Immunomodulatory Effects of Cimicifugae Rhizoma Extracts in Macrophages

  • Cimicifugae Rhizoma (CR) belongs to the Ranunculaceae family, which has been traditionally used to treat climacteric complaints, antipyretics and diaphoresis as an alternative medicine for estrogen hormone replacement therapy with estrogens. Recently, it has been reported that different extract fractions of CR have various effects such as anti-allergic, anti-inflammatory and anti-proliferative activities. The current study investigated the immunomodulatory effects of Cimicifugae Rhizoma water extracts (CRE) in the macrophage-like cell line, Raw 264.7. Our results showed that CRE ($1\sim50\;{\mu}g/mL$) stimulated tumoricidal activity and NO production, whereas phagocytic activity was inhibited at the same concentrations. Additionally, iNOS mRNA expression was significantly increased in Raw 264.7 exposed to CRE as demonstrated by RT-PCR. These results indicate that the tumoricidal activity induced by CRE may be mediated by the production of NO and these activities may be useful for the treatment of diseases such as cancer.

  • Febrile seizure (FS) is the most common seizure disorder of childhood, and occurs in an age-related manner. FS are classified into simple and complex. FS has a multifactorial inheritance, suggesting that both genetic and environmental factors are causative. Various animal models have elucidated the pathophysiological mechanisms of FS. Risk factors for a first FS are a family history of the disorder and a developmental delay. Risk factors for recurrent FS are a family history, age below 18 months at seizure onset, maximum temperature, and duration of fever. Risk factors for subsequent development of epilepsy are neurodevelopmental abnormality and complex FS. Clinicians evaluating children after a simple FS should concentrate on identifying the cause of the child's fever. Meningitis should be considered in the differential diagnosis for any febrile child. A simple FS does not usually require further evaluation such as ordering electroencephalography, neuroimaging, or other studies. Treatment is acute rescue therapy for prolonged FS. Antipyretics are not proven to reduce the recurrence risk for FS. Some evidence shows that both intermittent therapy with oral/rectal diazepam and continuous prophylaxis with oral phenobarbital or valproate are effective in reducing the risk of recurrence, but there is no evidence that these medications reduce the risk of subsequent epilepsy. Vaccine-induced FS is a rare event that does not lead to deleterious outcomes, but could affect patient and physician attitudes toward the safety of vaccination.

  • Background and Purpose : To investigate the wild plants to be used for survival on tactical activity of Special Forces soldiers at deep enemy's territory Material and Methods : To investigate the wild plants, I selected various native plants in the forests and fields that can be used for medical cure and eatables. And I selected some poisonous plants and some plants containing water or salts, after due consideration of tactical activity and use in the battle-fields. Results and Conclusion : There are many wild plants to be used for multiple purpose in our country. We can get from these plants emergency medicines, foods, even water and salts. I chose 6 species of digestives, 11 species of analgesics, 9 species of antiphlogistics, 7 species of hemostatics, 5 species of antidiarheal, 5 species of diuretics, 2 species of laxatives, 7 species of antipyretics, 1 species of vermicide and 45 species of eatable plants and 12 species of poisonous plants. and I summarized general name, scientific name, component, picking season, used portion, taking method of these plants. Building up accurate knowledge on the wild plants could contributed to raise survival rate of Special Forces soldiers in the battle-fields.

배합해열진통제(配合解熱鎭痛劑)의 전임상시험(前臨床試驗)에 관(關)한 연구(硏究) (The Pre-Clinical Experiments of the Compounding Antipyretic Analgesics)

  • The studies of the compounding analgestic antipyretics were examined by the converted Koster's method (mice) and the converted Mac Donald's method (mice) induced on the three assumption crossover test. And the following results were found. 1. The same effect of the writhing inhibition in this compounding antipyretic dosage by it's oral administration is as follows. Aminopyrine 100mg/kg. (standard), aminopyrine 50mg/kg compounding with chlorpheniramine maleate 2mg/kg., compounding with diphenhydramine hydrochloride 8mg/kg., compounding with atropine sulfate 0.2mg/kg., or compounding with scopolamine hydrobromide 0.2mg/kg. And aspirin80mg/kg., Salicylamide 90mg/kg., sulpyrine 60mg/kg., or phenacetin 70mg/kg. compounding with the same dosage of the adjutants above. 2. The elevation-rate of the reaction threshold in this compounding antipyretic dosage by it's oral administration calculate as follows. When the elevation-rate (ER) of aminopyrine (100mg./kg.) is 1.00 (Standard), ER of aminopyrine (50mg./kg.) compounding with chlorpheniramine maleate (2mg./kg.) calculated 1.42, aspirin (80.mg./kg.) compounding with diphenhydramine hydrochloride (80mg./kg.) calculated 1.18, salicylamide (90mg./kg.) compounding with chlorpheniramine maleate (2mg./kg.) calculated 1.15, sulpyrine (60mg./kg.) compounding with chlorpheniramine maleate(2mg./kg.) calculated 1.28, and ER phenacetin (70mg./kg.) compounding with chlorpheniramine maleate (2mg./kg.) calculated 1.19.

성인 발열환자의 발열관리 실태 조사 (A Descriptive Study for Fever Management in Adult Febrile Patients)

  • Purpose: The specific aims of this study were to define fever from the nurse's and physician's perspectives, describe fever management method and decision-making, and identify differences of definition of fever and fever managements between nurses and physicians. Methods: A cross-sectional survey using a 23-item questionnaire was used. A total of 305 nurses and physicians were surveyed, and 246 useful questionnaires were returned. Results: Body temperature of $36.5{\sim}38.3^{\circ}C$ were considered as adult febrile, and fever managements were initiated when the temperature was $37.0{\sim}38.0^{\circ}C$. More than half of the nurses and physicians, made a clinical decision regarding fever managements based on their own experiences. The most frequently used interventions in nursing practice were antipyretics, intravenous hydration, and external cooling. Nurses provided external cooling to lower body temperature than physicians (Z=-4.67, p<.001). Most participants reported that they needed a fever management protocol. Conclusion: Based on the information obtained, nurses respond with sensitivity to lower body temperature. We found significant differences among nurses, between nurses and physicians in approaching care of adult febrile patients.

의도적 중독으로 응급센터에 내원한 환자에 대한 연령별 비교 분석 (Comparison of Epidemiology and Outcomes of Patients with Intentional Poisoning by Age Groups: Single Center Observation Study)

  • Purpose: This study was conducted to compare demographic and characteristic differences in self-poisoned patients by age groups using a comprehensive trauma database. Methods: This study included 1,823 patients who were admitted to the emergency room following self-poisoning. Three age groups were defined: young patients (less than 19 years), adult patients (19 to 64 years) and elderly patients (65 years and old). From January 2011 to December 2015, data were obtained from in a single emergency medical center. We investigated the materials of poisoning, ingestion time, GCS, systolic blood pressure, heart rate, and outcomes of patients. Results: A total of 1,823 self-poisoned patients visited the hospital during 5 years. Among these, 130 (7.1%) were categorized as young, 1,460 (80.1%) were adults and 233 (12.8%) were elderly. The most common drug of self-poisoning substances was antipyretics (50.0%) for those in the young group, sedative drugs (45.0%) for adults, and pesticides (41.2%) for the elderly. Mortality was 2.7% in the adult group and 14.6% in the elderly group. After adjusting for all factors related to mortality, the odds ratio (OR) of mortality was 2.63 in the elderly group (95% confidence interval [CI]: 1.44-4.81). Conclusion: There were definite differences in the characteristics of three groups. The younger patients used drugs that could be easily obtained while older patients used more lethal drugs.

양호일지를 이용한 학생보건진단 (Diagnosis of Health Problems in School Children Through the Analysis of Daily Health Records)

  • Based generally on the socio-economic status of the surrounding areas of Seoul daily health records from 6 randomly sampled primary schools were analyzed to evaluate the health problems of school children in 1992. Diseases were classified into 11 categories according to ICD-9. The mean number of visits to health care rooms during school per student was 0.95 during 215 school days from February to December in 1992 and the mean number of daily visits was 10.12. Female students visited health rooms more frequently than male students. The total spell base incidence rate was 947.3 per 1,000 students in a year; the incident rate from digestive diseases was 342.1; from injuries it was 333.6; and from respiratory diseases, it was 243.9. 85% of all diseases were from trauma, gastric symtoms, and common colds. The most frequent diseases for male students resulted from trauma and for female students from gastric symtoms. The average incident rate was highest in the Kangnam area, and the lowest in the Kangbuk area and this result is statistically significant. The incident rates of 5th and 6th graders were significantly higher than the rest. Emergency cases refered to hospital were 140(1.07%), and drugs used for treatment consisted of digestives, drugs for common colds, analgesics and antipyretics, eye drops, and external ointments for trauma. In conclusion the above results suggest that the school health service program and health education program should be based on the health status of school children.

알파 아마니틴에 의한 간독성에 대한 갯방풍의 보호 효과 (In vitro Protective Effects of Glehnia Littoralis on Alpha-amanitin Induced Hepatotoxicity)

  • Purpose: Glehnia littoralis has been used to treat ischemic stroke, phlegm, cough, systemic paralysis, antipyretics and neuralgia. The pharmacological mechanisms of Glehnia littoralis include calcium channel block, coumarin derivatives, anticoagulation, anti-convulsive effect, as well as anti-oxidant and anti-inflammatory effects. Alpha-amanitin (${\alpha}$-amanitin) is a major toxin from extremely poisonous Amanita fungi. Oxidative stress, which may contribute to severe hepatotoxicity was induced by ${\alpha}$-amanitin. The aim of this study was to investigate whether Glehnia littoralis ethyl acetate extract (GLEA) has the protective antioxidant effects on ${\alpha}$-amanitin -induced hepatotoxicity. Methods: Human hepatoma cell line HepG2 cells were pretreated in the presence or absence of GLEA (50, 100 and $200{\mu}g/ml$) for 4 hours, then exposed to $60{\mu}mol/L$ of${\alpha}$-amanitin for an additional 4 hours. Cell viability was evaluated using the MTT method. AST, ALT, and LDH production in a culture medium and intracellular MDA, GSH, and SOD levels were determined. Results: GLEA (50, 100 and $200{\mu}g/ml$) significantly increased the relative cell viability by 7.11, 9.87, and 14.39%, respectively, and reduced the level of ALT by 10.39%, 34.27%, and 52.14%, AST by 9.89%, 15.16%, and 32.84%, as well as LDH by 15.86%, 22.98%, and 24.32% in culture medium, respectively. GLEA could also remarkably decrease the level of MDA and increase the content of GSH and SOD in the HepG2 cells. Conclusion: In the in vitro model, Glehnia littoralis was effective in limiting hepatic injury after ${\alpha}$-amanitin poisoning. Its antioxidant effect is attenuated by antidotal therapy.

목표체온유지치료를 적용한 심정지 후 증후군 환자에서 반동성 고체온 발생군과 비발생군의 임상결과 비교 (Comparison of Clinical Outcomes between Rebound Hyperthermia and Non-Rebound Hypertherma Groups in Postcardiac Arrest Syndrome Patients Undergoing Targeted Temperature Management)

  • Purpose : This retrospective study aims to provide basic data for intervention to improve clinical outcomes and identify the characteristics of the rebound hyperthermia (RHG) and non-rebound hyperthermia (NRHG) groups by checking body temperature in patients with post-cardiac arrest syndrome. Method : The study involved 118 patients who completed target temperature management (TTM) in an acute-care unit. Data were analyzed for frequency, percentages, mean, standard deviation, median, and quartiles, and compared using the chi-squared test and Mann-Whitney U-test. Results : Rebound hyperthermia (RH) was observed in 74 (62.7%) patients, predominantly male (69.5%), with an average age of 64.54 ± 15.98, and a body mass index of 23.22 ± 4.75kg/m2 (overweight). Hypertension (50%) was the most common co-morbidity, followed by diabetes and heart disease (33.1%). Neuron-specific enolase levels were higher in the NRHG 24, 48, and 72 hours after recovery of spontaneous circulation (p = .037, p < .001, p = .008). The APHCHE IV was also higher in the NRHG (p < .001). RH occurred 25.49 (7.28-52.96) hours after TTM completion, lasting for 2 (1-3) hours. Temperature reduction strategies included notifying doctors, administering antipyretics, and nursing intervention, with the latter being the most common at 94.6%. Half of the subjects in the RHG and 77.3% in the NRHG fell into cerebral performance categories 3, 4, and 5 (p = .003). Conclusion : RH is more likely a body mechanism related to CPR and TTM than a result of pathogenic infection. Therefore, we require an active intervention for hyperthermia, and a patient-specific nursing intervention protocol.

소아의 열에 대한 부모의 반응과 이해 및 처치 (Parental response, recognition and management about fever in children)

  • 목 적 : 열은 소아에서 매우 흔한 증상 중 하나이지만 부모들은 열 자체를 위험한 질환으로 인식하는 경우가 많다. 열에 대해 올바로 아는 것은 부모의 불필요한 염려를 덜고 환아의 질환과 진단 및 치료 과정을 이해하는데 도움을 줄 것이다. 이에 저자들은 소아의 열에 대한 부모들의 인식을 조사하고 과거와 비교하여 변화가 있는지 알아보고자 하였다. 방 법 : 2006년 5월부터 7월까지 3개월 동안 가톨릭대학교 성바오로병원 소아과 외래를 방문한 소아의 보호자들에게 열에 대한 지식, 태도, 대처 방법을 묻는 19문항의 설문조사를 실시하였다. 결 과 : 136명이 답하였으며 10년 전에 비하여 열에 대한 정보, 태도, 대처 방법 등에서 크게 개선된 점이 없었다. 열에 대한 기본 지식인 열의 기준과 측정 방법에 대해서 잘못 알고 있는 보호자가 많았고, 열성경련과 뇌손상의 가능성에 대한 염려는 여전히 지속되고 있었다. 열은 항상 해롭고 위험한 것이라고 생각하고 있었으며, 따라서 치료가 필요하지 않은 체온에서도 실제보다 심각한 결과를 걱정하며 해열제를 투여하고 병원에 방문하고 있음을 보여주었다. 결 론 : 보호자들은 열이 이로운 점을 가진 생리 반응중의 하나라는 사실을 인식하지 못하고 실제보다 심각한 결과를 예상하며 따라서 불필요한 우려와 치료를 택하게 된다. 이런 잘못된 인식은 시간의 경과에도 불구하고 큰 변화가 없었으며 따라서 소아과 의사들의 역할이 중요하다고 할 수 있다. 부모들에게 해열제를 사용하도록 할 때는 열이 해롭지 않으며 해열제의 목적은 열로 인한 아이의 불편감을 덜어주는 것임을 반복하여 알려주고, 더 나아가서는 소아과 의사들이 좀 더 체계적인 정보 및 교육 프로그램을 제공한다면 열에 대한 올바른 인식 형성에 도움이 되리라고 생각된다.
