  • Objectives: The aim of this study was to evaluate and quantify the risk of both individual and combined health behaviors on premature mortality in middle aged men in Korea. Methods: In total, 14 533 male subjects 40 to 59 years of age were recruited. At enrollment, subjects completed a baseline questionnaire, which included information about socio-demographic factors, past medical history, and life style. During the follow-up period from 1993 to 2008, we identified 990 all-cause premature deaths using national death certificates. A Cox proportional hazard regression model was used to estimate the hazard ratio (HR) of each health risk behavior, which included smoking, drinking, physical inactivity, and lack of sleep hours. Using the Cox model, each health behavior was assigned a risk score proportional to its regression coefficient value. Health risk scores were calculated for each patient and the HR of all-cause premature mortality was calculated according to risk score. Results: Current smoking and drinking, high body mass index, less sleep hours, and less education were significantly associated with all-cause premature mortality, while regular exercise was associated with a reduced risk. When combined by health risk score, there was a strong trend for increased mortality risk with increased score (p-trend < 0.01). When compared with the 1-9 score group, HRs of the 10-19 and 20-28 score groups were 2.58 (95% confidence intervals [CIs], 2.19 to 3.03) and 7.09 (95% CIs, 5.21 to 9.66), respectively. Conclusions: Modifiable risk factors, such as smoking, drinking, and regular exercise, have considerable impact on premature mortality and should be assessed in combination.

  • Objectives: A web-based survey has been administered annually since 2005 throughout Korea to assess the prevalence of adolescent health risk behaviors among middle and high school students. The aim of this study was to evaluate the test-retest reliability of the Korea Youth Risk Behavior Web-based Survey (KYRBWS) questionnaire. Methods: A convenience sample of 2298 middle and high school students participated in self-administered questionnaire surveys twice, approximately two weeks apart, in 2008. The percent agreement, kappa statistics, and prevalence rates at the first and second surveys were computed for the core subset of 39 self-reported health risk behavior indices of the KYRBWS. Results: Among 39 indices, seven indices had kappas $\geq$ 0.81 and all of the indices had kappas $\geq$ 0.41. Based on nonoverlapping 95% confidence intervals, three indices had significantly different prevalence rates between the first and second surveys. In the subgroup analyses by school grade and gender, two indices had significantly different reliability estimates between middle and high school students. There were no significantly different reliability estimates between male and female students, except for one index. Conclusions: This study demonstrated that the reliability estimates for the KYRBWS questionnaire are varied, but generally reliable over time. The indices with low reliability estimates need to be evaluated further in order to determine whether the indices should be modified or deleted from future versions of the KYRBWS.

  • Objectives: This study estimated the association of cardiovascular health behaviors with the risk of all-cause and cardiovascular disease (CVD) mortality in middle-aged men in Korea. Methods: In total, 12 538 men aged 40 to 59 years were enrolled in 1993 and followed up through 2011. Cardiovascular health metrics defined the following lifestyle behaviors proposed by the American Heart Association: smoking, physical activity, body mass index, diet habit score, total cholesterol, blood pressure, and fasting blood glucose. The cardiovascular health metrics score was calculated as a single categorical variable, by assigning 1 point to each ideal healthy behavior. A Cox proportional hazards regression model was used to estimate the hazard ratio of cardiovascular health behavior. Population attributable risks (PARs) were calculated from the significant cardiovascular health metrics. Results: There were 1054 total and 171 CVD deaths over 230 690 person-years of follow-up. The prevalence of meeting all 7 cardiovascular health metrics was 0.67%. Current smoking, elevated blood pressure, and high fasting blood glucose were significantly associated with all-cause and CVD mortality. The adjusted PARs for the 3 significant metrics combined were 35.2% (95% confidence interval [CI], 21.7 to 47.4) and 52.8% (95% CI, 22.0 to 74.0) for all-cause and CVD mortality, respectively. The adjusted hazard ratios of the groups with a 6-7 vs. 0-2 cardiovascular health metrics score were 0.42 (95% CI, 0.31 to 0.59) for all-cause mortality and 0.10 (95% CI, 0.03 to 0.29) for CVD mortality. Conclusions: Among cardiovascular health behaviors, not smoking, normal blood pressure, and recommended fasting blood glucose levels were associated with reduced risks of all-cause and CVD mortality. Meeting a greater number of cardiovascular health metrics was associated with a lower risk of all-cause and CVD mortality.

  • Objectives : The physical activity of Korean adolescents and its distribution based on social characteristics have not yet been fully assessed. This study intends to reveal the distribution of physical activity by its subgroups and offer possible explanatory variables. Methods : The 3rd Korea Youth Risk Behavior Web-based Survey was analyzed for this study. The appropriateness of physical activity was defined by Korea s Health Plan 2010 and physical inactivity was assessed independently. Family affluence scale, parents education levels, subjective economic status, grade, and school location were considered explanatory variables. All statistical analysis was conducted using SAS ver. 9.1. Results : The proportion of participants engaging in vigorous physical activity was high in males (41.6%), at a low grade (38.5%), within the high family affluence scale group (35.5%). The distribution of participants engaging in moderate physical activity showed similar patterns, but the overall proportion was lower (9.8%). Low family affluence and students with lower subjective economic status reported a higher prevalence of physical inactivity. In multiple logistic regression analysis for physical activity, significant factors included family affluence scale (p<0.05). For physical inactivity, family affluence scale, parents education levels, and subjective economic status were included as significant factors (p<0.05). Conclusions : The results suggest that the physical activity and inactivity of adolescents may be affected by socioeconomic variables, such as family affluence scale. This implies the need to take proper measures to address these socio-economic inequalities.

  • In order to obtain basic data for a health promotion program of hair dressers, the health promoting behavior of some hair dressers was assessed with the related factors such as general characteristics and cognitive-perceptual factors. The subjects were 245 hair dressers working at beauty shops having at least two hair dressers. The data was collected by a questionnaire from October 14 to October 20, 2001. The results were as follows: 1. The mean score of health promoting behavior conducted by the hair dressers was 31.17 out of 50.00 in total score. 2. The score of the health promoting behavior was statistically different according to marriage, on-duty hours, holidays, satisfaction in the job, perceived health status, perceived fatigue, health concept, and self-efficacy (p<.05), while it was not statistically different according to gender, religious faith, residence, eduation, economic levels, career in the job, average income, medical care insurance, employment insurance, control of fortuity, and control over other persons. 3. The health promoting behaviors in the hair dressers showed positive correlation with self-efficacy and internal control, while it showed negative correlation with control over other people, control of fortuity, health concept, and perceived fatigue. 4. The Regression analysis results of the questionnaire showed that perceived fatigue, health concept and office hours were significant factors but the age, the fact of being single or couple, position at the job, holidays, satisfaction of the job, self-control and stresses from other people and self-efficiency appeared insignificant. In conclusion, the hair dressers with less perceived fatigue and better cognitive health concept and short duty hours showed better health promoting behavior. So, In order to conduct health promoting behaviors of hair dressers, the method will need to control of working time and perceived fatigue.
