Etiology of Bacteremia in Children with Hemato-oncologic Diseases from a Single Center from 2011 to 2015 (단일 기관에서의 소아 혈액종양 환자에서 발생한 균혈증의 원인균 및 임상 양상: 2011-2015년)

  • Purpose: The aim of this study was to identify the pathogens of blood stream infection (BSI) in children with hemato-oncologic disorders, to analyze susceptibility patterns of microorganisms to guide empirical antimicrobial therapy, and to compare temporal trends of the pathogen and antimicrobial susceptibility with those of previous studies. Methods: We retrospectively analyzed the medical records of children with hemato-oncologic disorders whose blood culture grew pathogens at the Seoul National University Children's Hospital between 2011 and 2015. Results: A total of 167 patients developed 221 episodes of bacteremia. Among 229 pathogens, gram-negative bacteria (GNB) accounted for 69.0% (64.0% in 2002 to 2005, 63.4% in 2006 to 2010); gram-positive bacteria (GPB) accounted for 28.8% (31.3% in 2002 to 2005, 34.6% in 2006 to 2010); and fungus accounted for 2.2%. Among GNB, Klebsiella species (53.2%, 84/158) and Escherichia coli (19.6%, 31/158) were common. Staphylococcus aureus (48.5%, 32/66) and viridans streptococci (21.2%, 14/66) were frequently isolated among GPB. The susceptibilities of oxacillin and vancomycin in GPB were 54.8% and 96.9% (51.5% and 95.5% in 2002 to 2005; 34.1% and 90.5% in 2006 to 2010), respectively, whereas in GNB, the susceptibilities of cefotaxime, piperacillin/tazobactam, and imipenem were 73.2%, 77.2%, and 92.6% (75.9%, 82.8%, and 93.4% in 2002 to 2005; 62.8%, 82.9%, 93.8% and in 2006 to 2010), respectively. There were no significant differences in the proportion of etiologic agents or the antimicrobial susceptibilities between the current study and that of the previous two studies from 2002 to 2010. Overall fatality rate was 13.1%. Conclusions: GNB predominated in BSI among children with hemato-oncologic disorders. The etiology of bacteremia and antimicrobial susceptibility were comparable to those of the previous studies. Thus, piperacillin/tazobactam can be used as the initial empirical antimicrobial agent in febrile neutropenia.

Antimicrobial susceptibility and genetic characteristics of Streptococcus uberis isolated from bovine mastitis milk (젖소 유방염 유즙에서 분리한 Streptococcus uberis의 항생제 감수성 및 유전학적 특성)

  • Streptococcus spp. comprising Streptococcus (S.) uberis S. dysgalactiae strains is major causeof bovine mastitis from particularly well-managed or low somatic cell count herds that have successfullycontrolled contagious pathogens. In this study, antimicrobial susceptibility and genetic characteristics of S.uberis isolated from clinical or subclinical mastitis milk at 2003 were investigated. Eighty seven isolatesof Streptococus spp. were identified by the conventional biochemical methods. The antimicrobialsusceptibility by disk diffusion method was determined for 46 S. uberis, 11 S. bovis, 10 S. oralis, 6 S.uberis and 14 other Streptococcus spp.. Overall, the tested strains were susceptible to tetracycline (11.5%),amikacin (14.9%), streptomycin (16.1%), neomycin (26.4%), kanamycin (35.6%), gentamicin (65.2%),oxacillin (70.1%), ampicillin (75.9%), chloramphenicol (78.2%), and cephalothin (97.7%). Additionally, S.uberis strains were susceptible to pencillin G (97.8%), but resistant to erythromycin (76.0%) by minimalinhibitory concentration test. The multiple-drug resistance rate of isolated bacteria to 4 more thanamplification fingerprinting patterns amplifed with primer 8.6d showed that 3 to 8 number of distinguishableDNA fragments ranged from 180 bp to 1,20 bp. Thirty seven isolates of S. uberis strains were subtypedinto 8 distinct patterns. Each subtype revealed a typical pattern of antimicrobial susceptibilities. Thesefindings demonstrate that S. uberis isolates were mastitis pathogens of diverse serotypes, and oftenencountered the diverse resistant patterns.

Molecular Characterization of Pseudomonas aeruginosa Isolates Resistant to All Antimicrobial Agents, but Susceptible to Colistin, in Daegu, Korea

  • Multi-drug resistant Pseudomonas aeruginosa has been implicated in a variety of serious therapeutic problems in clinical environments. Among the 968 P. aeruginosa isolates obtained from two hospitals in Daegu, Korea, we acquired 17 isolates that were resistant to all available tested antimicrobial agents, with the exception of colistin (colistin-only sensitive). We characterized the antimicrobial susceptibilities, $metallo-{\beta}-lactamases$, and epidemiological relatedness among the colistin-only sensitive P. aeruginosa isolates. All colistin-only sensitive isolates were positive in the modified Hodge test and imipenem-EDTA synergy test, thereby indicating the production of $metallo-{\beta}-lactamases$. 11 isolates from the secondary hospital and six isolates from the tertiary teaching hospital harbored $bla_{VIM-2}$ and $bla_{IMP-1}$, respectively. The pulsed-field gel electrophoretic analysis of the SpeI-digested DNA from P. aeruginosa isolates indicated that two different clones of colistin-only sensitive P. aeruginosa originated from each hospital, and had spread within the hospital environment. Overall, colistin-only sensitive P. aeruginosa was detected in Korea for the first time, but no pan-drug resistant bacteria were identified. Nationwide surveillance is required in order to monitor the emergence of colistin-only sensitive or pan-drug resistant bacteria.

Biochemical properties and antimicrobial drug susceptibility of Pasteurella haemolytica isolated from pneumonic calves and goats (호흡기 증상을 나타낸 송아지 및 산양에서 분리한 pasteurella haemolytica의 생화학적 특성 및 약제감수성)

  • The present study was conducted to investigate biochemical properties and antimicrobial drug susceptibilities of 57 strains of Pasteurella haemolytica (P haemolytica) isolated from pneumonic calves and goats in Youngnam province during the period from September 1989 to February 1991. P haemolytica was isolated from 95 of 142(31.7%) pneumonic calves of 1 to 6 months of age and from 9 of 24(37.5%) pneumonic goats of 2 to 10 months of age. Seasonal isolation frequency of P haemolytica in calves and goats varied from 16.6 to 35.7%, and it was higher in winter. The majority of biochemical and cultural properties of P haemolytica isolated from calves and goats were identical to those of the reference strains employed. All isolates were susceptible to ceftiofur, erythromycin, kanamycin, gentamicin, and chloramphenicol (MIC: <$25{\mu}g/ml$), some of them were resistant to amikacin, sulfadimethoxin, and streptomycin (MIC: >200IU or ${\mu}g/ml$).

  • The present study was conducted to investigate biochemical properties and antimicrobial drug susceptibilities of 47 strains of E rhusiopathiae isolated from the cases of acute septicemic swine erysipelas in Youngnam and Kyunggi provinces during the period from June 1988 to December 1989. The isolants were identified as E rhusiopathiae on the basis of cellular and colonial morphology, and characteristic reactions in some biochemical tests. All the organisms produced hydrogen sulfide in triple sugar iron agar and showed the characteristic "pipe cleaner" type of growth in gelatin stab cultures. The majority of biochemical and cultural properties of E rhusiopathiae isolated from pigs affected with acute erysipelas were identical to those of the reference strains employed. All the isolates were highly susceptible to penicillin G, ampicillin, erythromycin (MIC:0.025~0.39IU or ${\mu}g/ml$), and moderately susceptible to oleandomycin, oxytetracycline, chloramphenicol(MIC:$0.78{\sim}25{\mu}g/ml$). Kanamycin and sulfadimethoxine showed no activity against the isolates(MIC:>$400{\mu}g/ml$). The MICs of dihydrostreptomycin presented two distribution peaks; of 47 strains, 5(10.6%) were resistant to dihydrostreptomycin (MIC:$400{\mu}g/ml$), while the majority of them were susceptible to the drug.

Investigation of ${\beta}$-Lactamase-producing Multidrug-resistant Pseudomonas aeruginosa Isolated from Non-Tertiary Care Hospitals in Korea

  • A total of 2,280 nonduplicate clinical isolates of Pseudomonas aeruginosa, obtained nationwide from Korean non-tertiary care hospitals from 2002 to 2005, were identified and their susceptibilities to aminoglycosides, antipseudomonal penicillins, carbapenems, cephalosporins, monobactams, and quinolones were studied, together with their production of ${\beta}$-lactamases. Using disk diffusion and minimum inhibitory concentration tests, it was found that 2.9% of isolates were multidrug-resistant (MDR) P. aeruginosa. An EDTA-disk synergy test, PCR amplification with specifically designed primers, and direct sequencing of the PCR products showed that the $bla_{OXA-10}$, $bla_{VIM-2}$, $bla_{OXA-2}$, $bla_{OXA-17}$, $bla_{PER-1}$, $bla_{SHV-12}$, and $bla_{IMP-1}$ genes were carried by 34.3%, 26.9%, 3.0%,3.0%, 1.5%, 1.5%, and 1.5% of 67 MDR P. aeruginosa isolates, respectively. The prevalence of MDR P. aeruginosa was three-fold higher, compared with that from the United States. More than two types of ${\beta}$-lactamase genes were carried by 10.4% of isolates. The most prevalent ${\beta}$-lactamase genes were $bla_{VIM-2}$ and $bla_{OXA-10}$. This study is the first description of MDR P. aeruginosa trom non-tertiary care hospitals in Korea and the coexistence of the $bla_{VIM-2}$, $bla_{IMP-1}$, or $bla_{PER-1} in these clinical isolates.

  • The present study was conducted to investigate the incidence of Pasteurella multocida infection in cheju swine from March 1994 to December 1994 isolated organisms were identified by the biochemical properties, cellulose serological type and antibiotic susceptibilities. Pasteurella multocida was isolated from the Lungs of 96 pigs with pneumonia(51 %) among 188 slaughtered pigs. The majority of p multocida isolates were identical to those of the standard strains. On the classification of the capsular type of the isolated p multocida it consist of the 88 isolates of type A(91.6%) 2 isolates of type D (2%) and un classified 6 types(6.2%). The majority of the 96 isolates of p multocida highly susceptible to the antibiotics including ampicillin(Am), cephalotin(Ce), erythromycin(Em), gentamycin(Gm), kanamycin(Km), lincomycin(Lm), neomycin(Nm), penicillin(Pc), streptomycin(Sm), solfametoxazol/trimethoprim(Sxt) and tetracycline(Te)

  • Habituation to alkali condition and susceptibility to antimicrobial agents were investigated to determine methods t inactivation Escherichia coli O157:H7 associated with food poisoning. The investigation showed that Escherichia coli O157:H7growth at pH 9.0 The susceptibilities of Escherichia coli O157:H7 to 13 drugs examined were found highest in regad to saicin and ceftriaxone and lowest in regard to erythromycin and josamycin. No significant changes in minimal inhibitory concen-tration(MIC) values of bactericidal effect was found when saicin and ceftriaxone were irradiated with doses of 5-100KGy.

Genetic Diversity of Metallo-β-lactamase Genes of Chryseobacterium indologenes Isolates from Korea

  • This study was performed to characterize the chromosomal metallo-${\beta}$-lactamases (MBLs) of Chryseobacterium indologenes isolated from Korea and to propose a clustering method of IND MBLs based on their amino acid similarities. Chromosomal MBL genes were amplified by PCR from 31 clinical isolates of E. indologenes. Nucleotide sequencing was performed by the dideoxy chain termination method using these PCR products. Antimicrobial susceptibilities were determined by the agar dilution method. PCR experiments showed that all 31 E. indologenes isolates contained all $bla_{IND}$ genes. DNA sequence analysis revealed that E. indologenes isolates possessed ten types of $bla_{IND}$ gene, including seven novel variants ($bla_{IND-8}$ to $bla_{IND-14}$). The most common combination of MBL was IND-2 (n = 18). Minimum inhibitory concentrations of imipenem and meropenem for the isolates harboring novel IND MBLs were ${\geq}16{\mu}g/mL$. IND MBLs were grouped in three clusters, based on amino acid similarities.

Antimicrobial Activities of Corn Silk Extract of Klebsiella pneumoniae (옥수수수염 추출액의 Klebsiella pneumoniae에 대한 항균활성)

  • Klebsiella pneumoniae is found in the normal flora of the skin, mouth, respiratory tract, urinary tract, and intestines in human. However, the stain is opportunistic pathogen, which is the causative agent of community acquired pneumonia. Corn silk has been known to be effective for antimicrobial activity against pathogenic bacteria, including K. pneumoniae, Staphylococcus aureus, Bacillus subtilis, Shigella spp., Salmonella spp., Escherichia coli, Pseudomonas aeruginosa, et al. In this study we focused on the antimicrobial properties of con silk water extract of K. penumoniae. K. pneumoniae isolates K. pneumoniae ATCC 13883 and broad-spectrum β-lactamase (BSBL), exteded-spectrum β-lactamase (ESBL), carbapenemase-producers. Antimicrobial susceptibilities were determined by the disk diffusion method. Searches for bla genes were performed by PCR amplication and direct sequencing. MacConkey agar plate medium was prepared using the corn silk extracts (50% or 100%) instead of distilled water for antimicrobial activity test. The microbial growth inhibitory potential of K. pneumoniae was determined by using the MacConkey agar plate spreading method, and the plate was incubated 18 hr at 37℃. Genes encoding β-lactamases including SHV-1 (n=8), SHV-2a (n=8), SHV-5 (n=2), SHV-11 (n=2), SHV-12 (n=18), TEM-1 (n=10), CTX-M-3 (n=2), CTX-M-14 (n=2), CTX-M-15 (n=1), GES-5 (n=5), KPC-2 (n=6), KPC-3 (n=4), and NDM-1 (n=2) were detected. The corn silk extract showed significantly antimicrobial activity against K. pneumoniae ATCC 13883, but BSBLs, ESBLs, and carbapenemase producers were not. Therefore, corn silk extract is thought to be able to assist in the prevention and rapid recovery of infectious disease caused by K. pneumoniae.
