Calcifying odontogenic cyst(COC) is comparatively rare in occurrence. COC represents about 1% of jaw cysts, and although it may occur in soft tissue, it is most commonly found within bone. Both the intraosseous and extraosseous forms occur with about equal frequency in the maxilla and mandible, mainly in the incisor and canine areas The most notable features of this pathologic entity are histopathological and include a cyst lining demonstrating characteristic "ghost" epithelial cells with a propensity to calcify and the occasional association of this finding with certain odontogenic tumors including the odontoma and the ameloblastoma. In this case, COC was associated with anterior wall of the maxillary sinus which appeared in the anterior maxilla of 64-year-old woman, was reported. We report that the clinical experience of COC with review of literatures.
This study was designed to investigate the difference between craniofacial characteristics of the normal occlusion and those of Class II Div. 1 malocclusion. The sample was divided into 2 groups, the 50 subjects of Normal occlusion, the 50 subjects of Class II Div. 1 malocclusion in both sexes. Both groups aged from 11 to 14 years. The results of this study were as follows; 1. No significant difference was observed in cranial base shape between both groups, but anterior cranial base size of Class II Div. 1 malocclusion group was larger than that of normal group. 2. No significant difference in antero-posterior position of Maxilla to cranial base was founded between both groups. 3. No difference in Mandibular shapes and Mandibular plane angles to the cranial base was observed between Class II Div. 1 malocclusion and normal occlusion, but Mandibular position in Class II Div. 1 malocclusion was posterior to that of normal group. 4. Antero-posterior relationship of Maxilla and Mandible was significant difference between both groups, but vertical relationship of those was no difference. 5. Maxillary incisor position to cranial base of Class II Div. 1 malocclusion was anteior to normal occlusion, and Maxillary posterior teeth was posterior. Mandibular incisor and mandibular posterior teeth position was no difference. 6. Upper and lower lip position to esthetic line of Class II Div. 1 malocclusion was anterior to normal occlusion.

Dental implants are the first option for replacement of missing teeth. Failure usually involves additional cost and procedures. As a result, the physician should limit the risk factors associated with implant failures. Implant site is one of many factors that can influence the success or failure of dental implants. The association between early implant failure (EIF) and implant site has yet to be documented. This review aims to estimate the impact of insertion site on the percentage of EIFs. An electronic and manual search of studies that reported early failure of dental implants based on collection site. A total of 21 studies were included in the review and examined for the association between EIF and alveolar site. Subgroup analysis, including a comparison between implants inserted in four alveolar ridge regions of both jaws was performed. The early failure rate was higher for maxillary implants (3.14%) compared to mandibular implants (1.96%). Applying a random effect, risk ratio (RR), and confidence interval (CI) of 95% revealed higher failure in the maxilla compared to the mandible (RR 1.41; 95% CI [1.19, 1.67]; P<0.0001; I2=58%). The anterior maxilla is more critical for early implant loss than other alveolar bone sites. Implants in the anterior mandible exhibited the best success rate compared of the sites.

Nasoalveolar molding (NAM) device is an effective treatment for protruding maxilla in infants with cleft palate. However, only a few studies have investigated the effect of NAM devices on the treatment of protruding maxilla in infants with cleft lip only. We have designed a combination treatment using NAM devices prior to cheiloplasy for cleft lip-only patients with severe anterior protrusion of the premaxilla. Three cleft lip-only infants with 1-cm or more of premaxilla protrusion were included. Definitive cheiloplasty was performed at 6 months of age without any preoperative correction in infant 1. Cheiloplasty was performed in conjunction with the use of NAM device and lip adhesion in infants 2 and 3. Postoperative columella length and anterior-posterior dimension of the protruding premaxilla were compared amongst the infants. We were able to obtain satisfactory postoperative columella length and general nasal appearance.
Objective: This study aimed to analyze the effect of changing various parameters of the bone-borne rapid palatal expander (RPE) using the finite element method (FEM). Methods: In eight experimental groups, we investigated the effect of the number, position, and length of miniscrews; positional changes of the expander; and changes in the hook length on maxillary expansion. In finite element analysis, we compared the magnitude and distribution of stress, and the displacement changes following expansion of the bone-borne RPE. Results: When we compared the number and position of miniscrews, placing miniscrews in the anterior and posterior sides was advantageous for maxillary expansion in terms of stress distribution and displacement changes. Miniscrew length did not significantly affect stress distribution and displacement changes. Furthermore, anteroposterior displacement of the expander did not significantly affect transverse maxillary expansion but had various effects on vertical changes of the maxilla. The maxilla rotated clockwise when the miniscrews were placed in the anterior region. The hook length of the expander did not show consistent results in terms of changes in stress distribution and magnitude or in displacement changes. Conclusions: The findings of this study suggest that changes in the location and length of the miniscrews and displacement of the bone-borne RPE could affect the pattern of the maxillary expansion, depending on the combination of these factors.
Purpose: The purpose of this study was to assess the relationship between gingival biotype and underlying crestal bone morphology in the maxillary anterior region. Materials and Methods: The maxillary anterior teeth from 40 subjects (20 thin biotype, 20 thick biotype) with ages from 20 to 50 years were included in this study. All subjects had healthy gingiva in the maxillary anterior region and had no history of orthodontic treatment, periodontal treatment, or hyperplastic medication. Using the probe transparency method, the scalloped distance (SCD) between the contact point-bone crest and the midface-bone crest was measured for each maxillary anterior teeth of two groups. Result: The mean SCD was $3.00{\pm}0.21mm$ in thin biotype and $2.81{\pm}0.20mm$ in thick biotype. The SCD value in the thin biotype was statistically significantly greater than in the thick biotype (t=2.982, P<0.01). Comparing the degree of crestal bone scallop in each maxillary anterior teeth in the two groups, all six teeth in the thin biotype showed higher bone scallop than in the thick biotype. Conclusion: A simple procedure using a probe could to determine gingival biotype and to predict the underlying crestal bone morphology was introduced. This may be useful for effective treatment planning.
The purpose of this study was to find out the craniofacial skeletal characteristics and to establish standards in facial patterns of Korean adolescences with normal occlusion. The subjects consisted of 54 males and 71 females ranging in age from 12 to 16 years. To classify facial patrons, number the clinical deviations from the normal five key measurements: the facial axis, facial angle, mandibular plane angle, lower face height, mandibular arc, are added and averaged with the proper sign to divide subjects into three groups ; brachyfacial, mesofacial dolichofacial groups. For the comparison of each group, a total of 43 morphologic variables were employed and the data were analyzed by statistical methods. The findings of this study can be summerized as follows; 1. The mesofacial group was $54.4\%$, the brachyfacial group was $29.6\%$ and the dolichofacial group was $16.0\%$ in this subjects. 2. There were no significant differences in size and shape of cranial base among each groups. 3. The brachyfacial group manifested the forward positioned maxilla and mandible to anterior cranial base, smaller cant of the mandibular plane, square shaped mandible, and prominent symphysis as compared with dolichofacial group. 4. There were no significant differences in the cant of the maxilla to the anterior cranial base among each groups. 5. Ramal height and madibular body length of brachyfacial group were larger than those of dolichofacial group. 6. Brachyfacial group indicated the smallest degree in divergency of maxilla and mandible while dolichofacial group showed the largest degree.
Objective: To investigate the three-dimensional (3D) surgical accuracy between virtually planned and actual surgical movements (SM) of the maxilla in two-jaw orthognathic surgery. Methods: The sample consisted of 15 skeletal Class III patients who underwent two-jaw orthognathic surgery performed by a single surgeon using a virtual surgical simulation (VSS) software. The 3D cone-beam computed tomography (CBCT) images were obtained before (T0) and after surgery (T1). After merging the dental cast image onto the T0 CBCT image, VSS was performed. SM were classified into midline correction (anterior and posterior), advancement, setback, anterior elongation, and impaction (total and posterior). The landmarks were the midpoint between the central incisors, the mesiobuccal cusp tip (MBCT) of both first molars, and the midpoint of the two MBCTs. The amount and direction of SM by VSS and actual surgery were measured using 3D coordinates of the landmarks. Discrepancies less than 1 mm between VSS and T1 landmarks indicated a precise outcome. The surgical achievement percentage (SAP, [amount of movement in actual surgery/amount of movement in VSS] × 100) (%) and precision percentage (PP, [number of patients with precise outcome/number of total patients] × 100) (%) were compared among SM types using Fisher's exact and Kruskal-Wallis tests. Results: Overall mean discrepancy between VSS and actual surgery, SAP, and PP were 0.13 mm, 89.9%, and 68.3%, respectively. There was no significant difference in the SAP and PP values among the seven SM types (all p > 0.05). Conclusions: VSS could be considered as an effective tool for increasing surgical accuracy.
Purpose: The aim of the present clinical trial was to compare pain during injection of anterior middle superior alveolar (AMSA) technique with that of infiltration injection technique in the maxilla in periodontal flap surgeries of patients referring to the Department of Periodontics, Faculty of Dentistry, Tabriz University of Medical Sciences. Methods: Twenty subjects with an age range of 20 to 40 years were selected for the present study. One side of the maxilla was randomly selected as the test side and the other as the control side using a flip of a coin. AMSA technique was used on the test side and infiltration technique was used on the control side for anesthesia. On both sides 2% lidocaine containing 1:80,000 epinephrine was used for anesthesia. The operator obtained the visual analogue scale for each patient immediately after the injection and immediately after surgery. Data was analyzed using descriptive statistical methods (frequency percentages, means and standard deviations) and Wilcoxon's test using SPSS ver. 13 (SPSS Inc.). Statistical significance was defined at P<0.05. Results: There were no statistically significant differences in pain during injection between the two techniques (P=0.856). There were statistically significant differences in postoperative pain between the two injection techniques (P=0.024). Conclusions: Postoperative pain in AMSA injection technique was less than that in the infiltration technique. Therefore, the AMSA technique is preferable in the periodontal surgeries for the anesthesia of palatal tissues given the fact that it has other advantages, too.
