The Surgery of Pulmonary Tuberculosis: 163 cases experience (폐결핵 수술: 163례 보고)

  • With the decreasing incidence of new cases and the highly effective results with antituberculous drug therapy, there is a marked decline in the need for surgery which was formerly such an important part in the successful program of management of this disease. During the period of two years and a half from Jun. 1984 to Dec. 1986, this study represents an analysis of 163 cases of several surgical management for eventual control of pulmonary tuberculosis at National Kon-ju tuberculosis Hospital. 1. Mode of surgical treatment was: Resection; 123 cases [Pneumonectomy: 83, lobectomy: 35, lobectomy plus segmentectomy; 4 segmentectomy: 1], thoracoplasty: 20 and others: 20. 2. Age distribution ranged 16and 68 with average of 34 years. Male and female ratio was 1.2: 1. 3. Surgical indications were: totally destroyed lung; 64, Destroyed lobe of segment; 13, cavity positive sputum; 10, cavity c negative Sputum; 6, Bronchostenosis c atelectasis; 2, empyema c or s BPF; 46, Aspergilloma; 8, Questions of Associated tumor; 4 and other 5. 4. Incidence of Complications was 10.4% and the mortality was 5.5 percent. The cause of mortality were analyzed. The main causes of death were respiratory insufficiency; 4, fulminant hepatitis; 1, hemorrhage; 1 and unknown; 1 in pneumonectomy, and asphyxia; 1 in lobectomy and sepsis; 1 in other procedure. 5. Conversion rare of positive sputum to negative state related to resectional surgery was 91.5%. In pneumonectomy, drug resistant group preoperatively showed 88.1% conversion rate postoperatively and drug sensitive group showed that 100% conversion rate. In lobectomy, both drug resistant and sensitive groups showed that 100% conversion rate postoperatively.

Morphological and Functional Outcomes after Vertical Hemilaryngeal Transplantation in the Canine (개에서 후두수직절반 이식 후 형태 및 기능에 대한 연구)

  • In this study, the author developed a new animal model to examine morphological changes and functional recoveries after vertical hemilaryngeal transplantation in the canine. Seven vertical hemilaryngeal transplantations were carried out in the canine. After preparing the host dog removing right sided hemilarynx, hemilarynx of the donor dog was transplanted by hooking up the arteries, veins, nerves and hypopharyngeal mucosa. Especially, recurrent laryngeal nerve was anastomosed at the branch level(anterior and posterior) respectively. After 7 days, for the first evaluation of the transplantation, four out of seven dogs were considered successful. Three dogs survived more than one month, which is the critical period to evaluate the functional recovery after transplantations. After EMG examination, two dogs(#3, #5 dog) showed some functional recoveries. The five-transplanted hemilarynges were sectioned at the arytenoid cartilage region to examine the morphological changes. The results showed that the transplanted hemilarynx appeared normal as control in #5 dog. In addition, #2 dog showed fairly good condition even though died from asphyxia after 9 days out of transplantation. The other. three dogs(#3, #6, #7) showed various levels of atrophy and disappearance of the muscles and cartilages in their larynges. It can be suggested that this model could contribute an advance to preparing human laryngeal transplantation in the future.

  • A case of giant fibrovascular polyp of the esophagus with a review of the literature is presented. A 52 year old man with into rmittent dysphagia was found to have an intraluminal esophageal lesion of remarkable size by the radiological studies, but overlooked at esophagoscopy. A giant esophageal polyp w s successfully re- moved surgically by transthoracic approach, although preoperative evaluation of the location and characteristics of the lesion was problematic. These pedunculated intraluminal polyps are rare and characterized by slow growing. benign nature that almost always originate at the level of. the cricopharyngeus muscle, and often attain giant proportions. Symptoms are related to esophageal ob- struction and sudden death by asphyxia can occur. Surgical removal is the choice of treatment.

Thoracic Trauma: Analysis of 150 Cases (흉부손상 150례에 대한 임상적 관찰)

  • Clinical observations were performed on 150 cases of chest trauma, those were admitted and treated at the Department of Surgery, Korea University Woosok Hospital, during the past 8 years period from August 1965 to August 1972. 1. The ratio of male to female patient of chest trauma was 3.4:1 in male predominence and age from 20 to 50 occupied 62% of the total cases. 2. The most common cause of chest trauma was traffic accident in this series. Eighty-one cases[54%] were injured by traffic accident and total cases due to blunt trauma [non-penetrating injury] were 113 cases[75.4%]including the cases with traffic accident, and remaining 37 cases[24.6%] were due to penetrating injury including 25[16.6%] cases of stab wounds. 3. Hemopneumothorax were observed in 645/[96 cases] of the total cases, and etiologic distribution revealed 78.1% due to non-penetrating trauma and 20.8% due to penetrating injury. 4. Rib fracture was found in 50% of cases. Common injuries associated with rib fracture were lung, brain and liver. 5. Most common symptom was chest pain and respiratory difficulty, and common sign associated with chest injury was decreased respiratory sound and subcutaneous emphysema. 6. Conservative non-operative treatment was performed in 94 cases[62.6%] and 56 cases [37.4%] were treated with operative treatment including 31 cases[20.6%] with open thoracotomy. 7. Overall mortality was 9.3%[14 cases] and most common causes of death were due to brain edema,asphyxia and shock.

  • 일반적으로 출생 후 1세 전후의 영유아들은 신체의 발달이 온전치 않아 수면중 질식사하는 영유아 돌연사증후군(SIDS)에 이를 수 있어, 보호자의 세심한 관심이 필요하다. 더욱이 보호자가 있더라도 오히려 더 높은 질식에 의한 사망률이 늘어나고 있다. 이를 위해서, 본 논문에서는 수면시 발생할 수 있는 질식사 방지를 위한 하드웨어 장치와 이와 연계된 스마트폰 앱(App) 시스템을 설계하고 구현한다. 제안된 시스템으로써 하드웨어 장치는 초음파 거리 센서(sonsor)를 이용하여 실시간으로 뒤집힘 여부의 정보를 수집하여 무선 통신을 통해 스마트폰 앱(App)에 전달한다. 또한 스마트폰 앱 시스템은 수집된 정보를 분석 판단하여, 비정상적인 상태의 영유아에 대해 긴급 경보와 긴급 전화를 자동 호출한다. 따라서 여러 위험에 노출되어 있는 영유아들의 수면중에 뒤척이다 엎드려 자는 경우, 호흡기를 막거나 가슴압박으로 인한 질식사를 미연에 방지한다. 향후, 본 논문에서 제안된 시스템은 졸음방지나 낙상방지시스템 같은 유비쿼터스 헬스케어(ubiquitous healthcare) 환경 구현에도 적용될 수 있다.

Assessing the forensic knowledge of 119 emergency medical technicians (119 구급대원의 법의학 지식에 대한 연구)

  • Purpose: It is crucial that 119 emergency medical technician (EMT) have adequate knowledge regarding forensic science. This study aimed to assess the forensic knowledge of EMTs in the Republic of Korea. Methods: This study was conducted on EMTs of five fire stations in D metropolitan city between August 6 and August 13, 2018. A questionnaire consisting of items on crime-related incident recognition (i.e., mechanical asphyxiation, trauma, sex crimes, and child abuse), forensic evidence management (i.e., evidence collection, preservation, and recording) was administered to the participants. Finally, 119 questionnaires were analyzed. Results: The ratio of correct answers for crime-related incidents recognition was 55.82%, which was lower than that for forensic evidence management(84.5%). In particular, the scores for the type of neck compression(16.5%) and wound assessment(44.0%) were low. The percentage of correct answers for forensic evidence management was 78.4% for evidence collection, 84.4% for evidence preservation, and 90.6% for evidence recording. Previous forensic education experience was not a significant variable. Conclusion: Specific and practical forensic science education on perception of crime-related incident, including asphyxia and wound identification, and forensic evidence collection is needed.

Development and Evaluation of a Dysphagia Assessment Tool and an Intervention Program for the Elderly in the Long-Term Care Facilities (노인요양시설 노인의 연하곤란 사정도구, 중재프로그램 개발 및 평가)

  • The purpose of this study was to develop a dysphagia assessment tool and a intervention program for elderly in the long-term care facilities and to evaluate its effect. The dysphagia assessment tool consists of 20 items was developed through literature review, previous studies and discussion with nursing/medical members. The intervention program consisted of 72 items and participants for evaluation were 50 elders who were registered at long-term care facilities in 4 regions of Seoul. Collected data were analyzed using SPSS Statistics 18.0 and Spearman's Correlation, t-test, and $x^2$-test. After the application of the dysphagia assessment tool and intervention program, the weight in experimental group was increased (t=4.913, p=.000) and the total time of swallowing was reduced (t=-4.557, p=.000) than control group. These findings were statistically significant. Considering these results, by applying the dysphagia assessment tool and the intervention program, the lives of the elderly in long-term care facilities are expected to be improved and emergency situations such as aspiration pneumonia and asphyxia will be decreased.

  • Hypoxic ischemic encephalopathy(HIE) is a condition that occurs when the entire brain is deprived of an adequate oxygen supply and associated in most cases with oxygen deprivation in the neonate due to birth asphyxia. Children with HIE develop problems such as cerebral palsy, mental retardation, learning difficulties, and other disabilities. We hereby report the case of treating a child patient with HIE whose chief complaint of severe lingual ulceration. A 12-month-old boy referred to our hospital to treat ulcers of ventral surface of tongue. He has been suffering from lingual ulceration due to persistent tongue thrusting. He has bitten his own tongue involuntary. The ulceration was apparently seemed to be not healed because of the constant irritation by lower incisors. So we designed tongue protector to prevent lingual ulcers. The appliance was proved to be effective to reduce tongue trauma by lower incisors and periodic follow-up is needed.

  • Hypoxic ischemic encephalopathy (HIE) is an important cause of permanent damage to central nervous system cells, most cases with oxygen deprivation in the nenonate due to birth asphyxia. Survival children with HIE develop problems such as cerebral palsy, mental retardation, learning difficulties, and other permanent neuro-developmental disabilities. This article report two cases of the children with HIE who had be traumatized on their teeth from oropharyngeal airway (OPA) in the emergency situation. The patients with the disease accompanying seizure or convulsion, needs special consideration for the prevention from dental trauma in emergency airway management.

  • Incidence and mortality rate of maxillofacial infection is relatively low in the era of antibiotics. Despite the use of antibiotics, delayed treatment, underlying systemic diseases, drug-resistant microorganisms may result in life-threatening situations. The deep neck infection developed from odontogenic infection may result in sepsis, mediastinitis, aspiration pneumonia, asphyxia. Sepsis is the most dangerous complication which can quickly result in a number of lethal situations. The treatment of sepsis includes awareness of such complication, use of sensitive antibiotics, removal of infection source, and hemodynamic, respiratory and metabolic support. We experienced a patient who died of sepsis, which developed from odontogenic infection. The initial diagnosis was a buccal space cellulitis. However, in spite of medical and surgical treatment, this progressed to Ludwig's angina and then deep neck infection and finally sepsis. On the 10th hospital day, the patient died of multiorgan failure caused by sepsis.
