Dynamic contrast enhanced (DCE) magnetic resonance (MR) imaging plays an important role in non-invasive detection and characterization of primary and metastatic lesions in the liver. Recently, efforts have been made to improve spatial and temporal resolution of DCE liver MRI for arterial phase imaging. Review of recent publications related to arterial phase imaging of the liver indicates that there exist primarily two approaches: breath-hold and free-breathing. For breath-hold imaging, acquiring multiple arterial phase images in a breath-hold is the preferred approach over conventional single-phase imaging. For free-breathing imaging, a combination of three-dimensional (3D) stack-of-stars golden-angle sampling and compressed sensing parallel imaging reconstruction is one of emerging techniques. Self-gating can be used to decrease respiratory motion artifact. This article introduces recent MRI technologies relevant to hepatic arterial phase imaging, including differential subsampling with Cartesian ordering (DISCO), golden-angle radial sparse parallel (GRASP), and X-D GRASP. This article also describes techniques related to dynamic 3D image reconstruction of the liver from golden-angle stack-of-stars data.
신체조성과 혈관탄성의 변화로 인한 심혈관질환과 근감소로 인한 낙상의 발생은 노화로 인하여 가장 빈번하게 발생하게 된다. 그래서 탄력밴드 저항운동은 노인들이 쉽고 안전하게 수행할 수 있으며 신체조성과 혈관탄성에 긍정적인 영향을 미칠 수 있을 것이다. 따라서 본 연구는 고령여성(>65 years)을 대상으로 12주간 저항운동을 실시한 후 신체조성, 혈관탄성, 낙상위험도지수에 어떠한 효과를 미치는지에 대하여 살펴보았다. 저항운동은 주3회 한 회당 60분으로 구성되었고 운동 강도는 Borg의 운동자각도(6 - 20)에서 11-14로 설정되었다. 신체조성에 해당하는 체중, 근육량, 체지방률, BMI(Body Mass Index)와, 혈관탄성(맥파속도 (Pulse wave velocity (PWV)), 낙상위험도지수(Risk of falling)는 훈련 전과 후에 측정이 되었다. 연구 결과 신체조성에서 운동집단의 체중(p=.003), BMI(p=.002), PWV(p=.017), 낙상위험도지수(p=.037)는 유의하게 감소하였으나 통제집단에서 체중(p=.009), BMI(p=.009)는 유의하게 증가하였다. 따라서 여성노인들은 탄력밴드 저항운동을 통해 체중과 BMI를 감소시키고 혈관탄성에 긍정적인 효과를 얻게 되었다. 그리하여 탄력밴드 저항운동은 노인들의 대사증후군, 심혈관질환, 낙상사고의 발생률을 낮출 수 있는 매우 효과적인 운동이라 할 수 있다.
Pulse conduction velocity is determined by areterial compliance, which is changed by lateral pressure of arterial wall. Hydrostatic pressure of the limb vessel is changed by body position, especially in elevated arm. The arterial pulse in the finger causes the blood volume to change, changing the optical density of the blood. Photoplethysmograph of index finger was obtained by LED and phototransistor. Pulse transmission time(PTT) was measured by the interval between the peak of ECG R wave and the peak of the finger plethysmogram. PTT was increased by upward position of arm, and decreased by downward position of the arm compared to horizontal position. This result suggests that relationship between finger plethysmography and postural change could be applied to evaluate clinical cardiovascular status.
PPG signal measured by pulse oximeter can measure pulse and the oxygen saturation of arterial blood. But the PPG signal is distorted by finger movement or other movement in the body. To detect pulse from the PPG signal with motion artifact, we use band pass filter(BPF), Independent component analysis(ICA) and nonlinear autocorrelation(NAC). BPF is used to remove DC component and high frequency noise in the PPG signal with motion artifacts. ICA is used to separate pulse signal and motion artifact. However, pulse signal separated by ICA have no choice but to accompany signal distortion because pulse signal and motion artifact are not completely independent. So, we use nonlinear autocorrelation to emphasize the pure pulse signal from the distorted signal.
Objectives: Cardiovascular disease is a major cause of death in the elderly in Korea. Increased arterial stiffness is linked to risk of cardiovascular disease and mortality. The purpose of this study was to investigate the relationship between arterial stiffness and physical activity in the elderly. Methods: A total of 209 older adults (110 men and 99 women) participated in this study. Arterial stiffness of subjects such as brachial-ankle pulse wave velocity (baPWV) and ankle brachial pressure index (ABI) was measured using a non-invasive vascular screening device (VP-1000 Plus, Omron, Kyoto, Japan). The interviewed Korean version of the international physical activity questionnaire short form (IPAQ-SF) was used to evaluate subject's physical activity level and classify subjects as active or inactive group based on the time spent doing moderate to vigorous physical activity (MVPA). Results: The mean age of total subjects was 75.3 ± 5.6 years. There was no significant difference in sex distribution between the active group (39.7%) and inactive group (60.3%). The baPWV (1,758.1 ± 375.2 cm/sec) of the active group was significantly lower than that (1,969.7 ± 372.3 cm/sec) of the inactive group (P < 0.05). There was a significant inverse association between time spent in MVPA and baPWV (r = -0.245, P < 0.01). Conclusions: This study suggests that physical activity programs for older adults are needed to prevent arteriosclerosis.
뇌혈관 질환은 우리나라의 주요 10대 사망 원인 중 3대 사인에 해당하며, 이러한 질환은 동맥경화와 관련이 있으므로 동맥경화를 예측할 수 있는 인자의 진단 방향의 제시가 필요하다. 본 논문에서는 연령대별 상완-발목동맥 맥파전달속도와 뇌동맥 혈관 협착 및 혈압, 비만도, 복부비만과의 유의성을 평가하였으며, 뇌동맥 혈관 협착과 연령대별 유의성 평가는 50대의 연령대에서만 유의한 결과를 나타내었으며, 동맥경화의 위험인자인 혈압, 비만, 복부비만의 경우 전체 연령대에서 유의한 결과를 나타내었다. 그리고 상완-발목동맥 맥파전달속도가 상승하였을 경우, 뇌동맥 혈관 협착은 전체 1,376명중 788명으로 57.2%의 분포를 나타내었으며, 위험인자인 혈압의 경우, 전체 7,557명중 3,255명으로 43%, 비만도는 2,708로 35.8%, 복부비만은 2,941명으로 38.9%의 분포를 나타내었다. 이러한 결과를 통하여 상완-발목동맥 맥파전달속도 검사 시, 동맥경화가 의심된다면 우선적으로 뇌동맥 혈관 협착을 의심하고 뇌동맥 혈관 검사를 시행할 것을 권한다. 동맥경화의 위험인자인 혈압, 비만, 복부비만과의 유의성 평가를 통하여 향 후 유사 연구 시, 기초자료로 활용될 것이라고 사료된다.
펄스옥시메트리는 서로 다른 두 파장의 빛을 이용하여 동맥혈의 산소포화도를 측정하는 비침습성의 광에 의한 측정 방법이다. 동맥혈의 맥동성분은 정맥혈, 피부 그리고 기타의 비맥동성분들에 의해 감쇠되는 직류 성분에 중첩되어 있다. 본 논문에서는 맥동 성분의 적분비를 이용하여 산소포화도를 계산하는 알고리즘을 사용하였다. 알고리즘에서는, 심장의 수축과 관련되는 맥동신호의 1/2 싸이클만을 이용한다. 이용하는 시간은 1 cycle의 약 1/4이다. 1/4 주기를 이용하는 본 알고리즘은 기존의 방식과 유사한 결과가 나타났다. 신호의 진폭이 낮고, 시간의 점유가 많은 부분을 계산에서 배제했기 때문에, 동잡음의 영향이 감소되었다.
본 연구에서는 심장의 세포 변화에서부터 혈류 순환의 시스템 변화까지 일련의 과정을 시뮬레이션 할 수 있는 통합모델을 개발하였다. 본 통합 모델을 이용하여 대동맥의 탄성도 변화 따른 Pulse Wave Velocity를 추정하였으며 심근의 수축 Mechanics의 변화를 시뮬레이션 하였다. 심장은 단순한 구 형상으로 모델링 되었다. 특히 동맥순환의 특성인 Wave propagation 과 Wave deflection의 현상을 모델링하기 위해 기존 모델에서 사용된 동맥계 순환 모델을 수정하였다. 즉 기존의 동맥 모델을 1차원의 운동방정식과 연속방정식을 기반으로 하는 Distributed arterial model로 대체하였다. Distributed arterial model은 혈액의 점성에 의한 에너지 손실, 혈관의 점탄성 효과 그리고 분지 되는 혈관에서의 에너지 손실을 포함하는 정교한 동맥 순환 모델이다. 정교한 동맥계 순환 모델의 동맥 탄성도 값을 조절함으로써 탄성도 변화에 대한 PWV를 계산 할 수 있었다. 이러한 수치적 방법을 사용하여 노화에 따른 동맥벽 탄성도의 저하가 심근세포의 Cross-bridge 동역학에 미치는 영향을 시뮬레이션 하였다.
The conventional studies of the aging effect on cardio-vascular system are mostly obtained from human experiences and can only provide local information. In this study, the effect of fatigue fracture of arterial wall on the blood pressure was investigated by hemodynamic analysis. The aging process was simulated with three stages. The pulse wave velocity increased with age. The systolic blood pressure increases consistently but diastolic blood pressure decreases with age. The simulated results show in good accordance with the clinical ones.
The acupuncture procedures in Hwangjenaekyung (黃帝內經) was different to modern ones in many aspects. Especially, the role of pulse diagnosis in acupuncture was totally different and the pulse diagnosis was essential part in acupuncture therapy in Hwangjenaekyung era. We found four noteworthy features on the pulse diagnosis before and after acupuncture in Hwangjenaekyung : (1) Pulse diagnosis was a mandatory process in acupuncture (2) Doctors who used pulse diagnosis in 12 meridians (十二經脈遍診脈法), pulse diagnosis in 9 points of 3 body parts (三部九候脈法), and comparative pulse diagnosis between radial artery and carotid artery (人迎寸口對比脈法) followed the former rule ((1)). (3) The major pulse features to detect before and after acupuncture were conversion between the vacuous pulse (虛脈) and the replete pulse (實脈), and conversion between slippery pulse (滑脈) and rough pulse (澁脈). (4) Deukki (得氣, Deqi) was synonym of Kiji (氣至), and it referred to the changes of arterial pulse, not the sensation followed by acupuncture manipulation.
