Clinical Case of Liver Cirrhosis, with Herbal Medicine, Acupuncture, and Moxibustion Treatment (간경변 환자의 한방치험례)

  • Object : Liver cirrhosis is a disease of the liver in which normal cells are replaced by scar tissue. The purpose of this case is to report the improvement of liver cirrhosis after herb-med, acupuncture & moxibustion complex therapy. Methods : We provided herb-med, acupuncture & moxibustion complex therapy to a patient who suffered from ascites and fatigue. We examined LFT & Child-Pugh class to evaluate the effectiveness of oriental treatment. Result and Conclusion : We observed that herb-mod. acupuncture & moxibustion complex therapy decreased symptoms of liver cirrhosis and improved general condition of a patient who suffered from ascites and fatigue. In the LFT result, Child-Pugh class was improved.

Meconium Peritonitis: A Rare Treatable Cause of Non-Immune Hydrops

  • Meconium peritonitis as a cause of non-immune hydrops in neonates is rarely reported. Here we report such a rare occurrence. In our case, a routine antenatal scan at 25 weeks revealed isolated ascites. By 31 weeks of gestation, all features of hydrops were observed in scans. However, antenatal workup for immune and non-immune hydrops was negative. Subsequently, a preterm hydropic female baby was delivered at 32 weeks. She required intubation and ventilator support. An X-ray revealed calcification in the abdomen suggestive of meconium peritonitis. Ultrasound showed gross ascites, a giant cyst compressing the inferior vena cava, and minimal bilateral pleural effusion. Emergency laparotomy revealed meconium pellets and perforation of the ileum. Double-barrel ileostomy was performed, and the edema resolved and activity improved. The baby was discharged after 3 weeks. Ileostomy closure was done at follow-up. The baby is growing well.

Effects of arginine and guanidinoacetic acid with or without phenylalanine on ascites susceptibility in cold-stressed broilers fed canola meal-based diet

  • In order to evaluate the effects of ARG sources (arginine [ARG] and Guanidinoacetic acid [GAA]) and phenylalanine (PHE) supplementation on performance, susceptibility to ascites, intestinal morphology, and nutrient digestibility in the cold-stressed broilers fed a canola meal (CM)-based diet, a 2×2 factorial experiment with four treatments was conducted. The dietary treatments included CM-based diet + 2.57 g/kg ARG, CM-based diet + 2.57 g/kg ARG + 1.5 g/kg PHE, CM-based diet + 1.8 g/kg GAA and CM-based diet + 1.8 g/kg GAA + 1.5 g/kg PHE. The corn-CM diet without supplementation was used as a negative control (NC) group in the fifth treatment that excluded the factorial arrangement. The results showed that adding ARG to diets without PHE supplement increased (p < 0.05) feed intake. Also, birds fed diets containing ARG had higher (p < 0.05) body weight gain (BWG) compared to those fed GAA added diets. Supplementation of PHE improved (p < 0.05) the FCR compared to groups fed diets without added PHE. Further, ARG addition increased (p < 0.05) plasma nitric oxide (NO) concentration, carcass, breast and leg yields, duodenal, jejunal, and ileal villus height (VH) to crypt depth (CD, and dry matter digestibility, while decreasing (p < 0.05) ascites mortality and right ventricle (RV) to total ventricle (TV) ratio compared to GAA added groups. Supplementation of PHE also declined susceptibility to ascites by reducing (p < 0.01) RV to TV ratio while increasing (p < 0.05) plasma NO level. The digestibility of ether extract also increased (p < 0.05) in broilers fed GAA supplemented diets versus those fed ARG added diets. The findings suggested that ARG may improve BWG and lower ascites incidence in broilers fed a diet based on CM under cold stress because of its antihypertensive effects. Moreover, the findings of this study demonstrated the importance of including PHE formulation in ARG-deficient diets to attenuate the adverse effects of cold stress on broilers. It was also concluded that GAA could be efficaciously used in cold-stressed broilers fed an ARG-deficient diet.

A substance that inhibited the lipolytic action of toxohormone-L was isolated from white ginseng powder. This substance was an acidic polysaccharides It inhibited toxohormone-L-induced lipolysis in a dose dependent manner at concentrations higher than 100g/ml.

  • This study was devised to observe an inhibitory reaction toward an lipolytic action of toxohormone-L from KEUMSUNYEON powder. Toxohormone-L is known to be a lipolytic factor that was purified from the ascites fluid of sarcoma 180-bearing mice and of patients with hepatoma. KEUMSUNYEON powder was found to inhibit toxohormone-L induced lipolysis at its concentration of 0.5 mg/ml.

  • Toxohormone-L is a lipolytlc factor, found in ascites fluid of sarcoma 180-bearing mice and of patients with hepatoma. A substance that inhibited the lipolytic action of Toxohormone-L was isolated from Korean red ginseng powder. This substance had a pectin-like a-1,4-polygalacturonan backbone with some acetoxyl groups, and so was an acidic polysaccharide. Acidic polysaccharide was found to inhibit significantly toxohormone-L-induced lipolysis at its concentration of 10$\mu\textrm{g}$/ml.

Risk Factors for Appendiceal Metastasis with Epithelial Ovarian Cancer

  • Purpose: To investigate the risk factors for appendiceal metastasis of epithelial ovarian cancer and compare findings with the previous studies. Materials and Methods: One hundred and thirty-four patients with epithelial ovarian cancer were assessed in this study. All of them had undergone a surgical procedure including appendectomy. Of these, 21 (15.7%) patients who had appendiceal metastasis were analyzed as the case group and the patients with no metastasis were the controls, compared according to stage, grade, histology of tumor, preoperative Ca125 levels, presence of ascites, peritoneal cytology, diameter and site of tumor considered as risk factors. Results: We found statistically significant differences between the groups in terms of stage, grade, right-sided tumor location, presence of ascites, diameter of tumor${\geq}10cm$ and positive peritoneal cytology (p<0.05). In the logistic regression model, stage, grade, presence of ascites, right-sided location and diameter of tumor were independent risk factors. ROC curve analysis showed that stage, grade and diameter of the tumor were discriminative factors for appendiceal metastasis. Conclusions: In epithelial ovarian cancer, stage, grade, presence of ascites, right-sided location and large tumor size have importance for estimation of risk of appendiceal metastasis. As we compare our findings with previous studies, there is no definite recommendation for the risk factors of appendiceal metastasis in epithelial ovarian cancer and more studies are needed.

  • Pleural effusion due to hepatic cirrhosis with ascites is well known, although hepatic hydrothorax in the absence of ascites is a rare condition, the pathogenesis of which is still unknown. We report a case of hepatic hydrothorax without ascites confirmed by the intraperitoneal injection of Tc-99m mecroaggregated serum albumin (Tc-99m MAA) that demonstrated the passage of Tc-99m MAA into the right pleural cavity.

Analysis of Effect and Prognostic Factors Associated with Catheter Intervention on Ascites in Terminal Cancer Patient (복수가 있는 말기 암환자에서 복수 배액관 시술의 효과와 예후요인 분석)

  • Purpose: Malignant ascites is a common complication in terminal cancer patients. Less-invasive pigtail catheter insertion is the most frequent procedure in patients who need repeated ascites drainage. This study investigated effects and adverse events associated with catheter insertion for ascites drainage and evaluated prognostic outcomes. Methods: We reviewed medical records between 2010 and 2013 of hospice and palliative care institutions in Seoul, South Korea. Among 2,608 inpatients, 67 patients received ascites pigtail catheter drainage. We reviewed demographic data, palliative performance scale, laboratory data, duration of catheter insertion, prevalence and type of complications, use and duration of antibiotics, and survival time. Univariate and multivariate Cox regression models were used to evaluate prognostic outcomes related with catheter insertion. Results: Ascites drainage was performed most commonly in hepatobiliary and gastric cancer patients. Ascites symptoms improved in 55 patients after the catheter drainage. Adverse events included pain (19.4%), leakage (14.9%), disconnection (7.5%), catheter occlusion (6%) and fever (4.5%). In Cox regression analysis, survival time from the catheter insertion was significantly associated with Palliative Performance Scale (PPS) (HR 0.73; P value 0.045) and serum sodium level (HR 2.77; P value 0.003) in a multivariate model. Conclusion: Patients' PPS and serum sodium level should be considered before making a decision of pigtail catheter insertion.

A Case of Alcoholic Liver Cirrhosis Treated with Injinoryeong-san (인진오령산(茵蔯五苓散)을 투여한 알코올성 간경변 환자 치험 1례)

  • Objectives: To introduce the effect of traditional Korean medicine (TKM) treatment with Injinoryeong-san on alcoholic liver cirrhosis (ALC).Methods: A 37-year-old man with a history of alcohol ingestion for the past five years was diagnosed with ALC based upon his clinical symptoms, sonogram results, and laboratory studies. The patient received TKM treatment with acupuncture, moxibustion, and Injinoryeong-san in parallel to Western medicine for two weeks. The clinical assessment was monitored based upon changes in the Child-Pugh score and ultrasonography, body weight, and abdominal circumference for measuring the amount of ascites.Results: The patient’s clinical symptoms improved with decreases in abdominal circumference and body weight. Ultrasound revealed a regression of the amount of ascites in the abdominal cavity. Laboratory results also improved significantly, and the Child-Pugh score increased from class B to A.Conclusions: This case report showed a significant improvement of ALC with relatively simple treatment for only a short period and therefore supported the potential of TKM treatment in ALC.
