Risk Factors for Lymphedema Patients (림프부종 환자에서의 위험 인자)

  • Purpose: Lymphedema is edema developing mainly in the arms and legs due to an abnormal lymphatic system, over one hundred millions of patient worldwide suffer from it. At present, prevention is the best treatment. Thus, It is important to know which patient are more pone to develop it in order to prevent it. By evaluating the risk factor for lymphedema, we intended to find the effective prevention. Methods: We have investigated that outpatients who chiefly complained of lymphedema visited the lymphedema clinic at one university hospital from September 1 in 2003 to August 31 in 2005. We evaluated the risk factor for lymphedema by questionnaires. Questionnaire consists of demographic data, disease factor, treatment factor, posttreatment factor. We evaluated the correlation between lymphedema with each factor. Results: The total number of patients was 50, 19 patients with breast cancer and 31 patients with cervical cancer. In terms of stages of edema, 12 patients were below stage 1 and 38 patients were above stage 2. The incidence of lymphangitis was more frequent, more obese and the impairment of the site of edema site more severe, the stage of cancer was higher in the patients higher than stage 2 than in the patients below stage 1 (P>0.05). Conclusion: Lymphedema is more severe when the cancer stage in higher, accompanying more frequent infections, more obesity and more impairment at the site of edema site. Above all, cancer stage and the impairment of edema site are impossible to correct but obesity and infection may be corrected. Therefore we speculate that we are able to protect the aggravation of edema by weight reduction and infection control.

Two Cases of Cerebral Cysticercosis Treated with Albendazole (Albendazole을 이용한 뇌낭미충증(腦囊尾蟲症) 치험(治驗) 2예)

  • Two patients with confirmed cerebral cysticercosis were treated with Albendazole(Zentel$^{(R)}$) at a daily dose of 1.200mg t.i.d. for 14 consecutive days and evaluated for tolerance and therapeutic effects. First case was 29 year old male, who had experience of 4 times of grand mal seizures during 1 year period before administration in Korea University Hospital. His chief complaints were seizure and moderate degree headache. He also had 4 subcutaneous nodules on the thorax, right and left upper arms. Among them one nodule was biopsied and confirmed microscopically as Cysticercus cellulosae hominis. Computed tomography of the brain showed four round low density lesions in right postero-frontal area, sylvian area, intra-occipital area and left parietal area. Second case was 48 year old male, who also had experience of seizures at 3 years, 5 months and 3 months before administration. In this case, no subcutaneous nodules and no headaches were noted. Brain CT showed four round low density lesions in right postero-parietal area and temporo-parietal area, and left temporo-parietal and parietal area. Serum antibody against cystic fluid antigen was detected by ELISA in both cases. The efficacy of the treatment of cerebral cysticercosis was assessed by the frequency of convulsions after treatment for 22 months follow-up. by the disappearance of the densities in cystic lesions at brain CT for 6 months follow-up, and disappearance of subcutaneous nodules, headache and so on. As the results, all low density lesions in both cases were disappeared in films of brain CT, and 4 nodules in first case were also disappeared. No more seizure and complain of headache occurred during the last 22 months after treatment in both cases. Post-treatment complete blood count and liver function test revealed no remarkable change compared to pre-treatment test. In the nations of Latin America, the physicians do not initially recommended the simultaneous administrations of steroids, reserving them only for patients whom the adverse reactions such as severe headache and/or seizures are occurred. According to them, in most patients these symptoms are controlled with aspirin and symptomatic drugs. But our experience using praziquantel is different, and most cerebral cysticercosis patients who takes PZQ had complaint of severe headache and sometimes seizure. So we simultaneously used dexamethasone as 6mg q.i.d. for 14 consecutive days and 6 days tapering thereafter in both cases for prevention of reactions produced by the host in response to the deaths of the parasites. As the conclusion, albendazole is effective in patients who presented cerebral cysticercosis and albendazole may help in the control of cysticercosis.

Effects of Arabinoxylan Rice Bran and Exercise Training on Immune Function and Inflammation Response in Lipopolysaccharide-stimulated Rats (유색미 겨 아라비녹실레인과 운동트레이닝이 Lipopolysaccharide 처치된 흰쥐의 면역인자 및 염증억제에 미치는 영향)

  • Arabinoxylan (Ara) rice bran has been shown to be a potent biological response modifier as manifested by stimulation of different arms of the immune system. We examined the effects of Ara rice bran and exercise on the immune function and cytokines in lipopolysaccharide (LPS)-stimulated rats. As the results, tumor necrosis factor-${\alpha}$ as representative inflammatory cytokines showed a significantly lower in Ara supplement group, thus the Ara rice bran had a higher inhibitory activity than the both exercise and control group. However, 4 weeks of exercise training significantly increased inflammatory reactions rather than treatment with Ara in LPS-treated rats. The Ara rice bran acted to decrease the inflammatory reaction. These results suggest that the supplement of Ara rice bran is likely contribute to inflammation response and the Ara rice bran can be used as a possible safe alternative to the immunotherapeutic modalities.

Trend of Space Development and Issue (우주개발동향과 주요 이슈)

  • October 4, 1957 the Soviet Sputnik 1 was launched into space the first time in the history of mankind. After launching, the realm of humankind was expanded to space. Today all countries of the world wage a fierce competition in order to utilize space for various purposes. World powers of space such as United States, Russia, China, and Japan, put reconnaissance satellites and ocean surveillance satellites into orbit, being able to easily see equipment and troops movement on earth. Each country makes efforts to occupy space assets through the militarization of space and expand national interests. Recently private companies or individuals involved in commercial space activities are becoming more prevalent. Thus, in addition to space activities for military purposes, commercial space activities become widespread. Individuals and private companies as well as nations are also involved in space activities. Outer space is not the monopoly of space powers such as the United States and Russia. The whole human race can benefit from free access to space, being the common heritage of mankind. In particular, outer space becomes an indispensable element of military activities and human life. Many countries are now entering space development, putting a lot of budget into new development programs. Republic of Korea also built the Narodo Space Center, starting its space development with budget and manpower. We have to find out ways to use space not only for military purposes but also for commercial space activities that can contribute to the national economy. In addition, through the joint efforts of the international community, we have to make efforts for preservation and peaceful use of space. Various issues relating to space activities and research should be studies in order to contribute to the progress of humanity. Those issues include the definition of outer space, space debris reduction and environmental conservation issues, non-bind measure cooperation - European International Code of Conduct, space law and national legislation related empowerment issues, arms control measures in space, and restrictions on the use of nuclear fuel. We also need to be involved in the discussion of those issues as one of responsible space countries. In addition, we try to find out regional cooperation schemes such as the ESA in the Europe actively. Currently in the Northeast Asia, cooperation bodies led by Japan and China respectively, are operated in the confrontational way. To avoid such confrontation, a new cooperative body needs to be established for cooperation on space exploration and information. The system to allow the exchange of satellite information for early warning of natural disasters needs to be built as well. In addition, efforts to enhance the effectiveness of the relevant international treaties on space, and fill in the blanks in international space laws should be made at the same time. To this end, we have to do a leading role in the establishment of standards such as non-binding measures (resolution) - Code of Conduct, being discussed in the UN and other organizations, and compliance with those standards. Courses in aerospace should be requires in law schools and educational institutes, and professional manpower need to be nurtured. In addition, the space-related technology and policy needs to be jointly studied among the private, public, and military groups, and the cross exchange among them should be encouraged.

Development of New Analytical Method Evaluating Working Memory on Y Maze (Y-미로에서 작업기억을 평가하는 새로운 방법 개발)

  • The Y-maze is widely used to test working memory in behavioral science. For this purpose, spontaneous alternation behavior is monitored, and an increased percentage of spontaneous alternation is regarded as enhanced working memory. However, in some cases, the percentage of spontaneous alternation does not accurately reflect the extent of working memory in rodents. To complement the short-comings of this measure, we developed a new method to evaluate working memory on the Y-maze. This is done by defining all spontaneous alternation cases and Pi, the probability that the rodent achieved spontaneous alternation from each alternation case. After all Pi-values acquired in each animal are summarized, the result is considered as entropy. To validate the new analytical method, mice were raised under either control or an enriched environmental condition for 10 weeks, and working memory behavior on the Y-maze was monitored. The results showed that the new analytical method successfully reproduced significance. In addition, the new method turned out to be more accurate than measurement of the percentage of spontaneous alternation, meaning that, to get higher entropy, alternation should be recorded in all arms and directions. Together, these data indicate that the new analytical method is a useful supplement to the method that compares the percentage of spontaneous alternation, and thus is a good tool with which to evaluate working memory in rodents.

The Trend and Implications of the Publication of China's Defense and Security White Papers (중국의 국방·안보백서 발간의 추이와 함의)

  • This paper is to analyze the trend and implications of the publication of China's defense and security white papers. To this end the paper is composed of 5 chapters titled instruction; publication details and composition of China's defense and security white paper; the gist of the white paper on national defense and security during the Chinese president Xi Jinping era; the implications of Chinese white paper on national defense and security during the Chinese president Xi Jinping era; and conclusion. The Chinese Ministry of National Defense had published seven books every two years since 1998 to 2010. In 1995, the ministry published a white paper titled 'China's arms control and disarmament' for the first time. In 2013, it published a white paper titled 'The Diversified Employment of China's Armed Forces' and 'China's Military Strategy' in 2015. All have the common characteristic of being a propaganda policy reflecting China's strategic calculations. It seems that China has began to publish defense white papers in a proactive manner, due to the following factors: (1)pressure on China to demand military transparency from neighboring countries such as the United States; (2)the erosion of the 'China threat.' and (3)confidence in the achievement of China's military modernization. The 'active defensive strategy' and the 'strong defense strategy' of Si Jinping are implied in the words "China's dream is a dream of a powerful country and dream of a strong nation is essential to construct a strong nation." His these strategies have raised security concerns for neighboring countries. We need to maintain and reinforce strong ROK-US security cooperation, and hedging strategies to harmoniously promote ROK-China economic cooperation.

The Future of Countermobility Capability with a Literature Analysis from FASCAM to Terrain Shaping Obstacle(TSO) (미래 대기동 작전 능력의 발전방안 연구 -살포식지뢰(FASCAM)로부터 지형 조성 장애물(TSO) 전력을 중심으로-)

  • In this study, the future of countermobility capability is presented by analyzing the status of the countermobility obstacles focusing on the history of landmines and munitions. The conventional landmine was forbidden globally by the CCW and Ottawa Treaty because it caused civilian damage after the war. Because the inhumanity of those mines had been acknowledged, shatterable mines with a self-destruct (SD) function and M93 "HORNET" anti-tank munition with enhanced sensors have been fielded. In 2016, the Obama administration announced a policy that banned all antipersonnel landmines, leaving a considerable gap in the countermobility capability. To deal with these problems, the developments of "SAVO" and the SLEP program of Volcano mines were conducted. In the sense of a long-term approach, the countermobility obstacles, including mines, were chosen as fundamental forces for Multi-Domain Operations and were improved to Terrain Shaping Obstacles (TSO). TSO has improved sensors and mobility kill capabilities and features an enhanced remote control over each munition on the battlefield through a network established with satellite communication. The combined arms countermobility might be fully capable until 2050 if the TSO program can be completed successfully.

Effects of Working Postures on Headache/Eye Strain in Korean Waged Workers: The 5th Working Condition Survey(2017) (근무자세가 두통/눈의 피로에 미치는 영향: 제5차 2017년 근로환경조사)

  • Objectives: The aim of this study was to examine the association of working posture with headache/eye strain in Korean waged workers. Methods: Data were collected from the 5th Korean Working Condition Survey. A total of 30,955 workers participated in this study. A four-items of working postures were measured using a 7-point Likert scale, and we categorized them into three groups ('normal', 'moderate', and 'severe') each 4 items. Headache/eye strain were measured using a self-administrative questionnaire ('yes' vs. 'no'). To examine the relationship between the total working postures score and headache/eye strain, we summed a 4-items of working postures. It was categorized into five groups according to the severity of exposures('normal', 'mild', 'moderate', 'severe', and 'very severe'). The multivariate logistic regression analysis was performed using the IBM SPSS(Ver. 25), and a p<.05 was considered significant. Results: The results show that 'fatiguing and painful posture' and 'posture repetitively using hands or arms' were associated with headache/eye strain after controlling for control variables. The total working posture score was positively associated with headache/eye strain. People with higher levels of the total working posture score were more likely to increase the risk of headache/eye strain compared to those of normal(OR: 1.844, 95% CI: 1.549~2.195 for 'mild'; OR: 2.564, 95% CI: 2.152~3.503 for 'moderate'; OR: 4.140, 95% CI: 3.432~4.994 for 'severe'; OR: 7.613, 95% CI: 5.625~10.304 for 'very severe') (p<.05 for trend). Conclusions: These results indicate that inappropriate working postures might play a crucial role in developing headache/eye strain. It is needed to improve the working environment, especially focusing on reducing inappropriate working posture at the organizational level.

The Effect of Glycolic acid peeling and Seaweed peeling on keratosis pilaris (글릭콜릭산 필링과 해초 필링이 모공각화증 피부에 미치는 영향)

  • This study was conducted to investigate the effects of seaweed peeling (SP), glycolic acid peeling (GP) and general scrub (GS), which are widely known as cures for acne in both medicine and esthetics on the keratosis pilaris skin and provide basic data for a keratosis pilaris improvement program. For the experiment, subjects were categorized into control (GS) and experimental (GP and SP) groups, and tests were performed on arms and legs with relatively high keratosis pilaris symptoms (5 parts for each group) for 6 weeks. The keratin quantity, sebum content, moisture level and pigmentation were measured before and after (2, 4 and 6 weeks) the experiment and comparatively analyzed. The GP group showed an increase in moisture level (t=-4.064, p<0.01) but a decrease in pigmentation (t=3.536, p<0.01), while a decrease in keratin quantity (t=2.370, p<0.05) and pigmentation (t=4.017, p<0.01) was observed in the SP group and a decrease in keratin quantity (t=2.834, p<0.05) and an increase in moisture level (t=-7.589, p<0.001) was observed in the control group (GS). Additionally, the skin irritation reaction was lowest in the GS group. The SP group had the highest satisfaction with the improvement in response to keratosis pilaris care. When asked if they were willing to get the treatment with the same product, both SP and GP groups were high. In other words, keratosis pilaris care was needed in both experimental and control groups. Overall, the results of this study indicate that SP, GP and GS, which are commonly used in remedying acne, normalize turnover cycle by removing the dead cells from around the pores and improve keratosis pilaris symptoms by increasing moisture in the skin. Therefore, to improve keratosis pilaris skin, it is important to keep removing keratin and using a moisturizer that provides a skin barrier on a regular basis. The results presented herein will be useful as basic data for a keratosis pilaris improvement program.

Development of Supplemental Equipment to Reduce Movement During Fusion Image Acquisition (융합영상(Fusion image)에서 움직임을 줄이기 위한 보정기구의 개발)

  • Purpose: Patients' movement during long image acquisition time for the fusion image of PET-CT (Positron Emission Tomography-Computed Tomography) results in unconformity, and greatly affects the quality of the image and diagnosis. The arm support fixtures provided by medical device companies are not manufactured considering the convenience and safety of the patients; the arm and head movements (horizontal and vertical) during PET/CT scan cause defects in the brain fundus images and often require retaking. Therefore, this study aims to develop patient-compensation device that would minimize the head and arm movements during PET/CT scan, providing comfort and safety, and to reduce retaking. Materials and Methods: From June to July 2012, 20 patients who had no movement-related problems and another 20 patients who had difficulties in raising arms due to shoulder pain were recruited among the ones who visited nuclear medicine department for PET Torso scan. By using Patient Holding System (PHS), different range of motion (ROM) in the arm ($25^{\circ}$, $27^{\circ}$, $29^{\circ}$, $31^{\circ}$, $33^{\circ}$, $35^{\circ}$) was applied to find the most comfortable angle and posture. The manufacturing company was investigated for the permeability of the support material, and the comfort level of applying bands (velcro type) to fix the patient's head and arms was evaluated. To find out the retake frequency due to movements, the amount of retake cases pre/post patient-compensation were analyzed using the PET Torso scan data collected between January to December 2012. Results: Among the patients without movement disorder, 18 answered that PHS and $29^{\circ}$ arm ROM were the most comfortable, and 2 answered $27^{\circ}$ and $31^{\circ}$, respectively. Among the patients with shoulder pain, 15 picked $31^{\circ}$ as the most comfortable angle, 2 picked $33^{\circ}$, and 3 picked $35^{\circ}$. For this study, the handle was manufactured to be adjustable for vertical movements. The material permeability of the patient-compensation device has been verified, and PHS and the compensation device were band-fixed (velcro type) to prevent device movements. A furrow was cut for head fixation to minimize the head and neck movements, fixing bands were attached for the head, wrist, forearm, and upper arm to limit movements. The retake frequency of PET Torso scan due to patient movements was 11.06% (191 cases/1,808 patients) before using the movement control device, and 2.65% (48 cases/1,732 patients) after using the device; 8.41% of the frequency was reduced. Conclusion: Recent change and innovation in the medical environment are making expensive medical image scans, and providing differentiated services for the customers is essential. To secure patient comfort and safety during PET/CT scans, ergonomic patient-compensation devices need to be provided. Therefore, this study manufactured a patientcompensation device with vertically adjustable ergonomic ROM according to the patient's body shape and condition during PET Torso scan. The defects in the basal ganglia images due to arm movements were reduced, and retaking was decreased.
