cities, South Korea. Also the data collection and management of theme maps such as actual vegetation, landuse and a soil must be done preliminary.

No-reference Image Blur Assessment Based on Multi-scale Spatial Local Features

  • Blur is an important type of image distortion. How to evaluate the quality of blurred image accurately and efficiently is a research hotspot in the field of image processing in recent years. Inspired by the multi-scale perceptual characteristics of the human visual system (HVS), this paper presents a no-reference image blur/sharpness assessment method based on multi-scale local features in the spatial domain. First, considering various content has different sensitivity to blur distortion, the image is divided into smooth, edge, and texture regions in blocks. Then, the Gaussian scale space of the image is constructed, and the categorized contrast features between the original image and the Gaussian scale space images are calculated to express the blur degree of different image contents. To simulate the impact of viewing distance on blur distortion, the distribution characteristics of local maximum gradient of multi-resolution images were also calculated in the spatial domain. Finally, the image blur assessment model is obtained by fusing all features and learning the mapping from features to quality scores by support vector regression (SVR). Performance of the proposed method is evaluated on four synthetically blurred databases and one real blurred database. The experimental results demonstrate that our method can produce quality scores more consistent with subjective evaluations than other methods, especially for real burred images.

  • In these days, as the scale of our technology becomes larger and more highly advanced, the magnitude of an accident tends to be large. For this reason, in some systems whose accident may cause a large-scale and long-term catastrophe, a preliminary and quantitative safety assessment has become required before its construction. A method developed for this requirement is risk assessment. In this study, we focused on the methodology of probabilistic risk assessment, which has been developed mainly in the field of nuclear power industry, and considered the process to adopt this method to railway system in order to establish a scientific and comprehensive way of safety assessment.
