Streptococcus mutans is the major causative factor in dental caries and has been known to induce dental caries by the process of initial attachment proliferation and acid production. The pH of Streptococcus mutans(S. mutans) media and glucosyltransferase(GTase) activity were determined to evaluate the anticariogenic activity of Dioscorea batatas and Prunella vulgaris hexane fraction. In the experiment of hexane fraction of Dioscorea batatas, the relative growth ratio(RGR) against Streptococcus mutans (S. mutans) was determined as 93% in concentration of 0.125 mg/ml, 100% in 0.25 mg/ml. The hexane fraction of Prunella vulgaris revealed relative growth ratio(RGR) against Streptococcus mutans (S. mutans) as 86% in concentration of 0.25 mg/ml, 100% in 0.5 mg/ml. The relative growth ratio(RGR) against of hexane fraction were determined as 92% in concentration of 0.25 mg/ml, 100% in 0.5 mg/ml. The acidities were pH 7.2 on Dioscorea batatas and 7.0 on Prunella vulgaris in 2.0 mg/ml in contrast to pH 5.6 on Dioscorea batatas and 5.2 on Prunella vulgaris in control. The inhibitory effect to activity revealed 35% on Dioscorea batatas and 25% on Prunella vulgaris in 2.0 mg/ml. The inhibitory effect of Dioscorea batatas was more potent than Prunella vulgaris.
As an effort to select powerful anti-cariogenic materials from natural resources, various plant extracts were examined for their anti-S. mutans and anti-glucosyltransferase (GTase) activities. The ethanol extracts of licorice bark, which was produced after water extraction of licorice, showed the most powerful anti-S. mutans as well as anti-GTase activities. When licorice bark was consecutively fractionated with n-hexane, chloroform, ethylacetate, and butanol, the chloroform fraction exhibited the strongest anti-S. mutans activites. This fraction was further fractionated into 4 fractions through a silica gel column, and according to HPLC analysis, anti-S. mutant activities seemed to come mostly from relatively hydrophobic materials.

In the present study, we investigated the anticariogenic compounds from Aralia continertalis (A. continnentalis) has been isolated and identified by MS, $^1H-NMR$ and $^{13}C-NMR$. The MeOH extract was suspended in $H_2O$ and sequentially Partitioned with $CHCl_3$, EtOAc, and n-BuOH. The $CHCl_3$ fraction showed remarkable antibacterial activity against S. mutans. The antibacterial activity compounds against 5. mutans by MIC was isolated successively through the screening system and various chromatography methods. From this active chloroform subfraction, isolation and identification finally gave continentalic acid by spectroscopic methods (MS, $^1H-NMR$ and $^{13}C-NMR$) as an active principle. The compound, continentalic acid, showed significant growth, acid production, adhesion and water-insoluble glucan synthesis inhibitory effect against 5. mutars. These results suggest that continentalic acid from A. continentalis may inhibit cariogenic properties of S. mutans and these properties may provide some scientific rationales that the local inhabitants used the extracts for treatment of dental diseases.
Xylitol is a sugar alcohol with a variety of functions including bactericidal and anticariogenic effects. However, the cellular mechanisms underlying the role of xylitol in bone metabolism are not yet clarified. In our present study, we exploited the physiological role of xylitol on osteoclast differentiation in a co-culture system of osteoblastic and RAW 264.7 cells. Xylitol treatment of these co-cultures reduced the number of tartrate-resistant acid phosphatase (TRAP)-positive multinucleated cells induced by 10 nM $1{\alpha},25(OH)_2D_3$ in a dose-dependent manner. A cell viability test revealed no marked cellular damage by up to 100 mM of xylitol. Exposure of osteoblastic cells to xylitol decreased RANKL, but not OPG, mRNA expression in the presence of $10^{-8}M$$1{\alpha},25(OH)_2D_3$ in a dose-dependent manner. Furthermore, bone resorption activity, assessed on bone slices in the coculture system, was found to be dramatically decreased with increasing xylitol concentrations. RANKL and OPG proteins were assayed by ELISA and the soluble RANKL (sRANKL) concentration was decreased with an increased xylitol concentration. In contrast, OPG was unaltered by any xylitol concentration in this assay. These results indicate that xylitol inhibits $1{\alpha},25(OH)_2D_3$-induced osteoclastogenesis by reducing the sRANKL/OPG expression ratio in osteoblastic cells.

Numerous chemical agents have been developed to reduce the activity of cariogenic bacteria. Of these, chlorhexidine is acknowledged as the most effective. Gel and mouthrinse have been the traditional method of its application in the mouth. It has been reported that chlorhexidine varnish has prolonged inhibitory effect on the number of streptococcus mutans in saliva and plaque. Recently, chlorhexidine varnish and polyurethane sealant have been developed to promote prolonged anticariogenic effect of chlorhexidine. Products containing 10% chlorhexidine varnish and polyurethane sealant have been developed to prevent caries by reducing the number of streptococcus mutans in the oral cavity. The purpose of this study is to investigate the inhibitory effect of chlorhexidine varnish and polyurethane sealant on streptococcus mutans in the primary dentition. Children with primary dentition containing no active carious lesion were divided into two groups. To the experimental group (n=11), chlorhexidine varnish and polyurethane sealant ($Chlorzoin^{(R)}$, Knowell Therapeutic Technologies, Inc. Canada.) was applied once a week for four weeks according to the manufacturer's instruction. Only oral prophylaxis was performed on the control group(n=7). Caries activity was measured after using $Cariescreen^{(R)}$SM (Knowell Therapeutic Technologies, Inc. Canada,) to incubate streptococcus mutans before and 5, 12, 24 weeks after initial varnish application. The following results were observed.; 1. There was statistically significant decrease in the number of streptococcus mutans in the experimental group for 5 weeks(P<0.01), 12 weeks(P<0.05) after the initial application. but, by 24 weeks significant difference had disappeared. 2. As the inhibitory effect of chlorhexidine varnish and polyurethane sealant application is not everlasting, reapplication at 12-24 weeks should be needed.

It has been well established that S. mutans is the major etiological agent in dental caries, one of the most common oral diseases worldwide. The present study was designed to investigate the effect of Saururus chinensis (S. chinensis) ethanol extracts on the growth, acid production, biofilm formation, adhesion, and insoluble glucan synthesis of S. mutans. The ethanol extracts of S. chinensis showed concentration dependent inhibitory activity against the growth and acid production of S. mutans, and produced significant inhibition at the concentration of 0.025, 0.05, 0.1, 0.2 and 0.4 mg/ml compared to the control group. The extracts markedly inhibited S. mutans adherence to HA treated with saliva, and cell adherence was repressed by more than 80% at the concentration of 0.05 mg/ml and complete inhibition was observed at the concentration of 0.4 mg/ml. On the activity of glucosyltransferase which synthesizes water insoluble glucan form sucrose, ethanol extract of S. chinensis showed more than 10% inhibition over the concentration of 0.025 mg/ml. The synthesis of insoluble glucan was decreased in the presence of 0.025 ${\sim}$ 0.4 mg/ml of the ethanol extract of S. chinensis. Our research strongly suggested S. chinensis was a promising natural product for the prevention of dental caries.
In this study, to investigate the anticariogenic effect of red ginseng, the antibacterial effect, cell adherence and biofilm formation inhibitory effect of n-hexane extract of red ginseng marc (HERGM) against Streptococcus mutans, the causative bacteria of caries, were measured. The growth of S. mutans was inhibited in proportion to the concentration of HERGM, and was hardly observed at a concentration above 125 ㎍/mL (MIC =125 ㎍/mL). It was found that HERGM acts on the cell membrane and the nucleic acid component of the cell was leaked. In addition, HERGM inhibited the adherence and biofilm formation of S. mutans by more than 90% at a concentration of 125 ㎍/mL. GTase activity was completely inhibited at a concentration of 50 ㎍/mL of HERGM. In conclusion, it was found that HERGM commonly inhibited the growth and biofilm formation of S. mutans.
Erythritol is a sweetener produced by yeast from glucose and a natural sugar found in fermented foods such as mushrooms, wine, fruits, rice wine, and soy sauce. Correct information and basic data when producing or using products for preventing dental caries by checking the gene expression patterns of glucosyl transferase (GTF) and fructosyl transferase (FTF) of Streptococcus mutans in erythritol and other sweeteners it was implemented to provide. Erythritol inhibited the growth of Streptococcus mutans, which is involved in dental caries. When used as a sweetener to replace sucrose, erythritol had an excellent caries-preventative effect. In particular, erythritol reduced the expressions of gtfB, gtfC, gtfD, and FTF, which are related to the synthesis of extracellular polysaccharides, and thereby reduced the formation of dental plaque and the attachment rate of bacteria to tooth surfaces. The study shows erythritol has potential use as an anticariogenic sweetener that inhibits the mechanism underlying caries caused by Streptococci.

The aim of this study was to evaluate the preventive effect of commercially available anticariogenic products, specifically, the tooth cream containing Casein phosphopeptide-amorphous calcium phosphate(CPP-ACP), fluoride varnish and low-level fluoride mouthrinse on enamel erosion induced by carbonated beverage in a short period of time. Enamel specimens were treated as follows and were then kept in artificial saliva for 24 hours followed by further processing by alternately soaking them in Cola beverage and in distilled water for 1 minute each five times. Group 1: control group (no treatment) Group 2: tooth cream with CPP-ACP Group 3: fluoride varnish (1,000 ppm F) Group 4: low-level fluoride mouthrinse (227 ppm F) Group 5: fluoride varnish + tooth cream with CPP-ACP Group 6: low-level fluoride mouthrinse + tooth cream with CPP-ACP Microhardness and erosion depth were measured and the mineral loss of each specimen was evaluated by measuring the volumetric fluorescence change(${\Delta}Q$) against the stable fluorescent grid using quantitative light-induced fluorescence(QLF). The experiment lasted for 6 days repeated each day. The results were as follows: 1. The microhardness was increased as follows: Group $1{\leq}2{\leq}4$<6<$3{\fallingdotseq}5$. 2. The mean erosion depth was increased as follows: Group $5{\fallingdotseq}3$<6<$4{\fallingdotseq}2{\fallingdotseq}1$. 3. The ${\Delta}Q$ was increased as follows: Group $1{\fallingdotseq}2{\leq}4{\leq}6{\leq}3{\fallingdotseq}5$. The decrement of ${\Delta}Q$ was similar between group 1 and 2, group 4 and 6 and group 3 and 5. 4. The ${\Delta}Q$ showed positive correlation with microhardness (r=0.96, p<0.05), while it was negatively correlated to erosion depth (r=-0.96, p<0.05).
Dental caries is one of the most prevalent dental diseases in Korea and its prevention is very important in orthodontic therapy. For the cleansing action of saliva itself and/or tooth-brushing is lowered in patient with fixed orthodontic appliance, oral hygiene of the patient becomes worse, which provides more favorable environment for micro-organisms. Chlorhexidine, one of the series of bisguanide, has been reported to be strong antimicrobial agent and very effective on Streptococcus mutans. The purpose of this study is to evaluate the possibility of chlorhexidine as a anticariogenic agent in fixed orthodontic therapy. We used the varnish containing chlorhexidine as a main ingredient for the chemical control of salivary S.mutans in patients with fixed appliance therapy We applied the varnish containing chlorhexidine on the labial and interproximal surface of the teeth before bonding and banding teeth of our patients(N=20) and compared to control group patients(N=20). Before the application of chlorhexidine varnish and four times periodically after the completion of fixed appliance set-up, we sampled saliva of both group patients and incuvate S.mutans for 24 hours. In the culture study of sampled saliva, counting the number of S.mutans colonies, we founded as follows : 1. In patients with fixed appliance therapy, the risk of dental caries increase when it compared to that of preorthodontic treatment ; The number of salivary S.mutans increase in Patient's oral cavity. 2. The experimental agent that contain chlorhexidine is effective to reduce the number of salivary S.mutans. 3. For the effect of this agent is not ever-lasting, periodical application is needed, and additional study for economical interval and number of application is needed.
