한방병원에 입원한 환자 147명의 간기능 검사에 대한 후향적 관찰 (Retrospective Observation of Liver Function Test in 147 Patients Admitted to Korean Medicine Hospital)

  • The purpose of this study was to evaluate the changes of liver function test before and after treatment in patients admitted to Korean medicine hospital. We checked liver function test level (aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, total bilirubin, direct bilirubin) of 147 patient who admitted in Korean medicine hospital from July 1, 2015 to June 30, 2018. The subjects were selected those who took herbal medicine continuously during the admission period and who performed liver function test on admission and before discharge. And the subjects were excluded those who had a history of liver and biliary disease at the time of admission or who took hepatoprotectants. Aspartate aminotransferase, alanine aminotransferase and alkaline phosphatase significantly decreased compared with the values of admission(p<0.05), but total bilirubin and direct bilirubin were not significantly changed(p>0.05). On admission 31 patients(21.1%) had abnormal liver function and 6 patients(4.1%) had liver injury while 19 patients(12.9%) had abnormal liver function and 6 patients(4.1%) showed liver injury before discharge. This study suggests that herbal medicine may not injure liver function.

흰쥐의 알코올 유발성 간손상에 실비음(實脾飮)이 미치는 보호 효과 (The Protective Effects of Silbi-um Extract on the Alcoholic Liver Injury in Rats)

  • Objectives: The objective of this study is to investigate the effects of Silbi-um (SBU) extract on the alcoholic fatty liver induced by EtOH administration for 8 weeks. Methods: Male Sprague Dawley rats were used. All animals were randomly divided into 3 groups; Normal, EtOH and EtOH+SBU. The rats of EtOH group were daily treated with ethanol of 25% (v/v) for 8 weeks (n=10). EtOH+SBU group was orally treated with SBU water extract after ethanol administration (n=10). The rats of Normal group were treated with saline (n=10). After 8 weeks, the mean body weight, liver weight, and liver-body weight ratio were calculated. The serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) of all groups were measured. The morphological alterations were observed using hematoxylin and eosin (H&E) and Oil Red O staining. Moreover, the alteration of tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) levels were analyzed immunohistochemistrically. Results: The histological data showed that liver sections from EtOH group displayed severe steatosis. SBU extract significantly inhibited the progression of the alcoholic liver injury. The increased serum level of ALT and AST induced by ethanol administration were decreased by SBU extract. Furthermore, SBU extract significantly decreased the liver concentrations of $TNF-{\alpha}$. Conclusions: SBU water extract attenuated the alcohol induced fatty liver by improving hepatic lipid metabolism via suppression of $TNF-{\alpha}$ protein. SBU could be effective in protecting the liver from alcoholic fatty liver.

Four Case Reports on Treatment of elevated aminotransferase levels with Herbal medicine containing Artemisia capillaris as Principal Component

  • Background: Many herbal medications have been used to treat various liver diseases. But the concerns of herbal medicine induced liver injury also existed. In this respect, we would like to report several cases with imporved elevated liver aminotransferase after treating herbal medicine including Artemisia capillaris. Case Reports: We report four patients with elevated aminotransferase levels (ATLs), which indicate hepatocellular damages. After receiving herbal medicines therapy containing Artemisia capillaris as principal component (HMA), the patients' ATLs were improved. In the first case, the patient's ATLs decreased into normal range after administration of HMA, although they have not been improved with hepatotonics for a long period. In the second case, the patient's ATLs have been elevated after taking anticoagulants. The ATLs were improved with HMA without stopping anticoagulants. In the other two cases, the patients' ATLs were also improved after taking HMA. In addition, there were no changes of previous drugs for treating the corresponding underlying diseases and no adverse events during HMA applications. Result and Conclusion: The four patients received the herbal medicine containing Artemisia capillaris as principal component and showed improvement of ATLs. These cases suggest that HMA can be considered as alternative or complementary remedies to improve various liver diseases.

Correlation between Transient Elastography (Fibroscan®) and Ultrasonographic and Computed Tomographic Grading in Pediatric Nonalcoholic Steatohepatitis

  • Purpose: This study aimed to examine the advantages and usefulness of transient elastography (Fibroscan®) in diagnosing non-alcoholic steatohepatitis in children and adolescents compared to those of abdominal computed tomography and liver ultrasonography. Methods: Forty-six children and adolescent participants aged between 6 and 16 years who underwent transient elastography (Fibroscan®) as well as liver ultrasonography or abdominal computed tomography were included. Thirty-nine participants underwent liver ultrasonography and 11 underwent computed tomography. The physical measurements, blood test results, presence of metabolic syndrome, and the degree of liver steatosis and liver fibrosis were analyzed, and their correlations with transient elastography (Fibroscan®), abdominal computed tomography, and liver ultrasonography, as well as the correlations between examinations, were analyzed. Results: Thirty-six participants (78.3%) were boys, and the mean age was 12.29±2.57 years, with a mean body mass index of 27.88±4.28. In the 46 participants, the mean values for aspartate aminotransferase, alanine aminotransferase, and total bilirubin were 89.87±118.69 IU/L, 138.54±141.79 IU/L, and 0.77±0.61 mg/dL, respectively. Although transient elastography (Fibroscan®) and abdominal computed tomography grading had a statistically significant positive correlation with aspartate aminotransferase and alanine aminotransferase values, the correlations between the results of grading performed by transient elastography (Fibroscan®), abdominal computed tomography, and liver ultrasonography were not statistically. Conclusion: We confirmed that each examination was correlated with the results of some blood tests, suggesting the usefulness and possibility of diagnosis and treatment of steatohepatitis mediated by transient elastography (Fibroscan®) in the department of pediatrics.

수온별 넙치(Paralichthys olivaceus)의 스트레스 반응 및 산소소비 변화 (Change on Stress Responses and Oxygen Consumption of Olive Flounder, Paralichthys olivaceus at Different Water Temperature)

  • 급격한 수온의 변화는 어류의 생리학적인 측면에서 스트레스를 유발한다. 본 연구에서는 넙치(Paralichthys olivaceus)로부터 각 수온별(9, 12, 15, 18 및 21℃) 조건에 따라 24 및 48시간 동안 노출시킨 후에, 혈액생리학적 분석, 스트레스 단백질로 알려진 Hsp70 mRNA 발현 및 산소 소비량을 조사하였다. 혈액학적 분석에서 hematocrit (Ht) 및 hemoglobin (Hb), 혈장 코티졸 및 글루코스의 변화, aspartate aminotransferase (AST) 및 alanine aminotransferase (ALT), NH3, 삼투질농도(osmolality) 및 총단백질(total protein, TP)은 9℃ 및 12℃에서 다른 수온별 실험구에 비해 대부분의 항목에서 유의적인 차이를 보였다. Hsp70 mRNA 발현은 9℃ 및 12℃에서 다른 실험구에 비해 높은 발현량을 확인하였고, 산소소비량은 9℃ 및 12℃에서 21℃에 비해 낮았다. 이러한 결과는 넙치 종자의 장거리 수송을 위한 수온자료로 활용할 수 있다.

Effects of Artichoke Supplementation on Liver Enzymes: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

  • Studies examining the effect of artichoke on liver enzymes have reported inconsistent results. This systematic review and meta-analysis aimed to assess the effects of artichoke administration on the liver enzymes. PubMed, Embase, the Cochrane Library, and Scopus databases were searched for articles published up to January 2022. Standardized mean difference (Hedges' g) were analyzed using a random-effects model. Heterogeneity, publication bias, and sensitivity analysis were assessed for the liver enzymes. Pooled analysis of seven randomized controlled trials (RCTs) suggested that the artichoke administration has an effect on both alanine aminotransferase (ALT) (Hedges' g, -1.08; 95% confidence interval [CI], -1.76 to -0.40; p = 0.002), and aspartate aminotransferase (AST) (Hedges' g, -1.02; 95% CI, -1.76 to -0.28; p = 0.007). Greater effects on ALT were detected in trials that lasted ≤8 weeks. Also, greater effects on AST were detected in trials using > 500 mg artichoke. Overall, this meta-analysis demonstrated artichoke supplementation decreased ALT and AST.

저수온 스트레스시 자바리, Epinephelus bruneus에서의 vitamin E의 생리적 효과 (Physiological effects of dietary vitamin E on kelp grouper, Epinephelus bruneus under low water temperature stress)

  • The physiological response of the longtooth grouper, Epinephelus bruneus to low water temperature (LWT) stress while on three different concentrations of dietary vitamin E (1 mg/diet g, 5 mg/diet g, and 10 mg /diet g) were investigated. Plasma cortisol, glucose, and aspartate aminotransferase (AST) levels were higher in the control and sham control compared with the vitamin E supplemented groups. The most addition with vitamin E in the diet (10 mg/diet g) showed the lowest levels of cortisol, glucose, and AST. However, plasma alanine aminotransferase (ALT) showed no significant differences across all experimental conditions (P>0.05). Also the more disposing vitamin E into feeding the better resistance against stress on the LWT was shown through cortisol, glucose, ALT, and AST.

  • Effects of ${\alpha}$-tocopherol and perilla oil on the toxicity of polychlorinated biphenyls (PCB) in male rat were studied. Rats were fed ad libitum for 6 weeks with the animal diet which contains PCB 30 ppm and 100 ppm. Perilla oil (0.5 g/kg body weight) and ${\alpha}$-tocopherol (30 mg/kg body weight) were administered intraperitoneally twice a week for 6 weeks. Rats fed with PCB showed enlargement of liver and spleen, increase in aspartate aminotransferase, alkaline phosphatase, sereum lipid and cytochrome P 450 and decrease in body weight and glutathione. When perilla oil was administered to rats fed with PCB increase in aspartate aminotransferase, alkaline phosphatase, serum lipid and cytochrome P45O and decrease in body weight and glutathione were significantly augmented, compared to rats fed with PCB alone. This means that perilla oil potentiates the toxicity of PCB. On the other hand when ${\alpha}$-tocopherol was administered to rats fed with PCB increase in aspartate aminotransferase, alkaline phosphatase, serum lipid and cytochrome P45O and decrease in body weight and glutathione were signigicantly reduced, compared to rats fed with PCB alone. This means that u-tocopherol reduces the toxicity of PCB. From the above results, it may be concluded that PCB is metabolized by microsomal mixed function oxidase and the metabolite causes the toxicity and microsomal glutathione plays a role of protection on the toxicity of PCB.

CYTOTOXICITY OF D-GALACTOSAMINE ON PRIMARY CULTURES OF ADULT RAT HEPATOCYTES

  • Primary cultures of adult rat hepatocytes were used to study the cytotoxicity of D-galactosamine. Hepatocytes were isolated by a collagenase perfusion technique and maintained as monolayers in serum-free medium on collagen-coated culture dishes. Treatment of galactosamine to the culture markedly inhibited the uptake of ${\alpha}$-aminoisobutyric acid (AIB) inducible with glucagon and dexamethasone. At0.1 mM of galactosamine, AIB uptake was inhibited significantly when treated for 12 hr. At higher doses (0.25, 0.5 and 1.0mM), a significant inhibition was noticed after 1 hr exposure. Generally the magnitude of the inhibition was related to the dose and treatment time of galactosamine. Treatment of galactosamine also produced a dose- and treatment time-related suppression of the tyrosine aminotransferase (TAT) induction caused by dexamethasone. Meanwhile, uptake of ouabain was not affected by the treatment of galactosamine. The viability of the hepatocytes was decreased only slightly by the treatment of galactosamine; more than 87% of the cells excluded tryphane blue when treated 1 mM galactosamine for 12 hr. Galactosamine induced depressions of AIB uptake and TAT activity were prevented by the simultaneous addition of uridine to the culture. Predictors of nonresponse to intravenous immunoglobulin therapy in Kawasaki disease

  • Purpose: It has been reported that 10% to 20% of children with Kawasaki disease (KD) will not respond to intravenous immunoglobulin (IVIG) treatment. In this study, we aimed to identify useful predictors of therapeutic failure in children with KD. Methods: We examined 309 children diagnosed with KD at the Kyungpook National University Hospital and the Inje University Busan Paik Hospital between January 2005 and June 2011. We retrospectively reviewed their medical records and analyzed multiple parameters in responders and nonresponders to IVIG. Results: Among the 309 children, 30 (9.7%) did not respond to IVIG. They had significantly higher proportion of neutrophils, and higher levels of aspartate aminotransferase, alanine aminotransferase (ALT), total bilirubin, and N-terminal fragment of B-type natriuretic peptide than did responders. IVIG-nonresponders had a significantly longer duration of hospitalization, and more frequently experienced coronary artery lesion, and sterile pyuria. No differences in the duration of fever at initial treatment or, clinical features were noted. Conclusion: Two independent predictors (ALT${\geq}$84 IU/L, total bilirubin${\geq}$0.9 mg/dL) for nonresponse were confirmed through multivariate logistic regression analysis. Thus elevated ALT and total bilirubin levels might be useful in predicting nonresponse to IVIG therapy in children with KD.
