Page kidney after botulinum toxin injection during chiropractic care

  • Page kidney refers to the phenomenon of hypertension secondary to long-standing compression of renal parenchyma caused by renal subcapsular collection. The most common cause of renal subcapsular collection is a hematoma which usually occurs after a history of blunt trauma. A 42-year-old female patient who received botulinum toxin injection in her back during chiropractic care was admitted to the emergency room with sudden bilateral flank pain and hypertension. The computed tomography (CT) images demonstrated the presence of bilateral subcapsular renal hematoma. The patient was treated conservatively and recovered well. The follow up CT images showed markedly resolved bilateral hematoma.

Effect of Acupuncture Stimulation on Heart Rate Variability in Stroke Patients (침자극이 뇌졸중 환자의 심박변이도에 미치는 영향)

  • Objectives : This study was performed to observe effect of acupuncture stimulation on heart rate variability in stroke patients. Methods : Data were obtained from heart rate variability of 29 stroke patients. Heart rate variability was measured total five times(before and after needle insertion, before and after needle removal, and 10 minutes after needle removal) for five minutes each time. Results : Heart rates were continuously decreased until 10 minutes after needle removal. SDNN, Total power, LF, and HF were increased until 5 minutes after needle removal, but deceased between 5 and 10 minutes after needle removal. There was no significant changes between male and female and among age groups. Conclusion : This study showed that acupuncture stimulation could effect on heart rate variability in stroke patients. Heart rate variability could be a objective tool to verify the mechanism of acupuncture effect in various condition.

  • This study describes a fetal heart rate(FHR) estimation algorithm using phonogram. Using a phonogram amplifier, various fetal heart sounds are collected in a university hospital. The FHR estimation algorithms consists of a lowpass filter, decimation, envelop detection, pitch detection, and post-processing. The post-processing is the FHR decision procedure using all informations of fetal heart rates. Using the algorithm and other parameters of fetal heart sound, a fetal monitoring software was developed. This can display the original signals, the FFT spectra, FHR and its trajectory. Even though the fetal phonogram amplifier detects the fetal heart sounds well, the sound quality is not so good as the ultrasonography. In case of very week fetal heart sound, autocorrelation of it showed clear periodicity. But two main peaks in one period is an obstacle in pitch detection and peaks are not so vivid. The proposed FHR estimation algorithm showed very accurate and stable results. Since the developed software displays multiple parameters in real time and has convenient functions, it will be useful for the phonogram-style fetal monitoring device.

  • A 7-month-old girl with no medical history was treated with mechanical circulatory support due to myocarditis. Her cardiac contractility did not improve despite more than one week of extracorporeal membrane oxygenation treatment. Thus, we planned a heart transplant. However, a high level of cytomegalovirus was found in blood laboratory results by quantitative polymerase chain reaction. The patient's heart contractility recovered to normal range four days after ganciclovir treatment. She was discharged with slightly decreased cardiac contractility with a left ventricular ejection fraction of 45%.

Effects of a Recreation Therapy Program on Mental Health and Heart Rate Variability in Burn Rehabilitation Patients (치료레크리에이션 프로그램이 화상재활환자의 정신건강 상태와 심박동 변이에 미치는 효과)

  • Purpose: This study was done to evaluate the effects of a recreation therapy program on burn rehabilitation patients to determine if the program is an effective nursing intervention which can affect mental health problems and heart rate variability. Methods: Subjects were 54 hospitalized burn rehabilitation patients (25 in the control group, 29 in the experimental group). The experimental group participated 6 times in a recreation therapy program led by a qualified instructor. Brief symptoms inventory-18 (somatization, anxiety, depression) questionnaire, and heart rate variability were checked before and after the recreation therapy program. Results: The mental health scale showed significant differences in somatization (p<.001), anxiety (p<.001) and depression (p<.001). There was no significant difference in heart rate variability (autonomic activity, autonomic balance, stress resistance, stress parameter and fatigue, mean heart rate, electro-cardiac stability). Conclusion: The findings from this study suggest that a recreation therapy program is an effective nursing intervention to decrease the level of mental health problems of burn rehabilitation patients. However, a subsequent study is needed to develop an intervention program that will induce the effect of physiological parameters like heart rate variability (HRV).

Perfusion Techniques Using the Modified Isolated Working Rat Heart Model (흰쥐의 심장을 이용한 Modified Isolated Working Heart Perfusion Technique)

  • We have modified an isolated perfusion rat heart model of cardiopulmonary bypass, with which we are able to screen the effects of various cardioplegic solutions and hypothermia upon the ability of the heart to survivie during and recover from period of ischemic arrest. The modified experimental model was differed from the original as follow : a heat coil chamber of atrial and aortic reservoir provided temperature control, and the perfusate was gassed with each pure oxygen and pure carbon dioxide in 95:5 ratio. The Langendorff perfusion was initiated for a 10 minute period by introducing perfusate at $37^{\circ}C.$ into the aorta from the aortic reservoir located 100 cm above the heart. The isolated perfused working rat heart model was a left heart preparation in which oxygenated perfusion medium (at $37^{\circ}C.$) entered the cannulated left atrium at a pressure of 20 cm $H_{2}O$ and was passed to the ventricle, from which it was sponeously elected(no electrical pacing) via an aortic cannula, against a hydrostatic pressure of 100cm $H_{2}O$. during this working period various indices of cardiac functin were measured. The cardiac functions were stable for over 3 hour with perfusion of Krebs-Henseleit bicarbonate buffer solution containing only glucose (11.1 mM/L). The percentage of cardiac functins were maintained about 94% on heart rate, 80.6% on peak aortic pressure, 87.7% on coronary flow and 76.3% on aortic flow rate after 3 hour of working heart perfusion at a pressure of 20 cm $H_{2}O$. We believe this preparation to be a good biochemical model for the human heart which offers many advantages including economic, speed of preparation, reproducibility, and the ability to handle large numbers.

Protective Effect of Lonicerae Flos Aqueous Extracts on a Pressure Overload-induced Heart Failure Model

  • Objectives: Lonicerae flos (LF), a dried flower part of Lonicera japonica Thunb., has been widely used in Korean medicine as anti-inflammatory and antioxidative agent. The purpose of this study was to determine the cardioprotective effects of LF, through potential antioxidant effects, on the pressure overload (PO)-induced heart failure (HF) in C57BL/6 mice after transverse aortic constriction (TAC) surgery. Methods: Resveratrol (10 mg/kg body weight) or LF (125, 250 or 500 mg/kg body weight) was orally administered, once daily for 14 days, starting 14 days after TAC surgery. Changes in the mortality, body weights, heart weights, histopathology of the heart, and antioxidant defense systems of the heart were analyzed. Results: Marked and noticeable increases of heart weights, mortalities, and hypertrophic, focal, and lytic fibrotic histological changes in the LVs were observed, with destruction of heart antioxidant defense systems after surgery. However, HF signs, induced by TAC surgery through PO, and destruction of heart antioxidant defense systems were significantly and dose-dependently inhibited by 14 days of maintained oral treatment with LF 500, 250 or 125 mg/kg. Treatment with 250 mg/kg LF was comparable to treatment with 10 mg/kg resveratrol. Conclusions: The results in this study suggest that oral administration of LF favorably relieves PO-induced HF following TAC, through increase of heart antioxidant defense systems. The overall effects of 250 mg/kg LF were similar to those of 10 mg/kg resveratrol. More detailed mechanistic studies should be conducted in the future, with screening of the biologically active compounds in LF.

  • In this article, the effects of stress on central nerve system and heart function and the concept of heart rate variability were reviewed. HRV(Heart Rate Variability), the periodical change of the heart rate, is indicated larger in the healthier because they respond flexibly to various sorts of facts influencing on HR. HRV analysis is largely composed of the time domain analysis and the frequency analysis. In the former the flexibility of heart function is analysed, while in the latter autonomic nerve function is examined, which is the degree of sympathetic and parasympathetic nerve activity and the state of balance. Furthermore, existence or nonexistence of disease and/or level of stress can be estimated by measuring the variability and normality of heart rate, and balance of autonomic nerve system, and through HRV biofeedback the symptoms of anxiety disorder or asthma can be reduced.

Dual-Phase Approach to Improve Prediction of Heart Disease in Mobile Environment

  • In this paper, we propose a dual-phase approach to improve the process of heart disease prediction in a mobile environment. Firstly, only the confident frequent rules are extracted from a patient's clinical information. These are then used to foretell the possibility of the presence of heart disease. However, in some cases, subjects cannot describe exactly what has happened to them or they may have a silent disease - in which case it won't be possible to detect any symptoms at this stage. To address these problems, data records collected over a long period of time of a patient's heart rate variability (HRV) are used to predict whether the patient is suffering from heart disease. By analyzing HRV patterns, doctors can determine whether a patient is suffering from heart disease. The task of collecting HRV patterns is done by an online artificial neural network, which as well as learning knew knowledge, is able to store and preserve all previously learned knowledge. An experiment is conducted to evaluate the performance of the proposed heart disease prediction process under different settings. The results show that the process's performance outperforms existing techniques such as that of the self-organizing map and gas neural growing in terms of classification and diagnostic accuracy, and network structure.

Assessment of the Cardioprotection Offered by Fisetin in H2O2-induced Zebrafish (Danio rerio)-Tg (cmlc2: egfp)

  • The aim of this study was to evaluate the protective function of fisetin, a natural flavonoid in zebrafish heart for the treatment of myocardial infarction in coronary and ischemic heart disease. For this purpose, we induced oxidative stress zebrafish (Danio rerio)-Tg (cmlc2: egfp) by $H_2O_2$ and then administered fisetin, the protective effect of fisetin was determined by measuring the heart rate following fisetin administration. After testing the toxicity of fisetin, we found that the heartt increased in a concentration-dependent manner, however there was no difference between the heart rates of embryos and adults. The improved heart rate demonstrated the cardioprotective effect of fisetin. The result showed that fisetin, at concentration of 3and $5{\mu}M$, significantly increased heart rate compared with the heart with $H_2O_2$ alone. This indicates that fisetin plays an important role in the prevention of heart damage and treatment of cardiovascular diseases caused by oxidative stress due to ischemia / reperfusion.
